Abstract

The maternal mortality rate (MMR) in the world based on data from the World Health Organization, according to the World Health Organization (WHO), in 2016 reached 289,000 people, divided into several countries, including the United States reaching 9,300 people, North Africa 179,000 people and Southeast Asia 16,000 soul. Approximately 80% of maternal deaths are the result of increased complications during pregnancy, childbirth and after delivery. Causes of maternal death include postpartum hemorrhage, eclampsia, prolonged labor and sepsis. One of the causes of maternal death is prolonged labor with an average number in the world of 8% and in Indonesia it is 9%. The general aim of this research is to determine the factors associated with the incidence of prolonged labor in women giving birth at the Djoelham Binjai Regional Hospital in 2023. Prolonged labor is a situation where the latent phase occurs for more than 8 hours or labor lasts more than 12 hours with the baby not yet being born. On a partograph, labor that is not progressing is indicated by cervical dilatation above the alert line (right side) in the active phase. The pathophysiology of prolonged labor is closely related to the concept of functional stages of labor which explains the physiological goals of labor. There are three functional stages of labor, namely the preparatory division, the opening/dilatation stage and the pelvic division. This research uses a quantitative type of research using retrospective research. The design of this research was carried out using a cross sectional approach. The sample in the study was women giving birth who experienced a long labor at Djoelham Binjai Regional Hospital in 2023. The proportion of respondents who had a long labor due to a weak vagina < 20' was 12 (40%), respondents who had a long labor due to CPD with In the No CPD category, there were 13 (43.3%) respondents who did not give birth for a long time due to their weight, with the weight category > 4000 grams being 12 (40%). The results (p value = 0.030 < 0.05) which means that this research shows that there is a significant relationship between His, CPD and Baby's Weight with Long Parturition at Djoelham Binjai Regional Hospital. The strength and weakness of the His during the birth process greatly influences the speed or Length of labor It is hoped that respondents will be more active in checking their pregnancies to avoid prolonged labor by health workers either at posyandu or other health service places. Abstract

Background: Optimal breast milk production meets infants’ nutritional needs, crucial in stunting prevention. Many postpartum mothers face insufficient milk production, which can affect infant weight gain and increase the risk of stunting. The innovation of combining oxytocin massage with Foxy Oil is expected to enhance breast milk volume, frequency of wet diapers, and infant weight gain—key indicators in stunting prevention. Methods: This study used a quasi-experimental design with purposive sampling, involving 60 postpartum mothers: 30 in the intervention group and 30 in the control group. Statistical analyses were conducted using SPSS 26 for univariate and bivariate data. Data normality was tested using the Kolmogorov-Smirnov test and bivariate analysis with the Mann-Whitney test. Breastfeeding optimisation was assessed based on breast milk volume, number of wet diapers, and infant weight over one month in the working area of Tanjung Morawa Community Health Centre. The husband or a close family member performed an Oxytocin massage twice daily for 10–15 minutes. Results: Bivariate analysis after oxytocin massage showed significant effects on breast milk volume (p = 0.000), number of wet diapers (p = 0.000), and infant weight (p = 0.039). The analysis indicates that the combination of Foxy Oil and oxytocin massage positively affects breast milk production, frequency of wet diapers, and infant weight gain. Conclusion: Foxy Oil's innovation, combined with oxytocin massage, can be widely applied in midwifery practice to support successful exclusive breastfeeding and contribute to stunting prevention.

