Abstract

Diabetes Mellitus (DM) is a serious health problem that continues to increase and can cause morbidity and mortality in many countries. India as a country with the second highest number of DM cases in the world after China has around 77 million cases in adults aged 20-79 years and is estimated to increase by 134 million in 2045. The cause of the incidence of DM in adults is multifactorial. The purpose of this study was to determine and analyze the risk factors associated with the incidence of DM in adults aged 15-49 years in India. This quantitative study uses a cross-sectional approach. This research data source was secondary data from the 2015-2016 National Family Health Survey (NHFS-4). The study sample size was a male adult population aged 15-49 years in 157 rural and urban areas in India. The independent variables studied were age, meat consumption, alcohol consumption, fried food consumption, smoking, economic status, education level, place of residence, and employment status, while the dependent variable is the incidence of DM. The results from the chi-square show that there were relationships between age, meat consumption, alcohol consumption, fried food consumption, smoking, economic status, education level, place of residence, and the incidence of DM in adult men aged 15-49 years in India. The results of the logistic regression test showed that age, smoking habits, and economic status had a significant effect. Meanwhile, the most influential factor was the higher economic status with an OR of 0.628 times higher having type 2 diabetes.

Abstract

One of the error conditions that are required to be met is the absence of autocorrelation problems. Autocorrelation is a correlation of errors between observations, the existence of an error correlation between observations will cause deviations from the actual statistical value. One of the statistical methods for overcoming autocorrelation is the Prais Winsten method. This study aims to explain the Prais Winsten method in overcoming the problem of autocorrelation on factors that affect the life expectancy of East Java Province in 2018. This research is a secondary data analysis, data obtained from the East Java Province's health profile in 2018 with the dependent variable, namely life expectancy, and independent variables, namely prevalence of diarrhea, clean and healthy living behavior, and mean years of school. The results of this research indicated the finding of autocorrelation problems in the factors that affected the life expectancy of East Java Province in 2018. Improvements with the Prais Winsten method showed that the Durbin Watson value was at the critical point limit, Mean Square Error and coefficient of determination (R2) value was decreasing. This research concludes that the Prais Winsten method can overcome autocorrelation.

Abstract

Overweight and obesity can be interpreted as abnormal fat accumulation that can cause health problems. The average BMI (Body Mass Index) in adult women has also continued to increase. This study aims to identify the relationship between age, alcohol consumption, cigarette consumption, employment status, economic status, ethnicity and area of "‹"‹residence with the incidence of obesity in women aged 15-49 years in South Africa and determine the most influential factors on obesity. This study is a quantitative study with a design cross-sectional using secondary data from The DHS Program 2016. Respondents in this study were women aged 15 - 49 years in South Africa as many as 1158 people. The dependent variable in this study is the nutritional status of obesity which is classified as obese if BMI ≥25 kg/m2. While the independent variables are age, alcohol consumption, cigarette consumption, employment status, economic status, ethnicity, and area of "‹"‹residence. Data were analyzed by chi-square and logistic regression. The results showed that the factors associated with obesity were age, employment status and economic status (p-value <0.05). The most influential factor is adult age with a range of 25 - 49 years. Therefore, there is a need for behavioral changes in adult women 25-49 years in South Africa and national monitoring evaluation of multisectoral programs for obesity prevention in South Africa.

Abstract

One of the countries in Asia that has a high prevalence of wasting is Timor-Leste. The prevalence of wasting in Timor-Leste is 24% in children aged 15-49 months. The purpose of this study is to identify the factors that influence the incidence of wasting in children aged 15-49 months in Timor Leste. This research is an analysis of secondary data from the 2016 Demographic and Health Survey (DHS) in Timor-Leste using a cross-sectional approach. Informants in this study were mothers of toddlers aged 15-49 years and had toddlers aged 15-49 months. the number of samples in this study were 1,207 mothers. Bivariate analysis was performed by Chi-Square test and multivariate test by logistic regression test. The results showed that the factors that influence wasting are low birth weight, history of breastfeeding and mother's education level. Meanwhile, after being analyzed using logistic regression, the most influencing variables were low birth weight babies with a significant value of 0.028 and an OR value of 0.620.

