Abstract

Background: Chronic periodontitis occurred mostly in productive ages. Chronic periodontitis was started by adherence and accumulation of plaque bacteria that caused vascular enhancement, PMN infiltration, and periodontal tissue destruction. This process caused bone calcium solubitily which went trough blood stream and excreted to gungiva sulci. Calcium in periodontal tissue could be found both in extracellular and intracellular matrix.Purpose: Purpose of this study was to know calcium level in gingival crevicular fluid of chronic periodontitis and compare calcium level in chronic periodontitis and gingivitis.Method: This study was analytic observational with cross sectional approach and got admission from ethic comission of Dental Faculty, Gadjah Mada University. The subjects were patients who came to dental hospital of Jember University. The subjects had to full fill informed consent. The subjects were selected according inclusion and exclusion criteria. Subjects  complying inclusion criteria were divided into gingivitis group (PI score 0 - 0.7) and chronic periodontitis (PI score 1 - 8.0). Gingival crevicular fluid samples were collected from teeth which got gingivitis and chronic periodontitis, GCF was absorbed using paperpoint and stored inside eppendorf tube. Paperpoint was added with 50 µL 0.02 M PBS pH 7.4 and 100 µL distilled water. Calcium level was measured by Atomic Absorption Spectrophotometer (AAS). Result: The result showed there was calcium in gingival crevicular fluid of chronic periodontitis. Mean of calcium level of chronic periodontitis was higher than  gingivitis.Conclusion: This study concluded there was calcium in gingival crevicular fluid of chronic periodontitis and calcium level of chronic periodontitis was higher than gingivitis.

Abstract

Backgroud. Increased levels of low density lipoprotein (LDL) and decreased levels of high density lipoprotein (HDL) in the blood is a risk factor for atherosclerosis leading to Coronary Heart Disease (CHD). Risk factor of atherosclerosis are multifactorial such as consuming high lipid diet, excessive eating, sedentary, and inflammatory response. According to the recent study indicated that inflammatory response could affect LDL and HDL blood levels that related to the risk of atherosclerosis lesion formation. Therefore, we suspected that periodontitis could affect LDL and HDL. Purpose. The purpose of this study was to show the effect of periodontitis in LDL and HDL. Methods. This experimental study used the post test only control group design. The object were 8 male wistar rats (Rattus norvegicus) with inclusion criterias were male rats, ±200 gr body weight, 3-4 months age, having a healthy condition, good appetite, and normal behavior. The rats were devided into two groups, i. e. control group and treatment group (periodontitis group) with 4 rats in each group. Periodontitis rat model was created by fitting cervical left low molar tooth with wire ligature and injected with 0,02 ml P. gingivalis ATCC 33277 (dosage 5µg/0,05 ml) in bucal sulcus, three times a week for 4 weeks (28 days), all of the rats were fed with standard diet. On day 29th all groups of rats were sacrificed and intracardial blood were taken 3 cc for examination of LDL and HDL blood levels by electrospectrophotometric method. Data were analyzed by T-test. Result. Result showed that periodontitis rat model had a significantly higher LDL levels compared to control group (P <0,05) and HDL levels were lower than controls, but the difference was not significant (P >0,05). Conclusion and suggestion. Based on this study periodontitis increase LDL blood levels. Further research related with duration of periodontitis are needed because periodontitis is usually a chronic disease and have a long lasting effect . In addition, the role of the severity of periodontitis are required to learn.   Keywords: HDL, LDL, Periodontitis.

