Abstract

Work demands affect employee compliance in the work environment. The varying age of employees also influence employee compliance in the workplace. The appropriate rewards for employees from management/owners also play a role in employee compliance. With good employee compliance, the company/clinic can meet its business objectives effectively. The aim of this research is to determine and analyze the effect between work demands and age on employee compliance, mediated by rewards as the mediating variable. The research conducted at clinic X in Cirebon Regency with a population of 40 people. The sampling technique used is total sampling with inclusion and exclusion criteria, resulting in a sample size of 28 respondents. This type of research is observational quantitative research. Data analysis using linear regression and mediation testing with prior validity and reliability tests. The results of the study show a significant effect of work demands on employee compliance, however there is no significant effect between work demands and age on employee compliance with reward as a mediating variable.The importance of management implementing communicative and participative leadership, fair and transparent rewards and team based work programs.

Abstract

Digital transformation has become a crucial element in the development of hospital strategic management in the modern era. Hospitals are no longer required solely to provide clinical services but must also manage complex organizations efficiently through the use of digital technologies. This study aims to describe the concept of hospital strategic management transformation in the digital era, identify emerging forms of conceptual innovation, and analyze how these innovations are implemented in operational practices. The research employs a Systematic Literature Review (SLR) method with a qualitative descriptive approach, reviewing relevant scientific publications from the last five years. The findings indicate that conceptual innovations such as the digitalization of clinical and administrative processes, the use of data analytics, and digitally integrated service models significantly enhance operational efficiency, service quality, and evidence-based decision making. However, the implementation process faces several challenges, including limited technological infrastructure, insufficient human resource competencies, organizational resistance to change, and data security issues. The study concludes that successful digital transformation depends on the alignment between strategic vision, technological capabilities, and adaptive organizational culture. Therefore, hospitals are recommended to strengthen organizational readiness through continuous human resource development, improved digital infrastructure, and ongoing performance monitoring and evaluation.

Abstract

Introduction: Hospitals in Indonesia face complex challenges in the digital era, demanding strategic management transformation to improve service quality and operational efficiency. This study aims to examine various aspects of hospital strategic management based on current literature. Methods: Through a systematic literature review approach of 12 open access scientific journal articles published between 2023-2025, a thematic analysis was conducted to identify patterns, trends, and gaps in the implementation of strategic management in Indonesian hospitals. Discussion: The principal findings highlight five key areas of strategic management: digital transformation, policy integration, operational efficiency, human resource development, and service quality improvement. Digital transformation is the primary catalyst for change, while innovation in human resource management and the implementation of Lean Hospital Management optimize operational efficiency. Integration of strategic policies has been proven to improve the quality of services and ultimately increase the competitiveness of hospitals. Conclusion: Strategic management is an essential foundation for hospitals navigating the complexities of the contemporary healthcare environment. Effective strategic management implementation has been shown to positively correlate with improved service quality, organizational competitiveness, and financial sustainability. Strategic hospital management requires a holistic approach that integrates technology, human resources, and operational policies to achieve high-quality and sustainable healthcare.

Abstract

Electronic Medical Records (EMR)constitute a strategic infrastructure in the digital transformation of healthcare services. Although they offer improvements in quality, efficiency, and patient safety, EMR implementation often encounters significant resistance from healthcare professionals. This resistance is not merely a technical barrier, but rather a strategic failure that threatens return on investment and the sustainability of digital transformation initiatives. This study employs a systematic literature review with a narrative (meta-synthesis) approach. A total of 10 internationally reputable scientific articles published between 2019 and 2024 were critically analyzed. The articles were sourced from the Scopus, PubMed, and ScienceDirect databases. Thematic synthesis was used to categorize resistance factors and intervention strategies. The findings indicate that healthcare professionals’ resistance is multidimensional, encompassing: (1) disruption of workflow and clinical autonomy; (2) cognitive burden and non–user-centered system design; (3) ethical and data security concerns; and (4) inadequate digital competencies and support systems. From a strategic perspective, this resistance leads to decreased productivity, increased turnover, patient safety risks, and stagnation of innovation. Effective interventions include transformational leadership, early user involvement, structured change management, and incentive alignment.Resistance to EMR represents a strategic management issue that requires an integrated sociotechnical approach. The success of EMR implementation is not determined by technological sophistication, but by the organization’s ability to manage human factors and change processes. Further evidence-based intervention research is needed within the Indonesian healthcare context

