Abstract

The quality of life in elderly hypertension with overweight and non-overweightBackground: Indonesia will enter the period of aging and an increase in several elderlies with chronic diseases. The biggest health problems of the elderly are degenerative diseases, namely hypertension which significantly disrupts the quality of life both in terms of physical and mental health. One of the causal conditions associated with hypertension is overweight and obesity, which is known at this time the prevalence of obesity is growing progressively in old age. However, the results of research related to the influence of obesity on quality of life are still controversial.Objective: To determine differences in quality of life between hypertensive elderly who are overweight and non-overweight.Method: Cross-sectional study conducted in Yogyakarta City in Januari-April 2017 with a subject of 200 elderly people aged 60-74 years. Overweight was determined based on BMI measurements (>23 kg/m2) with an estimation of elderly height using depa length, quality of life of the elderly was measured using WHOQOL-BREF questionnaire, and compared the quality of life by age, gender, education, occupation, income, marital status, comorbidity, and disease history. Data analysis used independent t-test and linear regression.Results: Mean of environmental domain score was significantly higher (p=0,012) in elderly hypertension with overweight (26,69±3,28) compared to elderly hypertension non-overweight (25,44±3,58). Overweight protective effect on the low quality of life of the environment domain because it is influenced by other factors such as marital status, income, and education. Multivariate analysis shows that income and marital status are external factors that consistently influence the quality of life of the elderly both in total scores and per domain. High-income elderly people with married status have a better quality of life score.Conclusions: The mean score of the environmental domain is higher in elderly hypertension with overweight. Quality of life scores will be better in high income elderly and married.

Abstract

Background: Osteoarthritis (OA) is a chronic condition characterized by the breakdown of joints cartilage. Approximately 25% of persons 55 years of age or older have knee pain on most days and about half of them have radiographic OA in the knee. Prevalence of knee OA increases with age and it is more common in women than men. It is not easy to establish the diagnosis of knee OA since other knee disorders have similar clinicalsigns and symptoms.Objective: The purpose of this study was to observe the diagnosis pattern of knee OA in rheumatology outpatient clinic at Dr. Sardjito Hospital based on clinical and radiographic criteria of American College of Rheumatology (ACR). Method: The design of this study was cross-sectional. Data of the patients with knee OA were investigated from their medical records. Results: There were 212 subjects diagnosed with knee OA during the year 2000–2010. Most of the subjects (90.56%) were more than 50 years old. Women were more frequent affected by OA than men. All of the subjects (100%) had knee pain. Crepitus was found in98.11% subjects. Morning stiffness less than 30 minutes was found in 86.79% subjects. Osteophyte appearances were found in 79.72% subjects.Conclusion: Knee pain, crepitus, and age more than 50 years old were the most frequent criteria used to diagnose knee OA. Morning stiffness less than 30 minutes and osteophyte appearances were also frequent in knee OA.

Abstract

Background: Yogyakarta became one of the cities with quite high prevalence of childhood obesity at 8,98%. In other hand, shifting socio-economic conditions can alter maternal role model of healthy eating practices. These changes related to food intake and incidence of getting obesity in preschool children. Objective: To analyzed maternal eating practices as risk factor of obesity and identified the dominant risk factor related to obesity in preschool children aged 3-5 years. Method: This study was a case control study. Case of this study was obese preschool children (Z-score WHZ > 2 SD) and control of this study was non-obese preschool children (Z-score WHZ -2 ≤ SD ≤ 2). There were 101 obese children and 101 non-obese children based on the nutritional status screening result of 53 early childhood education (ECD) schools in Kota Yogyakarta. Control group was taken by an age and gender matching with the case group. Using Home environment Survey (HES) questionnaire to asses maternal eating practices. The data were analyzed using McNemar test and conditional logistic regression. Results: Maternal eating practices was significantly related to risk of obesity (OR=2,45; p=0,01; 95% CI=1,25-4,84). Maternal eating practices (OR=2,42; p=0,017) and maternal obesity (OR=2,62; p=0,003) were the most dominant risk factors related to the incidence of obesity in preschool children aged 3-5 years. Conclusion: Maternal eating practices and maternal obesity were significantly increase the risk for getting obese in preschool children aged 3-5 years. Abstract

Background : The use of herbal medicine in Indonesia is getting increase. People try to keep their health or to prevent some diseases using herbal medicines. However, the safety of herbal medicine used in community has not known yet, due to the limited data and believe that herbal medicine is safer than modern medicines. Objective : This study was aimed to understand the safety of herbal medicine used in Yogyakarta. Methods : We used retrospective data of patients who were prescribed herbal medicine by the physicians in two public health centers in Yogyakarta. We searched for the patients identity in the medical records then we did the interview and gave questionnaire to the patients at their homes. The Adverse Drug Reaction was identified using Naranjo algorithm. This study has been approved by ethical committee of Universitas Ahmad Dahlan, Yogyakarta. Results : We identified 47 patients prescribed with herbal medicine as complementary treatment. There are 10 patients (21.3%) experienced ADR. The category of ADR are: probable (7 patients) and possible (3 patients). Tensigard is the most phytopharmaca cause pruritus in this study. Conclusion : Around 20% patients experienced ADR due to the herbal medicine prescription in complementary treatment. Thus, the study of pharmacovigilance is needed to be conducted in bigger sample size and cohort design to understand the safety profile of herbal medicine used.

