Abstract

Objective: The purpose of the research was to analyze the influence of selective grinding frequency towards premaxilla position of infant with complete unilateral cleft lip and palate using prosthetic feeding aid.Material and Methods: Ten infants with complete unilateral cleft lip and palate with wide alveolar cleft and rotating position of premaxilla were treated with prosthetic feeding aid. Impression was done twice to each subject before and after prosthetic feeding aid placement. The photograph was taken from each working model. Tracing was made on each photograph using photoQuad program on the assigned postulate spots. Adjustment towards the prosthetic feeding aid device was done every two week with selective grinding on the anatomical surface of acrylic on the palatolateral major and anteromedial minor defects, and relining with soft liner on the labial of anatomical surface was also done. Anatomical surface selective grinding was not exceeded 1–1.5 mm on every visit. The correlation analyze between selective grinding frequency of prosthetic feeding aid and premaxilla position used the Pearson correlation coefficient.Results: The correlation analysis between selective grinding prosthetic feeding aid frequency with premaxilla position on infant with complete unilateral cleft lip and palate have positive correlation with value of 23% or r = 0.479 and t-count = 1.544 or p-value = 0.1611.Conclusion: The more frequent the selective grinding on prosthetic feeding aid, the higher the premaxilla position change will increase towards midsagittal.

Abstract

Signs and symptoms of temporomandibular disorders/TMD often occur in edentulous patients. Etiology and clinical symptoms are different so the diagnosis is different. The diagnosis of TMD can be determined using Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) Axis I (clinical examination) and Axis 2 (psychological examination). The purpose of this study was to obtain a description of temporomandibular joint disorders in edentulous patients at the Dental Hospital of Padjadjaran University based on DC/TMD Axis I. This research was a descriptive study with a survey technique. The sample size was 32 patients that were collected by consecutive sampling. Temporomandibular disorders in edentulous patients were obtained by filling out symptom questionnaires and clinical examination based on DC/TMD Axis I, then grouped using the DC/TMD Axis I algorithm. The results showed that from 32 edentulous patients who visited the Dental Hospital of Padjadjaran University, there were 23 subjects diagnosed with temporomandibular disorders, with disc displacement with reduction (40.63%) as a major diagnosis, followed by local myalgia and myofascial pain in 2 subjects (6.25%) each, and arthralgia in 1 subject (3.12%). Subjects who had more than one diagnosis as many as 5 people (15.62%). According to the result of the study, most of the edentulous patients visiting the Dental Hospital of Padjadjaran University were diagnosed with temporomandibular disorders, and disc displacement with reduction was the most common diagnosis. Abstract

Peg shape is one of the abnormalities of dental anatomy, i.e. tooth size is smaller than average and tapered shape. These peg-shaped teeth are often found in the maxillary lateral incisors and third molars. The form on the lateral incisor often causes aesthetic problems because of its anterior position. One of the treatments for peg-shaped teeth is Veneer. Case report: 26 years old woman had a peg-shaped shape on both lateral incisors. Indirect veneer aesthetic treatment was performed using Ceramage to correct the form and cover the diastema to achieve better aesthetics. Wax-up is done to see the suitability of the shape and proportion of the Veneer. Patients get a visual picture of the expected treatment results. The treatment results in Ceramage gave an excellent condition in improving the shape of the Peg-shaped incisor. The veneer colour resembles natural teeth, and the patient is satisfied with the treatment results. Ceramage can be used as a treatment option for anterior teeth with deformities. Ceramage Veneer is an alternative that can be done to improve the aesthetic appearance at a more economical cost for anterior teeth. DOI : 10.54052/jhds.v1n2.p121-130

