Abstract

ABSTRACT Background: Indonesia comes in the fifth for the greatest number of preterm births. Preterm infants may inflict various complication as the result of underdeveloped immunity, affecting their growth and development in the long run until they reach adult phase. Such complications could be prevented through adequate nutrition fulfillment. Purpose: This article aimed to elaborate the characteristics of growth and development of premature babies, long term effect on the development and the impact of immunity and gut health of preterm infants in supporting their growth and development. Methods: References cited in this article were obtained from the latest primary literature within the last 10 years. Discussion: The rate and ability of infants to perform catch-up growth depends on the birth weight and gestation age, at which the lower birth weight and lower gestational age had slower rate. Brain structures that of preterm infants differ compared to the term, and these changes give rise to various clinical outcomes, including long term emotional, behavioral changes, cognitive and executive functioning. Immature immune system in preterm infants reduces the protective ability by innate and adaptive immunity in overcoming pathogens compared to term infants, including gut microbiota prematurity which affects nutrition absorption and growth and development catch up ability. Appropriate and adequate nutrition supplementation has shown beneficial effects in promoting the growth of normal gut flora, which allow better absorption of nutrition and therefore enhancing growth rate and supporting the development of preterm infants. Conclusions: Optimal growth and development of preterm infants are supported by sufficient nutrition supplementation to support the growth of gut microbiota, facilitating the catch-up growth and development of premature infants and immune system maturity.

Abstract

Latar belakang. World Health Organization (WHO) menetapkan Corona virus disease 2019 (COVID)-19 sebagai pandemi pada Maret 2020. COVID-19 telah menyebar dengan sangat cepat ke berbagai belahan dunia karena sangat infeksius. Hal ini mendorong pemerintah mengeluarkan beberapa kebijakan guna mengontrol penyebaran COVID-19 seperti penutupan sekolah maupun pembatasan sosial. Dampak kebijakan tersebut dan adanya ketakutan terhadap infeksi COVID-19 diperkirakan berdampak negatif terhadap kesehatan jiwa terutama pada kelompok rentan yaitu remaja.Tujuan. Mengetahui apakah ada kaitan antara pandemic COVID-19 dengan masalah jiwa pada remaja.Metode. Dilakukan pencarian di PubMed, Google Scholar, dan Cochrane dengan menggunakan kata kunci “depression”,”mental health”,”adolescent”, “COVID-19”. Hasil pencarian dievaluasi menggunakan kriteria eksklusi dan inklusi. Selanjutnya dilakukan telaah kritis dengan memerhatikan validitas, kepentingan, dan penerapan pada pasien terhadap artikel lengkap dari studi yang terseleksi.Hasil. Diperoleh satu studi yang relevan dengan pertanyaan klinis dan memenuhi kriteria inklusi and eksklusi. Luaran dari studi ini memperlihatkan bahwa prevalensi masalah kesehatan jiwa cukup tinggi pada remaja selama pandemi COVID-19. Kesimpulan. Angka kejadian depresi dan ansietas lebih tinggi pada kelompok perempuan, populasi yang tinggal di pedesaan, dan murid yang lebih senior. Namun, studi ini belum dapat membuktikan apakah pandemi COVID-19 menyebabkan masalah kesehatan jiwa pada remaja.

Abstract

Background Optimal child development is needed for adequate learning. Children, particularly toddlers, require iron for brain development, and consequently, overall development. Objective To analyze for an association between iron status and developmental status in children aged 24–36 months. Methods This explorative cross-sectional study was held in Kampung Melayu, Jakarta. Subjects were recruited using a total population sampling method. Data were collected through interview with parents, anthropometric examinations, and blood tests. Developmental status was determined using the Ages and Stages Questionnaire-3 (ASQ-3) and iron status was based on ferritin, high sensitivity C-reactive protein (hs-CRP), and hemoglobin levels. Data analyses included Chi-square/Fisher’s exact, Mann-Whitney, and logistic regression tests. Results Of 80 subjects, 17.5% had developmental delay and 41.3% had deficient iron status. There was no significant association between iron status and developmental status in bivariate analysis, but the logistic regression analysis revealed that iron status (OR=6.9; 95%CI 1.328 to 35.633; P=0.022) and nutritional status (OR=11.75; 95%CI 1.551 to 88.979; P=0.017) contributed to developmental delay. Conclusion Better iron status and nutritional status are associated with better child development of children aged 24–36 months. So efforts are needed to maintain iron status as well as nutritional status.