Abstract

Background: Cytokine Interleukin (IL) 1ß is increased in synovial fluid and cartilage of patients with osteoarthritis (OA). Curcuminoid blocks lipopolysaccharides causing the suppression of IL 1ß production. The capability of curcuminoid to decrease IL 1ß secretion by the synovial fluid monocytes has not been previously studied. Objective: This study aimed to explore the activity of curcuminoid of turmeric rhizome extract (C. domestica Val.) in blocking the IL 1ß secretion by the knee synovial fluid monocytes of patients with osteoarthritis.Methods: This research was a prospective randomized open and blinded endpoint (PROBE) study. The subjects of the study were osteoarthritis patients who were divided into 2 groups. Subjects were administered 30 mg of turmeric rhizome curcuminoid extract (treatment group) and 25 mg diclofenac sodium capsules (control group) twice a day for 4 weeks. Knee synovial fluid was taken before and after treatment. The capability examination of IL 1ß secretion by synovial fluid monocytes was conducted by culturizing the monocyte cells. Level of IL 1ß was measured by ELISA Results: There were 80 subjects eligible to participate in the study according to the inclusion/exclusion criteria. The decrease of means of IL 1ß concentration of the curcuminoid group after the 4-week therapy was 70.24±81.46 pg/ml, while the decrease of means of diclofenac sodium was 61.90±60.42 pg/ml, with no significant difference between groups (p=0.691;95%CI:0.681-0.699).Conclusion: Curcuminoid of turmeric rhizome (C. domestica Val.) extract has the capability which is not weaker than that with diclofenac sodium in decreasing the IL 1ß secretion by synovial fluid monocytes of the OA patients’ joints.

Abstract

The Relationship between Social Support and the Degree ofPain in Patient with Acute Phase of Gout ArthritisBackground: Pharmacological therapies currently used for pain relief in patient with acute phase of gout arthritis often cause serious side effects, such as gastrointestinal bleeding and disturbance of kidney or liver function. The psychological stress is able to increase the pain severity. Some researches conclude that the social support is able to decrease the pain. Other than pharmacological treatment it should be necessary to address the social support in management of patients with acute phase of gout arthritis.Objective: The objective of this research is to determine the extent to which the relationship between social support on the degree of pain in patients with acute phase of gout arthritis.Method: This research used a cross sectional design. There were 21 patients with acute phase of gout arthritis recruited in this research. The questioner of social support and the visual analogue scale were used in this research. Analysis of the relationship of social support and the degree of pain used Pearson correlation.Result: The results of Pearson correlation analysis showed that there was a significant relationship between the social support and the degree of pain (r = - 0.437 and p = 0.047) in patients with acute phase of gout arthritis.Conclusion: This study showed that the higher the social support, the lower the degree of pain in patients with acute phase of gout arthritis.Keywords: social support, degree of pain, acute phase of gout arthritis

Abstract

According to the Indonesian Lupus Foundation (YLI) in the last decade, the number of people with SLE (Systemic Lupus Erythematosus) has continued to increase every year. This research was undertaken to assess the medication knowledge and quality of life among SLE outpatients who had been counselled by pharmacist. A cross-sectional, non-experimental, descriptive study design was used in this research. A total of 121 respondents from Semarang and Yogyakarta met the criteria. Seventy (57,85%) respondents had previous counseling, while 51 (42,15%) respondents never. Their knowledge score was 32,17 ± 3,52 and the quality of life was 59,23 ± 17,04. There was any correlation between the knowledge score and their quality of life. *P 0,014. Their average age was 31,53 ± 7,77 years. It was positively related with their Quality of life *P 0,031 but not to their knowledge score. The participants’ education level and disease duration were not considerably related with QoL and knowledge. Previous counseling by the pharmacists was positively related to their knowledge score, but not to their quality of life (*P 0,034 and 0,793). The conclusion that can be drawn is the SLE outpatients had low knowledge and their quality of life is not good enough. Pharmacists’ counseling can improve SLE outpatients’ knowledge scores significantly but not their QoL.