Abstract

Background: Anemia in pregnancy remains a major cause of maternal complications, low birth weight, and stunting. Although iron–folic acid supplementation is widely implemented, there is a lack of culturally appropriate, food-based interventions using locally available ingredients that can sustainably improve hemoglobin levels in low-resource settings. This gap highlights the need for innovative, community-adaptable nutritional interventions that complement existing programs. The objective was to evaluate the effectiveness of Pisbro cookies (banana–broccoli cookies) in improving hemoglobin levels among pregnant women with anemia. Methods: This study used a quasi-experimental two-group pretest–posttest design and followed the TREND reporting guideline. A purposive sampling method was used to recruit 40 pregnant women with mild–moderate anemia from the Sentosa Baru Health Centre, Medan. Inclusion criteria included: (1) confirmed anemia based on Hb examination, (2) gestational age 12–36 weeks, (3) willingness to participate for 30 days, (4) adherence to Fe tablet consumption. Exclusion criteria included pregnancy complications, chronic illness, allergies to ingredients, and incomplete follow-up. The intervention group received 5 Pisbro cookies/day for 30 days, while the control group received routine antenatal care without additional intervention. Hemoglobin levels were measured at baseline and after 30 days using a calibrated hemoglobin meter. Data were analyzed using an independent t-test at α = 0.05. Results: The intervention group showed an increase in mean hemoglobin levels from 9.56 ± 0.48 g/dL to 9.79 ± 0.51 g/dL, while the control group decreased from 9.62 ± 0.47 g/dL to 9.40 ± 0.50 g/dL. The difference between groups was statistically significant (p = 0.019), indicating a positive effect of Pisbro cookies on maternal hemoglobin levels. Conclusion: Pisbro cookies were effective in improving hemoglobin levels among pregnant women with anemia. As a low-cost, culturally acceptable food-based intervention using locally available ingredients, Pisbro has potential to complement maternal nutrition programs and support anemia and stunting prevention efforts.

Abstract

Abstract

Pregnancy-related low back pain (LBP) is prevalent in late gestation and can negatively affect physical comfort and psychological well-being. Non-pharmacological interventions such as antenatal yoga are increasingly incorporated into routine care; however, pragmatic evidence from community-based settings remains limited. This study examined within-subject changes in LBP severity among third-trimester pregnant women participating in a structured antenatal-yoga program delivered at a community midwifery clinic. Researchers conducted a single-group pre-experimental pretest-posttest study at an independent midwifery clinic in Gunungsitoli, Indonesia. Forty pregnant women ≥28 weeks' gestation were recruited through purposive sampling. The intervention consisted of supervised prenatal-yoga sessions held three times per week over four weeks. Pain intensity was assessed before and after the program using the Numeric Rating Scale (NRS) and categorical severity bands (mild, moderate, severe). Pre-post differences were analysed using the Wilcoxon signed-rank test. Participation in antenatal yoga was associated with a clinically meaningful reduction in pain severity. Prior to the intervention, 62.5% of participants reported moderate pain and 37.5% reported mild pain; no cases of severe pain were observed. Following the intervention, 82.5% reported mild pain and 17.5% reported moderate pain. The change in NRS scores was statistically significant (p < 0.001). In this pragmatic third-trimester cohort, engagement in a structured antenatal yoga program was linked to reduced LBP severity and a shift toward milder pain categories. These findings support the feasibility of integrating antenatal yoga into routine prenatal care as a non-pharmacological option. Future controlled studies incorporating effect sizes, confidence intervals, functional outcomes, adherence andsafety monitoring, and short-term follow-up are recommended to strengthen casual inference and inform optimal program design. Abstract

Nutritional problems among primary school children remain a significant public health concern, particularly in the context of the dual burden of malnutrition. This study aimed to examine changes in students’ nutritional knowledge following a school-based healthy plate education intervention. A quasi-experimental pre–post design without a control group was implemented among 63 students from grades IV to VI. The intervention integrated visual media, interactive instruction, and practical demonstrations based on the “My Plate” concept. Knowledge levels were assessed using structured pre-test and post-test instruments and analyzed using the Wilcoxon Signed-Rank Test. The results showed a substantial shift in knowledge distribution, with the proportion of students in the “good” category increasing from 23.8% to 71.4%. Statistical analysis revealed a significant difference between pre-test and post-test scores (Z = -5.543, p < 0.001), with a large effect size (r = 0.69). No participant experienced a decrease in score after the intervention. These findings indicate that school-based interactive nutrition education incorporating visual and experiential components can effectively enhance primary school students’ nutritional knowledge. Further longitudinal studies are recommended to assess the sustainability of behavioral changes.
