Abstract

Loss of teeth in adult needs to be replaced with dentures to restore masticatory and esthetic functions. Dentures are made immediately before extraction and used immediately after extraction, so that they do not change in appearance or function. If patient has restorable teeth, the teeth can be used as denture support. Overdentures supported by one or more restored teeth or implants as periodontal and mucosal support to improve durability of the denture. This arti-cle discusss maxillary immediate denture and mandibular overdenture on a woman who felt uncomfortable with the denture that has been used for 4 years. Patient wants a new denture without changing appearance. Intraoral exa-mination found tooth 17, 16, 26 GIC fillings, 13 dowel crown, 31, 32 PFM crowns, 42 composite filling with mobili-ty grade I, and 44 caries. This case was managed with extraction teeth 16, RCT 26, and extraction 13 before insert-ion the maxillary immediate denture with cusil #17 and overdenture 26, and also RCT was performed on 44, 42, 32, 31 followed by insertion the magnetic overdenture 44, 42 and metal coping overdenture 31, 32. It is concluded that immediate denture and overdenture provide optimal results for esthetic improvement and patient satisfaction.

Abstract

ABSTRAKBruxism dapat menyebabkan nyeri pada otot mastikasi atau sendi temporomandibula (TMJ). Beberapa penelitian menunjukan bahwa bruxism dapat memberikan beban besar pada TMJ. Namun, penelitian mengenai efek bruxism pada prosesus kondilaris masih minim. Penelitian ini bertujuan untuk mengetahui apakah terdapat perbedaan luas permukaan prosesus kondilaris pada penderita bruxism dibandingkan dengan bukan penderita bruxism berdasarkan radiograf panoramik. Penelitian ini merupakan penelitian analitik cross-sectional pada radiograf panoramik penderita dan bukan penderita bruxism di Instalasi Radiologi Rumah Sakit Gigi dan Mulut Universitas Padjadjaran (RSGM Unpad). Kelompok bruxism terdiri dari 24 subjek sementara kelompok bukan penderita bruxism terdiri dari 20 subjek. Luas permukaan prosesus kondilaris diukur menggunakan software ImageJ. Hasil penelitian dianalisis menggunakan independent t-test, dan chi-squared test. Reliabilitas ditentukan menggunakan intra-class correlation coefficient. Penelitian ini mendapatkan rata-rata luas permukaan prosesus kondilaris pada penderita bruxism di regio kanan dan kiri adalah 201,38 39.36 mm dan 191,05 41,90 mm. Pada kelompok bukan penderita bruxism, rata-ratanya adalah 251,35 49,52 mm dan 234,34 43,86 mm di regio kanan dan kiri Penelitian ini menemukan rata-rata luas permukaan prosesus kondilaris pada pasien bruxism lebih kecil dari bukan penderita bruxism tetapi perbedaannya tidak signifikan. Selain itu, perbedaan luas permukaan tidak terikat dengan kondisi bruxism.ABSTRACTBruxism can cause pain in the masticatory muscles and the temporomandibular joint (TMJ). Studies have shown that bruxism can exert large load on the TMJ. However, research on the effects of bruxism on the condylar process is still rare. This study aims to determine whether there is a difference in the surface area of the condylar process in patients with bruxism compared to patients without bruxism based on panoramic radiographs.This was a cross-sectional analytic study conducted on panoramic radiographs of bruxism and non-bruxism patient at the Radiology Unit in Rumah Sakit Gigi dan Mulut Universitas Padjadjaran (RSGM UNPAD). The bruxism group consisted of 24 subjects, while the non-bruxism group consisted of 20 subjects. Condylar surface area was measured using ImageJ software. The results were analyzed with independent t-test and chi-squared test. Reliability was determined using intra-class correlation coefficient. The average surface area of the right and left condylar process in bruxism patients was 201.38 39.36 mm and 191.05 41.90 mm. In non-bruxism patients, the mean surface area was 251.35 49.52 mm and 234.34 43.86 mm in the right and left region. This study found that the average surface area of condylar process in bruxism patients was smaller than non-bruxism patients. but the difference was not significant. In addition, the difference in surface area was not related to the condition of bruxism.

