Abstract

Women adapt to pregnancy through multi-organ system physiologic changes, including cardiovascular adaptations. These changes affect those with pre-existing cardiovascular problems differently, and subsequently lead to higher probability of death caused by cardiovascular diseases during pregnancy. Therefore, detection of cardiovascular disease early in pregnancy is important to lower maternal morbidity and mortality by providing prompt and adequate management. This study aimed to evaluate and test the feasibility of integrating 12-lead electrocardiogram (ECG) examination and antenatal care (ANC) screening as a simple and effective method for early detection of heart abnormality in pregnant woman. Pregnant women were recruited in this study in any trimester who attended ANC for a routine pregnancy examination in Puskesmas Tegalrejo Yogyakarta. The subjects underwent primary screening which focused on cardiac auscultation and 12-lead ECG examinations, and those who had abnormal findings were further followed-up in secondary screening by using trans-thoracic echocardiography to confirm heart abnormality. A total of 523 pregnant women from Puskesmas Tegalrejo were included in this study. 15 (2.8%) pregnant woman were suspected to have heart abnormalities; from those, 3 (0.5%) were found with heart murmurs with abnormal ECG readings, 1 (0.19%) had heart murmurs with normal ECG results, and 11 (2.1%) had abnormal ECG readings only. The secondary screening of those patients resulted in 1 (0.19%) pregnant woman who was diagnosed with Atrial Septal Defect. Our study found that among 15 patients identified with suspected ECG abnormalities, one mother who underwent ANC was newly diagnosed with a pre-existing cardiac abnormality. Our study concluded this screening method is a simple and feasible integrated heart screening program that can be implemented widely. We hope this integrated heart screening program may benefit pregnant women who may have cardiac abnormalities to be detected as early as possible, thus reducing maternal morbidity and mortality.

Abstract

Background: Low pregnancy weight gain is one of the risk factor of low birth weight (LBW). Weight gain rate per week can be used gain during pregnancy. However, some studies show differences in cut-off point weight gain in pregnancy of second and third trimester to minimize the risk of LBW.Objective: To identify changes of weight gain rate per week in the second and third trimester of pregnancy in relation to the risk of LBW.Method: The study was observational study using case control design. The study population was women giving birth at Ulin Hospital of Banjarmasin during the period of November 2006 – August 2007. Subject of the study was having LBW infants 98 mothers as case group and giving birth to infants >2500 grams 245 mothers as control group. Data medical record of obstetric and gynecology unit, mother and child health monitoring books, so did the direct interview. Chi square and double logistic regression methods were used to analyse the relationship between the objective variabels.Result: Weight gain <250 grams/week in the second and third trimester of pregnancy had significant relationship with the risk of LBW (OR 7.1; 95%CI 4.0 – 12.5): other variables having risks of LBW were gestational age (<37 weeks), hypertensive disorder during pregnancy, parity, having history of preterm delivery and prenatal care < 5 times. The result of multivariable analysis showed that weight gain <250 grams/week in the second and third trimester of pregnancy had significant relationship with LBW; other variables were preterm delivery, hypertensive disorder during pregnancy and primipara.Conclusion: Pregnant mothers with weight gain <250 grams/week during second and third trimester of pregnancy had probability 7.1 times of having LBW infants than those giving birth to infants >2500 grams. Other variables having risks of LBW were gestational age (<37 weeks), hypertensive disorder during pregnancy, parity, having history of preterm delivery and prenatal care < 5 times.Keywords: weight gain rate per week, low birth weight

Abstract

Compliance of antenatal care and election of birth helpers in NatunaPurposeThis study aimed to determine the association of ANC (Antenatal care) compliance with birth attendant choice in Natuna.MethodsThis study used a cross-sectional method. The subjects of the research were mothers in Natuna regency 2014. The variables were: independent variable (ANC compliance), the dependent variable (the birth attendant choice), and the control variables (age, maternal education, distance to the health facilities, parity). Data collection used questionnaires and interviews. The data analysis used descriptive and inferential analysis. The bivariable analysis used the chi-square test and multivariable analysis used logistic regression test with 95% of confidence interval (CI) and a significance level of p <0.05.ResultsBivariable and multivariable analysis showed significant correlations between ANC compliance with birth attendant choice. Pregnant women who did not comply doing ANC according to the standard that has been set at least four times had a chance two times more likely to give birth with the traditional birth attendant (TBA) than the pregnant women who complied to implement the ANC during pregnancy. Mothers with low education had a chance two times greater of choosing the TBA as birth attendant than women who are highly educated. Mothers with parity of >2 had a chance 1.9 times greater of choosing the TBA as birth attendant than women with parity ≤2.ConclusionsThere needs to be a control program from the health department working with community health workers to socialize awareness of the importance of the ANC. There needs to be the addition of TBA who are trained and partnered especially in remote areas because it cannot be denied there are still many people who choose TBA as a birth helper.