Abstract

Polymethyl methacrylate (PMMA /acrylic) has beenused extensivelyfor many yearsasthe maincomponent of denturebases. Clinical problemscommonly seen in the acrylic dentureis usually afractureat the midline. This occursdue toheavyocclusal forcesoracci dental damagesuch asfalling down while brushing the denture. Hence, it is necessary to strengthen the denture base especially the midline.The a forementioned issue can be solved by using reinforcement. Since those reasons above so that literature review will be presented on reinforcing materials ofacrylic denture. Denture base strengthening can be done using metal sandfibers. Each material has some advantages and disadvantages. The disadvantages of the metal reinforcement arein terms of aesthetics, can not chemically attached to the acrylic and can cause allergic reactions, furthermore, it is prone to corrosion. While the fiber reinforcement has aesthetics advantages due to its color and good adhesion to the acrylic. The addition of fiber or metalon the acrylic denture base can increase the strength of acrylic. The selection of a reinforcing material to be used should be considering the advantages and disadvantages of each material besides the availability in the dental laboratories.

Abstract

Gangguan sendi temporomandibula adalah gangguan yang banyak diderita pada populasi masyarakat. Persentase gangguan hampir mendekati 75% populasi dengan adanya minimal satu gejala gangguan sendi yang diderita, tetapi banyak penderita tidak menyadari gangguan sendi ini. Penyebab gangguan sendi ini multifaktor. Banyak cara untuk mendiagnosa gangguan sendi ini, salah satunya adalah research diagnostic criteria for temporomandibular joint disorders/RDC-TMD. RDC-TMD mulai dikenal sejak tahun 1992, dan terus diperbaiki sampai akhirnya, pada tahun 2010 dilakukan penyempurnaan penetapan diagnosis gangguan sendi tersebut. Walaupun kriteria ini merupakan kriteria untuk riset, tetapi juga dapat digunakan untuk penetapan diagnosis pasien di klinik. Berdasarkan RDC-TMD, gangguan sendi pada pasien dapat didiagnosis sehingga arah perawatan untuk gangguan ini akan lebih tepat. Banyak penelitian mengenai reliabilitas dan validitas dari kriteria ini, dan hasilnya menunjukkan nilai yang cukup baik sehingga kriteria ini sedang mengarah menjadi diagnostic criteria for temporomandibular joint disorders (DC-TMD). RDC-TMD tahun 2010 dapat digunakan sebagai alat untuk mendiagnosis gangguan sendi temporomandibula, sehingga diagnosis yang tepat dapat memberikan arah perawatan yang tepat.

Abstract

Abstract: Malocclusion is considered as one of the causative factors of temporomandibular joint disorder. However, there are still pros and cons to the role of occlusion as a pathophysiological factor for temporomandibular joint disorders (TMD). This study aimed to review the literature on the extent to which research had been carried out related to TMD in patients with malocclusion published from 2012 to January 2022. The method used in collecting data was guided by PRISMA-Scr through the Pubmed NCBI and Sciencedirect databases with relevant keywords. The obtained literature was screened and assessed for feasibility. Detail of data extracted with Microsoft Excel software. Based on the search, a total of 21 kinds of literature were suitable with the inclusion category as study material. The included literature consisted of cross-sectional studies (n=6), case-control (n=4), cohort (n=3), retrospective study (n=3), systematic review (n=2), systematic review and meta-analysis (n=1), and review study (n=2). Most of the literature said that there was no significant relationship between variations in occlusion irregularities and TMD associated with various signs and symptoms of TMD as well as variations in temporomandibular joint (TMJ) anatomy. TMD was actually more influenced by habits, personality, and psychological conditions. Treatments for TMD patients, either surgical or orthodontic, had a positive effect but were not significant. In conclusion, there is no relationship between temporomandibular joint disorders and malocclusion. Keywords: temporomandibular joint disorder; malocclusion Abstrak: Maloklusi dianggap menjadi salah satu faktor penyebab gangguan sendi temporomandibula. Hingga kini, masih terdapat pro dan kontra peran oklusi sebagai faktor patofisiologi gangguan sendi temporomandibular (temporomandibular joint, TMJ). Penelitian ini bertujuan untuk mengkaji literatur penelitian yang telah dilakukan berkaitan dengan gangguan sendi temporomandibula (temporoman-dibular joint disorder, TMD) pada maloklusi yang dipublikasikan pada 2012 hingga Januari 2022. Metode pengumpulan data berpedoman pada PRISMA-Scr melalui database Pubmed NCBI dan Sciencedirect dengan kata kunci yang relevan. Literatur yang didapat diseleksi dan dilihat kelayakan-nya. Detail data diekstraksi dengan software Microsoft Excel. Hasil pencarian mendapatkan sebanyak 21 literatur memenuhi kategori inklusi sebagai bahan kajian, terdiri dari jenis studi cross-sectional (n=6), kasus kontrol (n=4), kohort (n=3), studi retrospektif (n=3), tinjauan sistematis (n=2), systematic review dan meta-analysis (n=1), serta studi review (n=2). Sebagian besar literatur menyatakan bahwa tidak terdapat hubungan bermakna antara variasi tidak teraturnya oklusi terhadap TMD terkait dengan ragam tanda dan gejala TMD serta variasi anatomi TMJ (temporomandibular joint). Didapatkan TMD justru lebih dipengaruhi oleh kebiasaan, kepribadian, serta keadaan psikologis. Perawatan yang dilakukan pada penderita TMD, baik berupa bedah maupun perawatan ortodontik memiliki pengaruh yang cukup positif namun tidak bermakna. Simpulan penelitian ini ialah tidak terdapat hubungan bermakna antara gangguan sendi temporomandibula dengan maloklusi. Kata kunci: gangguan sendi temporomandibular; maloklusi

