Abstract

Latar Belakang: Coronavirus merupakan penyakit yang muncul pada akhir tahun 2019 disebabkan oleh  Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) yang kini telah ditetapkan sebagai pandemic oleh WHO sejak 11 Maret 2020. Indonesia melaporkan kasus pertama COVID-19 pada tanggal 2 Maret 2020 dan jumlahnya terus bertambah hingga sekarang. Sampai saat ini penelitian tentang infeksi SARS-CoV-2 pada kehamilan dan luaran maternalnya masih terbatas. Dikatakan ibu hamil memiliki risiko lebih tinggi untuk terjadinya morbiditas dan mortalitas dibandingkan pada populasi umum.Tujuan: Mengetahui apakah terdapat hubungan antara ibu hamil konfirmasi COVID-19 dengan luaran maternal di Daerah Istimewa Yogyakarta. Metode: Desain penelitian ini adalah kohort retrospektif. Subjek penelitian adalah   pasien hamil dengan konfirmasi COVID-19. Subjek penelitian diambil di RSUP Dr. Sardjito dan RS Akademik UGM sejak periode Maret 2020 sampai dengan Agustus 2021Hasil: Dari 436 subjek penelitian didapatkan 340 sampel yang memenuhi kriteria untuk dilakukan analisis. Karakteristik subjek penelitian menunjukkan 58,24% pasien berusia >35 tahun dengan mayoritas usia kehamilan ≥ 37 minggu sebanyak 79,41%. Terdapat hubungan yang signifikan (p<0.05) antara ibu hamil dengan COVID-19 simptomatik terhadap luaran persalinan prematur (RR 2,42, CI 95% 1,44–4.04), kebutuhan ICU (RR 53.89, CI 95% 3.33–870.45), lama rawat inap (RR 1.97, CI 95% 1,05–3,68) dan kematian maternal (RR 38.08, CI 95% 2,34–619,83) dibandingkan dengan ibu hamil COVID-19 asimptomatik. Tidak terdapat hubungan bermakna (p>0.05) antara ibu hamil COVID-19 dengan luaran metode persalinan. Usia, obesitas, hipertensi dan diabetus melitus tidak memiliki hubungan signifikan terhadap luaran maternalKesimpulan: Kehamilan merupakan faktor risiko independent COVID-19 simptomatik yang tidak bergantung pada komorbiditas lain. Ibu hamil dengan COVID-19 simptomatik memberikan luaran maternal yang lebih buruk dibandingkan dengan ibu hamil COVID-19 asimptomatik. Metode persalinandiputuskan berdasarkan indikasi medis dan kebidanan.Kata Kunci: COVID-19, kehamilan, komorbid, luaran maternal

Abstract

Background: Vitamin D deficiency is inversely related to blood pressure and may contribute to the genesis and maintenance of hypertension. However, in many clinical studies the relationship between vitamin D status and blood pressure has not shown consistent results. This study aims to analyze differences in vitamin D status in Madurese ethnic pregnant women with hypertension and non-hypertension. Subjects and Method: This research is an analytical observational design with a case control study. The target population in this study were pregnant women who came from the Madurese ethnicity. The affordable population is pregnant women who come from the Madurese ethnicity in the Work Area of the Bangkalan District Health Office. The study was conducted in the period from May 2018 to February 2020. The sampling technique used was consecutive sampling. The sample size was 105 pregnant women with hypertension and 105 controls. The independent variables included consumption of food sources of vitamin D and exposure to ultraviolet B rays. The dependent variable was vitamin D levels. Data on consumption of foods that were sources of vitamin D was obtained using the Food Frequency Questionnaire (FFQ). Data on exposure to ultraviolet B (UV B) rays were obtained by questionnaire. Data on vitamin D levels were obtained from the collection of blood serum which was then analyzed by ELISA Kit. Independent Sample T and chi square test were used to analyze the differences in variables between the two groups. Results: The mean value of vitamin D levels in the case and control groups were 37.87 ng/mL and 37.89 ng/mL, respectively. Subjects with vitamin D insufficient status were more in the case group (19.00%) than the control group (16.20%). The results showed that there was no significant difference between the two groups in vitamin D levels (p= 0.984) and vitamin D status (p= 0.587). Insufficient exposure to UV B provides a 3.95 times greater risk of experiencing insufficient vitamin D. Conclusion: The conclusion of this study is that the role of vitamin D in the incidence of hypertension in pregnancy has not been determined.

Abstract

Every nation on earth is experiencing an increase in the number and proportion of elderly people. This vulnerable population may experience a decline in health-related quality of life (HRQOL) following a natural disaster. This scoping review aimed to have a better understanding on the elderly’s HRQOL following natural disasters, its influencing factors, and the state of the research on this topic. A literature search was conducted using four databases (PubMed/MEDLINE, Cochrane Library, Science Direct, and ProQuest) for the last 10 years (2013–2022). The summary of the selected relevant articles and analysis data were used to present the findings. There were seven articles included in our review. One article was about floodsand the rest was about earthquake.Two out of the six earthquake articles involved tsunamis. All of the studies were conducted during the recovery phase and revealed that the elderly had poor HRQOL after natural disasters. Several aspects, including demographics, physical, psychological, social, and environmental factors, were identified as having impacts on the elderly‘s HRQOL following a disaster. The relocation had a significant negative impact on the mental health of the elderly, with a mean difference of -3.69 (-5.60, -1.77). In conclusion, the HRQOL of the elderly after a natural disaster is low and affected by several factors. To improve it, a stronger research agenda and disaster management programs that take these factors into account are required.