Abstract

The extraction of impacted third molars is one of the most common treatments performed in oral and maxillofacial surgery. Most third-molar surgeries are performed without complications, although still can cause serious complications in patients, one of are temporomandibular joint disorders (TMD). This study aims to determine the effect of third molar extraction on TMD that con-ducted using preferred reporting items for systematic reviews and meta-analyses (PRISMA) protocol through the databases PubMed, Ebsco, Sage, Scopus, and Springer with relevant keywords and published 2012-2022. The articles were then selected and evaluated eligibility. Detailed data extracted by google spreadsheet. Six articles were included in this rapid review, consisted of retrospective study (n=1), prospective cohort study (n=1) retrospective cohort study (n=1), review (n=2), and syste-matic review (n=1). It is concluded that there was an effect but not significant between extraction of impacted third molars and TMD related to various influencing factors. These factors include the complexity of the procedure, age, and gender. There is no definitive relationship due to the variation in the sample and research methods, and no quality assessment was conducted.

Abstract

The objective of this article is to discuss the role of stabilization splints in overcoming the temporomandibular joint disorders (TMD) experienced by a 24-year-old woman who came to the Prosthodontics clinic of RSGM FKG Unpad with chief com-plaints of pain and sounds that appear in the temporomandibular joints (TMJ) when opening the mouth, limited mouth opening, and muscle tension in the neck and face region. The patient wanted her problem to be resolved. The treatment procedure start-ed with the extraction of the impacted right and left upper third molars followed by a stabilization splint with a control dura-tion every two weeks. During the treatment phase, the patient was instructed to perform self-management therapy for elimi-nating the bad habits. After using the stabilization splint for 3 months the TMD gradually recovered. It is concluded that the use of stabilizing splints as occlusal therapy was considered an effective method in overcoming TMD.

Abstract

Introduction: Temporomandibular joint disorder are reported to affect 50 until 75% of the adult population and can affect an individual’s quality of life. The etiology of this disorder is multifactorial; it can be caused by parafunctional habit, chewing gum. 13% of adolescents and adults were reported chewing gum regularly. This study aims to determine the relationship between chewing gum and temporomandibular joint disorder. Review: This study was conducted using the rapid review method with screening and search referring to the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guidelines. Article searches were performed using keywords and inclusion criteria in the Google Scholar, PubMed, and ScienceDirect databases. A total of 4 articles using observational research methods were included. Evaluation of the habit of chewing gum was carried out using questionnaires and interviews, while the paramenters used to evaluate temporomandibular joint disorders were anamnesis and clinical examination based on the Research Diagnostic Criteria for TMD (RDC/TMD). Three articles indicated that there was a significant association between gum chewing and temporomandibular disorder, whereas 1 article concluded that further studies on this topic were necessary. The heterogeneity that occurs can be caused by differences in sample characteristics and methods. Conclusion: The habit of chewing gum can cause temporomandibular joint disorders

Abstract

Background : The increase of life expectancy occurred along the demand of a denture that provides high masticatory efficiency to promotes high quality of life among the elders. Implant supported overdentures has been an attractive procedures because of its simplicity and minimal invasive, in which both the attachment part and mucous provide support, retention and stability. Objective : The purpose of this article is to show how to implement an implant and mucous supported overdenture with CBCT guidance to improve parallelized implant placement. Case report : a 55 years old female came referred to UNPAD Dental Hospital with complete loss of teeth. First phase of treatment was to make complete denture, following by duplicating the denture and used it as guidance in CBCT. Second phase was the implant stage which is two stage surgery. First stage surgery aimed to put two paralleled implant and followed by second stage surgery after three months. Conclusion : Implant overdentures has many advantages to elder patient, which is less invasive, simple reconstruction, provides facial support, retention, stability and easily removed for hygiene.

Abstract

Acrylic artificial teeth are still widely used in denture fabrication. Several brands marketed in Bandung claimed that their product has met the standard but all the brands do not give the mechanical properties data including hardness. The ADA specification standard no. 15 requires a hardness of minimum 15.00 KHN for acrylic artificial teeth. This study is aimed to find out which acrylic artificial teeth have a standardized hardness value. The study is an analytic descriptive study performed on five brands of acrylic artificial teeth marketed in Bandung. The samples were given 10 indentation spots on the upper and lower surfaces. The results of the study were analysed using Anova test and Dunnet test. The analysis showed that the hardness value of the upper and lower surfaces was as followed, respec-tively: A, 17.95 KHN and 17.46 KHN; B, 17.01 KHN and 17.49 KHN; C, 18.24 KHN and 17.41 KHN; D 17.61 KHN and 17.01 KHN; E 17.01 KHN and 16.59 KHN. The two-ways Anova showed that the hardness value of both sur-faces does not differ significantly. It was concluded that the five brands have met ADA specification standard no. 15 and there were differences in hardness values among the brands.

