Abstract

The Effect of Food Calcium Supplementation on the Blood Pressure Level of Pregnant Women in Bogor District.Study on the calcium supplementation to prevent the increase of blood pressure level of pregnant women was conducted in 6 Community Health Centers in Bogor District. Seventy-seven pregnant women between 18-23 weeks of pregnancy were included in this study. They were randomly assigned to receive 2 kinds of food (biscuit and syrup) which contain 700 mg of calcium for the intervention group and 200 mg for the control group. The subjects were asked to consume every 5 days in a week for 16 weeks. The results shows that no significant different in body weight, clinical status and biochemical status, calcium in urine and feces between 2 group. The energy and calcium intake were still below the indonesian RDA level. Diastolic blood pressure in supine position was decreased by 1.4 mg Hg in intervention group and 0.9 mg Hg in control group after 16 weeks of the study. However there was no significant difference between intervention and control group. This study observed there was a tendency that calcium supplementation maintains diastolic blood pressure level.Keywords: calcium, blood pressure, pregnant women.

Abstract

Association Between Zinc Level ol Breastmilk and Infant Blood in Three Different Areas.Zinc is an essential micro nutrient in many body metabolism. Breast feeding mothers should suffice their zinc consumption as their infants need zinc for maximum growth and development. The study aimed to look for a correlation between zinc level of breastmilk and of infant blood in three different areas. The sampels were 90 breast feeding mothers with their infants aged 4-6 months from three different areas: (i) urban, (ii) rural, and (iii) coastal. Information was gathered including food pattern and consumption, social economic, anthropometry, zinc level of breastmilk and infant blood. Breastmilk sample was collected in day time from the two nipples of mother, while blood sample was taken from arm vein of infant. This study found that zinc level in breastmilk differed in the three different areas. In urban area, zinc level of breastmilk was 0.91 ± 0.16 µg/ml, while in rural area was 0.75 ± 0.13 µg/ml, and in coastal area was 1.07 ± 0.40 µg/ml. Zinc level of breastmilk in urban area was siginificantly different to that in rural area (p<0.01), and zinc level of breastmilk for coastal area was significantly different to that in rural area (p<0.01). In respect to the level of infant blood, there were also differences. In urban area, zinc level of infant blood was 0.88 ± 0.17 µg/ml, while that in rural area was 0.66 ± 0.07 µg/ml, and in coastal area was 0. 73 ± 0.17 µg/ml. Infant blood zinc level in urban area was significantly different (p<0.05) compared to coastal area. However zinc level of infant blood for coastal area was not significantly different (p>0.05) from that in rural area. The study concluded zinc level of breastmilk in coastal area showed the highest, and zinc level in infant blood in urban area showed the highest. There was a strong correlation (r=0.5165, p<0.05) between breastmilk zinc level and infant blood zinc level only in rural area, but not in both urban and coastal area.Keywords: zinc level, breastmilk, infant blood, urban, rural and coastal.

Abstract

HOMOCYSTEINE AND FOLIC ACID STATUS OF ANEMIC PREGNANT WOMEN.Background: Folic acid deficiency among pregnant women in Indonesia is quite prevalent. It is believed to cause the anemia and the risk of infant neutral tube defect. Folic acid plays a certain role in homocysteine convertion into methionine. Serum homocysteine will increase with the decrease folic acid.Objective: To obtain folic acid consumption, homocysteine and folic acid status in pregnant women suffering from anemia using recent method of determination, and relationship of the three variables.Methodology: Pregnant women in the study area were screened by their Hb values under 11 g/dL. Thirty anemic pregnant women consumption was obtained by used 2x24 hours recall method. Folic acid was analysed by Ion Capture Method and homocysteine was analysed by Fluoresence Polarization Immuno assay.Results: Folic acid consumption was 61 ± 50 µg/mL with a range of 3.5 to 15.1 µg/mL. Normal value of serum folic acid was 3.1 to 12.4 µg/mL. Therefore, the value folic acid of pregnant women in this study was still in the range of normal value. Serum homocysteine value in pregnant women was 7.1 ± 1.95 µg/mL with a range of 3.0 to 10.9 µg/mL, while normal value was 5 to 15 µg/dL.Conclusion: homocysteine, folic acid, pregnant women, anemia.

Abstract

THE IMPACT OF AEROBIC EXERCISE AND IRON TABLETS ON PHYSICAL FITNESS AND IRON STATUS AMONG ADOLESCENT.Background: Iron deficiency anemia (IDA), as measured by hemoglobin level below normal value, is the most prevalent micronutrient deficiency in Indonesia. IDA among adolescent has been shown in some studies not only affect health status but also on decreasing learning concentration and physical fitness. So far, iron supplementation has been shown to improve iron status effectively and theoretically may also improve physical fitness among high risk groups especially adolescent. Objectives: To determine the effect of aerobic exercise and iron supplementation on physical fitness and iron status among adolescent. Methods: The  subjects were adolescent boys aged 13-19 years old suffering from IDA In three Islamic boarding schools (Pondok Pesantren). A total of 181 selected subjects were assigned into two groups. The intervention group received iron tablet and aerobic exercise 3 times a week, while the control group received only iron tablet 3 times a week for the period of 7 weeks. Variables included in the study were health status assessed by physical examination by medical doctor, hemoglobin concentration, serum transferrin level, and cardiovascular endurance which were assessed before and after the intervention. Cardiovaskular endurance was measured by time needed to run for the distance of 2400 meters. Results: The results showed that the prevalence of IDA among adolescent of Islamic boarding school was 40 percent. Hemoglobin concentration in the intervention group increased  significantly (2.02 ± 1.26 gram/dL) compared to that in the control group (0.83 ± 0.93 gram/dL) Serum transferrin receptor decreased by -0.41 + 2.3 and -0.28 ± 4.9 ug/L in the intervention and control groups respectively. Cardiovaskular endurance was significantly better in the intervention group (1.9 ± 0.3 minute faster) compared to that in the control group (0.5 ± 0.3 minute slower) after the intervention. Conclusions: Iron supplementation and aerobic exercise regularly significantly improve iron status and physical fitness among adolescent. Keywords: Iron tablet, physical fitness, aerobic exercise

