Abstract

UMKM Kripik Tempe RICO merupakan salah satu UMKM kripik tempe terbesar yang berada di Desa Sadang, Kabupaten Ngawi. Dalam proses produksinya seringkali terjadi kecelakaan kerja. Diketahui penyebab kecelakaan kerja berasal dari kelalaian manusia Human Error seperti pekerja yang lalai dalam mengendalikan mesin, terburu-buru dalam bekerja, dan tidak menggunakan APD secara lengkap. Tujuan pada penelitian ini yaitu Menganalisis Human Error, menentukan tingkat risiko proses produksi UMKM Kripik Tempe RICO serta memberikan rekomendasi perbaikan sistem dengan metode SHERPA untuk mengurangi kecelakaan kerja pada kegiatan yang disebabkan oleh Human Error. Berdasarkan SHERPA Error Mode teridentifikasi human error pada tipe action error sebanyak 21 error atau sama dengan 49%, tipe checking error sebanyak 5 error atau sama dengan 12%, tipe retrieval error sebanyak 9 error atau sama dengan 21%; tipe communication error sebanyak 6 error atau 14%, tipe selection error sebanyak 2 error atau sama dengan 5%.

Abstract

Program pengabdian kepada masyarakat ini dilaksanakan untuk meningkatkan kesadaran kesiapsiagaan bencana pada siswa dan guru sekolah menengah kejuruan melalui pembelajaran imersif berbasis Virtual Reality (VR). Kegiatan ini dilatarbelakangi oleh keterbatasan edukasi kebencanaan yang umumnya masih menggunakan metode ceramah konvensional sehingga kurang efektif dalam membangun kesiapan nyata. Sebanyak 30 peserta, yang terdiri dari siswa dan guru SMK Negeri 1 Sidoarjo, mengikuti kegiatan ini. Rangkaian kegiatan meliputi sosialisasi, pelatihan, serta praktik langsung menggunakan simulasi VR dengan skenario tanggap darurat gempa bumi, mencakup prosedur evakuasi dan protokol keselamatan. Untuk mengukur efektivitas, dilakukan evaluasi melalui pre-test dan post-test. Hasil menunjukkan adanya peningkatan pemahaman peserta sebesar 89% terhadap sistem tanggap darurat gempa bumi setelah mengikuti simulasi berbasis VR. Program ini membuktikan bahwa teknologi imersif efektif dalam meningkatkan kualitas pendidikan kebencanaan serta mendukung pencapaian Sustainable Development Goal tentang Good Health and Well-Being, khususnya dalam upaya meningkatkan keselamatan, resiliensi, dan tindakan preventif di lingkungan pendidikan vokasi.

Abstract

Industrial Revolution 4.0 increases the demand for batteries but also increases the risk of workplace accidents in factories such as PT TMB. Production processes involving chemicals and heavy equipment cause potential hazards. In the last three years, accidents have increased, disrupting production and adding costs. Therefore, this research aims to identify risks and provide risk mitigation in the battery production process. This research uses Fuzzy Fault Tree Analysis (FFTA) to identify hazards and reduce risks, in order to improve the safety and competitiveness of the company. This research was conducted at PT TMB by combining qualitative analysis to understand general conditions and quantitative to analyze the risk of workplace accidents, such as fire, fatigue, operator injury, and worker health. The Fuzzy Fault Tree Analysis (FFTA) method was used to map and reduce potential hazards, as well as develop more effective work safety policies. The results of the research explained that there are four top events with different probabilities and impacts: fire risk (A) 26.82%, occupational fatigue (B) 11.57%, occupational injury (C) 40.58%, and occupational health (D) 21.03%. From the four top events mentioned above, mitigation is focused on the 3 highest positions based on the assessment of the matrix table, which are fire risk, occupational injury, and occupational health.

Abstract

This study aims to evaluate the usability level of the Virtual Reality (VR) simulation system for earthquake preparedness training at the Environmental Agency of East Java Province using the System Usability Scale (SUS) method. The testing was conducted with nine respondents who performed two task scenarios in the simulation and subsequently completed the SUS questionnaire. The analysis covered three main aspects of usability: effectiveness, efficiency, and user satisfaction. The results showed an effectiveness level of 100% (“Highly Effective” category), an average efficiency of 0.01758 goals/second (“Very Fast” category), and an average SUS score of 73.88, which falls into the “Acceptable” category with an adjective rating of “Good.” These findings demonstrate that the VR simulation has strategic potential as an effective and efficient interactive training medium for improving the understanding of earthquake evacuation procedures. Nevertheless, improvements are needed in interface consistency and in reducing the initial learning curve to optimize the user experience.

Abstract

The benchmark for the success of a business or product is to measure the satisfaction of customers who have used or felt the goods or services. The customer's expectations of the function and emotional happiness factors that arise from the use of goods or services are entirely the responsibility of the producer. So that manufacturers need to focus on what attributes must be presented in a product or service to increase customer satisfaction. The decrease in customer interest in providing vehicle service at an automotive company in Sidoarjo is the problem studied in this study, to analyze the level of customer satisfaction and attributes that are priority improvements as recommendations for the company. Data generated from customers is processed and analyzed using the Service Performance (SERVPERF) and Lean Service methods. Based on the results of data processing, it can be concluded that the level of customer satisfaction with service quality is 82.3%, and three service attributes are classified as a top priority and two types of critical waste.

