Abstract

Achalasia is a rare motor disorder of the esophagus and lower esophageal sphincter. The incidence is approximately 1/100,000 per year and the prevalence rate is 10/100,000. Achalasia is quite difficult to establishbecause the symptoms might be insidious and therefore not many people come to seek medical attention until it deteriorates to final stage of the disease. There are several modalities that can be used as diagnostic toolssuch as manometry, barium esophagogram, esophagoduodenoscopy, esophageal CT-scan, until the recent one, high-resolution manometry that can classify achalasia into three different types. The treatment options are the pharmacologic intervention, endoscopic treatment, minimal invasive surgery, and radical surgery.We reported a case of 20 year old female with achalasia who came with dysphagia symptom since three years before. The diagnosis was made by historytaking, physical examination and barium meal and esophagogastroduodenoscopy. The patient underwent pneumatic dilatation and since then the symptom was relieved.Keywords: achalasia, diagnostic, treatment

Abstract

Patients diagnosed with hematology malignancy and solid tumor who underwent chemotherapy, frequently encountered hepatitis B reactivation. Patients with blood cancer, including lymphoma, had higher risk of reactivation compared to those with solid tumor. Steroid and immunosuppressant drugs contained in chemotherapy regimens were responsible for those events. Hepatitis B reactivation during chemotherapy treatment could contribute to acute liver failure and increase mortality. Administration of analog nucleoside therapy in this group of patient prior to the initiation of chemotherapy could prevent reactivation of hepatitis B.A 43 year-old male patient were diagnosed with diffuse large B cell non-Hodgkin lymphoma stadium II BE (oropharynx) under chemotherapy and had hepatitis B. In this evidence based case report, we reported a critical appraisal of the role of lamivudine as preemptive therapy in blood cancer and solid tumor.Keywords: chemotherapy, hepatitis B reactivation, malignancy

Abstract

Clostridium difficile (C. difficile) is the main pathogen causing antibiotics associated diarrhea and colitis. This bacterium increases with hospitalization with incidence of 20-60 cases per 100,000 patients/day. C. difficile is gram positive bacilli which produce toxins in 2,700 cases in every 100,000 exposures to particular antibiotics, such as clindamycin, cephalosporin, and ampicillin. These antibiotics disrupt the intestinal normal flora and predispose to colonization of C. difficile. This case described a 53-year old male patient with squamous cell carcinoma in his left ear who came to Department of Internal Medicine, Cipto Mangunkusumo Hospital, with the complain of diarrhea since two weeks after one month hospitalization in Department of Ear, Nose, and Throat. The characteristics of the diarrhea were 10 times per day ± 100 mL, watery consistency, green yellowish in color, and no blood in the feces. Additionally, the patient also complained of pain in all parts of his stomach, especially in the epigastric area. Earlier, patient was given ceftazidime for 30 days of hospitalization. The serology examination of C. difficile in the feces showed positive result (titer = 0.790 and control = 0.190). During the colonoscopy examination, pancolitis was found and the pathologic anatomy result was found appropriate for infective colitis. Thereafter, antibiotic administration was ceased and metronidazole was administered intravenously three times a day. The diarrhea stopped after seven days and the patient was discharged. Abstract

Five percents of patient presenting with gastrointestinal (GI) bleeding, the etiology of bleeding could not be found by upper endoscopy and colonoscopy. Almost 75% of which, the abnormality is detected in small bowel. One of the etiologies in small bowel bleeding is jejunal diverticle. In this paper, we reported a female, 38 years old, came with upper GI bleeding since one month ago. She had undergone several diagnostic procedures, such as abdominal ultrasound, abdominal computed tomography scan (CT-scan), upper and lower endoscopy, but there were no conclusion to explain the cause of bleeding. However, barium follow through examination found a diverticle, pouch-like shape, at jejunal proximal projection. Then, she underwent surgical treatment. Small intestine bleeding is best investigated by capsule endoscopy and double balloon enteroscopy. However, in limited conditions, small bowel follow through can be used to screen the source of bleeding in small intestine. The specific diagnosis of small intestine diverticle is possible by radiologic contrast study using various form of barium. Small bowel diverticle does not require surgical treatment, unless refractory symptoms or complications occur. Jejunal diverticle is one of sources in small intestinal bleeding. Abstract

