Abstract

Abstract

Background: Diphtheria cases spread in almost all regions in Indonesia. In 2018 the highest number of diphtheria cases in East Java was 695 cases with a Case Fatality Rate (CFR) of 1.44%. Diphtheria is an acute infectious disease caused by Corynebacterium diphteriae bacteria. Environmental and behavior factors can be a cause of the spread of diphtheria. Objectives: The purpose of the study was to apply the Generalized Poisson regression to identify the factors that influence of diphtheria cases in 2018. Methods: The study used secondary data that had been published by the Provincial Health Office through data from the health profile of East Java Province in 2018. The population was a population in East Java Province in 2018 whose region consisted of 38 districts/cities. The sample in the study was diphtheria sufferers in East Java Province in 2018. The variable used in the study was the number of diphtheria cases as a dependent variable. The independent variable is the density of the population, the percentage of healthy homes, the percentage of households in a clean and healthy life behavior (CHLB), and the percentage of complete basic immunization. The data analysis used in the study is Generalized Poisson Regression Result: The results of the study get the best model of diphtheria case = Exp [2,995+ 0.01381 (CHLB household)]. Conclusion: The conclusion of the study is a significant variable affecting the diphtheria case in East Java Province in 2018 is a CHLB household.

Abstract

Hypertension has the potential to cause serious complications such as heart disease, stroke, and kidney failure if not treated properly. Apart from physical factors, hypertension sufferers also often face psychological problems such as stress, which can worsen their condition and affect their quality of life. This study aimed to explore the relationship between stress and quality of life in hypertension sufferers. This systematic review searched for relevant articles using MeSH-based keywords in databases such as Scopus, PubMed, Sage Journal, and Web of Science. The article inclusion criteria were: population: patients with hypertension, intervention: no intervention, comparison: no comparison, results: the article's results showed a relationship between stress and quality of life in people with hypertension. Article quality assessment was carried out using the Joanna Briggs Institute guidelines. Descriptive analysis was carried out using a narrative approach by considering articles published from 2012 to 2024. Based on a literature search, 36229 articles were found with appropriate keywords from Scopus (2578), PubMed (683), Sage Journal (31999), and Web of Science (969). After screening for inclusion and exclusion criteria, 11 articles were identified for inclusion in this review. These eleven articles show that there is a relationship between stress and quality of life in people with hypertension. The importance of a comprehensive approach that considers socioeconomic, demographic, and psychological factors to improve the quality of life of hypertension sufferers.

Abstract

Breast cancer recurrence remains a major cause of mortality among women worldwide. Early identification is essential for improving patient outcomes, and prediction modelling has emerged as an approach to support this objective. This study aimed to develop and compare machine learning models for predicting breast cancer recurrence. A retrospective secondary analysis was conducted on data from 286 patients in the University of California, Irvine (UCI) Machine Learning Repository. Nine predictors were used to develop logistic regression (LR), artificial neural network (ANN), and extreme gradient boosting (XGBoost) models. The dataset was split into training and testing sets using a 70:30 ratio. Model performance was evaluated using the area under the receiver operating characteristic curve (ROC-AUC), Brier score, and calibration analysis, with 95% confidence intervals estimated through bootstrap resampling. LR demonstrated the best discriminatory performance, achieving a test AUC of 0.790. In contrast, XGBoost and ANN showed lower generalization performance, with test AUCs of 0.727 and 0.748, respectively. LR also achieved the highest testing recall (0.731) and F1-score (0.623), indicating superior sensitivity for identifying recurrent cases. Meanwhile, XGBoost demonstrated the highest precision (0.727), accuracy (0.756), and calibration performance. The relatively small sample size and reliance on structured clinical predictors may have contributed to the superior performance of LR in this study. In conclusion, LR demonstrated the most reliable predictive performance for this dataset.  Future research should use larger, diverse datasets and incorporate a broader range of predictors, including imaging and genomic data, to optimize the benefits of more complex machine learning models.
