Abstract

      CUPPING AS PAIN ALTERNATIVE THERAPYABSTRACTPain is still a world’s problem. Current pain treatment is still unsatisfactory due to its chronicity and existing drugs’ side effects. Due to dissatisfaction with the existing treatment, many patients are choosing a traditional therapy for their pain treatment. Cupping therapy is one of the traditional therapies that is widely used to relieve pain. Cupping therapy has been used in many countries in the world. Some of these countries have used cupping therapy as a pain treatment in hospitals. Neurologists are handling pain cases in their daily practice. Nearly 80% of patients come to the doctor with one of their complaint related to pain directly. The knowledge of cupping therapy for pain is important for the development of new therapies in order to solve the pain problems that so far have not reached a satisfactory level.Keyword: Alternative therapy, cupping therapy, pain, pain problemsABSTRAKNyeri masih merupakan masalah dunia. Pengobatan nyeri saat ini masih belum sampai tahap yang memuaskan terkait kronisitas dan efek samping obat yang ada. Akibat ketidakpuasan dengan pengobatan yang ada, banyak pasien yang memilih terapi tradisional untuk pengobatan nyerinya. Terapi bekam merupakan salah satu terapi tradisional yang banyak dipakai untuk meredakan keluhan nyeri. Terapi bekam telah dipakai di banyak negara di dunia. Sebagian negara tersebut telah menggunakan bekam sebagai terapi nyeri di rumah sakit. Bekam menurunkan nyeri melalui efek antinosiseptifnya, dengan cara stimulasi sistem saraf perifer dan menurunkan stres oksidatif. Studi terbaru menunjukkan bahwa terapi bekam basah dapat meningkatkan ekspresi ß-endorphin dan HSP70 keratinosit pada lokasi bekam. Dalam praktek sehari-hari dokter spesialis saraf menangani kasus nyeri. Hampir 80% pasien datang ke dokter dengan disertai salah satu keluhan yang berkaitan dengan nyeri secara langsung. Pengetahuan tentang terapi bekam untuk nyeri penting diketahui untuk pengembangan terapi baru guna menyelesaikan permasalahan nyeri yang sampai saat ini belum sampai pada tahap yang memuaskan.Kata kunci: Masalah nyeri, nyeri, terapi alternatif, terapi bekam

Abstract

Introduction: Testicular tuberculosis is a very rare type of urogenital TB. Case Presentation: A 36 year old man came to the emergency room because of pain in the right testicle accompanied by swelling and discharge. Orchiectomy on the dextra testis was continued with treatment using category 1 OAT for 9 months after the diagnosis was made using anatomic pathology examination. Discussion: The physical examination needs to be done carefully because most general practitioners consider testicular enlargement to be a malignancy. Urogenital tuberculosis requires 6 months of treatment and 9 months of treatment in patients with certain conditions. Conclusion: Prompt and precise diagnosis if testicular TB is necesssary to prevent infertility, the most common complication

Abstract

In the case of advanced stage gynecological disorders, most patients have a disturbance in the upper urinary tract. Most patients with uterine cervical carcinoma are already in stage 3B and require urine diversion. The purpose of this study is to find and determine the profile of gynecological disorders that require urine diversion, the type of urine diversion, and urine diverted output in patients with cervical carcinoma. This descriptive analytical retrospective study was conducted at Dr. Soetomo Hospital, Surabaya, Indonesia, between 2012 and 2015. The samples of this study were all patients with gynecological disorders who underwent retrograde DJ stent procedures, underwent ureterocutaneostomy and nephrostomy at the Integrated Central Surgery Building, Dr. Soetomo Hospital, Surabaya, for three years between 2012 and 2015. Data was collected based on the medical record number of patients with gynecological disorders who underwent urine diversion procedures and were recorded in the book on the operation schedule. We find that the most common cause of gynecological abnormalities in kidney obstruction is cervical carcinoma. There are three types of urine diversions used, namely DJ stent, percutaneous nefropyelostomy, and ureterocutaneostomy, with DJ as the most commonly performed. Urine diversion can improve creatinine without being influenced by the type of diversity.

