Abstract

Introduction: The aging process causes a decline in physiological functions, including the cardiovascular system, which increases the risk of hypertension and heart disease in the elderly. Physical activities like elderly gymnastics have been shown to maintain fitness and reduce the risk of these diseases. However, mobility limitations and concerns about injury make it difficult for some elderly individuals to participate. Therefore, safe and effective modifications of gymnastics are needed, one of which is seated exercises using fabric to assist movements without adding excessive strain to the body. Objective: This study aimed to determine the effect of seated elderly exercise using a cloth as an aid on cardiovascular load in older adults.. Method: This quantitative study used a pretest–posttest single group design. The subjects were older adults in Mantren Hamlet, Klodran Village, Colomadu District, Karanganyar Regency, who met the inclusion criteria. The intervention was conducted in 10 sessions, each lasting 1 hour (10 minutes warm-up, 40 minutes core exercise, and 10 minutes cool-down). Blood pressure, oxygen saturation, and pulse rate are measured before and after the intervention using a sphygmomanometer and oximeter. Data are analyzed using the Shapiro–Wilk normality test and paired t-test or Wilcoxon test as appropriate. Results: Analysis of the study shows that the average systolic and diastolic blood pressure decreased in the first week and fourth week after the intervention, while SpO₂ values remained within the normal range. The normality test indicated that the data did not follow a normal distribution, so the test was continued using the Wilcoxon test. The Wilcoxon test results showed a significant difference between the values before and after the intervention in all physiological parameters (p < 0.05), marked by a decrease in blood pressure and an increase in SpO₂ values after the intervention. Conclusion: The intervention applied was proven to have a positive effect on cardiovascular and oxygenation parameters in the elderly, as indicated by a significant decrease in blood pressure and an increase in oxygen saturation. This study shows that the intervention has the potential to be an effective approach to support cardiovascular health in the elderly, although further research with a larger sample and more optimal control of variables is still needed to strengthen the generalization of the findings

Abstract

This case report aimed to describe physiotherapy management and clinical changes in a patient with pressure ulcer following postoperative right femoral neck fracture. A 60-year-old female after open reduction internal fixation presented with sacral and bilateral gluteal pressure ulcers, type 2 diabetes mellitus, severe anemia, systemic infection, and massive melena. Physiotherapy interventions included positioning, repositioning, breathing exercise, passive and active range of motion exercise, sensory stimulation, gradual mobility training, and family education. Clinical outcomes were evaluated descriptively across three sessions using the Behavioral Pain Scale, pressure ulcer observation, range of motion, manual muscle testing, and Functional Status Score for the Intensive Care Unit. During the intervention period, the Behavioral Pain Scale increased from 8 to 9, and pressure ulcer size remained unchanged. Passive range of motion was maintained from T1 to T3; however, muscle strength decreased from 3/5 to 2/5, and functional status declined from 2 to 0. These findings indicate that physiotherapy in this medically complex case primarily supported joint mobility maintenance, comfort, and prevention of further complications rather than functional recovery. Because this study involved a single patient with a short observation period, the findings should be interpreted descriptively and not as evidence of intervention effectiveness.

Abstract

Intoduction: Dupuytren’s Contracture et causa post burn injury is a condition characterized by fibrosis and contracture of the palmar fascia following burn trauma which causes pain, limited joint range of motion, decreased muscle strength, impaired skin integrity, and reduced functional activity. Objective: to determine the effectiveness of physiotherapy management using Transcutaneous Electrical Nerve Stimulation (TENS), ultrasound, scar massage, and exercise in patients with Dupuytren’s Contracture et causa post burn injury. Methods: the research method used a case study conducted on one 56 year old male patient diagnosed with Dupuytren’s Contracture et causa post burn injury. The measurement tools used included the Numeric Rating Scale (NRS) for pain assessment, goniometer for measuring joint range of motion, Manual Muscle Testing (MMT) for muscle strength, Vancouver Scar Scale (VSS) for scar evaluation, SWEAT for skin integrity assessment, and Wrist and Hand Disability Index (WHDI) for evaluating functional ability. Results and Discussion: after three sessions of physiotherapy intervention, the patient showed decreased pain intensity, increased joint range of motion, improved skin sensitivity, and slight improvement in functional activity, although muscle strength and scar characteristics showed no significant changes. Conclusion: physiotherapy interventions consisting of TENS, ultrasound, scar massage, and exercise were effective in reducing pain and improving functional ability in cases of Dupuytren’s Contracture et causa post burn injury

Abstract

Luka bakar sering menyebabkan pembentukan jaringan parut yang berujung pada kontraktur dan keterbatasan lingkup gerak sendi (Range of Motion/ROM), terutama pada ekstremitas atas. Kontraktur pasca luka bakar dapat menimbulkan gangguan fungsi dan menurunkan kualitas hidup, sehingga diperlukan intervensi rehabilitasi yang efektif untuk memulihkan mobilitas sendi. Penelitian ini bertujuan untuk mengevaluasi efektivitas kombinasi stretching dan scar massage terhadap peningkatan lingkup gerak sendi pada pasien dengan kontraktur pasca luka bakar. Penelitian ini menggunakan desain case report berdasarkan CARE Guidelines. Subjek penelitian adalah seorang laki-laki berusia 23 tahun dengan kontraktur pasca luka bakar listrik pada jari ke-IV tangan kiri. Intervensi berupa kombinasi latihan stretching dan scar massage diberikan selama tiga minggu dengan frekuensi satu kali per hari. Stretching dilakukan dengan intensitas sedang selama 10 detik sebanyak tiga set, sedangkan scar massage diberikan selama 15 menit menggunakan teknik effleurage, kneading, dan friction. Pengukuran lingkup gerak sendi dilakukan menggunakan goniometer sebelum dan sesudah intervensi. Setelah tiga minggu intervensi, lingkup gerak sendi jari ke-IV meningkat dari 10° menjadi 20°, yang menunjukkan peningkatan mobilitas sendi yang bermakna secara klinis. Pasien juga melaporkan penurunan kekakuan dan peningkatan fleksibilitas jari. Hasil evaluasi tindak lanjut menunjukkan bahwa peningkatan ROM tetap stabil setelah jeda latihan. Kombinasi stretching dan scar massage efektif dalam meningkatkan dan mempertahankan lingkup gerak sendi pada pasien dengan kontraktur pasca luka bakar. Pendekatan ini dapat dipertimbangkan sebagai intervensi fisioterapi yang aman dan bermanfaat dalam program rehabilitasi pasca luka bakar.
