Abstract

ABSTRACTIntroduction: Therapeutic exercise is the main rehabilitation program in patients with ChronicObstructive Pulmonary Disease (COPD). Shortness of breath due to persistent obstruction causesdisability in the form of decreased functional capacity. Hairmyers is an isolated group of exercise torecondition the limb muscles without weights. The purpose of this study was to assess the efficacy oflow-intensity Hairmyres exercise in improving Six-Minute Walk Distance (6MWD).Methods: This is a pre-post interventional study held from January to April 2005. Hairmyres exerciseswere given to the patients (n=14) with moderate and severe COPD for eight weeks, five times a weekwith two exercises performed in front of the researcher. Six-Minute Walk Distance was measured before,at the end of the 4 th week, and after the exercise program.Results: Compared to the beginning, the 6MWD increased by 27 meters at the end of the 4th week and62.29 meters at the end of the program in moderate COPD patients, while the increase in severe COPDpatients were 39.71 and 76 meters, respectively. There was a significant difference (p <0.05) betweenthe first and the last measurement in both groups. This difference also surpassed the minimal clinicallyimportant difference (MCID) of 6MWD.Keywords: COPD, Hairmyers, Low-Intensity Exercise, 6MWD

Abstract

Postpartum family planning coverage in Central Java is below the national standard, or it only reached 26.8%. Postpartum mothers are at risk of getting pregnant if they do not use contraception. Modern contraceptives prevent 3.2 million of 5.6 million under-five deaths and 109,000 of 155,000 (70%) maternal deaths. Continuity of Midwifery Care during the extended postpartum period encourages mothers to use modern contraception to manage the pregnancy gap. This study aimed to explore midwifery services for Family Planning Postpartum from the perspective of providers and clients in three health facilities: hospitals (RS), Public Health Centers, and Independent Midwifery Practice (PMB). The research was implemented through a qualitative descriptive, a case study approach design in 2021. informants were ten postpartum mothers, nine midwives, two heads of Puskesmas, head division of Family Planning, Women and Children Empowerment (KBPPA) conducted an in-depth interview. The interviews were digitally recorded, transcribed, and analyzed using the Miles and Huberman method. Providers and clients identified several benefits of Postpartum Family Planning (KBPP). Midwives stated several obstacles in providing services, such as limited counseling time during labor and lack of support from hospital management. The competence of midwives constrained KBPP services at the Puskesmas. There were still few midwives at the Puskesmas certified to provide Long Term Contraception Method services. Midwives were also overloaded with work during the pandemic. Obstacles in the Independent Midwifery Practice were the lack of IMP with clinical networks and the limited authority of the midwife. Meanwhile, postpartum mothers' barriers included lack of knowledge about fertility and KBPP, worry about side effects, and the husband's lack of support. Therefore, optimizing the referral mechanism for long-term contraception in advanced health facilities is necessary. Continuity of care midwives and integration of services will increase coverage of postpartum contraception.

Abstract

Background: This study aimed to investigate gastric mucosa mucous layer thickness in portal hypertensive gastropathy (PHG) compare to normal mucosa in functional dyspepsia and its correlation with several variables such as child class, severity of esophageal varices and gastropathy. Methods: Biopsy specimens were taken from the antrum and corpus from both group of patients with PHG and functional dyspepsia. The specimen was given cryometric for frozen section. Tissue were sliced by sagital section 11 µm, placed in object glass, fixed and stained to evaluate mucous thickness and giemsa stained to observe Helicobacter pylori. Measurement of mucous thickness was done upward muscularis mucosa started from upper epithelial layer from foveale tip until outer mucous layer on 15 points which were marked randomly and calculate the mean value by micrometer (µm). Results: Mean value of antral mucous thickness in PHG was 13.30 ± 6.5 µm, while in the functional dyspepsia it was 25.59 ± 5.66 µm. Statistical analysis for both kinds of mucous thickness was p0.001. Mean corpus mucous thickness in PHG was 10.6 ± 6.81 µm, while mucous thickness in dyspepsia was 32.54 ± 6.51 µm. Statistical analysis revealed p0.001. This result showed significant difference of mucous thickness of antrum and corpus statistically between PHG and dyspepsia as control group. Conclusion: The study had proven the presence of decreased gastric mucosa mucous layer thickness in corpus and antrum in PHG. Thus, therapeutic approach to increase mucous thickness must be considered in patients with PHG. Keywords: Gastric mucosa, PHG, liver cirrhosisemistry

