Abstract

Background: Chronic renal failure  (CRF) had became  a concern  because of the  increasing  globally for incidence, prevalence, and mortality and hypertension was  one of the main causes of CRF. magnitude of the risk of hypertension with chronic renal failure. This study aims to assess the magnitude of the risk of hypertension with chronic kidney failure at Dr. Djasamen Saragih City of Pematang Siantar.Methods: This research was an analytic observational study with a case control design. The research subjects were 260 kidney failure patients who were treated at RSUD Dr. Djasamen Saragih Kota Pematang Siantar with as many as 52 cases of patients diagnosed with chronic kidney failure by doctors who can be seen from medical records. As many as 208 hospital controls were patients without CRF (gastritis, gastroenteritis, hernia, and trauma or minor infections). The inclusion criteria of the study subjects were ≥ 18 years of age and had completed medical record data. The exclusion criteria for study subjects were if they had a history of kidney transplantation or a history of kidney transplantation and patients with incomplete medical records.Results: the results of the analysis of bivariate hypertension variables, patients aged > 45 years, patients with diabetes, patients with anemia, patients with gout, and patients with a history of kidney stones showed a relationship with the incidence of kidney failure. The multivariate results showed that the incidence of hypertension in patients with chronic renal failure was 5.52 times higher than those without chronic renal failure with an OR of 5.52 (95% CI: 2.10-14.53).Conclusion: Hypertension has a relationship with the incidence of chronic renal failure with an OR value of 5,52 (95% CI: 2.10-14.53).

Abstract

Banyaknya beban masalah kesehatan yang dihadapai dan terbatasnya sumber daya yang dimiliki tidak memungkinkan untuk dapat ditanggulangi semuanya dalam waktu yang sama. Oleh Karena itu perlu ditentukan masalah apa saja yang perlu segera ditanggulangi berdasarkan analisis situasi. Tujuan penelitian ini adalah untuk mendapatkan gambaran tentang situasi masalah Penyakit Potensial Kejadian Luar Biasa (KLB) di Provinsi DKI Jakarta tahun 2021. Analisis situasi ini menggunakan desain observasional deskriptif dengan menggunakan pendekatan kuantitatif dan kualitatif. Tehnik penetapan prioritas masalah yang di gunakan adalah metode PAHO adaptive Hanlon. Hasil dari total 24 jenis penyakit potensial KLB, 19 jenis penyakit potensial wabah muncul di Provinsi DKI Jakarta periode tahun 2021. 5 Jenis penyakit potensial wabah tidak ditemukan laporan kasus pada periode tahun 2021. 10 urutan tertinggi prioritas masalah penyakit potensial wabah di Provinsi DKI Jakarta adalah: COVID-19, Campak, Pnemonia, Acute Flacid Paralysis (AFP), Dengue Fever, Difteri, Malaria, Hepatitis A, ILI (Influenza like illness), dan Diare Akut. Sedangkan prioritas utama masalah penyakit potensial wabah di Provinsi DKI Jakarta adalah COVID-19.

Abstract

Tuberculosis is still a major cause of health problems and death due to infectious agents. The success rate of TB treatment varies globally where Asia is 81.6% the second highest after Oceania 83.9%. The prevalence of tuberculosis with bacteriological confirmation in Indonesia is 759 per 100,000 population. This study aims to determine the relationship between the independent variabel with the success of TB treatment in Indonesia in 2018. Methods This study was conducted using a retrospective cohort study design. The study population was all TB Drug Sensitive patients who started treatment in 2018 and were recorded on SITT. Data analysis was performed using Chi Square. 392,706 TB patients were included in the analysis of 476,744 patients. With treatment results 45.2% recovered, 43.35% complete treatment, 3% died, 0.8% failed, 5.6% dropped out of treatment, 2.1% people moved. Age> 45 years RR 1,028 (95% CI; 1,009-1,049), female gender 1,022 (95% CI; 1,004-1,041), HIV negative 1,229 (95% CI; 1,169-1,292), patients with new treatment status RR 1,072 (95% CI; 1.033-1.112) and did not have a DM RR comorbid 1.027 (95% CI; 1.003-1.051) were predictors of the success of TB treatment. The results of this study indicate that patients who do not have HIV or DM co-morbidities are more likely to succeed in TB treatment.

