Abstract

Background: Osteoarthritis is a joint disease most often found in the community. Patients with osteoarthritis are frequently drink non steroidal anti inflammatory drugs such diclofenac sodium. Data has shown too many side effects of diclofenac sodium especially to the kidney function. A research data in Malang shows that people who use herbs were 476 persons in city area and 580 persons in distric area per thousand patients with arthritis. Curcuma domestica Val. is one of Asia's native plants used to make herbal medicine mainly used to reduce inflammation. The purpose of this study was to assess the safety of curcuminoid from Curcuma domestica Val. rhizome extract compared to diclofenac sodium to the kidney function of patients with osteoarthritis.  Methods: This was a prospective randomized open and blinded evaluation (PROBE) study. Subjects with knee osteoarthritis were divided randomly into two groups: the group who received 30 mg 3 times daily of curcuminoid from Curcuma domestica Val rhizome extrac (curcuminoid group) and group who received 25 mg 3 times daily of diclofenac sodium (diclofenac group). Assessment of results including serum blood urea nitrogen (BUN) and creatinine level was performed before and after 4 weeks period of treatment. Data analysis compared the change of those levels in each group during the treatment period by student t test analysis. Results: In the curcuminoid group there was no significant decrease of serum BUN level (p=0.52) and there was a significant decrease of serum creatinine level (p=0.03). In the diclofenac group there was a significant increase of serum BUN level (p<0.01) and no significant increase of serum creatinine level (p=0.39). Increasing the serum level of BUN and creatinine in diclofenac group were significantly different compared to decreasing of those level in the curcuminoid group with p=0.01 and p=0.03 respectively. Conclusion: Treatment with Curcuminoid from Curcuma domestica Val. rhizome extract was significantly decreased the serum BUN and creatinin level compared to those increased level in diclofenac sodium treatment. AbstrakLatar belakang: Osteoartritis merupakan penyakit sendi yang paling banyak dijumpai di masyarakat. Pasien osteoartritis sering menggunakan obat anti inflamasi non steroid seperti natrium diklofenak. Data menunjukkan banyaknya efek samping natrium diklofenak khususnya terhadap fungsi ginjal. Data penelitian di Kotamadya dan Kabupaten Malang menunjukkan bahwa penduduk yang menggunakan jamu masing-masing adalah 476 orang dan 580 orang per seribu pasien. Kunyit (Curcuma domestica Val.) adalah salah satu tumbuhan asli Asia yang biasa dibuat jamu yang utamanya digunakan untuk mengurangi peradangan. Tujuan penelitian ini adalah untuk menilai keamanan terapi kurkuminoid ekstrak rimpang kunyit dibandingkan dengan natrium diklofenak terhadap fungsi ginjal penderita osteoartritis. Metode: Penelitian ini dirancang sebagai prospective randomized open and blinded evaluation (PROBE). Subjek dengan osteoartritis lutut yang memenuhi syarat dan bersedia ikut dalam penelitian ini dibagi menjadi dua kelompok secara random yaitu kelompok yang mendapatkan terapi kurkuminoid ekstrak rimpang kunyit 30 mg diminum 3 kali sehari (kelompok kurkuminoid) dan kelompok yang mendapatkan terapi natrium diklofenak 25 mg diminum 3 kali sehari (kelompok diklofenak). Penilaian hasil terapi yang meliputi kadar blood urea nitrogen (BUN) dan kreatinin serum dilakukan sebelum dan setelah 4 minggu terapi. Analisis data untuk membandingkan perubahan kadar BUN dan kreatinin pada masing-masing kelompok perlakuan dilakukan dengan student t test. Hasil: Pada kelompok yang mendapat terapi kurkuminoid ekstrak rimpang kunyit terjadi penurunan kadar BUN serum yang tidak bermakna (p=0,52) dan penurunan kadar kreatinin serum secara bermakna (p=0,03). Pada kelompok yang mendapat terapi natrium diklofenak terjadi peningkatan kadar BUN serum secara bermakna (p<0,01) dan peningkatan kadar kreatinin serum yang tidak bermakna (p=0,39). Peningkatan kadar BUN dan kreatinin serum pada terapi natrium diklofenak berbeda bermakna dengan penurunan kadarnya pada terapi kurkuminoid (p=0,01 dan p=0,03). Kesimpulan: Terapi kurkuminoid ekstrak rimpang kunyit menurunkan kadar BUN dan kreatinin serum penderita osteoartritis secara bermakna dibandingkan dengan peningkatan kadarnya pada terapi dengan natrium diklofenak.