Abstract

Background: Upper gastrointestinal tract (GIT) bleeding on liver cirrhosis patients will increase morbidity and mortality. Recurrent bleeding’s risk rise after the first episode of variceal bleeding. The mortality risk also rises on each bleeding. Purpose: This study was done in order to identify the risk factors for the first episode of recurrent bleeding of upper GIT on liver cirrhosis patient. Evaluation of risk factors was based on preliminary data prior to ligation. Method: Evaluation of the upper GIT bleeding was done using anamnesis on the patients or their relatives by letter, home visits or telephone. The data on recurrent bleeding was obtained from medical records. They were evaluated on the 3rd month then 1st year after ligation. This study was a cross sectional study with retrospective data and a consecutive sampling method. Result: Bivariate analysis revealed the 3rd month’s risk factors for first episode of upper GIT bleeding were ascites, total bilirubin level of 2 mg/dL, hepatoma, Child-Pugh C classification of the liver function and red color sign on esophageal varices. The risk factors for the first episode of upper GIT bleeding on first year were age £ 60 years old, hepatoma, and red color sign (RCS) on esophageal varices. The differences between risk factors on upper GIT bleeding on the 3rd month and 1st year were likely due to intervention, collateral para-esophageal varices, medication that irritated GIT, physical activities, and differences on variceal obliteration rate related to variceal ligation. Conclusion: Risk factors for recurrent upper GIT bleeding that could be minimized were ascites, total bilirubin level, Child-Pugh classification and RCS. It was expected with parascentesis, diuretics, hepatoprotector medications and drugs that lowers portal hypertension (such as propranolol and isosorbid mononitrate), might improve those risk factors thus decreasing the risk for recurrent upper GIT bleeding. Abstract

Community health centers are health service providers that are oriented towards minimum service standards, one of which is service to diabetes patients. This study aims to review various literature on factors affecting minimum service standards for people with diabetes mellitus, which were published in 2018 to 2022. A literature search was conducted on the Proquest, PubMed, Google Scholar and Sage Journal databases. The keywords used in the search are “diabetes” AND “minimum service standards”, AND “puskesmas”. There were 7 articles that met the criteria. Some literature shows that there are several factors that affect the minimum service standards at puskesmas for diabetes mellitus patients, namely high staff workload, lack of training, SOP implementation, culture and community awareness, collection of diabetes screening data, and facilities and infrastructure.Keywords: community health center; minimum service standards; diabetes mellitus ABSTRAK Pusat kesehatan masyarakat merupakan pemberi pelayanan kesehatan yang berorientasi pada standar pelayanan minimal, yang salah satu di antaranya adalah pelayanan terhadap pasien diabetes. Studi ini bertujuan untuk meninjau berbagai literatur mengenai faktor-faktor yang mempengaruhi standar pelayanan minimal pada penderita diabetes mellitus, yang diterbitkan pada tahun 2018 sampai 2022. Pencarian literatur dilakukan pada database Proquest, PubMed, Google Scholar dan Sage Journal. Kata kunci yang dipakai dalam pencarian adalah “diabetes” AND “standar pelayanan minimal”, AND “puskesmas”. Didapatkan 7 artikel yang memenuhi kriteria. Beberapa literatur menunjukkan bahwa ada beberapa faktor yang mempengaruhi standar pelayanan minimal di puskesmas pada pasien diabetes mellitus yaitu beban kerja staf yang tinggi, kurangnya pelatihan, SOP pelaksanaan, budaya dan kesadaran masyarakat, pengumpulan data skrining diabetes, serta sarana dan prasarana.Kata kunci: pusat kesehatan masyarakat; standar pelayanan minimal; diabetes mellitus

Abstract

Penggunaan metode kontrasepsi merupakan salah satu upaya menurunkan angka kematian ibu. Namun, dalam 5 tahun terakhir capaian peserta aktif KB MKJP di Provinsi Jawa Timur masih belum mencapai target yang ditentukan oleh BKKBN. Penelitian ini bertujuan untuk menganalisis faktor yang berhubungan dengan capaian peserta aktif KB MKJP yang dilihat dari efek keruangan pada tingkat kabupaten/kota di Provinsi Jawa Timur tahun 2021. Desain penelitian ini adalah studi ekologi dengan menggunakan data Laporan BPS Tahun 2021 dan dianalisis menggunakan aplikasi GeoDa. Hasil pemetaan menunjukkan Sampang, Pamekasan dan Sumenep sebagai daerah dengan capaian rendah yang dikelilingi oleh daerah dengan karakteristik serupa. Selain itu hasil uji regresi spasial menunjukkan bahwa persentase penduduk miskin dan proporsi klinik KB secara statistik mempengaruhi capaian peserta aktif KB MKJP di Provinsi Jawa Timur. Oleh karena itu pemerataan pembangunan di Pulau Madura dan wilayah tapal kuda perlu menjadi prioritas Pemerintah Provinsi Jawa Timur untuk memastikan lebih banyak masyarakat dapat mengakses pelayanan KB dengan mudah.