Abstract

Bruxism is a condition that involves grinding and clenching which can place a large load on the mandible, so it can change the morphology of the mandible. Based on the Levandoski analysis, this study aimed to determine whether there are differences in the mandibular morphology between bruxism and non-bruxism patients. This study was a cross-sectional analytic study conducted on the panoramic radiographic samples of bruxism and non-bruxism patients with a total sample of 30 patients (n = 30) in each group, obtained from the radiographic archives at the Radiology Installation of RSGM Universitas Padjadjaran which have been confirmed as bruxism and non-bruxism patients. Measurements were carried out using ImageJ software based on the reference lines by Levandoski. The results of the study were analyzed using SPSS software by performing normality and homogeneity tests, followed by a statistically independent t-test or Mann-Whitney test. The results of the independent t-test showed that three lines had a p-value < 0.05, namely the line from the point gonion to the maxillary vertical line on the right side, the distance from the top point of the condyle to the maxillary interincisal point on the right side, and the distance from the top point of the condyle to the mandibular interincisal point on the right side. The Mann-Whitney test results showed that two lines had a p-value < 0.05, namely the maxillary vertical midline and the distance from the condyle to the left maxillary vertical line. The other line had a p-value> 0.05. There were differences in the mandibular morphology between bruxism and non-bruxism patients based on the Levandoski analysis, namely the length of the maxillary vertical midline, the distance from the condyle top point to the maxillary vertical midline, the distance from the point gonion to the maxillary vertical midline, the distance from the top point of the condyle to the maxillary interincisal point on the left side, and the distance from the top point of the condyle to the mandibular interincisal point on the left side. On the other four lines, there was no significant difference between bruxism and non-bruxism patients.

Abstract

ABSTRACT Introduction: Anterior implant placement is always challenging, especially when it is caused by trauma. Treatment preference is not only because of bone sufficiency but also aesthetic considerations. The complications aggravated by trauma and temporomandibular joint disorder need particular emphasis. The aim of this case report is to describe a new insight into dental implant treatment for trauma-induced edentulism and associated temporomandibular joint disorders. Case report: A 24-year-old woman had a motor vehicle accident. Her four anterior mandibular teeth were extracted through fractures. The patient also complained of clicking in both jaws during mouth opening. A bone graft and modification provisoris-stabilization splint were applied before definitive treatment. Furthermore, a 4-unit Porcelain Fused to Metal (PFM) implant-supported-fixed partial denture was done to replace the missing teeth. Conclusion: New insight into dental implant treatment for trauma-induced edentulism and associated temporomandibular joint disorders with replacement of several anterior mandibular missing teeth due to trauma with temporomandibular disorders could be treated with dental implant modification splint that functions as a temporary restoration as well, which was an alternative treatment option. Abstract

Seiring dengan meningkatnya popularitas esthethic dentistry, semakin banyak pasien yang membutuhkan perawatan yang dapat meningkatkan penampilan gigi anterior. Salah satu perawatan estetik yaitu porcelain laminate veneer yang merupakan lapisan tipis, sedikit tembus cahaya, terbuat dari bahan porselen, dan berfungsi untuk melaminasi atau menutupi gigi yang mengalami kerusakan, kelainan bentuk atau perubahan warna. Pada kasus diastema gigi 12 dan peg shape gigi 22, secara anatomis gigi terlihat lebih kecil dan akan terbentuk ruang atau diastema antar gigi. Pada kasus ini dilakukan perawatan estetik porcelain laminate veneer untuk memperbaiki bentuk dan menutupi diastema ini sehingga akan dicapai estetik yang lebih baik. Penatalaksanaan kasus yaitu wax up untuk melihat kesesuaian bentuk dan proporsi veneer yang telah didesain dengan digital smile design. Pasien menyetujui untuk dilakukan crown lengthening untuk mendapatkan level gingival margin yang lebih proporsional. Dibantu dengan diagnosis wax up dan smile designing yang direncanakan pada awal perawatan, pasien mendapatkan gambaran visual dari hasil perawatan yang diharapkan. Pasien merasa puas dengan hasil perawatan. Kesimpulan yang dapat diambil yaitu porcelain laminate veneer merupakan suatu alternatif yang dapat dilakukan untuk meningkatkan penampilan estetik pada gigi anterior agar pasien mendapatkan senyum yang lebih indah.