Abstract

Background : As one of important markers of cardiovascular complication in patients with type 2 diabetes mellitus, low density lipoprotein (LDL) shall be maintained to be in normal range. Based on previous research, consumption of probiotic yogurt contains Lactobacillus acidophilus and Bifidobacterium animalis subsp. Lactis is presumed can improve LDL in patients with type 2 diabetes mellitus but the scientific evidence is still few.Objectives: To investigate the effect of probiotic yogurt on LDL in patients with type 2 diabetes.Methods: This study was double blinded randomized controlled trial which was conducted on 32 patientss with type 2 diabetes mellitus. Subjects were divided randomly into 2 groups and given 4 weeks intervention. Subjects in control group were given conventional yogurt while subjects in intervention group were given probiotic yogurt contains L. acidophilus LA5 and B. animalis subsp. Lactis Bb12. Food intake data was collected 4 times using 24-hour recall method during intervention period. LDL of subjects was measured before and after intervention.Results: There was no significant difference between LDL before and after intervention in control group (128 – 148.5 mg/dL) and intervention group (130 – 120.5 mg/dL). Meanwhile, LDL after intervention in control group and intervention group were significantly different (p<0.05). LDL between energy and macronutrients intake categories were not significantly different.Conclusion: Probiotic yogurt has a better LDL reduction effect in patients with type 2 diabetes mellitus than conventional yogurt, although the reduction is not significant.

Abstract

Background: Generally, patients with type 2 diabetes mellitus (T2D) have dysbiosis condition. Dysbiosis can increase oxidative stress that leads to hyperglycemia. Previous researches showed that yogurt consumption can reduce blood glucose in T2D, so it can be used as an alternative healthy snack for T2D patients.Objectives: To investigate the effects of probiotic and conventional yogurt with dosage 100ml/day on fasting blood glucose (FBG) in T2D patientsMethods: Randomized controlled trial, double blind with pre-post group design. The 30 T2D patients from 3 publics health centre in Yogyakarta, were assigned to two groups. Each group, either control or intervention group, received 100 ml/d of yogurt for 4 weeks. FBG samples was assessed before and after intervention periodResults: FBG significantly decreased in both group, which was -27 mg/dL in control group (p<0.05) and -19 mg/dL in intervention group (p<0.05). No significant difference in FBG change between intervention and control group, but control group has greater reduction in FBG compared to intervention group. Conclusion: Conventional yogurt has no significant difference effect in FBG change compared to probiotic yogurt.

Abstract

Mind body interventions (MBIs) are exercise that emphasizes the use of brain and body to assist the healing process and to manage symptoms in order to improve wellbeing. The MBIs are expected to be a supporting therapy for patients with autoimmune disease such as systemic lupus erythematosus (SLE), to improve patients’ activities, reduce fatigue, stress, and depression.One type of MBIs that could be potentially applied in Indonesia is latihan pasrah diri (LPD). Studies showed that MBIs can affect the expression of pro- and anti-inflammatory mediators. Nuclear factor-κB(NF-κB) is a transcription factor that controls gene expression related to many physiological responses including inflammation, proliferation, cell differentiation, and apoptosis. NF-κB can be activated through canonical and alternative pathway. This literature review aimed to identify the role of NF-κB as consequence of practicing MBIs in SLE patients. We searched for relevant publications in the MEDLINE/Pub Med and Google Scholar with no date restriction. This review revealed that abnormal NF-κB could mediate autoimmune condition in SLE pathogenesis. MBIs are expected to be a supportive treatment that can help to control NF-κB expression in SLE patients. LPD as an Indonesian original MBI is expected as a suitable techniques that can be applied in patients with SLE in Indonesia. Further studies on the effect of LPD on NF-κB expression in patients with SLE need to be further explored.