Abstract

Seiring dengan meningkatnya popularitas esthethic dentistry, semakin banyak pasien yang membutuhkan perawatan yang dapat meningkatkan penampilan gigi anterior. Salah satu perawatan estetik yaitu porcelain laminate veneer yang merupakan lapisan tipis, sedikit tembus cahaya, terbuat dari bahan porselen, dan berfungsi untuk melaminasi atau menutupi gigi yang mengalami kerusakan, kelainan bentuk atau perubahan warna. Pada kasus diastema gigi 12 dan peg shape gigi 22, secara anatomis gigi terlihat lebih kecil dan akan terbentuk ruang atau diastema antar gigi. Pada kasus ini dilakukan perawatan estetik porcelain laminate veneer untuk memperbaiki bentuk dan menutupi diastema ini sehingga akan dicapai estetik yang lebih baik. Penatalaksanaan kasus yaitu wax up untuk melihat kesesuaian bentuk dan proporsi veneer yang telah didesain dengan digital smile design. Pasien menyetujui untuk dilakukan crown lengthening untuk mendapatkan level gingival margin yang lebih proporsional. Dibantu dengan diagnosis wax up dan smile designing yang direncanakan pada awal perawatan, pasien mendapatkan gambaran visual dari hasil perawatan yang diharapkan. Pasien merasa puas dengan hasil perawatan. Kesimpulan yang dapat diambil yaitu porcelain laminate veneer merupakan suatu alternatif yang dapat dilakukan untuk meningkatkan penampilan estetik pada gigi anterior agar pasien mendapatkan senyum yang lebih indah.

