Abstract

Introduction: The prevalence of diabetes mellitus increases over time in line with the prevalence of diabetic retinopathy is also expected to increase. It is not possible to determine which diabetic patients will have retinopathy, so therefore screening is needed. This study aims to have a better understanding in managing DM patients in the future, especially in cases of diabetic retinopathy in primary, secondary & community health care centers. Methods: A descriptive analytical study. Data collected cross sectional study based of characteristic and portable photo fundus from DM patients while doing social service at a health service center. Results: Portable photo fundus in this study, screening 138 DM patients, with majority of subjects is in the range 44-64 year age group (68.1%). There was the same number of patient between gender or type. History of DM in the family (63.0%) with a duration of DM ≤ 5 years (56.5%), mostly the quality of photo fundus that can be assessed (97.1%), patients who needed further treatment were referred to the ophthalmologist 59 cases (42.8%). Conclusion: People who live far from cities and have limited access to health facilities can be reached using this portable photo fundus, so that blindness in peripheral areas will be more easily detected and this can be used as a screening modality and blindness due to diabetic retinopathy can be prevented.

Abstract

Introduction: To determine the difference in the quality of life and cognitive function in patients with and without AMD. Methods: This research is an observational analytic study, a prospective cross sectional study in the polyclinic of RSUP Sanglah, Denpasar. Result: Study samples’s total are 66 people selected by consecutive sampling with 33 people in the group with AMD, and 33 people in the group without AMD. The results of the study showed the average age of the group with AMD 68 years and the group without AMD 67 years old. Most of the male sex and residence in denpasar in both groups. Most AMD with the wet type by 60.6 %. On the measurement of quality of life, it showed decreasing in all subscale in the group with AMD compared to the group without AMD with total average 27.2 % and also decreasing in cognitive function measured by MMSE in the group of AMD compared to the group of non – AMD with 6.8%, even though the MMSE score of the groups are within normal limits. All these things stated significant differences (p<0.05) statistically. Conclusion: There are decreasing quality of life and cognitive function in AMD groupd compared to Non-AMD group which is needed more treatment and screening in the future.

Abstract

Introduction: Vascular endothelial growth factor (VEGF) plays a crucial role in the development of proliferative diabetic retinopathy. Elevated levels of VEGF in the vitreous have been found to be associated with the severity of ischemia and neovascularization, which can lead to a decline in visual acuity. This study aims to determine the association between vitreous VEGF levels and improvement in visual acuity before and after PPV in PDR patients. Methods: This research is an analytic observational study with a pre-post single group design. The subjects of this study were all PDR patients who received PPV therapy at three hospital in Bali Province, Indonesia. Consecutive sampling method were conducted. The independent variable is vitreous VEGF, whilst pre and post-PPV visual acuity is the outcomes. We performed mean comparison and multivariable statistical test using IBM SPSS version 25. Result: 45 people were included in this study. Improvement in visual acuity after PPV compared to before PPV with an average improvement of 0.54 logMAR (p=0.001). Based on the ANCOVA multivariate analysis, factors affecting visual acuity improvement after PPV were preoperative vision (p<0.001), postoperative vision (p<0.001), HbA1c level (p=0.036), and DM duration (p=0.024). There was no association between high vitreous VEGF levels and visual acuity improvement (PR=0.95; 95% CI=0.55-1.63;p=0.841). Conclusion: This study concluded that there is an association between PPV and visual acuity improvement. However, clinicians should be aware of several confounding factors that affect visual acuity improvement, including pre-PPV visual acuity, post-PPV visual acuity, duration of DM, and HbA1c level. There is no relationship between vitreous VEGF and visual acuity before and after PPV in PDR, but it is necessary to keep good records of lens status and intraocular pressure status. Further research is needed and the research time is extended to evaluate a better visual outcome.