Abstract

General Background: Manual handling in warehouse picking involves repetitive activities and non-ergonomic postures that can place physical burdens on workers and potentially cause Musculoskeletal Disorders (MSDs). Specific Background: Picking activities include retrieving goods from storage shelves, moving goods to pallets, carrying hand pallets to the staging area, and repacking, with repeated bending, reaching, and squatting postures. Knowledge Gap: Previous studies have generally applied the Loading on the Upper Body Assessment (LUBA) and Ovako Working Posture Analysis System (OWAS) separately, while studies combining both methods for warehouse picking with frequent manual handling remain limited. Aims: This study analyzes work postures using LUBA and OWAS and formulates ergonomic recommendations for manual handling activities. Results: LUBA scores were 13 for retrieving goods from shelves, 12 for moving goods to pallets, 9 for carrying a hand pallet to the staging area, and 11 for repacking. The first, second, and fourth activities were classified as Level III, while hand-pallet movement was Level II. OWAS classified shelf retrieval and hand-pallet movement as action category 2, whereas pallet movement and repacking were action category 3. Nordic Body Map results also showed that eight of 16 workers were in the moderate-risk category and eight were in the high-risk category. Novelty: The study combines LUBA and OWAS to examine upper-body postural loading and overall working posture within warehouse picking activities. Implications: The findings support prioritizing ergonomic corrective actions for activities with higher postural risk and developing recommendations based on LUBA, OWAS, and worker complaints. Highlights: Three manual-handling activities reached LUBA Level III. Pallet movement and repacking reached OWAS action category 3. Abstract

PT XYZ is a company engaged in warehousing. In every process, there are various activities that can cause fatigue in workers. Currently, researchers want to analyze the effect of body condition, material, and warehouse conditions on the fatigue level of warehouse workers. To do this, companies can use multiple linear regression tests with SPSS software. In the multiple linear regression test that has been carried out, it is known that the body condition and material factors do not have a strong influence on the level of fatigue that occurs. This is because the Pvalue >0.05. While the warehouse condition factor has a strong influence on the level of fatigue, H0 is rejected because it has a Pvalue of 0.015 <0.05. From the results of the anova test conducted, it can be seen that the three existing factors simultaneously affect the fatigue level of PT XYZ workers. This is because the significance value in the anova test is 0.004; smaller than the value of 0.005. The benefit of this research is that PT XYZ can determine the right solution in order to reduce the level of fatigue so that warehouse productivity increases.

Abstract

This study presents the development of a virtual reality-based simulation aimed at improving earthquake disaster preparedness at the East Java Provincial Environmental Service Building. The simulation introduces proper emergency response procedures, realistic earthquake scenarios, and comprehensive knowledge of building layouts, evacuation route signs, and designated assembly points. The research methodology involved direct observations and interviews with employees to gather detailed information about existing preparedness measures. The simulation was developed through stages of problem identification, data collection, object modeling using Blender 3D, and integration into a cohesive system via Unity. The final implementation resulted in a dynamic, interactive, and educational simulation that closely mirrors real-life emergency situations. Testing results indicate that the simulation operates effectively, providing immersive experiences and facilitating user interaction. This tool is expected to enhance the understanding of emergency procedures for employees, particularly new staff and visitors, while fostering a safer, disaster-prepared work environment. The study underscores the potential of virtual reality technology as a strategic approach to disaster risk reduction and education.

Abstract

PT Lamipak Primula Indonesia is a company that produces various packaging made of plastic. The main product of PT Lamipak Primula Indonesia is toothpaste laminated tube packaging. PT Lamipak Primula Indonesia wants to minimize defects in products to minimize waste, reduce costs, increase efficiency, and maximize profits. This can also increase customer confidence in the product. PT Lamipak Primula Indonesia improves the quality of its products by applying Six Sigma with the DMAIC method. In this study, DPMO of 15766 was obtained and then converted to a sigma level of 3.7 which shows that the sigma level is below the 6 sigma level. This shows the achievement of sigma that has not been consistent and still shows the need for quality improvement in the laminated tube packaging production process in order to achieve zero defects. Based on FMEA analysis, it shows that the most significant failure occurs in machine conditions that cause defective shoulders with a value RPN of 210. This failure is caused by the lack of regular machine inspection, by the machine not being supervised in realtime, not following the SOP and the lack of worker skills.

Abstract

The identification of work accidents in the steel plate production process poses a serious risk to workers and disrupts operational activities. This research analyzes the factors causing work accidents at PT XYZ in the steel plate production process using the Fault Tree Analysis (FTA) method and the Plan-Do-Check-Act (PDCA) method. FTA is applied to identify and decipher the root causes of accidents, while PDCA serves as a framework for continuous improvement. Data were collected through accident reports, observations. The results showed that human error, including being pinched by a plate, hit by a ganco, and exposed to sparks were the main contributors to the occurrence of accidents. Through the PDCA cycle, several preventive measures were proposed, including enhancing worker training, improving equipment maintenance, and strengthening safety protocols. This study provides actionable insights to improve workplace safety and reduce the risk of accidents in the steel plate production process.