Abstract

Patients with generalized myasthenia gravis (MG) often show restrictive spirometry results. Although regular exercise and physical fitness are linked to better respiratory function, there is limited research assessing the effects of aerobic exercise on lung function in MG patients. The aim of this study was to analyze the effect of low-intensity aerobic exercise using a cycle ergometer on lung function parameters in MG patients. A randomized controlled trial with pre- and post-test was conducted at the Medical Rehabilitation Outpatient Clinic of Dr. Soetomo General Academic Hospital in Surabaya, Indonesia, in 2023. MG patients classified as I−IIb based on the Myasthenia Gravis Foundation of America (MGFA) classification were recruited and randomly divided into treatment and control groups. The treatment group was given low-intensity aerobic exercise using a cycle ergometer, education on lifestyle changes, and breathing exercises (deep and pursed lip breathing). Lung function parameters, including forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and forced expiratory volume ratio (FEVR), were then measured. Measurements were conducted before and after eight weeks of low-intensity aerobic cycle ergometer exercise and compared with those of a control group. A total of 17 MG patients were included in this study. The results showed a significant increase in FVC in the treatment group (p=0.003), whereas no significant change in the control group (p=0.068). A significant increase in FEV1 was observed both in the treatment (p=0.029) and the control group (p=0.016). There was no improvement in FEVR in either group. After the intervention, significant differences were observed in FVC (p=0.009) and FEV1 (p=0.029) between the treatment and control groups. There was no significant difference in FEVR values after the intervention between both groups (p=0.491). In conclusion, eight weeks of low-intensity aerobic cycle ergometer exercise led to significant improvements in FVC and FEV1 among MG patients.

Abstract

Background: Diphtheria cases spread in almost all regions in Indonesia. In 2018 the highest number of diphtheria cases in East Java was 695 cases with a Case Fatality Rate (CFR) of 1.44%. Diphtheria is an acute infectious disease caused by Corynebacterium diphteriae bacteria. Environmental and behavior factors can be a cause of the spread of diphtheria. Objectives: The purpose of the study was to apply the Generalized Poisson regression to identify the factors that influence of diphtheria cases in 2018. Methods: The study used secondary data that had been published by the Provincial Health Office through data from the health profile of East Java Province in 2018. The population was a population in East Java Province in 2018 whose region consisted of 38 districts/cities. The sample in the study was diphtheria sufferers in East Java Province in 2018. The variable used in the study was the number of diphtheria cases as a dependent variable. The independent variable is the density of the population, the percentage of healthy homes, the percentage of households in a clean and healthy life behavior (CHLB), and the percentage of complete basic immunization. The data analysis used in the study is Generalized Poisson Regression Result: The results of the study get the best model of diphtheria case = Exp [2,995+ 0.01381 (CHLB household)]. Conclusion: The conclusion of the study is a significant variable affecting the diphtheria case in East Java Province in 2018 is a CHLB household.

Abstract

Good scheduling arrangements are needed to prevent physical, emotional, and psychological exhaustion in nurses which will harm their performance. The aim is to see an assessment of the characteristics of nurse work scheduling in the hospital ward. This study used a descriptive survey design. The population in this study were all nurses in the ward of Lavalette Hospital - Malang. The sampling technique used was simple random sampling. Data collection using a scheduling characteristic questionnaire. Most of the respondents considered that the work scheduling criteria currently applied in the ward were in the sufficient category (80.2%). There are sub-variable coverage (309) and quality (321) less than the average value (322.08). The problems that arise in the nurse scheduling in the ward of Lavalette Hospital - Malang are the suitability of amount and distribution of nurses, quality of the schedule, an exchange between nurses, and time to prepare the schedule.