Abstract

Penelitian ini bertujuan meningkatkan hasil belajar mata pelajaran PPKn siswa kelas X AP1 SMK Negeri 1 Sukasada semester genap  tahun pelajaran 2017/2018 dengan penerapan model pembelajaran  Kooperatif tipe Role playing. Jenis penelitian ini adalah Penelitian Tindakan Kelas (PTK). Penelitian dilaksanakan sebanyak 2 siklus. Subyek penelitian adalah siswa kelas X AP1 SMK Negeri 1 Sukasada tahun pelajaran 2017/2018 semester genap yang berjumlah 30 orang. Data dianalisis menggunakan kuantitaif dan kualitatif. Berdasarkan hasil penelitian didapatkan terjadi peningkatan hasil belajar PPKn siswa siklus 1 dengan materi ?Wawasan Nusantara dalam konteks Negara kesatuan Republik Indonesia? pada siklus I dengan rata-rata sebesar 74,8  yang termasuk ke dalam kategori kurang, sedangkan ketuntasan klasikal sebesar 30 %. Sedangkan pada materi yang sama siklus II rata-rata PPKn siswa sebesar 85,81 yang termasuk klasifikasi baik, dan ketuntasan klasikal sebesar 100% dari 30 siswa. Dari analisis data di atas model pembelajaran kooperatif tipe Role Playing terbukti efektif dalam meningkatkan hasil belajar siswa. Kata Kunci: Hasil Belajar PPKn, Model Pembelajaran Kooperatif tipe Role Playing

Abstract

Herbal medicine has been an alternative treatment, instead of modern medicines. However, some of patients could experience adverse drug reaction (ADR) due to the use of herbal medicines. The ADR can decrease patients quality of life (QoL). Objective. This study was aimed to understand the QoL of patients prescribed with herbal medicines. Methods. Patients prescribed with herbal medicines as complementary treatment, in two Public Health Centers of Yogyakarta were recruited. The patients were prescribed with herbal medicines one year before this study was started. Naranjo algorithm was used to identify the ADR. The SF-36 questionnaire was given to the patients as the instrument to measure patients QoL. Results. There were 47 patients participated in this study. Most of the patients are female (62.5%).Ten patients experienced ADR. There are no significant differences between QoL domains in patients experienced ADR and those without ADR. However, the scores of physical function, role physical, general health, social function, role emotional, and mental health in patients without ADR are higher than those with ADR ( p0.05) . Conclusion. There are no significant differences of QoL between patients with and without ADR. In general, the QoL of patients without ADR is higher than those with ADR.

Abstract

Increasing the secretion of reactive oxygen intermediates (ROI) by monocytes in the synovial fluid is anindicator to determine the severity of joint inflammation. Previous studies have shown that curcumin inhibitthe osteoarthritis progression with its ability to inhibite the activity of the nitric oxide synthase (NOS) enzymefrom macrophages. In this prospective randomized open end blinded evaluations = PROBE study, 80 patientswith knee osteoarthritis were eligable. The subject were devided in to two group: group who received 3 x 30mg of curcuminoid from Curcuma domestica Val. extract (curcuminoid group) and group who received 3 x 25mg of diclofenac sodium (diclofenac group) as comparison. The treatment was for 4 weeks time. The secretionof ROI by sinovial fluid monocytes was calculated by scoring the amount of formazan formation after neutralred staining in nitrobleu tetrazolium reduction assay. The result of this study showed that the secretion of ROIby synovial fluid monocytes was significantly decreased in both groups (p <0.001) respectively. There wasno significant difference in decreasing of ROI secretion of synovial fluid monocytes between both treatmentgroups (p = 0.92).

