Abstract

Introduction: The Infant and young child feeding (IYCF) counselling program is an international strategy to improve IYCF practices in the community. This study aimed to explore the implementation of the program through the logic model approach. Methods: A qualitative method was employed for this evaluation study. Fifteen participants were selected by purposive random sampling, and data collection was conducted by in-depth interview, focus group discussion, observation, and document study. Data were processed from transcription, reduction, coding, categorizing to data interpretation and assessed by content analysis. Results: IYCF training is a cascading training and a combination of adult and fun learning that effectively produces a mixture of counsellors who can provide counselling using the counselling cards. However, the gap in the number of cadre counsellors and the target of counselling, as well as the limited time, low financial support, over workload, and inadequate supervision, resulted in poor recording and low numbers of clients who had been exposed to counselling. Nevertheless, this counselling has a positive effect on maternal knowledge and attitude. Still, the existence of traditional community practices, family and environmental factors are barriers to the implementation of proper IYCF practices. Conclusion: IYCF counselling effectively improved IYCF practices, so it has to be continued with strengthening the number of counsellors, recording and supervision activities. Changing strategy in counselling by involving other adult household members like grandmother or neighbours are explicitly targeted for more effective in improving the practices of infant and young child feeding.

Abstract

The Community Satisfaction Survey is mandatory based on Ministry of Administrative and Bureaucratic Reform Regulation number 14 in the year of 2017. An alternative measurement tool has been developed by expanding the measurement items. The research objective was to explore the validity and reliability of public satisfaction instruments in health institutions. The design was non-experimental, with a quantitative and cross sectional approach. The research was carried out in two entities, namely primary health care and hospital. Data collection using the IKM-29 questionnaire. Data processing with Rasch modeling and winsteps application. The results showed that the item reliability at PHC and hospital was 0.95 and 0.97 with Cronbach's alpha 0.95. The separation is more than 3 and the raw variance is more than 40%. In the two entities there are several different misfit items. Item 8 related to the perception of waiting time has a high degree of difficulty and is paradoxical. The results of the analysis show that the instrument is valid and reliable enough to be used as an alternative measurement of community satisfaction in PHC and Hospitals.

Abstract

A high prevalence chronic energy deficiency (CED) in pregnant women exist in Indonesia, which approximately 16.2%. The government had made various efforts to eliminate CED in pregnant women. Therefore, our study aimed to analyze the efficacy of eel cookies on improving nutritional status of pregnant women with CED risk. An experiment with a pre-post design of 36 pregnant women with risk of CED in Ciletuh, Sukabumi was carried out for 30 days. Pregnant women in intervention group were given eel cookies, while control group were given cookies without eel. At the beginning and end of the study, MUAC were measured and food intake was assessed through a 24 hour recall. The research data was analysed using t-test, Mann-Whitney test and chi square. Significant difference in body weight between intervention and control groups was not found (p > 0.05). The mean bodyweight in intervention group after intervention (48.5) was higher than in control group (48.3). The Mean bodyweight and MUAC in the two groups before and after intervention also did not show a significant difference  (p> 0.05). The risk of CED in pregnant women who received eel cookies were lower compared to the control group. Consumption of eel cookies can reduce CED risk in pregnant women by 35%, while cookies without eel only reduce the risk by 29.4%. Eel cookies had higher efficacy compared to cookies without eel on reducing CED risk in pregnant women.

Abstract

Background: Perceptions of patient satisfaction influence the quality of the public health center (Pusat Kesehatan Masyarakat, Puskesmas) services. This study aimed to determine the differences in the level of patient satisfaction with national health insurance and the accreditation status of Puskesmas. Methods: This quantitative analytical study using a cross-sectional method was conducted at four Puskesmas in Majalengka Regency from October to November 2021 using a purposive sampling technique. The instrument used was the community satisfaction instrument (IKM-29) questionnaire , with the variable measured being the level of satisfaction. Data was transformed into numerical form using Rasch modelling and analyzed using the Chi-Square, independent-t, and one-way ANOVA tests. Results: A total of 273 respondents consisted of insured (Jaminan Kesehatan Nasional, JKN) and non-insured (non-JKN) respondents, resulting in the satisfaction mean value between JKN and non-JKN patients of 2.50 and 2.51 (p-value= 0.926). Satisfaction at Puskesmas levels 1 (Dasar), 2 (Madya), 3 (Utama), and 4 (Paripurna) was 2.15, 2.23, 2.56, 3.03, respectively (p-value = 0.002), indicating an increase in the level of satisfaction at the Puskesmas accreditation level. Conclusion: There is no difference in satisfaction between respondents using JKN and non-JKN. However, there is an increase in satisfaction related to the Puskesmas accreditation level. It is recommended for each Puskesmas to maintain the same service to all patients and improve the quality of service, especially in service dimensions that are considered inferior. Additionally, it is necessary to review the minimum standard value for accreditation programs.

Abstract

Background: Stunting remains a critical public health issue in Indonesia, threatening the well-being of future generations. Community involvement, particularly by health cadres, is essential for effective prevention. However, structured assessments and follow-up initiatives to enhance their participation remain limited. Objective: This study aimed to evaluate health cadres' knowledge, roles, and skills in stunting prevention programs. Methods: A mixed-methods design with an embedded approach was applied in three phases. Phase 1: A quantitative survey assessed stunting knowledge among 193 cadres across 10 stunting locus villages in Bangka Belitung Province, Indonesia (October–December 2022). The survey tool was adapted from previous studies and analyzed using the RASCH IRT model and descriptive statistics. Phase 2: Qualitative data were collected through semi-structured interviews with 20 cadres selected from the survey pool, and analyzed thematically. Phase 3: The same 20 cadres were observed conducting anthropometric measurements to assess skill accuracy. Results: Knowledge levels were generally low, with 52% of cadres scoring low to very low and only 48% achieving moderate to high scores. Three themes emerged: 1) cadres’ perspectives on the causes and consequences of stunting, 2) their roles and actions in stunting prevention, and 3) their experiences during the COVID-19 pandemic. Observations revealed significant inaccuracies in anthropometric measurement techniques. Conclusion: There is a clear need to improve cadres’ knowledge and skills, especially in anthropometric assessment. Recommendations include implementing certification-based training, introducing innovative and standardized measurement tools, and offering accessible web-based learning platforms. Nurses play a crucial role in supporting cadre training and ensuring accurate growth monitoring at the community level.