Abstract

Latar belakang. Kasus pertama pasien COVID-19 di Indonesia dilaporkan pada bulan Maret 2020. Pemerintah memberlakukan Pembatasan Sosial Berskala Besar termasuk penutupan sekolah tatap muka serta mengeluarkan protokol kesehatan untuk diterapkan oleh masyarakat. Pandemi menyebabkan perubahan besar dalam tatanan kehidupan, kebiasaan dan pola hidup remaja. Tujuan. Penelitian ini ingin melihat pengetahuan dan kepatuhan remaja terhadap implementasi protokol kesehatan pada tahun ke dua pandemi COVID-19.Metode. Penelitian ini menggunakan metode potong lintang menggunakan kuesioner yang diisi secara daring dan dilakukan pada tahun ke dua pandemi COVID-19 dari bulan April hingga Juli 2021. Subjek merupakan remaja usia 10-18 tahun di DKI Jakarta. Pemilihan sekolah dilakukan secara cluster sampling. Hasil. Sebanyak 406 responden dari 20 sekolah mengikuti penelitian. Hanya sekitar 5,42% responden yang menjawab dengan benar cara transmisi SARS-CoV-2 dan hanya 43,84% responden yang menjawab dengan tepat 6 langkah cuci tangan. Sebanyak 45,81% responden memiliki nilai total pengetahuan yang baik (nilai >7). Terdapat tren yang menunjukkan persentase remaja yang mendapatkan skor pengetahuan di atas tujuh lebih banyak pada kelompok remaja dengan nilai kepatuhan yang lebih tinggi.Kesimpulan. Pada tahun ke dua pandemi COVID-19 masih banyak remaja yang kurang pengetahuannya mengenai COVID-19 dan belum menerapkan protokol kesehatan dengan baik.

Abstract

Background Quality of life is an important outcome in the management of children with chronic conditions such as rheumatic diseases. The Pediatric Quality of Life Inventory Rheumatology Module (PedsQL-RM) questionnaire has been proven valid and reliable, but a validated Indonesian version of the questionnaire is not yet available. Objective To translate the PedsQL-RM into the Indonesian language, perform a transcultural adaptation, and assess its validity. Methods A cross-sectional study was performed in patients aged 2 to 18 years with systemic lupus erythematosus (SLE) or juvenile idiopathic arthritis (JIA) in Dr. Cipto Mangunkusumo Hospital, Jakarta. The initial phase of the study consisted of forward translation from the original English version into Indonesian, synthesis by experts, backward translation, and cognitive debriefing, resulting in the final version of the questionnaire. The second phase was testing the final questionnaire on patients in each age group and their parents. Tests were carried out in two sessions with an interval of 2 to 4 weeks. The questionnaire consisted of a child report and a parent report, each measuring five dimensions: pain and hurt, activities, treatment, worry, and communication. We subsequently assessed validity and reliability of each dimension in the child and parent reports for the child and teen age groups. Validity was expressed as correlation coefficient (r) between dimension scores with the total score. Results The finalized Indonesian questionnaire was completed by 53 children aged 2-18 years with SLE or JIA and their parents. Due to small numbers of subjects in the younger age ranges, analysis was only performed in the 8-to-18-year age group. Validity varied from good to very good (r = 0.437 to 0.910) for the child report and from poor to good (r = 0.153 to 0.808) for the parent report. The questionnaire was deemed reliable, with a Cronbach’s alpha of 0.755 to 0.785. Conclusion The Indonesian version of the PedsQL-RM is valid and reliable for assessing quality of life in children aged 8 to 18 years with rheumatologic disease. Further study is needed to assess the validity and reliability of the tool for children aged 2 to 7 years.