Abstract

Indonesia is among the countries with the percentage of early marriages in the world, in 37th place. While in ASEAN, Indonesia submitted first place. Early marriage to a woman will have a negative impact because the growth of body organs, especially the reproductive organs are not ready for the process of pregnancy or birth so that it is categorized as a high risk for both pregnant women and fetuses in terms of health, psychological, and growth and development of children born. A cross-sectional study, subjects were the group of age 15-49 years who were followed by the interviews and had complete data. Data used was drawn from 2017 Indonesian Demographic Health Survey (IDHS). Logistic regression model was used for analysis. only 26.6% of the respondents had married before 18 years old. As compared to respondent high fertility (>2 child), all respondents with low fertility (<=2 child) were significantly about 1.19 times,  POR values indicate that women who get married early are 1.19 times higher risk to have >2 child (a-POR=1.19; 95% CI:1.16-1.23). As compared to respondent who use contraception, POR values indicate that women who get married are 1.10 times higher risk to use contraception (a-POR=1.10; 95% CI:1.05-1.13). Respondents who living with high wealth index, POR values indicate that women who get early marriage were about 1.10 times higher risk with low wealth index (a-POR=1.13; 95% CI :1.11 – 1.16).

Abstract

Global warming has an impact on climate change causing an increase in cases of mosquito borne disease. There are several factors that influence the incidence of dengue including climate consisting of humidity, rainfall, temperature, duration of sunshine, wind speed, and number of rainy days. The occurrence of changes in climatic parameters contributes to an increase in the number of vectors, increases the life span of vectors, and expands the spread of dengue vectors. This study aims to determine the relationship between climate and dengue incidence in Bandung City for 10 years (2011-2020). This research was an analytical research that uses ecological design according to place and time. The data used were climate parameter data and dengue incidence. The incidence of dengue during 2011-2020 fluctuated. The results of statistical tests showed that there was a correlation between dengue cases with maximum temperature (r= -0.36; p-value=0.0001), humidity (r= 0.26; p-value=0.004) and maximum wind speed (r= -0.27; p-value=0.003). From the results of this study, climate parameters have an effect on increasing the incidence of dengue so that strict PSN efforts are needed, especially starting in September when dengue cases begin to increase.

Abstract

Latar belakang: Human Immunodeficiency Virus (HIV) menjadi masalah utama kesehatan global yang sedang dihadapi oleh berbagai negara, termasuk Indonesia. Penderita HIV lebih rentan untuk terkena infeksi oportunistik, salah satunya tuberkulosis. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang berhubungan dengan infeksi oportunistik tuberkulosis pada pasien HIV di RSPI Prof. Dr. Sulianti Saroso tahun 2015-2019. Metode: Studi case control dilakukan dengan menggunakan data register pra-ART dan rekam medis. Jumlah sampel sebanyak 465 responden, yang terdiri dari 155 kasus dan 310 kontrol. Analisis yang dilakukan adalah analisis univariat, bivariat menggunakan chi-square, dan multivariat menggunakan regresi logistik. Hasil: Hasil uji regresi logistik menunjukkan ada hubungan yang bermakna secara statistik antara stadium HIV dengan infeksi oportunistik tuberkulosis (OR=33,03; 95% CI : 14,96 – 72,89) dengan nilai p <0,001, tetapi tidak ada hubungan yang bermakna secara statistik antara usia, jenis kelamin, jumlah CD4, jumlah viral load, pendidikan, status bekerja, perilaku seks berisiko, transfusi darah, dan penggunaan napza suntik dengan infeksi oportunistik tuberkulosis. Kesimpulan: Stadium HIV merupakan faktor determinan infeksi oportunistik tuberkulosis pada pasien HIV. Saran: Diperlukan skrining awal mendeteksi TB pada setiap stadium HIV khususnya pada stadium lanjut (III-IV) yang melakukan kunjungan di klinik Pokja.