Abstract

Background: The prevalence of periodontal disease in Indonesia is 60%. Chronic periodontitis is one of periodontal disease which affects Indonesian. The primary cause of periodontal disease is bacterial plaque. Adhesions and accumulation of bacterial plaque can cause inflammation and destruction of periodontal tissue following by increasing of gingival crevicular fluid flows and releasing of its components, one of the components is phosphorus that excreted from sulcular epithelium. Soluble phosphorus along blood stream is from gingival crevicular fluid which the flow is increased and excreted by sulcular epithelium. Objective: To know phosphorus levels in the gingival sulcus of chronic periodontitis. Methods: The subjects of the study were patients who came to the Dental Hospital University of Jember and occupied as inclusion and exclusion criteria. The subjects who had inclusion criteria were divided into 2 groups based intra-oral examination; gingivitis and chronic periodontitis. Gingival crevicular fluid sampling was taken from tooth with gingivitis or chronic periodontitis using paperpoint. Phosphorus level measurement used UV-Vis Spectrophotometer with a wavelength of 883 nm. The results were analyzed by Mann-Whitney test. Results: There were phosphorus in the gingival crevicular fluid of chronic periodontitis and it higher than gingivitis. Conclusion: There is phosphorus level in the gingival crevicular fluid in patients with chronic periodontitis.   Keywords: Chronic periodontitis, gingival crevicular fluid, phosphorus

Abstract

Background: Cardiovascular disease can be treated with synthetic or traditional antithrombotic drugs (herbal). Synthetic antithrombotic drugs have a lot of side effects, so as an alternative antithrombotic is used traditional drugs, star gosseberry. Star gosseberry have many active compounds which are suspected to inhibit platelet aggregation. Objective: to know the effect of star gosseberry on bleeding time of male Wistar rats as an alternative antithrombotic drugs. Methods of research: this study was experimental laboratory research with post-test only control group design. The research samples were 24 male wistar rats and divided into 4 groups: negative control (distilled water), positive control (aspirin 5,85 mg dose), treatment 1 (star gosseberry 4,5 mg/g dose), and treatment 2 (star gosseberry 2.25 mg/g dose). The treatment was given for 8 days then the tail of samples were cut by 0.5 cm from the tip of the tail. Every 30 seconds the blood that dripped out was absorbed on absorbent papers until the bleeding stops. Bleeding time was measured from initial point of bleeding until the cessation of the bleeding point. Result: star gosseberry dose 4,5 mg/g had the same effect as aspirin that was prolonged bleeding time. Conclusion: star gosseberry could be prolonged bleeding time so it can be used as an alternative antithrombotic drugs. Keywords: Katuk leaf, bleeding time, antithrombotic, platelet

Abstract

Background: Periodontal diseases have high prevalence in Indonesia. They are caused by bacteria plaque that induced host response to release pro inflammatory mediator. Pro inflammatory mediators and bacteria product cause degradation of collagen fibers in periodontal tissue. Deoxypyridinoline is one of pyridinoline cross-link of collagen type I that can be used as biomarker in bone metabolic diseases, however, their contribution to detect alveolar bone loss in periodontal diseases remains unclear. Purpose: This study was to evaluate deoxypyridinoline level in gingival crevicular fluid as alveolar bone loss biomarker on periodontal disease. Methods: This study used 24 subjects with periodontal diseases and 6 healthy subjects. Dividing of periodontal disease was based on index periodontal. Gingival crevicular fluid was taken at mesial site of maxillary posterior tooth by paper point and deoxypyridinoline be measured by ELISA technique. Results: We found increasing of deoxypyridinoline level following of the severity of periodontal diseases. There was also significant difference between healthy subjects and periodontal diseases subjects (p<0.05). Conclusion: Deoxypyridinoline level in gingiva crevicular fluid can be used as alveolar bone loss biomarker in periodontal disease subjects.Latar belakang: Prevalensi penyakit periodontal di Indonesia cukup tinggi. Ini disebabkan oleh bakteri plak yang merangsang respon tubuh untuk mengeluarkan mediator keradangan. Mediator keradangan dan produk bakteri menyebabkan degradasi serat kolagen jaringan periodontal. Deoksipiridinolin merupakan salah satu ikatan piridinium dari kolagen tipe I yang dapat digunakan sebagai biomarker penyakit metabolisme tubuh. Akan tetapi, penggunaan deoksipiridinolin untuk mendeteksi kehilangan tulang alveolar pada penyakit periodontal masih belum jelas. Tujuan: Tujuan penelitian ini untuk mengetahui bahwa kadar deoksipiridinolin pada cairan krevikular gingival dapat digunakan sebagai biomarker kehilangan tulang alveolar pada penyakit periodontal. Metode: Penelitian ini menggunakan 24 subyek penelitian yaitu 24 orang dengan penyakit periodontal dan 6 orang tidak menderita penyakit periodontal. Pembagian penyakit periodontal berdasarkan indeks periodontal. Cairan krevikular gingival diambil dari bagian mesial gigi posterior atas dengan menggunakan paper point dan diukur kadar deoksipiridinolin dengan menggunakan teknik ELISA. Hasil: Hasil penelitian menunjukkan ada peningkatan kadar deoksipiridinolin seiring dengan tingkat keparahan penyakit periodontal. Hasil statistik juga menunjukkan ada perbedaan rata-rata antara subyek penelitian yang tidak menderita penyakit dengan subyek yang menderita penyakit periodontal (p<0,05). Kesimpulan: Kadar deoksipiridinolin pada cairan krevikular gingival dapat digunakan sebagai biomarker kehilangan tulang alveolar pada penderita penyakit periodontal.