Abstract

Introduction: Iron is essential for child’s development and growth. Children’s iron requirements are secured from daily food intake that might be affected by Covid-19 pandemic. This study aims to determine iron intake and its relationship with ferritin and hemoglobin levels as indicators of iron status among children in Jakarta.Method: This cross-sectional study was conducted in Kampung Melayu, Jakarta from September to October 2020. Seventy seven healthy children aged 24–36 months were taken using total population sampling method. Interviews were conducted to obtain characteristics data and iron intake using a semi-quantitative-food frequency questionnaire (SQ-FFQ). Blood tests are performed to check the levels of ferritin (controlled by hs-CRP value), and hemoglobin.Pearson’s/Spearman’s correlation test was performed using SPSS version 20.0. Result: Median of iron intake was 9.6 (1.5–40,7) mg/day, in which 33.8% of subjects was below the Indonesian Recommended Dietary Allowance (RDA) recommendation. The median ferritin value was 18.1 (1.4–91.1) 𝜇g/L and the hemoglobin was 11.8 (6.6–15.2) g/dL, in which 40.3% and 27.3% subjects with iron insufficient-deficient and anemia, respectively. There were  positive correlations between iron intake and ferritin (r = 0.328, p = 0.002) and iron intake and hemoglobin (r = 0.308, p = 0.003). A strong positive correlation was found between ferritin and hemoglobin (r = 0.769, p < 0.001).Conclusion: Iron intake of children aged 24–36 months had a weak positive correlation with ferritin and hemoglobin level. Keywords: children 24–36 months, ferritin, hemoglobin, iron intake, Jakarta

Abstract

Dehydration is prevalent among athletes. Dehydration may cause fluid and electrolyte imbalance and impairment of an athlete physical and cognitive performance. Carbohydrate electrolyte solutions (CES) are often used by athletes as a rehydration fluid because it is claimed to have numerous beneficial effects on hydration status and performance. Studies on the use of carbohydrate-electrolyte solutions showed mixed results. Some studies also highlighted the side effects of carbohydrate-electrolyte solutions on gastrointestinal system. This review aimed to summarize the roles of carbohydrate-electrolyte solutions on hydration status and performance of athletes. The literature review was conducted in the narrative type. Database search was conducted using Pubmed Central search engine. The search terms used were "carbohydrate", "electrolyte", "athlete", "hydration", and "performance". The results show that carbohydrate-electrolyte solutions may prevent dehydration and electrolyte disturbance by increasing water absorptions and retentions. The maintenance of hydration status and the ergogenic effect exerted by carbohydrate can also prevent performance deteriorations. The formulation and administration of carbohydrate-electrolyte solutions must be done with caution and be given at the right timing to reduce side effects, primarily the gastrointestinal discomfort.

Latar belakang. Kemajuan dalam perawatan neonatal intensif telah memberikan kontribusi terhadap peningkatan angka kesintasan bayi prematur. Bayi yang bertahan hidup mempunyai risiko gangguan pertumbuhan dan perkembangan yang berhubungan dengan prematuritas dan morbiditas yang menyertainya. Tujuan. Mengetahui luaran pertumbuhan dan perkembangan bayi prematur usia gestasi 28-34 minggu pasca rawat dari Unit Neonatologi Rumah Sakit Ciptomangunkusumo.Metode. Penelitian kohort prospektif dengan wawancara kepada orang tua subjek penelitian, pencataan melalui rekam medis, dan pemeriksaan pertumbuhan dan perkembangan langsung oleh peneliti pada 90 bayi prematur pasca rawat pada bulan Juli? Desember 2016 dengan cara consecutive sampling. Kriteria inklusi adalah bayi prematur dengan masa gestasi 28-34 minggu, berdomisili di Jakarta, Bogor, Depok, Tangerang, serta Bekasi dan orang tua menyetujui mengikuti penelitian. Kriteria eksklusi adalah bayi memiliki kelainan kongenital atau bawaan seperti sindrom Down, sindrom Rubella dan lainnya, dan memiliki malformasi orofasial. Hasil. Hasil data usia koreksi 0 sampai 3 bulan, didapatkan rerata kenaikan berat bayi prematur 29,01±5,31 g/hari, hasil rerata kenaikan panjang badan 3,7±0,8cm/bulan dan rerata kenaikan lingkar kepala 1,6±0,6cm/bulan. Hasil perkembangan berdasarkan FSDQ CAT-CLAM (full-scale developmental quotient cognitive adaptive test- clinical linguistic and auditory milestone scale) menunjukkan skor yang cukup baik yaitu di atas 100% dengan rerata 148% (111-160% ).Kesimpulan. Pertumbuhan dan perkembangan bayi prematur usia gestasi 28-34 minggu pasca rawat Unit Neonatologi Rumah Sakit Ciptomangunkusomo Jakarta sampai usia 3 bulan koreksi baik dan dalam rentang normal.