Abstract

ABSTRACT Background: The prevalence of osteoarthritis (OA) in the community is high. This disease is the second most common cause of physical disability worldwide. Pain in OA is caused by several factors, such as inflammation. Non steroidal anti-inflammatory drugs (NSAIDs) were the most common drugs given worldwide to reduce pain in OA. NSAIDs were also associated with a high incidence of gastrointestinal side effects. An alternative to manage this problem is by using the combination of Curcuma xantorrhyza Roxb. (commonly known as temulawak) extract, ginger (Zingiber officinale) extract, soybean (Glycine max), and shrimp shell. Curcuma xantorrhyza contains curcumin which has anti-inflammatory effect by suppressing cyclo-oxygenase (COX-2) enzyme activity, suppressing lipo-oxygenase enzyme activity, and play a role as a free radical scavenger. Ginger can inhibit COX-2 activity in PGE-2 production. Shrimps shell contains glucosamine and chondroitin which can increase proteoglycan in articular chondrocytes and inhibit COX-2 synthesis. Isoflavone in soybean can inhibit articular cartilage degradation and COX-2 synthesis.Study Aims: The purpose of this study is to compare the effect of the combination to diclofenac sodium in reducing synovial fluid leukocyte count and joint pain in patients with osteoarthritis.Study Method:This study was a prospective randomized open end blinded evaluation (PROBE). Twenty one patients with knee osteoarthritis diagnosed by American College of Rheumatology criteria were included in this study. Patients were randomized into two groups to receive either diclofenac sodium 25 mg (control group) or the combination of Curcuma xantorrhyza extract 50 mg, ginger extract 100 mg, shrimp shell 100 mg, and soy bean flour 50 mg (treatment group) three times daily for 14 days. Independent t-tests and Mann-Whitney-Wilcoxon tests were used to evaluate changes between prior and post intervention.Results:  There were significantly reduction of synovial fluid leukocyte count in both control group (p=0.017) and treatment group (p=0.008) respectively.  The reduction of synovial fluid leukocyte count was not significantly different between control group and treatment group (p=0.929).  There were significant improvement of joint pain (VAS score) in both control group (p=0.012) and treatment group (p<0.001).  The reduction of VAS score was not significantly different between diclofenac group and treatment group (p=0.607).Conclution: These results indicate that the evicacy of this combiation was not significantly different with diclofenac sodium in reducing the synovial fluid leukocyte count and joint pain in patients with osteoarthritis.Keywords: osteoarthritis, Synovial fluid leukocyte count, Pain, VAS, Diclofenac sodium,Combinaion of curcuma, ginger, shrimp shell and soybean.

Abstract

ABSTRACTBackground: Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disorder that can be severe and life threatening. Mortality in SLE may due to lupus activity or long-term sequel. Systemic Lupus Activity Measurement (SLAM) score is a tool that can count lupus disease activity inpatients.Aims: To analyze the relationship between SLAM score and mortality in lupus inpatients.Methods: Retrospective cohort study used for reaching objective of the study. Lupus inpatients was used as research population. Medical record was used as study data collection over periods of 2006 until 2011. Independent variable was a SLAM score. The cut point of SLAM score was made based on the mean of SLAM score (16.7 point score). Dependent variable was mortality.Results: There were differences between number of ACR criteria fi ndings, pneumonia, heart rate with SLAM score (p value 0.001; 0.001; 0.002 respectively).There was a difference of median survival between less and more than 16.7 point score, 45 and 28 respectively (p 0.034). There was a relationship between SLAM score (more than 16.7 point score) and mortality HR 2.78 (96% CI 1.01-7.53). There was a difference of mortality incidence between more and less than 16.7 point score, 0.35 and 0.10 respectively. There was a relationship between SLAM score (more than 16.7 point score) and mortality RR 3.5 (95% CI). Mortality in lupus inpatients was 23%.Conclusion: There was a relationship between SLAM score and mortality on lupus inpatients.Keywords: SLE, SLAM, Mortality

Abstract

Severe Systemic Lupus Erythematosus patient was in the third of 28-days-cycle cyclophosphamide chemotherapy when the early symptoms of aspergillus infection occurred. This developed into extensive cranial bone damage. It was planned to get a biopsy and debridement for bone and soft tissue to diagnose and therapy palate osteonecrosis, followed by reconstruction surgery. This case report aimed to report the successful management of a severe patient on chemotherapy undergoing several operations so that the SLE aspergillosis is optimally managed.