Abstract

PurposeThe purpose of the study was to explore the experience of primipara mother when the emergency cesarean section was decided at RSUD Wates Kulon Progo Yogyakarta.MethodA phenomenological study was conducted involving indepth interview of 7 post emergency cesarean section mothers at Wates Kulon Progo Yogyakarta hospital in April-July 2014.ResultsThe study found three themes on the mother experience related to emergency cesarean section; the perception of the mother associated with emergency cesarean section, the influence of spirituality on positive perceptions of mothers and support for mothers in the face of emergency cesarean section.ConclusionMothers have a specific strategy in dealing with the emergency cesarean section which they will experience. Support for mothers in facing emergency cesarean section, spirituality influence on mother's positive perception and mother perception related to emergency cesarean section affects mother's readiness to undergo emergency cesaeran section. It is necessary to create a policy focusing on FCMC by involving family counselors when the mother is determined by emergency cesarean section.

Abstract

Efforts of high-risk pregnant women to search service for childbirth in public health center ofWaruroyom Purpose The study explored the behavior of women with a high-risk pregnancy to search service for childbirth in public health center of Waruroyom Cirebon.MethodA case study involved pregnant and maternal mothers with a high-risk pregnancy, parents, husbands, and midwives. The informants were chosen by purposive sampling, data collection through observation, in-depth interview and focus group discussion. ResultsWomen with high-risk pregnancy searched service for childbirth from midwives, public health center, and hospital. Pregnant women prefer midwives due to familiarity, close location, past experience, attitudes and quality of services. Women with high-risk pregnancy went to the public health center or hospital, did not because of the intentions of themselves but midwives' advice. ConclusionPregnant women did not know if they have a high-risk pregnancy. Midwives provide information about high-risk pregnancy at the end of pregnancy.

Abstract

This reseach aims to investigate the difference between umbilical cord treatment by using topical ASI and dry treatment towards the length of umbilical cord release in baby. This research used quasi experimental method with non-equivalent posttest design only. 44 samples of neonatal babies were taken by using consecutive sampling according to the inclusion criteria. The average length of umbilical cord release with topical ASI is 6.18 days and the one with dry treatment is 7.41 days. The length of umbilical cord release with topical ASI is 1.23 days faster compared to the one with dry treatment with p=0.010. The umbilical cord release with topical ASI is faster than the one with dry treatment and it can also reduce infection.

Abstract

Abstract: This study aims to determine the effect of type of delivery onthe risk of postpartum depression in mothers postpartum in hospitalsSleman. This type of research is analytic survey with a retrospective cohortdesign. Hospital research sites in Sleman, Yogyakarta. The population inthis study are all postpartum mothers who visited the poly obstetrics andgynecology in the months from October to December 2015. The totalsample of 110, the data analysis performed univariate and bivariate withchi-square test. The results showed the incidence of the risk of postpartumdepression in mothers postpartum in hospitals sleman was 36.3%, type oflabor significantly influence the risk of postpartum depression with thevalue (OR = 3.716, 95% CI 1.620 to 8.522).

Abstract

The study aims to analyze the influence of breast feeding towards the puerperal period in Primary Health Care Trucuk I Klaten in the year of 2015. This research is the type of analytical research that uses the design of retrospective cohort with the quantitative approach. The sample uses purposive sampling. The total sample are 86  mothers who breastfeed fully and 86 mothers who breastfeed partially. The result of the analysis shows that there are 103 (59,9%) cases of short peurperal period. The variable that significantly influence the puerperal period is breastfeeding (OR = 34,550; 95% CI = 12,925-92,353) and parity (OR = 0,404; 95% CI = 0,170-0,963). Meanwhile, the variables that have no significant influence the puerperal period is IMD (95% CI=0,756-11,045), mother age (95% CI = 0,221-1,096), and the status of mother employment (95% CI=0,744-2,731).

Abstract

Abstract: This study aims to find out the correlation of exclusive breast- feeding and the nutrional status and development of infant aged 6-12 months old. This is an observational analytical research. The data are analyzed using bivariate analysis of ChiSquare and multivariariate logistic regression. Based on bivariate analysis, there is a significant evidence regarding the correlation between exclusive breastfeeeding and infant nutrional status, as the value is OR 21.317;95% CI 2.761-164.565. The bivariate analysis in exclusive breastfeeding and infant development shows that there is a significant evidence in exclusive breastfeeding and infant development as the value is OR 6.000; 95% CI 2.548 – 14.130.

Abstract

Preeclampsia is one of the leading causes of maternal mortality. Research on the characteristics of mothers who are at risk for preeclampsia at the first pregnancy visit is important to prevent the incidence of preeclampsia. This study aims to determine the incidence of preeclampsia based on maternal characteristics. A cohort study design was used to determine the effect of maternal characteristics on the incidence of preeclampsia Sample was pregnant women who made their first pregnancy visit. The analysis was chi-square. The results showed that the factors that were significantly associated with the occurrence of preeclampsia were: primigravida (P-value: 0.00), family history of preeclampsia (P-value: 0.00), occupation (P-value:0.04), and insurance ownership (P-value:0.03). A risk assessment of preeclampsia in pregnant women can be carried out at the time of the first antenatal visit.