Abstract

 ABSTRAKBruxism dapat menyebabkan nyeri pada otot mastikasi atau sendi temporomandibula (TMJ). Beberapa penelitian menunjukan bahwa bruxism dapat memberikan beban besar pada TMJ. Namun, penelitian mengenai efek bruxism pada prosesus kondilaris masih minim. Penelitian ini bertujuan untuk mengetahui apakah terdapat perbedaan luas permukaan prosesus kondilaris pada penderita bruxism dibandingkan dengan bukan penderita bruxism berdasarkan radiograf panoramik. Penelitian ini merupakan penelitian analitik cross-sectional pada radiograf panoramik penderita dan bukan penderita bruxism di Instalasi Radiologi Rumah Sakit Gigi dan Mulut Universitas Padjadjaran (RSGM Unpad). Kelompok bruxism terdiri dari 24 subjek sementara kelompok bukan penderita bruxism terdiri dari 20 subjek. Luas permukaan prosesus kondilaris diukur menggunakan software ImageJ. Hasil penelitian dianalisis menggunakan independent t-test, dan chi-squared test. Reliabilitas ditentukan menggunakan intra-class correlation coefficient. Penelitian ini mendapatkan rata-rata luas permukaan prosesus kondilaris pada penderita bruxism di regio kanan dan kiri adalah 201,38  39.36 mm dan 191,05  41,90 mm. Pada kelompok bukan penderita bruxism, rata-ratanya adalah 251,35  49,52 mm dan 234,34 43,86 mm di regio kanan dan kiri Penelitian ini menemukan rata-rata luas permukaan prosesus kondilaris pada pasien bruxism lebih kecil dari bukan penderita bruxism tetapi perbedaannya tidak signifikan. Selain itu, perbedaan luas permukaan tidak terikat dengan kondisi bruxism.ABSTRACTBruxism can cause pain in the masticatory muscles and the temporomandibular joint (TMJ). Studies have shown that bruxism can exert large load on the TMJ. However, research on the effects of bruxism on the condylar process is still rare. This study aims to determine whether there is a difference in the surface area of the condylar process in patients with bruxism compared to patients without bruxism based on panoramic radiographs. This was a cross-sectional analytic study conducted on panoramic radiographs of bruxism and non-bruxism patient at the Radiology Unit in Rumah Sakit Gigi dan Mulut Universitas Padjadjaran (RSGM UNPAD). The bruxism group consisted of 24 subjects, while the non-bruxism group consisted of 20 subjects. Condylar surface area was measured using ImageJ software. The results were analyzed with independent t-test and chi-squared test. Reliability was determined using intra-class correlation coefficient. The average surface area of the right and left condylar process in bruxism patients was 201.38 ± 39.36 mm and 191.05 ± 41.90 mm. In non-bruxism patients, the mean surface area was 251.35 ± 49.52 mm and 234.34 ± 43.86 mm in the right and left region. This study found that the average surface area of condylar process in bruxism patients was smaller than non-bruxism patients. but the difference was not significant. In addition, the difference in surface area was not related to the condition of bruxism.

Abstract

Introduction: Bruxism is a parafunctional habit in the oral cavity area. Bruxism is distinguished from awake and sleep bruxism. Bruxism is caused by various etiological factors. Smoking habits are considered is one of the exogenous factors that cause bruxism. This study aims to determine the effect of smoking habits on bruxism. Review: This study was conducted using the rapid review of articles reporting the effect of smoking on the occurrence of bruxism. Article searches were conducted through PubMed, Google Scholar, and EBSCO Host from October until November 2022. Article screening was carried out by checking for duplication, reading titles and abstracts, and reading the contents of the entire text. A total of 1257 articles were identified at the initial stage of the research with total of 6 articles included into rapid review criteria. Research on articles analyzed qualitatively shows that smoking habits affect occurrence of bruxism in most of the articles reviewed. As many as 2 articles prove smoking influences bruxism in general and as many as 2 articles proving that smoking affects awake bruxism. Conclusion: Smoking habits have an influence on the occurrence of bruxism in general.

Abstract

Fixed partial denture is preferred because it provides better comfort than removable dentures. Conventional FPD requires a lot of abutment preparation, and not all patients can perform this procedure. Adhesive bridges allow mi-nimal tooth preparation with design modifications to increase retention and a good cleansing effect to avoid secon-dary caries. The purpose of this article is to provide an overview of the modified adhesive bridge design on missing mandibular molars to enhance retention and good cleansing effect. A 29-year-old male came to the Dental and Oral Hospital Unpad with desire to have FD made. Patient lost first molar mandibular after tooth extraction about 10 months ago and doesn’t desire excessive tooth reduction. The treatment was an adhesive bridge on tooth 46 with sanitary pontic design, metal mesh, metal wings on teeth 47 and 45, occlusal rest on mesial and distal 47 and distal 45. Retention obtained was in the form of mechanical, chemical, and combination retention. It is concluded that adhesive bridge with sanitary pontic is appropriate treatment option for loss of one mandibular molar. Minimal pre-paration of healthy teeth and better self-cleaning effect are the advantages of this treatment.