Abstract

Background. Graves' disease (GD) is an autoimmune disorder known to be the most common cause of hyperthyroidism. Thyrotropin receptor antibody (TRAb) might be involved in the occurrence and the disease process of Graves’ ophthalmopathy (GO).Objectives. This study aimed to evaluate whether TRAb levels are associated with the occurrence and severity of GO based on the clinical severity of The European Group on Graves' Orbitopathy (EUGOGO).Methods. A case-control study of 44 patients with newly diagnosed Graves’ disease (22 with GO compared to 22 without GO). Diagnosis of GO was made according to Bartley and Gorman’s criteria. Level of thyrotropin receptor antibody was tested with electrochemiluminescence immunoassay (ECLIA) method. Assessment of the clinical severity of GO was documented with EUGOGO scores.Results. Baseline characteristics were similar between 22 patients with GO compared to 22 patients non-GO group. Thyrotropin receptor antibody (TRAb) significantly increased in the GO group (11.223±7.116 IU/L) when compared to non-GO (6.720±3.442 IU/L; P=0.035). Multiple logistic regression analysis shows that 1 IU/L increase of TRAb has a 1.610-fold higher risk for developing GO. In the GO group, there is correlation between TRAb and the severity of GO-based on EUGOGO (r =0.794, P<0.001).Conclusion. Thyrotropin receptors antibody is a risk factor for the occurrence and severity of GO-based on EUGOGO.

Abstract

Background. Systemic Lupus Erythematosus (SLE) requires comprehensive and integrated treatment because it can manifest in various organ systems, both non-major and major organs. Pulse doses Methylprednisolone used as induction therapy, it provides dramatic improvement in prognosis in severe lupus through its nongenomic effects. Not all SLE patients who received pulse dose methylprednisolone therapy gives complete response, several factors may influence the therapeutic response, one of which is thought to influence the difference in therapeutic response is the number of major organ involvement.Objectives. To determine the relationship between the number of major organ involvement and therapeutic response of pulse dose methylprednisolone therapy in SLE patients at Dr. Sardjito Hospital Yogyakarta.Methods. This study used a retrospective cross-sectional study. The subjects of this study were adult patients with SLE treated in the internal medicine ward at Dr. Sardjito Hospital Yogyakarta and met the inclusion and exclusion criteria from January 1, 2016 to December 31, 2019. The data on the characteristics of the research subjects were taken from the patient's medical records and laboratory data before giving pulse methylprednisolone.Results. A total of 88 research subjects were taken from medical records. Patients with major organ involvement 1 experienced more complete response (53.8%), major organ involvement 2 mostly partial response (66.7%), major organ involvement 3 (48.6%) and major organ involvement 4 (53.8%) no response, and major organ involvement 5 (75%) partial response. There is a significant relationship with the number of major organ involvement with therapeutic response p<0.001. The correlation coefficient r=0.382 means that the more major organ involvement the less therapeutic response. The results of multivariate analysis showed that only the number of major organ involvement had a dominant effect on the therapeutic response, p=0.001. Regression coefficient 0.797.Conclusion. The number of major organ involvement significantly affects the success of pulse dose methylprednisolone therapy in SLE patients.

Abstract

ABSTRACT Systemic Lupus Erythematosus (SLE) adalah gangguan autoimun multisistem kronis ditandai dengan berkembangnya autoantibodi dan kompleks imun dalam berbagai manifestasi klinis dan kerusakan jaringan. Perjalanan penyakit SLE yang ditandai dengan eksaserbasi dan remisi memerlukan pemantauan yang ketat akan aktivitas penyakitnya. Evaluasi aktivitas penyakit ini berguna sebagai panduan dalam pemberian terapi. Pengukuran aktivitas penyakit dilakukan dengan Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) Evaluasi penggunaan obat dan pengukuran aktivitas penyakit pada pasien SLE merupakan penelitian non eksperimental dengan jenis penelitian bersifat deskriptif, pengambilan data secara concurrent. Sedangkan penelitian pengaruh konseling terhadap tingkat pengetahuan dan sikap pasien SLE merupakan penelitian eksperimental semu. Pasien SLE yang bersedia menjadi responden seluruhnya perempuan paling banyak pada kelompok usia 21-40 tahun, aktivitas penyakit sangat berat sebanyak 33,33%. Penggunaan obat pada pasien SLE diketahui bahwa tepat pasien sebesar 88,89%, tepat dosis sebesar 55,56%. penggunaan obat pada pasien SLE diketahui bahwa tepat pasien sebesar 88,89% , tepat indikasi sebesar 88,89%, tepat dosis sebesar 55,56%. Pemberian konseling dapat meningkatkan pengetahuan dan sikap pasien tentang SLE serta terapinya, dan dapat meningkatkan outcome terapi berupa peningkatan kualitas hidup dan penurunan tingkat depresi dan tingkat nyeri (VAS) pasien SLE.