Abstract

Iron deficiency is the leading cause of anemia during pregnancy, a major public health concern in many developing countries. To mitigate anemia, iron supplementation for at least 90 days is recommended for pregnant women. The aim of this study was to evaluate adherence to iron supplementation during pregnancy and to identify its key determinants in Cambodia. A cross-sectional study was conducted using secondary data from the 2021–2022 Cambodia National Demographic and Health Survey. Key variables assessed included maternal age, education level, ethnicity, wealth index, number of pregnancies, age at first pregnancy, prenatal care provider, timing of the first antenatal care (ANC) visit, and frequency of ANC visits. Multivariate logistic regression was employed to evaluate associations between adherence and independent variables. A total of 4,475 women aged 15–49 years who had been pregnant in the past five years were included in the analysis, with 91.2% adhering to iron supplementation for at least 90 days. Multivariate logistic regression showed that those with primary and higher education had greater odds of adherence (odds ratio (OR)=1.38; 95%CI: 1.00–1.90; OR=3.07; 95%CI: 1.39–6.79, respectively) compared to women with no education. There was a positive relationship between education level and adherence. Women who attended four or more ANC visits were more likely to adhere (OR=2.93; 95%CI: 2.27–3.77), and those who initiated ANC in the first trimester had higher adherence compared to those who started later (OR=1.78; 95%CI: 1.35–2.36). This study highlights that education level, frequency, and timing of ANC visits significantly influenced adherence to iron supplementation. Strengthening maternal education and promoting early and regular ANC follow-up should be prioritized to improve adherence and prevent anemia during pregnancy.

Abstract

Purpose: Infections in children can affect weight gain and linear growth by influencing metabolism and nutrition. Chronic inflammation results in growth failure mediated by pro-inflammatory cytokines and poor nutritional intake, which affects the GH/IGF-1 axis. This study aimed to investigate the impact of the inflammatory response on children's anthropometry, particularly HAZ, and the role of IGF-1. Methods: An observational study with a cross-sectional design was conducted from September 2021 to July 2022. The study involved children diagnosed with infections. The subjects had undergone a physical and laboratory investigation, which included a thorax photo, urine culture, Mantoux test, and complete blood test in a hospital setting. A complete medical history was obtained from the pediatrician. Results: The prevalence of undernutrition was 37.33%; the prevalence of underweight/severely underweight, stunted/severely stunted, and wasted/severely wasted was 25.33%, 30.66%, and 14.67% respectively. Stunted/severely stunted was more prevalent in subjects under 2 years old, compared to subjects more than 2 years old (22/41 vs. 7/34, p=0.014). SEM analysis revealed that the inflammatory response affected IGF-1 levels (r=0.850, p=0.000), while IGF-1 affected body composition (r=0.245, p=0.025), and then affected HAZ (r=1.000, p=0.000). The effect of IGF-1 on HAZ appears to be indirect, acting through body composition. Parental height has a weak, albeit significant, effect on body composition (r=0.101, p=0.025) and HAZ (r=0.192, p=0.040). Univariate analysis revealed strong correlations between IL-6 and IL-10 and IGF-1 (r=0.870, p=0.000 and 0.876, p=0.000, respectively). In contrast, parental height showed a correlation with HAZ/LAZ (r=0.319, p=0.000). Maternal height was correlated positively with WAZ (r=0.320, p=0.044). Conclusion: The prevalence of stunting among children under two years of age indicates that early childhood constitutes a critical period for intervention in growth and development. Inflammatory response has been demonstrated to influence IGF-1 levels in children. The role of IGF-1 on HAZ was mediated by body composition.

Abstract

Background: Intensive Care Unit-Acquired Weakness (ICU-AW) is a common neuromuscular complication in critically ill patients, leading to muscle atrophy, prolonged mechanical ventilation and impaired functional recovery. Effective rehabilitation strategies play crucial role in maintaining muscle strength. Neuromuscular electrical stimulation (NMES) had shown potential in preserving quadriceps femoris muscle thickness. This study aimed to examine the effect of NMES to quadriceps femoris muscle thickness in patient with ICU-AW.Methods: Patients that stayed in ICU more than 48 hours and used mechanical ventilation for at least 24 hours enrolled in one-group pretest-posttest design. Neuromuscular electrical stimulation applied to bilateral quadriceps femoris for 30 minutes daily over five consecutive days. Bilateral quadriceps femoris muscle thickness were assessed with USG day before and after therapy.Results: Twenty ICU-AW patients were enrolled from December 2023 to June 2024, and 13 of them completed the study. There were no significant changes in the thickness of the right and left quadriceps femoris muscle before and after 5-days of NMES therapy (p = 0.910 and p = 0.519, respectively). However, we preserved less than 5% decreased of muscle thickness.Conclusions: Early 5-days of NMES therapy did not increase quadriceps femoris muscle thickness but effectively prevented further muscle degradation in ICU-AW patients. The potential benefits of rehabilitation therapy in ICU-AW patients should be further explored in future studies with controlled indicators and larger sample sizes.