Introduction: COVID-19 outbreak has caused changes around the world with many polices remade to stop the spread of this virus since it started in 2019. Indonesia with the overall highest positive cases in South-East Asia has been challenged with prolonged restriction policy issued from early 2020 until now due to continuous increase of cases. This study aims to know the mental health of Indonesia citizens during early quarantine before it changed into restriction. Method: Indonesian version DASS-21 questionnaire was used in this cross-sectional study to assess Indonesian adults aged > 25 years old using an online platform from 22nd April, 2020 to 28th May, 2020, then analyzed using T-test and one-way ANOVA. Result: All participants have no depression and stress symptoms that meet the threshold for probable depression according to the DASS-21 instrument. On the other hand, 26.3% participants showed anxiety symptom with severity classified into mild (16.0%), moderate (8.9%), and severe (0.4%). Conclusion: This study also found relationship of depression, anxiety, and stress characteristic with gender, age marital status, and income in adults during pandemic. Where in this study shows male, younger age, unmarried status, and lower income people have higher scale of depression, anxiety, and stress characteristic. This finding may help Indonesia’s government and citizens to assess the restriction to mental health of Indonesian adults for further effective policy implementation.

Abstract

Background: To prevent the spread of COVID-19, authorities implemented health protocols such as quarantine, large-scale social restrictions, and lockdowns. Data on the psychological effects and sleep quality as a result of these health protocols are still limited.Objective: This study was aimed to determine the association between depression, anxiety, and stress with sleep quality during the COVID-19 pandemic in Indonesia.Methods: An online survey was used to do a cross-sectional study among students in Indonesia. Sociodemographic information, including age, gender, religion, level of education, and regions, was analyzed. Levels of depression, anxiety, and stress were measured using the DASS-21 questionnaire. Sleep quality was determined using Pittsburgh Sleeping Quality Index (PSQI) questionnaire. Chi-square was used to evaluate the variables for bivariate analysis and logistic regression for multivariate analysis.Results: A total of 913 participants from across Indonesia included to this study. Adolescent (OR=0.367, 95% CI=0.199-0.679), female (OR=1.437, 95% CI=1.095-1.886), and high school students (OR= 0.737, 95% CI=0.544-0.999) had a higher risk for having poor sleep quality. In the multivariate analysis there were significant relationship between sleep quality with depression (Adjusted OR= 1.887, 95% CI=1.330-2.679), anxiety (Adjusted OR=1.731, 95% CI=1.221-2.455), stress (Adjusted OR= 2.577, 95% CI=1.617-4.107), use of sleeping medication (Adjusted OR=9.070, 95% CI=2.062-39.896) and age (Adjusted OR= 0.414, 95% CI= 0.205-0.837).Conclusion: There was an association between depression, anxiety, stress, consumption of sleeping medication, age, and poor sleep quality during the pandemic in Indonesia.

Abstract

Background: COVID-19 is an unprecedented public health event. It started from Wuhan, China, in late December of 2019 and later spread worldwide. During the pandemic, negative impacts on mental health were found in all groups, including adults of their productive age. Objective: This study aimed to search for factors related to Post Traumatic Stress Symptoms (PTSS) during the quarantine due to the COVID-19 pandemic in Indonesia, specifically about the demographic background of respondents and its relation to PTSS. Methods: This study employed a cross-sectional design involving 257 Indonesian adults aged 26-67 years from 26 out of 34 provinces in Indonesia. The questionnaire used in this study consists of demographic questions and an Indonesian version of the Impact of Event Scale-Revised (IESR). The questionnaire was distributed through a link attached to the social media platform from 17 September to 5 October 2020. Data were analyzed using t-test, one-way ANOVA, and multiple linear regression using SPSS 25.0. Results: The data analysis showed a significant relationship between age and mean IES-R score during quarantine time in Indonesia (p-value 0.001). Other demographic factors such as gender, education level, marital status, work from home, income during the pandemic, and pandemic effects to income showed no significant relation to the IES-R score of the respondents. Adults aged 25-44 years old tend to have higher IES-R scores than those aged 45-64 and 65+ years old. Conclusion: Age was related to the IES-R score, while the other five independent variables included in the linear regression analysis were found to be confounders in this study.