Abstract

ABSTRACT / ABSTRAK Purpose of the Study: This research aimed to analyze legal considerations in malpractice cases carried out by ophthalmologists and analyze the differences between medical risk and medical malpractice Methodology: This research is normative legal research using the statute approach and case approach of medical malpractice cases. The research object discussed is the Supreme Court Judgment Number 277/PDT/2020/PT SBY and Supreme Court Judgment Number 338 K/Pdt/2020. The analysis technique implemented descriptive qualitative analysis. Results: The results of the research discovered that the judge's consideration in issuing nt in malpractice case by an ophthalmologist was inconsistent and defied the applicable laws and regulations, in which Articles 359 and 360 of the Criminal Code the judge should have imprisoned to the perpetrator. Apart from that, the judgment 277/PDT /2020/PT SBY is included in the medical malpractice action which is shown by the damages given to the patient and an apology from the doctor. Applications of this study: Research suggests the conformity of judgment with laws and punishment that should be given to doctors who committed medical malpractice from the proof and applicable laws and regulations. Novelty/Originality of this study: The study analyzed the judgments of medical malpractice cases through 6 court processes. Keywords: Medical Malpractice, Liability, Criminal Law   Abstrak / Abstrak  Tujuan Penelitian: Penelitian ini bertujuan untuk menganalisis pertimbangan hukum dalam kasus penyalahgunaan yang dilakukan oleh dokter mata dan untuk menganalisa perbedaan antara risiko medis dan penyimpangan medis Metodologi: Penelitian ini adalah penelitian hukum normatif menggunakan pendekatan hukum dan pendekatan kasus kasus kasus penyalahgunaan medis. Objek penelitian yang dibahas adalah Keputusan Mahkamah Agung Nomor 277/PDT/2020/PT SBY dan Keputusan Pengadilan Tinggi Nomor 338 K/Pdt/2020. Teknik analisis yang digunakan adalah analisis deskriptif kualitatif. Hasil penelitian menemukan bahwa pertimbangan hakim dalam mengeluarkan nt dalam kasus penyalahgunaan oleh seorang dokter mata tidak konsisten dan menantang hukum dan peraturan yang berlaku, di mana Pasal 359 dan 360 dari Kode Kriminal hakim harus menahan pelaku. Selain itu, putusan 277/PDT/2020/PT SBY termasuk dalam tindakan penyalahgunaan medis yang ditunjukkan dengan kerusakan yang dialami pasien dan maaf dari dokter. Aplikasi dari penelitian ini: Penelitian menunjukkan kesesuaian penilaian dengan hukum dan hukuman yang harus diberikan kepada dokter yang melakukan penyalahgunaan medis dari bukti dan hukum dan peraturan yang berlaku Novelty/Originality of this study: Studi ini menganalisis penilaian kasus penyalahgunaan medis melalui 6 proses pengadilan. Abstract

Introduction : Rhegmatogenous retinal detachment (RRD) is a common cause of visual impairments, with main treatment is a surgical reattachment. Persistent submacular fluid (SMF) can be found, even after ophthalmic confirmation that the retina has reattached and all retinal breaks have sealed. This condition can cause visual impairment and delay in visual recovery. Case Illustration : A 37-year-old female complained of blurred vision in her left eye for a week, with a history of LASIK four years ago in both eyes. On examination, left eye visual acuity (VA) was 6/9.5 with retinal detachment at 11 – 6 o’clock directions and break in 1 o’clock directions. The patient planned to undergo pars plana vitrectomy (PPV). The next day found the VA was 1/300. One month later, the VA was 6/6, but she felt glare and blurred when seeing a near object. Optical coherence tomography (OCT) was conducted and found a submacular fluid one month after the operation that persist two months after surgery. Discussion : Persistent SMF is mostly founds in RRD and treated by scleral buckle, rather than in patients treated with PPV. Other factors that can affect the incidence and delayed absorption is young age and macular status of retinal detachment. Uninvolved macular detachment shows no persistent SMF after PPV, a partly detached macula has a higher incidence of SMF than a detached macula after PPV. SMF is associated with poor initial visual outcome and visual improvement in its resolution. Conclusion : It remains unclear whether the final VA is affected by the persistent SMF or not.

