Abstract

Introduction: Puncture wound in diagnostic interventions permits the entry of bacteria into the skin or soft tissue, thus precipitating nosocomial infection, such as erysipelas. There are other risk factors of nosocomial infections including old age, immunosuppressive drugs, and underlying diseases. Pemphigus foliaceus (PF) is an autoimmune disease with corticosteroid treatment as the mainstay therapy, which could cause immunosuppression and predispose patients to infection. The objective of this paper was to report erysipelas as one of the manifestations of nosocomial infection in patients under immunosuppressive therapy. Case: A case of erysipelas acquired on the 9th day of hospitalization in a PF patient underwent intravenous dexamethasone injection, with history of puncture wounds on the previous day on the site of erysipelas was reported. The clinical findings of erysipelas were well defined, painful erythema and edema that felt firm and warm on palpation, with blisters and pustules on top. Gram staining from the pustules and blisters fluid revealed Gram (+) cocci. Patient was given 2 grams intravenous ceftriaxone for 7 days and saline wet compress. Improvement on the erysipelas was seen the day after ceftriaxone injection. The patient was discharged after 12 days of hospitalization with improvement both on the PF and the erysipelas. On the next visit 7 days later, the erysipelas lesion disappeared. Conclusion: Puncture wound and immunosuppresive treatment are the factors that could cause erysipelas as a nosocomial infection, and an appropriate treatment of the infection would decrease the functional disability of the patient.

Abstract

About 81% of pregnant women with trichomoniasis are asymptomatic, while trichomoniasis in pregnant women can increase the risk of complications, include premature rupture of membranes, preterm birth, and babies with low birth weight. Trichomoniasis can also increase the risk of other sexually transmitted infections (STIs) and human immunodeficiency virus (HIV) transmission. Trichomoniasis case in pregnant women could be influenced by demographic characteristics,, the sexual behavior, and also the diagnostic method used. Until now, there is no data about prevalence of trichomoniasis in pregnant women in Indonesia. The aim of this research was to determine the prevalence of trichomoniasis in pregnant women in Bandung, West Java, Indonesia. A descriptive cross-sectional study was performed in December 2016 until January 2017. The study participants were 50 pregnant women who visit antenatal care to Obstetric and Gynecology Clinic of 'Rumah Sakit Khusus Ibu dan Anak Kota Bandung', and meet the inclusion and exclusion criteria, through consecutive sampling. The study participants had a history taking, venereological examination, and Trichomonas rapid test from vaginal swabs. Trichomoniasis in this study was diagnosed based on Trichomonas rapid test, a test that uses color immunochromatographic, capillary flow, dipstick technology, and has high sensitivity and specificity in diagnosing trichomoniasis. Almost all participants in this study were low risk pregnant women to have STI based on demographic characteristics and sexual behaviour. The positive Trichomonas rapid test result was found from one of 50 study participants. In conclusion, prevalence of trichomoniasis in pregnant women in Bandung was 2%. Trichomoniasis case in low-risk pregnant women population is still found.

Abstract

Background: Coronavirus disease 2019 (COVID-19) is still becoming a problem in several countries with the emergence of new variants with the latest variant named Omicron. Studies show that patients with COVID-19 tend to have Acute Respiratory Distress Syndrome (ARDS) because of high pro-inflammatory cytokines and chemokines levels. Since its role as an antiviral agent, anti-cytokine, immunomodulator, antioxidant and has potential effect in maintaining body functions, vitamin C can be used as one of therapy. There is still arguable about giving treatment of COVID-19 patients with Intravenous Vitamin C (IVC). Previous research has not been discussed related to accelerating recovery and decreasing mortality. Purpose: To identify the impact of IVC intervention given to COVID-19 patients admitted in hospital by reviewing clinical outcomes particularly on complexity or mortality. Review: Literature information was sourced from PubMed, ScienceDirect, and Google Scholar databases through keywords to retrieve inclusion criteria published during 2019-2021. Result: We found that IVC influenced inflammatory biomarkers such as CRP, d-dimer, ferritin, and several inflammatory cytokines, mortality, length of stay, oxygenation, organ and immune system function but showed no significant difference between the control group. Conclusion: IVC treatment might be an essential adjuvant therapy in the aggravation of mild, severe, and critical COVID-19 patients, but did not understate complexity or mortality. Hence, further research and evaluation in clinical trials are needed.

Abstract

Introduction: Atopy is a genetic tendency of allergic condition with specific clinical manifestations such as atopic dermatitis (AD), allergic rhinitis (AR), and bronchial asthma (BA). Atopic individuals might experience other health problems that coincide with atopic manifestations, including the appearance of recurrent aphthous stomatitis (RAS) which is an oral mucosal disease that can also be influenced by allergies. This study was aimed to be the preliminary study regarding the level of IL-6 serum in atopic patients who were also having RAS. Methods: Thirty subjects were recruited and grouped into three groups with ten subjects each were group of atopic (dermatitis, allergic rhinitis, or bronchial asthma) with RAS, atopic without RAS, and control. Blood specimens were obtained with an approved institutional board review protocol. This research was cross-sectional research with consecutive sampling method conducted at Dr Hasan Sadikin Hospital Bandung. Results: There were significant differences in IL-6 log levels in all three groups (p < 0.001). The higher IL-6 levels in the group of atopic with RAS might be due to the influence of atopy because this condition can increase the acute inflammation of RAS. IL-6 is a pro-inflammatory marker found in the circulation of atopic and RAS patients. Conclusion: Serum IL-6 levels in the group of atopic patients with RAS is higher than in other groups. Keywords: Serum, IL-6, atopic, recurrent aphthous stomatitis