Abstract

Purpose:: To report one case of parameatal urethral cyst.Background : Parameatal urethral cyst is a scarce congenital condition that was first reported in two males in 1956, until now in reported literature only found less than 50 cases, in both adults and children. Our patient, case of parameatal urethral cyst in a 5 years old boy is reported. Complete excision with total removal of the epithelium of the cyst is required management for the treatment and prevention of cyst reocurrance.Case Report:a 5 years old male with a cystic lesion around urethral meatus since birth. At least 5 month the parents complain distorted urinary flow and poor appearance, and no other urinary symptom, no history of trauma. On physical examination, cystic mass with spherical shape which was about 0.5 cm in diameter was found around external meatus. There was no inflammatory sign. And there was normal blood laboratory (blood counts and blood chemistry) and urine laboratory (urine analysis and urine culture). The patient undergo completely excision of the cyst under general anaesthesia, and remove all of the lining epithelium. Good appearance results were obtained after 2 months follow up, without meatal strictures and urine stream problems, and no postoperative complications or recurrence. Pathological : Squamous epithelial, granulation tissue with chronic inflamationConclusions :Parameatal urethral cyst is a very rare benign condition that is asymptomatic in most of the cases. It may be present since birth or appear later and is prevalent in young males. Its etiology remains unclear and treatment is by complete surgical excision to avoid complications and recurrence. Abstract

Foreign objects may be present in the bladder due to deliberate insertion or displacement of the surrounding organs. There have been multiple cases of sharp objects such as nails, hair pins, and screws, as well as large objects such as AAA batteries, toothbrushes, and ballpoint pens found in the urethra and the bladder, which must have been aimed to induce sexual satisfaction associated with mental and psychic disorders. The diagnosis was established by means of  radiological examinations, ie thoracic x-ray, KUB, and urethrography. This article reported a 32-year-old man with headset inserted to penile until bladder and could not be pulled out again. The patient complained pain and bleeding out of the urinary tract. We performed vesicotomy and pulling the out headset from bladder. The operation was succesful. In conclusion, the case of a headset in the bladder has been treated successfully with vesicotomy.

Abstract

The natural hatching success rate of olive ridley sea turtle (Lepidochelys olivacea) eggs on Pulau Santen Beach is often hindered by predator disturbances, temperature fluctuations caused by human activities, and unfavorable environmental conditions, with a success rate of only 50–70%. This study aimed to analyze the hatching success rate of olive ridley sea turtle eggs using the INTAN BOX incubation device on Pulau Santen Beach, Banyuwangi. A quantitative descriptive method was employed to observe 365 eggs relocated from four nests to the INTAN BOX over an incubation period of 50–60 days. Parameters observed included temperature, humidity, and hatching success rates. Results indicated an average hatching success rate of 86%, with individual nest success rates ranging from 83% to 89%. These rates are significantly higher than the 50–70% success rate achieved through natural hatching. The INTAN BOX provides a stable environment, protects eggs from predators, and optimizes temperature and humidity conditions, thereby enhancing hatching success. Additionally, the device enables the management of hatchling sex ratios through temperature-dependent sex determination (TSD), which is critical for sea turtle population conservation. This study concluded that the INTAN BOX is an effective innovation for olive ridley turtle conservation while also supporting local community empowerment around Pulau Santen Beach.

Abstract

Introduction: Bladder trauma is the second most commonly injured organ in the genitourinary system after the kidney. The injury can be caused by a variety of mechanisms, including blunt trauma, penetrating trauma, and iatrogenic procedures. Case: A 21-year-old male with multiple injuries following a motorbike accident. Upon examination, a foreign body was detected in the right thigh. A CT scan revealed a metal rod impaled in the patient’s bladder without any contrast leakage. The patient was planned for foreign body removal and bladder repair. During the removal, bladder leakage was detected. The patient underwent foreign body removal and bladder repair. Discussion: This case underscores the importance of considering bladder injury in trauma patients, even in the absence of classic clinical signs such as haematuria. It also highlights the potential limitations of initial imaging techniques like cystography in detecting bladder injuries when foreign bodies are present. Conclusion: This case emphasizes the importance of maintaining a high index of suspicion and considering intraoperative exploration or additional diagnostic methods when imaging findings are inconclusive. Revising the diagnostic protocol for trauma patients with suspected bladder injury, especially if a foreign body is present, is essential to avoid missed diagnoses and improve patient outcomes.