Abstract

Introduction : Vitrectomy surgery affects zonular strength, which increases the risk of lens drop and can be sight- threatening if not managed correctly. Pars plana vitrectomy (PPV) and evacuation of dislocated lens fragments have to be done very carefully. Case Illustration : A 42-year-old female with blurred vision in the right eye for two months and a history of right-eye surgery three months ago due to proliferative diabetic retinopathy (PDR) was tamponed with SF6 gas. On examination was found visual acuity (VA) was 6/120, with cloudy lens and attached retina. The patient was diagnosed with a right-eye complicated cataract with advanced PDR post-PPV and planned to undergo phacoemulsification. While waiting for the surgery, the lens dropped into the posterior chamber, then planned to do the evacuation immediately. Dropped lens evacuation, PPV, retro-pupil IOL implantation, and silicone oil (SO) tamponing were done. Two months after the SO evacuation, the best corrected VA improved to 6/15. Discussion : Cataract progression after vitrectomy is common. In a vitrectomized eye, there is a lack of vitreous tamponade and a less stable lens. Peripheral zonules are attached to the pars plana, which can be disrupted during the vitrectomy surgery. Improvements in visual acuity and lower rates of complications were found if the lens fragment was evacuated soon. Conclusion : Cataract surgery is better done soon after PPV to prevent dropped lenses into the posterior chamber, whether the zonulisis sign is seen or not. The interval between the dropped lens incident and evacuation was associated with better final visual outcomes.

Abstract

Introduction : Vitreous Hemorrhage (VH) in proliferative diabetic retinopathy (PDR) is common cause of acute vision loss.If the retina cannot be visualized and the etiology of the VH is unknown, pars plana vitrectomy (PPV) is indicated. 1Intraocular gases like SF6 or C3F8 are one of the most useful adjuncts in vitreoretinal surgery. Exposure to intraocular gases additionally increases retrolental oxygen levels, resulting in development of lens opacities. Case Illustration : Patient female 52 y.o, came with sudden blurry vision in the left eye since two weeks ago. History of Diabetes Mellitus 10 years with insulin treatment. From the examination left eye visual acuity 1/60, zonulysis, VH, and negative fundus reflect. On ultrasonography found vitreous opacity. The patient was diagnosed with left eye VH ec advanced PDR and scheduled PPV with C3F8 gas tamponade. Patient difficult to maintain prone position after surgery. 2 month after the surgery the patient scheduled with phacoemulsification surgery because progressive lens opacity. While prepare to the second operation, the lens found drop to the vitreous. The second surgery done with lens evacuation and Intraocular lens implantation. Last follow up, best correction visual acuity was 6/18. Discussion : PPV and Gas tamponade are presdisposing lens opacity progression. Lens opacity after gas tamponade include longer intraocular longevity gas and patients who fail to maintain a prone position. Risk factors for zonulysis in this patient increase the occurrence of lens drop after PPV with gas tamponade. Conclusion : Regular slit-lamp examination needs to be performed for early detection of cataract after PPV with gas tamponade.

Abstract

Introduction : Retinal tears can be present in up to 2.4% of cases of lattice degeneration, which is considered an important risk of retinal detachment in a high-myopic patient. Laser photocoagulation has been shown to reduce the risks of retinal detachment before phacoemulsification thus improve in the visual outcome. We report a case of a high myopic patient with symptomatic retinal tears who underwent phacoemulsification that was treated prophylactically with barrage laser photocoagulation. Case Illustration : A 43 years-old female with a history of high myopia complained of flashes and floaters on both eyes. She had high myopia measuring -13.0 dioptres and -28.0 dioptres in the right and left eyes respectively. Unaided visual acuity was 2/60 in both eyes. Fundus examination showed lattice degenerations with a small tear found on both eyes with no retinal detachments. She was treated with a barrage laser on both eyes followed by phacoemulsification. On the last follow-up, the retina remained stable and the visual acuity was significantly improved with no development of macular edema or retinal detachment in both eyes. Discussion : Although most cases of lattice degeneration do not need intervention, some present a risk of retinal detachment. Barrage laser photocoagulation may have a place in high myopic patient lattice degeneration with symptomatic retinal tears to maintain retinal stability and to obtain the optimal outcome after phacoemulsification. Conclusion : Barrage laser photocoagulation is beneficial as a prophylactic treatment of symptomatic retinal tears before phacoemulsification in high-myopic patients.
