Abstract

Introduction: Health degree influenced by several factors and the offspring of the Ministry of Health and behavior and the environment. The behavior and environmental factors have contributed most in improving the degree of community health. The purpose of this research is to prove and analyzing the influence of the characteristics of the individual, self efficacy and team work against the commitment and productivity of health cadres. Methods: This research method using analytically research with explanatory research approach. Samples in this research totaled 200 respondents. The data was taken using simple random sampling. The collection of data use of the questionnaire, interview and observation. Data analysis was used the statistical test Partial Least Square (PLS). Result: Research results show the influence of the productivity variable health cadres of economic investment health is of 0,291 with the value of T-statistic of 4,645, where the value of the T-statistic was greater than 1.96 which means the productivity of health cadres significant effect of economic investment health, this shows the influence of the productivity of health cadre of economic investment health was positive, it means that when the productivity of health cadres are higher then the economic investment will also increasingly good health. Discussion: This research can serve as a reference for the district government of Tuban in increasing the degree of community health. Improving the health of the community must be done via promotif efforts and preventive measures through productivity program health cadres. The productivity of health cadres have an important role in efforts to improve the health of the community so that the government can save the funds for health or economic investment of health.

Abstract

Introduction: The tendency of non-communicable diseases is chronic, with the impact of disability including permanent disability. The purpose of this study is to analyze the determinants of the risk of father's death due to non-communicable diseases in families in Semanding District, Tuban Regency. Methods: This type of research is observational analytic with case control design. Using cluster sampling, 437 samples were taken. In the second stage, the sample was taken by determining the highest non-communicable diseases in the family, 0.82%, then the sample size was 182 with a case group of 82 (the highest non-communicable diseases) and a control of 100 (the father’s non-communicable diseases risk). Results: Data collection was done by interview. OR = 5,863 95% CI 3,087-11,136) with, consumption of foods high in excess salt (OR = 7,653 CI 95 % 3,951-14,821), consumption of food and drink high in excess sugar (OR 5, 582 CI 95% 2.9478-10.57) smoking (OR = 4.849 CI 95% 2.545-9.238).Conclusion: The health classification of respondents, namely the father as the head of the family suffering from non-communicable diseases, as the highest cause of death in the group of fathers at risk is smoking. The highest sufferers of non-communicable diseases is as the head of the family in the district. The highest in Kab.Tuban in 2018 is primary hypertension. This triggers the risk of death of the father because of the highest non-communicable diseases in families in the district. Notable in Kab. Tuban in 2018 is the lack of consumption of fruits and vegetables, consumption of foods high in excess salt, consumption of foods / drinks high in excess sugar and smoking.

Abstract

Introduction: The success of breastfeeding is a critical indicator of comprehensive adaptation during the maternal transition. Existing theoretical models address maternal adaptation separately, resulting in a lack of integration in predicting breastfeeding success. This research aimed to develop and validate the Maternal Role Adaptation System Model (MRASM) using Structural Equation Modeling. Methods: A cross-sectional study was conducted with 150 postpartum women recruited through purposive sampling from five community health centers in East Java, Indonesia. Eight latent constructs measured using validated instruments: focal stimuli (breast pain, fatigue, nutrition), contextual stimuli (spousal and healthcare support, information access, economic status), residual stimuli (social norms, cultural values, beliefs), coping mechanisms, emotional responses, self-concept, role function, and breastfeeding success. Data were analyzed using Structural Equation Modeling with Partial Least Squares (SEM-PLS) via Smart-PLS 4.0. Results: The MRASM demonstrated excellent psychometric properties (Cronbach's α > 0.678, composite reliability > 0.802, AVE > 0.508). Environmental stimuli (focal, contextual, and residual) significantly influenced maternal coping mechanisms, which in turn affected emotional responses, self-concept, and role function. Emotional responses (β = 0.356, p = 0.001) and self-concept (β = 0.268, p<0.001) are the strongest predictors of breastfeeding success, explaining 26.7% of outcome variance. These findings provide healthcare professionals with specific intervention targets: managing physical symptoms, enhancing coping resources, and supporting maternal emotional well-being and self-efficacy. Conclusions: MRASM integrates Roy's and Mercer's models to address postpartum adaptation holistically, providing healthcare professionals with an evidence-based framework to support maternal role transition and breastfeeding success.

Abstract

Introduction: This study aimed to identify the determinants associated with increased scabies severity among students in pesantren in Indonesia. Methods: A cross-sectional study was conducted to identify the determinants of increasing scabies severity among clinically confirmed cases in Islamic boarding schools, involving 120 students from six schools in Lamongan and Tuban. Data were collected using a modified and validated questionnaire, and nutritional status was assessed using body mass index (BMI). All participants were clinically diagnosed with scabies. Severity was categorized as mild, moderate, or severe based on the number of lesions and clinical manifestations. All instruments underwent content validity and reliability testing (Cronbach’s alpha ≥ 0.70), and ordinal logistic regression (proportional odds model) was applied to identify predictors of increasing scabies severity (p < 0.05) using SPSS version 25. Results: Frequent sharing of personal items was the strongest predictor of greater severity. Students who sometimes shared items had odds ratio (OR) of 3.01 (95% confidence interval [CI]: 1.45–6.60; p = 0.003), while frequent sharing increased the odds to 9.06-fold (95% CI: 3.00–27.40; p < 0.001). Good personal hygiene markedly reduced severity (OR = 0.04; 95% CI: 0.01–0.25; p = 0.002). Adequate residential density and longer duration of stay were also protective factors. Nutritional status and general knowledge were not consistently associated with severity. Conclusions: Scabies severity in pesantren is primarily driven by modifiable behavioral and environmental factors. Reducing shared-use practices and strengthening hygiene interventions may substantially mitigate disease severity in communal boarding school settings.