Abstract

Background: Menopause is a phase of a woman’s life marked by menstruation cycle cessation and an increased risk of periodontal disease. It can be caused by estrogen deficiency which alters the microenvironment in the sulcular gingival area and influences the composition and flow of gingival crevicular fluid (GCF). GCF has been widely studied as a non-invasive diagnostic and predictive tool for periodontal diseases. However, insufficient reports exist that explore its role as a predictive or diagnostic tool for bone loss detection in menopausal women. Purpose: This study aimed was to investigate deoxypyridinoline (DPD) and mineral levels that could be utilized as disorder indicators in menopausal women with periodontal disease. Methods: This study represents a form of analytical observation. Eighty-four patients of the Dental Hospital, University of Jember who fulfilled certain criteria were recruited. The subjects were divided into two main groups based on the presence of periodontal disease, (gingivitis=26; periodontitis=58) which were subsequently divided into three sub-groups based on their menopausal phase (pre-menopausal=26; perimenopausal=40; post-menopausal=18). GCF was collected using paper points from the buccal site of a posterior maxillary tooth with each subject having their GCF taken on only one occasion. DPD analysis was conducted by means of an ELISA test. The analysis of calcium, magnesium and sodium incorporated the use of an Atomic Absorption Spectroscope (AAS), while that of phosphor was by means of a spectrophotometer. Statistical analyses were performed using a comparison and correlation test (p<0.05). Results: There were significant differences in DPD and the mineral level of GCF in menopausal women with periodontal diseases (p<0.05). DPD and mineral levels showed significant correlation to those of menopausal women with periodontal diseases and a pH of GCF. Conclusion: DPD and mineral level in GCF could be used as disorder indicators in menopausal women with periodontal diseases.