Abstract

Background: Coronavirus Disease 2019 (COVID-19) is a new infectious disease and to date become a global pandemic. COVID-19 has symptoms varies in severity from mild to severe and can be fatal. Vitamin D deficiency could affect individual's immunity and increase the risk of mortality in COVID-19 patients.Methods: We did a literature review study to describe the association between vitamin D deficiency and mortality in COVID-19 patients. Literature search was done using PubMed database with keyword “COVID-19” AND “Vitamin D Deficiency” AND “Outcome”. Articles published before June 30th 2021 and published in English language were included in this study. Inclusion criteria is case control or cohort study which evaluate association between vitamin D deficiency and mortality in COVID-19 patients.Result: 13 eligible articles which met inclusion and exclusion criteria were included in this study. All subjects in included studies were COVID-19 patients confirmed by laboratory RT-PCR. Mean age of the subjects in included studies were range from 55-79 years old, with proportion of male more than female in most of the studies. 25(OH)D level <20 ng/ml was definition of vitamin D deficiency most often used. Vitamin D deficiency were significantly associated with COVID-19 mortality, with OR in 3 studies range from 3,87-6,84 and HR in 3 studies range from 4,15-14,73.Conclusion: This literature review concluded that vitamin D deficiency had the potency to be risk factor for mortality in COVID-19 patients.

Abstract

Background: Chlamydia is one of the most common sexually transmitted infections globally. Female sex workers (FSW), especially in urban areas, face a higher risk of infection due to their risky work environment and lifestyle. Jakarta is a city that has cosmopolitan and urban characteristics with an active sex industry. This research was conducted to determine the factors associated with the incidence of chlamydia infection among FSWs in West Jakarta.Methods: The research was conducted using a cross-sectional design by analyzing data from the 2018-2019 Integrated Biological and Behavioral Survey (IBBS). The data were analyzed using univariate and bivariate analysis with the chi-square test. Prevalence ratio (PR) was used as the measure of association.Results: Of the 283 FSWs involved in the study, the positivity rate of chlamydia infection in West Jakarta reached 42.8%. Significant risk factors for chlamydia infection among FSWs in West Jakarta include younger age, divorced status, recently becoming a sex worker, and having ≥ 5 customers per week.Conclusion: The high rate of chlamydia infection among FSWs in West Jakarta highlights the need to increase outreach to FSWs in order to educate them about STIs and encourage them to perform regular screenings, especially for young FSWs.

Abstract

Indonesia has a high intensity of disaster events. The prevalence of under-five stunting in Indonesia is also high. One of the targets of the Ministry of Health's Medium-Term Development Plan for 2020-2024 is to reduce stunting in children by 14% in 2024. This study aims to look at the relationship between disaster risk and the prevalence of stunting in 34 provinces in Indonesia. The study design is an ecological study approach sourced from the 2021 Indonesian Disaster Risk Index (IRBI), the 2021 Indonesian Nutritional Status Survey (SSGI), the 2018 Basic Health Research and the 2021 National Socioeconomic Survey (Susenas). This study conducted a spatial analysis, scatter plots, pearson correlation test and multivariable linear regression. From the results, it found that the higher risk of disaster increased the tendency for the prevalence of under-five stunting. It was concluded that disaster risk has a positive relationship with the prevalence of under-five stunting in Indonesia. We suggest that a disaster management program can be considered to reduce stunting in Indonesia.

Abstract

ARV therapy is important in treating HIV cases. Even though it does not cure, ARVs help suppress the growth of the virus. Adherence to therapy is essential to achieve optimal results. The objective was to decide the components that impact the adherence of PLHIV in ARV treatment. This could be a story writing audit think about conducted by looking at articles through the Science Coordinate and PubMed databases. The catchphrases utilized are Variables impacting adherence to antiretroviral treatment among HIV/AIDS patients, HIV patient adherence to ART, and "adherence" and ART HIV/AIDS patients, with inclusion criteria: Articles published in 2018-2022, published in Indonesian language or English, original research, available in full text. Exclusion criteria: articles published before 2018, review articles, and qualitative studies, do not discuss factors for adherence to ARV therapy. 20 articles satisfy the review criteria, various factors that affect adherence to ARV therapy are socio-demographic factors, clinical factors, therapeutic factors, healthcare facilities factors, and other contextual factors. Poor quality of health services, level of education, younger age, social support, community stigma, COVID-19 pandemic, and ability and understanding of ART act as risk factors for PLHIV’s adherence to ARV treatment. The discoveries of these different variables ended up reference for wellbeing offices to move forward the quality of administrations and public awareness of optimal ARV therapy adherence.