Abstract

This study analyzes the correlation between patient age with hospitalization cases and cost in Indonesia. The design of this study was cross-sectional quantitative using open sources data from Indonesia National Health Insurance Agency (BPJS Kesehatan). The sample units in this study were hospitalization cases from 2015-2021 of National Health Insurance program member. Sample size of this study are 43,921 that weighted to get 3,901,373 hospitalization cases. This study show an uptrend in cases and the total cost of cardiovascular disease hospitalization in 2015-2019, while it show downtrend during the Covid-19 pandemic. The average cost of cardiovascular disease hospitalization per case is 7.5 million rupiah (495.15 USD) per year. The highest average cost per case is 185.47 million rupiah (12,244.62 USD) per year; the lowest is 1.5million rupiah (99.03 USD) per year; and the average total cost is 3.99 trillion rupiah (263,417.41 USD) per year. The accumulated costs for 2015-2021 is 27.94 trillion rupiah (1,844,582.04 USD). Spearman's test shows there is a correlation between age and cases, average and total costs of cardiovascular disease hospitalization. Only correlation between age and average cost that show a negative pattern, which means that the higher the age, the lower the hospitalization average cost.

Abstract

The purpose of this study is to uncover the socioeconomic and demographic factors that influence the usage of contraceptives by analysing trends and patterns in Tanzania's family planning uptake using data from national surveys. The results show that while Tanzania has made progress in increasing contraceptive prevalence from 27% in 2010 to 39% in 2021-22, disparities persist across socioeconomic and demographic lines. Women in urban areas, from higher wealth quintiles, and with higher educational attainment have significantly higher contraceptive prevalence rates compared to their counterparts. The study also reveals that the majority of women are aware of family planning methods, and more than 75% of users get their methods from public sources. However, barriers such as sociocultural norms, spousal communication, and limited knowledge continue to hinder progress, particularly in rural areas. The study's findings offer evidence-based insights to guide the development and implementation of more successful family planning policies and programs in Tanzania, with the aim of resolving ongoing disparities in access and use of family planning services.

Abstract

Stroke merupakan masalah kesehatan di Indonesia dengan prevalensi 8,3 kasus per seribu penduduk. Penelitian ini bertujuan untuk memperoleh gambaran faktor risiko stroke pada lansia berdasarkan gender, dengan fokus pada faktor yang dapat dimodifikasi seperti aktivitas fisik, pola makan, kebiasaan merokok, dan pemeriksaan tekanan darah rutin. Penelitian ini menggunakan data sekunder dari Survei Kesehatan Indonesia (SKI) 2023 yang dirancang potong lintang. Sampel studi ini adalah subjek berusia ≥60 tahun (lansia) yang melaporkan diri atau didiagnosis stroke. Hasilnya memperlihatkan subyek laki – laki (54,7%) dan perempuan (45,3%) pada sampel lansia penderita stroke. Lansia stroke yang termasuk kategori lansia muda pada laki – laki (63%) lebih banyak dari pada perempuan (55,1%). Lansia perempuan lebih banyak tamat SD/MI ke bawah (76,9%) dibandingkan dengan lansia laki-laki (50,7%). Lansia laki – laki stroke lebih sedikit ditemukan tidak bekerja (36,7%) dari pada dengan lansia perempuan stroke (74,3%). Faktor risiko yang dapat diubah seperti aktivitas fisik dan merokok jumlahnya lebih banyak pada lansia laki – laki stroke dibandingkan lansia perempuan stroke. Diperlukan peningkatan kampanye kesehatan yang menekankan pentingnya berhenti merokok, rutin berolahraga, menjalani pola makan sehat, menghindari pola hidup sedentari, dan memeriksakan tekanan darah secara berkala untuk populasi lansia laki – laki stroke di Indonesia.