Abstract

Indonesian maternal mortality in 2012, 32.5% was caused by hypertension, preeclampsia, and eclampsia, while 24.22% in 2015 and 26% in 2016. Currently, there is no method of screening for preeclampsia based on maternal characteristics that were carried out during the first visit antenatal. The use of technology has great potential to improve health care at the community, especially in low-middle income countries. It is necessary to make a breakthrough in the form of developing a screening model for preeclampsia by utilizing information technology. This study aims to determine the detection rate of the application of preeclampsia.com as a community-based preeclampsia screening tool. This study was conducted by cohort study  on 549 pregnant women who had their first antenatal visit before 16 weeks followed up to delivery. Data analysis was data of factors for the occurrence of preeclampsia using Chi square, while to determine the strength of the “Preeclampsia.com” model as a screening method and early detection of preeclampsia using multilevel logistic regression,  analysis performed using SPSS version 23.0. The results of the analysis showed that the sensitivity value of the "Preeclampsia.com" application was 64.1% and the specificity value was 85.5% (95% CI). The Positive Predictive Value (PPV) from the "preeclampsia.com" application is 25.3% and the Negative Predictive Value (NPV) from the "preeclampsia.com" application is 96.9%. Based on the results of multilevel logistic regression analysis, it was found that the ability of the model to predict the incidence of preeclampsia was 96.4%. The use of the Preeclampsia.com application is quite good for community-based preeclampsia screening

Abstract

Increasing exclusive breastfeeding effort was continued in Jember district. Local government collaborates with USAID to increase breastfeeding promotion program. Health promotion program used multilevel approach. This study aimed to evaluate exclusive breastfeeding program and impact program to increasing breastfeeding in Jember district. This study method used cross sectional study in two population with mix method approach (qualitative and quantitative). Collecting qualitative data was using participative observation, indepth interview and focus group discussion to describing program. Collecting quantitative data was using questionare to evaluate impact program in two populations (120 respondent program and 125 respondent non program). Data was analyzed by open code in qualitative data. To analyzed quantitative data was used chi square.  The research result shows that Health promotion program increased exclusive breastfeeding was using multilevel approach. In top level, implementation of the program was regulation government policy number 17, 2013 about save childbirth, initiation breastfeeding and exclusive breastfeeding. In the middle level implementing program by Jember health department and local government clinic through peer breastfeeding support, mother pregnant class and promote by midwife. Lower level evaluation result was that two population program and non program have different significant. Population who give program have 2,4 higher OR value in exclusive breastfeeding than population not give the program. Based on this research it can be concluded that multilevel approach can used to increasing exclusive breastfeeding until six month. Government support was needed to successful program.

Abstract

Introduction and importance: Conjoined twins are a complication of monozygotic pregnancies with an incidence of 1:50,000-200,000 of life births. Pygopagus is found around 17% of all types of conjoined twins and has a mortality rate of 23%. Proper diagnosis and management in this case is important. Presentation of case: A 25-year-old primigravida woman presented at 22 weeks of gestation with a Pygopagus ultrasound result at 17 weeks of gestation. MRI was performed at 25 weeks of gestation, showed conjoined pygopagus twins with lumbosacral spina bifida, without visualized bone union, separated bladder, rectum and anus were difficult to evaluate. Caesarean section (CS) is planned at 36 weeks of gestation and postpartum MRI will be performed in preparation for the separation surgery. The patient was admitted to the hospital at 35 weeks of gestation in the active phase of the first stage of labor, then an urgent caesarean section was performed. Both babies were born with an APGAR score of 2/4/7. The second baby died a few hours after delivery. The separation surgery was performed as an emergency, but the first baby died during the procedure. Conclusions: Pygopagus requires holistic management, starting from diagnosis, preparation for delivery, to postpartum complications. The proper holistic management is expected to reduce infant and maternal morbidity and mortality.