Abstract

Background The asynchrony rapid phase of cognitive, physical and emotional development occurring in adolescence may result in various issues such as risk-taking behavior, psychosocial and emotional problems. School closure during the COVID-19 pandemic has caused a tremendous change in adolescent lifestyle and habits, including sleep patterns. causing added challenges to adolescents’ emotional and behavioral problems. Objective To identify the association between sleep disorders with emotional and behavioral problems during the COVID-19 pandemic in urban and rural adolescents. Methods We conducted a cross-sectional study among adolescents aged 12-18 years from November to December 2021 in two provinces, DKI Jakarta and Riau Islands. We recruited students consecutively from randomly selected junior and senior high schools. We used the Strengths and Difficulties Questionnaire (SDQ) and the Pittsburgh Sleep Quality Index (PSQI) to screen for emotional and behavioral problems and for sleep disorders, respectively. The questionnaires were sent electronically to each subject after obtaining parental consent. Results There were 400 subjects included in our study, of which 64% were female. The prevalences of COVID-19 amongst subjects in urban and rural areas were COVID-19 12.5% and 5.5%, respectively. Emotional and behavioral disorders as well as sleep disorders were more common in rural than urban areas (13 vs. 11.5% and 50.5% vs. 38%; P=0.012, respectively). Sleep disorders were associated with the SDQ subscales of emotional symptoms [PR 4.88 (95%CI 2.97 to 8.03); P<0.01), conduct problems [PR 3.71 (95%CI 1.53 to 9.04); P=0.02), hyperactivity/inattention [PR 6.05 (95%CI 2.58 to 16.17); P<0.01], and peer relationship problems [PR 3.28 (95%CI 1.33 to 8.09); P<0.01). Conclusion Sleep disorders among adolescents during COVID-19 pandemic were associated with emotional and behavioral problems. Adolescents who live in rural areas were more likely to have sleep disorders, emotional and behavioral problems.

Abstract

Background Since the survival of a patient with chronic physical illness has improved, major concerns arise about their quality of life and functioning. Childhood and adolescence are critical periods for promoting mental health because more than half of mental health problems begin at this stage, and many of those mental health problems will persist into adult life. Objective To analyze the risk of mental health problems among children hospitalized with chronic disease. Method An analytic observational study with a cross-sectional design was conducted using secondary data of 754 children aged 4-18 years admitted to the pediatric inpatient ward of Cipto Mangunkusumo Hospital, Jakarta between June 2021 and August 2022. We collected the results of subjects’ mental health screening conducted by the Growth, Development, and Social Pediatrics Division, Department of Child Health, Cipto Mangunkusumo Hospital during the subjects’ admission. Mental health problems evaluated in this study included psychosocial dysfunction screened using the Pediatric Symptom Checklist 17 (PSC-17) and depression screened using the Children’s Depression Inventory (CDI). Results Of the 754 subjects included, 520 (69.0%) were classified as having a chronic disease. The prevalence of psychosocial dysfunction and depression in children with chronic diseases was 19.6% and 6.3%, respectively. Gender, nutritional status, and height had no significant association with psychosocial dysfunction and depression in children. Children with chronic disease were more likely to develop psychosocial dysfunction (OR 2.13; 95%CI 1.31 to 3.59; P<0.001) and depression (OR 2.57; 95%CI 1.04 to 7.62; P<0.03) compared with those who did not have chronic disease. Conclusion Children with chronic disease were more likely to develop psychosocial dysfunction and depression compared to those without a chronic disease. These results highlight the importance of mental health support in pediatric patients with chronic disease.
