Abstract

ABSTRACT Tuberculosis (TB) is a major public health problem worldwide. Complications due to TB are still very high in Indonesia. This shows that there is still very low knowledge in preventing transmission and treatment of pulmonary TB. One of the efforts made to prevent transmission of pulmonary TB is to involve families living in the same house. This study aims to produce a family empowerment model that is expected to reduce the rate of pulmonary TB transmission. This study used qualitative methods to find a model. Informants in the qualitative study totaled 11 families with family members suffering from pulmonary TB who lived in the same house. Data collection was obtained through direct observation and in-depth interviews with informants who had been determined with a thematic analysis approach. The qualitative study was conducted to explore factors affecting preparedness to prevent pulmonary TB transmission. The family empowerment model was designed from the qualitative study with the stages of exploration, analysis, and modification. The results of the study found 7 themes 18 subthemes and 51 categories based on the results of interviews and FGDs found include family knowledge about pulmonary TB, attitudes in the care of family members suffering from pulmonary TB, utilization of health services, guidelines for handling pulmonary TB by families, pulmonary TB emergency plans by families, resource mobilization by families, family support in the care of patients with pulmonary TB. This model is expected to be implemented in the TB case management program and can be applied by the Deli Serdang District Health Office and North Sumatra Province in the Communicable Disease Program towards END TB in 2030. Keywords: Preparedness, Family Empowerment, Tuberculosis (TB)  ABSTRAK Tuberkulosis (TB) adalah masalah kesehatan masyarakat utama di seluruh dunia. Abstract

.Introduction: Digital transformation in the health sector requires hospitals to implement the Hospital Management Information System (HMIS); however, problems remain in its implementation. This study aimed to analyze the obstacles and challenges in implementing an HMIS in Padang City. Methods: This descriptive qualitative study was conducted between March and July, 2024. The study was conducted in two hospitals in Padang City: one public hospital and one private hospital. The research participants were 16 people who were identified through purposive sampling. Data were collected through in-depth interviews, observations, and review of documents. Data processing consisted of data collection, reduction, and presentation, with content analysis. Source and method triangulation were used to ensure data validity. Results: Public hospitals have conducted internal training and have an adequate IT team, while private hospitals do not have formal training and lack IT personnel. From an organizational perspective, public hospitals show strong managerial support in the form of budget allocation and cross-unit coordination, whereas private hospitals face limitations in the integration of Electronic Medical Records (EMR) and SATUSEHAT. In terms of technology, both hospitals face obstacles due to slow Internet networks, but public hospitals are better prepared in terms of hardware infrastructure. Conclusion: The success of an HMIS implementation depends heavily on the alignment of human, organizational, and technological aspects. Private hospitals require stronger support in terms of training, infrastructure, and policies to compete with public hospitals in the digital transformation of health care services.

Abstract

Indonesia is currently facing a double burden of disease, comprising both communicable and non-communicable diseases (NCDs), each of which has a significant impact on public health and national economic growth. NCDs such as hypertension and diabetes mellitus have become leading causes of morbidity and mortality, requiring serious attention. To ensure the quality of healthcare services, the government has established standards through Minister of Health Regulation No. 6 of 2024 concerning Technical Standards for Fulfilling the Quality of Basic Services under the Minimum Service Standards (SPM) in the health sector. This study aims to analyze the implementation of healthcare services for patients with hypertension and diabetes mellitus in an effort to improve SPM achievement at Alahan Panjang Public Health Center (Puskesmas), Solok Regency. The research employed a mixed-methods approach, combining quantitative and qualitative methods to gain a more comprehensive understanding. The results showed that 44.6% of patients had a low level of knowledge about their disease, while 55.4% had a high level of knowledge. The Puskesmas has implemented an NCD program based on Minister of Health Regulation No. 71 of 2015 and has its own Standard Operating Procedures (SOPs). The performance of healthcare personnel was generally adequate, but there were challenges due to scheduling conflicts with integrated Posyandu (community health post) activities. Budget constraints from the district health office also affected program funding. In addition, the availability of medical equipment, such as blood pressure monitors, remains limited. The implementation of integrated Posyandu services alongside NCD programs and other life cycle-based activities has been initiated, but these efforts do not always run optimally due to limited human resources and infrastructure. These findings highlight the need to strengthen support for NCD services to achieve the 100% SPM target.