Abstract

Latar Belakang: Depresi postpartum sering terjadi pada masa adaptasi psikologis ibu masa nifas, walaupun insidensinya sulit untuk diketahui secara pasti namun diyakini 10-15% ibu melahirkan mengalami gangguan ini. Faktor pada saat persalinan meliputi lamanya persalinan, jenis persalinan, serta intervensi medis yang digunakan mempengaruhi depresi postpartum, anak yang memiliki ibu depresi postpartum akan memiliki gangguan prilaku, rendah fungsi berfikir, mempengaruhi kognitif dan pertumbuhan anak. Tujuan: Mengetahui pengaruh jenis persalinan terhadap risiko depresi postpartum pada ibu nifas di RSUD Sleman Metode:Jenis penelitian yang digunakan adalah survey analitik dengan rancangan kohort retrospektif. Lokasi penelitian di RSUD Sleman Yogyakarta. Populasi pada penelitian ini adalah seluruh ibu nifas yang berkunjung pada poli kebidanan dan kandungan pada bulan Oktober-Desember 2015. Jumlah sampel 110, analisa data dilakukan secara univariat, bivariat dengan uji chi-square dan multivariat dengan uji regresi logistik Hasil : Kejadian risiko depresi postpartum pada ibu nifas di RSUD sleman adalah 36,3%, Jenis persalinan berpengaruh secara signifikan terhadap risiko depresi postpartum dengan nilai (OR=3,716, 95%CI 1,620-8,522), Pekerjaan berpengaruh secara signifikan terhadap risiko depresi postpartum dengan nilai (OR=2,411, 95%CI 1,084-5,366), umur, paritas, pendidikan, status ekonomi, dukungan keluarga dan status pernikahan tidak berpengaruh secara signifikan terhadap risiko depresi postpartum Kesimpulan :Ibu dengan persalinan bedah sesar mempunyai peluang risiko depresi postpartum 3,716 kali lebih besar dibandingkan ibu yang persalinan pervaginam, sehingga perlu dilakukan deteksi dini untuk melihat risiko depresi postpartum pada ibu nifas agar ibu dapat segera mendapatkan asuhan yang tepat.

Abstract

Universal precaution merupakan bagian dari keselamatan dan kesehatan kerja (K3) yang sangat diperlukan dalam aktivitas pelayanan kesehatan dan secara signifikan mengurangi jumlah insiden kerja karena paparan darah. Dikota Tangerang selatan pada tahun 2015 terdapat 15 kasus kematian ibu, penyebab infeksi intrapartum (6,7%) dan kematian bayi sebanyak 37 kasus, penyebab dari infeksi sebesar 13,5% dan 10,8% karena HIV/AIDS. Penelitian ini bertujuan untuk mengetahui hubungan pengetahuan dan sikap terhadap pelaksanaan universal precaution pada pertolongan persalinan oleh bidan.Pada penelitian ini terdapat variabel independent yaitu pengetahuan dan sikapdan variabel dependent yaitu pelaksanaan universal precaution pada pertolongan persalinan. Desain penelitian yang digunakan adalah Cross Sectional, berupa pengisian kuesioner dengan sampel 88 bidan. Uji statistik yang digunakan adalah Uji Fisher. Hasil penelitian yang didapat adalah bidan yang mempunyai pengetahuan baik sebanyak 89,9% , sikap positif sebanyak 78,4% dan melaksanakan universal precaution pada pertolongan persalinan dengan baik sebanyak 84,1%. Terdapat hubungan yang signifikan anatar pengetahuan terhadap pelaksanaan universal precaution dengan pvalue 0,041 dan tidak terdapat hubungan yang bermakna antara sikap terhadap pelaksanaan universal precaution dengan pvalue 0,491

Abstract

Objective: The aim was to assess pulsatility index of uterine artery (Ut-A PI) of normotensive pregnancy and preeclampsia between 28-40 weeks of gestation. Methods: This cross-sectional study was conducted at Dr. Sardjito Hospital and Harapan Kita Maternal Children Hospital as a tertiary referral hospital. The study included 220 women high risk pregnancies. Result: There were 188 pregnant women normotensive pregnancy (85.5%) and 32 preeclampsia cases (14.5%). Weight, Body Mass Index, blood pressure, Mean Arterial Pressure, chronic hypertension, history of preeclampsia, and baby weight showed an significant association with preeclampsia. Pulsatility index of uterine artery had a sensitivity of 87.50% and a specificity of 98.94% whereas the notching had a sensitivity of 46.88% and a specificity of 87.23%. Conclusion: The UtA PI is a complementary tool with good performance for the diagnosis of overall and severe preeclampsia with high sensitivity and specificity. Abstract

Background: Modified Early Obstetric Warning Score (MEOWS) is used to identify obstetric patients at risk of deterioration. MEOWS has long been used in the maternal ward of Dr. Sardjito Central General Hospital. Research in other countries found that MEOWS cards were not filled in completely and some MEOWS cards were not followed up.Objective: to determine the completeness of the MEOWS card and the suitability of the interventions carried out with the MEOWS card instructions.Methods: This study is a mixed method study (quantitative descriptive and qualitative phenomenology).Results: Out of 1285 study subjects, the proportion of complete MEOWS cards was 84.67%. The proportion of appropriate interventions was 95.05%. Appropriate intervention was associated with length of hospitalization ≥ 7 days, p value 0.043, prevalence ratio 0.456 (95% confidence interval 0.242-0.858). Reasons for staff not completing MEOWS cards and not performing appropriate interventions were management factors, lack of knowledge, compliance, vigilance, training and medical records.Conclusion: Most MEOWS cards were completed and appropriate interventions were performed. Appropriateness of MEOWS interventions is likely to reduce the prevalence of length of stay ≥ 7 days. MEOWS completion and interventions are influenced by staff management, knowledge, vigilance and compliance, which can be improved by training and practical use of medical records.