Abstract

ENERGY EXPENDITURE OF ELDERLY PEOPLE.Background: Physical activity is a factor to estimate the energy requirement. The elderly tend to reduce their activities which influence their energy requirement. Now aday the energy need for elderly is extrapolated from the adult. Energy expenditure based on their daily activities Is a method to estimate the energy requirement.Objectives: The purpose of the study was to assess energy expenditure of elderly people based on the daily physical activities.Methods: The study was conducted in 2 sub-sub districts of Bogar Country. A total of 92 elderly (42 men and 50 women) were included in this study. The inclusion criteria were BMI 18.5- 25.0, physically and hematologically healthy and agree to participate in this study. Physical activities data were collected 3 days respectively using method record and recall and energy intakes using 3 day food record by weighing.Results: The average age was 67.5 ± 5,1 years for male elderly and 65.4 ± 3.9 years of female elderly. Recreational activities (reading, watching TV, sitting) were the most activities done by male elderly (34.9% of the day) while female elderly 34.8% of the day were spent for sleeping activity. The highest energy expenditure of male elderly was contributed from reactional activities (570.3 ± 187.8) Kcal/day while female elderly the highest energy expenditure was contributed from household work activities. The average energy expenditure for male elderly was 1870.2 ± 261.2 Kcal/day or 34.4 Kcal/Body weight/day and female elderly was 1840.2 ± 255.7 Kcal/day or 38.2 Kcal/Body weight/day. The energy Intake of male elderly was 1858 ± 471.7 Kcal/day or 34.1 Kcal/Body weight/day and female elderly was 1472 ± 255.7 Kcal/day or 30.8 Kcal/Body weight/day.Conclusions: Conclusion of this research was the energy expenditure of male elderly balance with their energy consumption, while the energy expenditure of female elderly higher than their energy consumption.Recommendations: Further study on physical activities of elderly of different culture in Indonesia is needed.Keywords: elderly, physical activities, energy expenditure

Abstract

FACTORS RELATED TO PHYSICAL FITNESS OF NON-ANEMIC MALE ELDERLYBackground: Physical fitness can be used as indicator of the community health including elderly people. One of the benefit is elderly could live independently and do their daily activities without assistant. Since there were some factors related to physical fitness of the elderly people. This study aim to determine what factors were associated with physical fitness of non anemic male elderly in rural and urban area. Methods: 217 non anemic male elderly in rural area and 289 in urban area were participated in this study. After screening on blood hemoglobin level an agreed to participate the measurement followed by physical fitness measurements. Nutritional status was calculated from their body weight and height. Structured questionnaire was used to obtain information on sociodemographic factors, sosial activities, daily physical activities. Semi-FFQ was applied to assess their energy and protein intake. Results: The proportion of elderly who had low level physical fitness was 57.1% in rural area and 82.6% in urban area. Statistical analysis shows that the level of physical fitness in rural area was significantly associated with sociodemographic characteristic (age, current occupational and economic burden), sosial activities (religious activities), life style (current and previous smoking status), health status (systolic and diastolic blood level), energy and protein intakes. In Urban area shows that the level of physical fitness was significantly associated with life style (current smoking status, physical activities level, nutrtitional status, health status (systolic blood level) and energy and protein intakes. Conclusions: Several factors which cause the difference of level physical fitness between elderly at rural and urban area were job status, economic burden, physical activity, and diastolic blood pressure.Keyword: physical fitness, elderly.

Abstract

Vitamin A Breastmilk Postpartum Women In Kabupaten Serang.Background: Since the vitamin A status of most newborn is marginal, therefore, to avoid the infants suffering from vitamin A deficiency, it may be worth by ensuring that the concentration of retinol in breast milk is adequate.Objectives: To evaluate the effect of vitamin A supplementation on the concentration of retinol in breast milk of postpartum mothers.Methods: One hundred sixteen of postpartum mothers were randomly assigned to group 1 group 2. The mothers in group one (59 respondents) were given a single dose of vitamin A by 2 days consecutive, whereas the mothers in group two (57 respondents) were given double doses of vitamin A. Anthropometry, food consumption were assessed and maternal breast milk retinol level were serially assessed.Results: Breast milk retinol concentration showed greater at 24 h after supplementation in both groups. From 0.52 ± 0.29 umol/L in 0 h to 1.40 ± 0.65 umol/L in group 1 and from 0.41 ± 0.37 umol/L to 2.36 ± 0.89 umol/L in group 2. However, after 30 days of supplementation showed that the breast milk retinol concentration of both groups was lower then the zero hour concentration.Conclusions: Two ways vitamin A supplementation maintain higher breast milk retinol concentrabon of at least 30 days after supplementation.Keywords: retinol, postpartum, breastmilk