Abstract

In Indonesia, a critical issue facing the healthcare system today is the low availability of specialist doctors that serve regional hospitals while other areas have reported a physician-to-population ratio phase below the Indonesian national ideal. This study aims to prove the effect of intrinsic and extrinsic factors on specialist doctor retention in RSI Pati, a private Islamic hospital located in Central Java, Indonesia, where increasing numbers of specialists resigned after 2025. A qualitative case study design was used, and data were collected by conducting in-depth interviews with 11 participants from four occupational groups: active specialist doctors, former specialist doctors, and hospital managers. The results were further synthesized along a SWOT–Canvas framework to orient both internal and external factors to operational managerial activities. Loyalty of Doctors is determined by various Aspects and Interdependent factors such as clear and regular remuneration programs, Islamic culture in the organization that allows colleague-ship among doctors, supportive work environment (with HR-is appropriate accommodation) along with medical amenities eg. HMIS etc., career development opportunities etc. Based on these results, a range of managerial strategies was constructed including: rationalization of incentive schemes; strengthening human resource management systems; developing value-oriented professional services and expanding referral networks. These methods aim to ensure the long-term and competitive retention of specialist doctors in RSI Pati.

Abstract

Digital transformation in the healthcare sector through the implementation of Electronic Medical Records (EMR) aims to improve service efficiency, yet it also generates technostress among healthcare workers. This study is based on the increasing psychological pressure and digital workload experienced by healthcare professionals at RSIA Zainab Pekanbaru. The objective of this study is to examine the planning, implementation, and evaluation of technostress management strategies in the use of EMR. The research method used in this study is qualitative case study design involving 10 informants, consisting of 4 doctors and 6 nurses, with thematic analysis supported by NVivo 15. The results indicate that the planning stage is still dominated by a technical, reactive, and top-down approach, leaving psychological needs of healthcare workers insufficiently addressed. In the implementation phase, the organization prioritizes operational efficiency over adaptive capacity and psychosocial support. Evaluation remains focused on system performance without considering user experience or cognitive workload. This study concludes that: 1.Planning should shift toward a human-centered approach by involving healthcare staff; 2. Implementation requires strengthening human aspects through capacity building and digital well-being policies; 3. Evaluation must be holistic, integrating digital well-being indicators and structured feedback mechanisms.

Abstract

Penelitian ini bertujuan untuk menganalisis pengaruh retensi karyawan dan work-life balance (keseimbangan antara kehidupan kerja dan pribadi) terhadap loyalitas karyawan pada perusahaan online shop di Kota Bandung. Kepuasan kerja sebagai variabel intervening juga di analisis perannya dalam memediasi hubungan antara retensi karyawan dan work-life balance. Penelitian ini menggunakan metode penelitian kualitatif dengan metode analisis Structural Equation Modeling (SEM PLS) Data dikumpulkan melalui penyebaran kuesioner kepada karyawan online shop di Kota Bandung. Analisis data dilakukan menggunakan perangkat lunak statistik SmartPLS. Hasil analisis pada penelitian ini menunjukan bahwa retensi karyawan berpengaruh positif terhadap kepuasan kerja maupun loyalitas karyawan. Work-life balance juga memiliki pengaruh positif terhadap kepuasan kerja maupun loyalitas karyawan. Selanjutnya, kepuasan kerja memainkan peran sebagai variabel intervening yang memediasi hubungan antara retensi karyawan dan loyalitas karyawan. Penelitian ini memberikan konribusi dalam pemahaman tentang faktor-faktor yang mempengaruhi loyalitas karyawan. Implikasi dari penelitian ini adalah pentingnya manajemen retensi karyawan dan peningkatan work-life balance untuk meningkatkan kepuasan kerja dan memperkuat loyalitas karyawan.

