Abstract

Chronic noncommunicable diseases (NCDs), particularly hypertension and type 2 diabetes mellitus, are increasing in Indonesia’s coastal communities, including the Bajo population of Konawe. Limited healthcare access, cultural barriers, and low preventive health engagement exacerbate disease burden. Culturally tailored interventions are needed to enhance self-awareness and improve chronic disease management. This study aimed to develop and validate a culturally sensitive care model for chronic disease management in the Bajo coastal community using a modified Delphi approach. A three-round modified Delphi method was conducted with 26 panelists representing health professionals, community leaders, policymakers, and family members, recruited through Community Health Centers and community health worker networks. The first round gathered qualitative input on cultural values, traditional practices, barriers, and care model components. The second and third rounds quantitatively assessed proposed components for consensus, defined as ≥75% agreement with ratings ≥4 on a 5-point Likert scale. Eleven of twelve proposed components achieved full consensus. High-priority elements included integration of traditional and biomedical care, family and community involvement, culturally relevant health education, flexible service schedules, and routine free health checks. The panel emphasized communication in local language, engagement of trusted local figures, and culturally competent health workers. One component, the use of local broadcasting media, was rejected because it was perceived as having low feasibility. This study presents a culturally sensitive care framework that operationalizes cultural integration through traditional-biomedical collaboration, family engagement, culturally adapted education, and community-based screening for Indigenous and remote settings. Future studies should prioritize pilot implementation and evaluation of feasibility, acceptability, and health outcomes in coastal settings.

Abstract

Introduction: The increasing number of teenagers who are obese has become a public concern, considering the impact of obesity on physical and psychosocial health. The aim of this research is to investigate the relationship fast food consumption habits and physical activity with incident of obesity among urban high school students.Method: This study was analytical correlation design with a cross sectional approach. The sample in this study was 78 students at SMAN 1 Konawe grades X and XI selected using Stratified Random Sampling techniques. The Food Frequency Questionnaire, the Physical Activity Level, and body mass index were the instrument used in this study. The statistical test used is Pearson chi-square.Results: Most of the respondents had a habit of consuming excessive fast food (59.0%) and had low physical activity (50.0 %). There was a significant relationship between fast food, physical activity and obesity (p = 0.000 respectively). Conclusions: Fast food consumption habits and physical activity are significantly related to the incidence of obesity in urban high school students. It is necessary to carry out obesity prevention and control programs involving various sectors in society, including educational institutions.

Abstract

Latar Belakang: Gagal ginjal kronik merupakan penyakit progresif yang membutuhkan hemodialisis jangka panjang dan dapat berdampak pada kondisi fisik, psikologis, serta sosial pasien sehingga menurunkan kualitas hidup. Self-efficacy merupakan salah satu faktor yang berperan dalam membantu pasien mengelola penyakit dan menjalani perawatan. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan self-efficacy dengan kualitas hidup pada pasien gagal ginjal kronik yang menjalani hemodialisis di RSUD Bahteramas Provinsi Sulawesi Tenggara. Metode: Penelitian menggunakan desain analitik dengan pendekatan cross-sectional. Sampel berjumlah 126 responden yang dipilih menggunakan teknik purposive sampling. Kriteria inklusi meliputi pasien gagal ginjal kronik yang menjalani hemodialisis 1-3 kali per minggu, telah menjalani hemodialisis <1 sampai >3 tahun, memiliki kesadaran penuh, bersedia menjadi responden, serta kooperatif dan mampu berkomunikasi dengan baik. Self-efficacy diukur menggunakan Chronic Kidney Disease Self-efficacy Scale, sedangkan kualitas hidup menggunakan Kidney Disease Quality of Life Short Form. Analisis data menggunakan uji Chi-Square. Hasil: Sebagian besar responden memiliki self-efficacy kategori sedang (51,6%) dan kualitas hidup kategori sedang (49,2%). Uji Chi-Square menunjukkan terdapat hubungan yang signifikan antara self-efficacy dengan kualitas hidup (p=0,001). Nilai Cramer's V sebesar 0,718 menunjukkan hubungan yang kuat. Kesimpulan: Terdapat hubungan yang signifikan antara self-efficacy dengan kualitas hidup pasien gagal ginjal kronik yang menjalani hemodialisis di RSUD Bahteramas Provinsi Sulawesi Tenggara. Peningkatan self-efficacy melalui edukasi, motivasi, dan konseling dapat menjadi bagian dari upaya keperawatan untuk mendukung kualitas hidup pasien.