Abstract

Sickle cell disease (SCD) is a hemoglobinopathy inherited in an autosomal recessive pattern arising from a mutation causing substitution of valine for glutamic acid in the sixth amino acid of the β-globin chain. The clinical manifestation of SCD varies from acute pain, chronic anemia, to multi-organ infarct and failure. SCD is known to be rare in Indonesia. However, we recently encountered a SCD patient presenting with recurrent pain and hypesthesia since a year ago. The diagnosis of SCD in this patient was established based on the results of peripheral blood smear and hemoglobin electrophoresis. The patient had previously been diagnosed with thalassemia at the age of 19 years old due to a history of icterus, thus delaying prompt treatment. This further emphasizes the challenges in distinguishing SCD, which is relatively rare in Indonesia compared to thalassemia, especially considering the complexity and the variability in the phenotypic presentation of SCD. The patient was then given supportive therapy consisting of blood transfusion and symptomatic treatment, as well as consideration to administer hydroxyurea if the patient’s clinical condition worsens.

Abstract

Context: Coronavirus disease 2019 (COVID-19) is a newly discovered disease, caused by SARS-CoV-2 and currently has become a pandemic. Acute respiratory distress syndrome (ARDS) is the most common complication in COVID-19. There were numerous conflicting results among articles related to it. The aim of this study is to ascertain the most compelling evidence about ARDS in COVID-19.Evidence Acquisition: This systematic review was registered in PROSPERO (CRD42020180379). A systematic search was conducted in PubMed, PubMed central, and Google Scholar on April 16, 2020. Two reviewers independently searched and selected the articles. The risk of bias was evaluated using the Newcastle-Ottawa Quality assessment tool.Results: A total of 1,647 articles were screened, 9 articles were included. Patients were classified as having various degrees of ARDS, the diagnosis of COVID-19 was confirmed by PCR nasopharyngeal swab.  Risk factors of ARDS in COVID-19 reported were older age, male gender, and pre-existing medical conditions. Cytokine storm was thought to play a role in the mechanism of ARDS. The main treatment for COVID-19 was supportive and symptomatic. To date, there is no antiviral treatment recommended for COVID-19 and the given treatment for ARDS in COVID-19 was similar to other pneumonia-induced ARDS. No additional therapy specific for ARDS in COVID-19 has been proposed.Conclusion: Our synthesis of the literature showed that there was no good evidence in the mechanism and treatment of ARDS. Further translation research in the mechanism of ARDS and continuing with clinical trials evaluating drug efficacy for ARDS in COVID-19 is needed.

Abstract

Introduction: The World Health Organization (WHO) announced the Coronavirus 2019 (COVID-19) as a Public Health Emergency of International Concern (PHEIC) toward the end of January 2020. There is still limited evidence to explain the gastrointestinal involvement in COVID-19. In this study, we aimed to further investigate current evidence describing the gastrointestinal involvement in COVID-19 patients.Methods: This systematic review has been registered in PROSPERO (CRD42020181584). A systematic search of literature for observational and randomized controlled trial was conducted in PubMed, PubMed central, and Google Scholar through April 16, 2020. Two reviewers independently searched and selected. The risk of bias was evaluated using the Newcastle-Ottawa Quality assessment tool. Results: A total of 1,480 articles were screened from which 12 articles with 5584 subjects were selected. SARS-CoV-2 can invade human body by binding to angiotensin converting enzyme 2 (ACE-2) receptor which also located to small intestinal epithelial cells, crypt cells and colon. The virus itself may cause disorders of the intestinal flora. The diagnosis should be based on a set of symptoms diarrhoea, nausea, vomiting, abdominal discomfort or pain, combined with positivity of faecal PCR test. Treatment of COVID-19 mainly is supportive care. The probiotic may modulate the gut microbiota to alter the gastrointestinal symptoms and reduced enteritis, ventilator associated pneumonia, and reverse certain side effect of antibiotics.Conclusion: Our synthesis of literature showed that there was no good evidence yet in overall area of gastrointestinal manifestations in COVID-19. Future research is needed to explore all areas, especially in mechanism and treatments