Abstract

Penelitian ini bertujuan untuk menghitung unit cost layanan ekokardiografi di RSAL dr. Oetojo Sorong, Papua Barat Daya, dengan menggunakan metode Activity-Based Costing (ABC) dan membandingkannya dengan tarif INA-CBG’s yang diterapkan. Metode penelitian yang digunakan adalah pendekatan kuantitatif deskriptif dengan fokus pada analisis biaya langsung, biaya overhead, dan alokasi biaya berdasarkan aktivitas. Data yang digunakan meliputi data primer berupa observasi langsung dan wawancara, serta data sekunder seperti laporan keuangan dan rekam medis rumah sakit. Hasil penelitian menunjukkan bahwa unit cost layanan ekokardiografi yang dihitung menggunakan metode ABC lebih tinggi dibandingkan dengan tarif INA-CBG’s, yang mengindikasikan adanya disparitas antara tarif standar dan biaya aktual. Penerapan metode ABC memberikan manfaat signifikan bagi RSAL dr. Oetojo Sorong dalam mengidentifikasi aktivitas yang memakan biaya besar, meningkatkan efisiensi operasional, serta memberikan transparansi dalam pengelolaan biaya. Penelitian ini juga memberikan rekomendasi kepada manajemen rumah sakit untuk mengevaluasi tarif layanan dan BPJS Kesehatan untuk meninjau kembali kesesuaian tarif INA-CBG’s dengan biaya aktual layanan kesehatan.

Abstract

Process efficiency is a key factor in improving the quality of healthcare services, especially in outpatient facilities such as clinics. This study presents a case study at Promec Clinic in Jakarta, aiming to identify bottlenecks and design process improvement recommendations to reduce end-to-end lead time. A descriptive and explanatory quantitative approach was employed, integrating Time and Motion Study (TMS) and Value Stream Mapping (VSM) as primary analysis tools. Data were collected through direct observation of 100 patients, interviews with staff, and operational process satisfaction questionnaires. The results indicate that the "during examination" stage contributes 73.6% of the total service time, while the "before examination" stage is the primary source of operational errors, particularly in form filling (45.59%) and data entry (39.71%). Pareto and Fishbone analyses reveal that manual procedures, lack of standardization, and reliance on informal communication are root causes of inefficiency. Improvement recommendations include digitizing patient forms using a dedicated registration system, implementing self-service kiosks, conducting periodic staff training, and developing Lean-based Standard Operating Procedures (SOPs). These findings offer practical contributions for clinic management in designing more efficient processes and serve as a reference for applying Lean methodologies in primary healthcare settings in Indonesia.

Abstract

This study aimed to analyze the effects of hospital image and laboratory service quality on customer loyalty, considering the mediating role of customer satisfaction. The research was conducted at Restu Ibu Hospital Balikpapan using a quantitative approach with a causal design. A total of 163 respondents were selected through purposive sampling. Data were collected via questionnaires and analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM). Hospital image had a significant direct effect on customer loyalty (β = 0.325; p = 0.031), while laboratory service quality did not have a significant direct effect (β = 0.037; p = 0.683). Customer satisfaction had a significant effect on customer loyalty (β = 0.528; p < 0.001) and mediated the relationship between hospital image (β = 0.207; p = 0.001) and laboratory service quality (β = 0.246; p = 0.027) with customer loyalty. The model Goodness of Fit (GoF) value of 0.717 indicated an excellent model fit. These findings highlight the importance of strengthening institutional image and improving laboratory service quality as key strategies to enhance customer loyalty in the hospital setting.