Abstract

Inkontinensia urin merupakan kelainan yang ditandai dengan keluarnya urin secara involunter yang mempengaruhi 51% wanita di seluruh dunia. Prevalensi meningkat seiring usia, dari 13% pada wanita muda hingga 75% pada wanita tua. Di Indonesia, prevalensi inkontinensia urin mencapai 13% dari 2765 responden, dengan stress inkontinensia urin (4,0%), inkontinensia urgensi (4,1%), dan mixed inkontinensia urin (1,6%) sebagai tipe utama. Tinjauan pustaka ini bertujuan untuk memahami patofisiologi, klasifikasi, diagnosis, dan penatalaksanaan inkontinensia urin berdasarkan literatur terkini. Studi kepustakaan dengan menganalisis berbagai referensi mengenai anatomi kandung kemih, fisiologi normal berkemih, dan berbagai tipe inkontinensia urin dari database PubMed, Scopus, dan Google Scholar dengan rentang waktu 2014-2024. Inkontinensia urin diklasifikasikan menjadi tiga tipe utama: stress incontinence, urgency incontinence, dan mixed incontinence. Patofisiologi melibatkan gangguan pada otot detrusor, sphincter uretra, dan sistem saraf. Diagnosis ditegakkan melalui anamnesis, pemeriksaan fisik, dan pemeriksaan penunjang seperti urodinamik. Terapi bervariasi mulai dari modifikasi gaya hidup, terapi farmakologis, hingga intervensi bedah sesuai dengan tipe inkontinensia. Inkontinensia urin bukan merupakan proses penuaan normal namun kondisi medis yang dapat ditangani. Kemajuan dalam diagnosis dan terapi memberikan harapan bagi penderita untuk mengembalikan kualitas hidup normal.

Abstract

Objective: This study aims to evaluate the role of percutaneous nephrostomy (PCN) in treating patients with hydronephrosis due to complications of cervical cancer. Material & Methods: This research was a retrospective analytical cohort study. All medical record data of patients with a diagnosis of cervical carcinoma and undergoing percutaneous nephrostomy at Soetomo General Hospital in 2017 – 2020 were collected and analyzed. Results: A total of 45 subjects were included in this research. The mean age of the subjects was 55.07, with the youngest being 34 years old and the oldest being 75 years old. Most of the subjects were stage IIIB cervical carcinoma (95.56%), and the rest were stage IIB. Contralateral PCN was required in 75.56% of subjects, and the rest only required unilateral PCN. Before being PCN performed, some patients had moderate HN (64.4%), and after the patients underwent PCN procedure, the majority of patients had the HN resolved (48.89%) (p<0.05). There was also an improvement in the laboratory parameter of urea and creatinine serum before and after PCN (p<0.05). Conclusion: PCN improves the laboratory and radiologic parameters of cervical cancer patients with hydronephrosis.

Abstract

Objective: Research on the distribution of urology in Indonesia, especially in East Java, has not been widely discussed. There are still several areas in East Java that do not have a urologist at all. We try to find out the factors underlying this distribution through several indicators. Material & Methods: Data from all hospitals in East Java were compiled with data of urologists in East Java obtained from the Indonesian Urological Association, supplemented by information from social media and the telephone of the hospital operator. The number of urologists and hospitals with urology services is then correlated with the human development index, the education index, purchasing power index, health index, and regional minimum wage. Results: The ratio of urologists in each district and city in East Java is not evenly distributed, and likewise with the hospitals that provide urology services in each district and city in East Java. The ratio of urologists and the number of hospitals providing urology services are significantly related to the human development index, the education index, the purchasing power index, the health index, and the regional minimum wage. Conclusion: The location of a urologist's practice is related to the development of an area, which in this study is assessed through indicators of human development, education, health, and economy.