Abstract

Pendahuluan: Disfungsi ovarium merupakan kondisi yang menimbulkan defisiensi hormon estrogen dan progesteron. Penurunan hormonal ini menyebabkan peningkatan produksi tumor necrosis factor (TNF) yang dapat memicu peningkatan produksi matriks metalloproteinase-8 (MMP-8). Periodontitis yang disebabkan oleh bakteri gram negatif akan memicu makrofag melepaskan TNF-α yang berkontribusi dalam pembentukan MMP-8. MMP-8 ini berperan dalam degradasi kolagen jaringan ikat gingiva. Meningkatnya MMP-8 dapat menyebabkan terjadinya periodontitis. Gejala periodontitis karena disfungsi ovarium dan induksi bakteri ini dapat diperlambat dengan bahan alam, yaitu daun singkong. Tujuan penelitian ini adalah untuk menganalisis potensi ekstrak daun singkong (Manihot esculenta Crantz) terhadap ekspresi MMP-8 fibroblas pada model tikus disfungsi ovarium dan periodontitis. Metode: Penelitian ini merupakan penelitian eksperimental laboratoris dengan post-test only control group design. Sampel yang digunakan adalah tikus Sprague-Dawley betina yang dibagi dalam 5 kelompok yaitu (1) kelompok kontrol (K), (2) Kelompok yang diinduksi bakteri Porphyromonas gingivalis dan diberi aquades, (3) Kelompok yang diinduksi bakteri Porphyromonas gingivalis dan diberi ekstrak daun singkong, (4) Kelompok yang diberi perlakuan ovariektomi dan diberi aquades, (5) Kelompok yang dilakukan ovariektomi dan diberi ekstrak daun singkong. Pengambilan jaringan gingiva setelah tahap euthanasia dilakukan untuk pembuatan preparat histopatologi dengan pewarnaan imunohistokimia. Pengamatan dan penghitungan ekspresi MMP-8 dilakukan dengan menggunakan software ImageJ dan Immunoratio. Hasil: Hasil analisis data one-way ANOVA, ekspresi MMP-8 fibroblas gingiva menunjukkan perbedaan yang signifikan (p=0,000). Hal ini menunjukkan bahwa ekstrak daun singkong dapat menurunkan ekspresi MMP-8 fibroblas gingiva tikus yang mengalami disfungsi ovarium dan periodontitis diinduksi P. gingivalis. Simpulan: Ekstrak daun singkong (Manihot esculenta Crantz) dapat menurunkan ekspresi MMP-8 sel fibroblas gingiva pada model tikus disfungsi ovarium dan periodontitis.Kata kunci: Disfungsi ovarium, periodontitis, ekspresi MMP-8, ekstrak daun singkong. ABSTRACTIntroduction: Ovarian dysfunction is a condition that causes estrogen and progesterone deficiency. This hormonal decrease causes an increase in the production of Tumour Necrosis Factor (TNF), which can trigger an increase in the production of matrix metalloproteinase-8 (MMP-8). Periodontitis caused by gram-negative bacteria will trigger macrophages to release TNF-α, which contributes to the formation of MMP-8. MMP-8 plays a role in collagen degradation of the gingival connective tissue. An increase in MMP-8 can cause periodontitis. Periodontitis symptoms due to ovarian dysfunction and bacterial induction can be slowed down by natural ingredients, such as cassava leaf. The purpose of this study was to analyse the potential of cassava leaf extract (Manihot esculenta Crantz) on MMP-8 expression of gingival fibroblast in rats model with ovarian dysfunction and periodontitis. Methods: This study was an experimental laboratory with post-test only control group design. The samples were female Sprague-Dawley rats divided into five groups: (1) control group (K); (2) Porphyromonas gingivalis induced group and given aquadest; (3) Porphyromonas gingivalis induced and given cassava leaf extract; (4) Group with ovariectomy treatment and given aquadest; (5) Group with ovariectomy treatment and given cassava leaf extract. Gingival tissue retrieval after the euthanasia was carried out for the histopathology preparations by immunohistochemical staining. Observation and calculation of MMP-8 expressions were performed using ImageJ and Immunoratio software. Results: The results of the one-way ANOVA analysis of MMP-8 expression of gingival fibroblasts showed a significant difference (p = 0.000); thus cassava leaf extract reduce the MMP-8 expression of gingival fibroblasts of rats with ovarian dysfunction and P. gingivalis induced periodontitis. Conclusion: Cassava (Manihot esculenta Crantz) leaf extract can reduce the MMP-8 expression of gingival fibroblast cells in rats’ model with ovarian dysfunction and periodontitis.Keywords: Ovarian dysfunction, periodontitis, MMP-8 expression, cassava leaf extract.

Abstract

Pendahuluan: Disfungsi ovarium merupakan keadaan ovarium yang mengalami kegagalan dalam sekresi hormon seks steroid. Penurunan sekresi hormon terutama estrogen dapat memicu peningkatan sitokin proinflamasi seperti IL-1, IL-6 dan TNF-α yang berperan dalam resorpsi tulang dan reaksi inflamasi periodontal. Salah satu drug of choice dari periodontitis adalah metronidazole. Penggunaan metronidazole secara sistemik dapat mengakibatkan efek samping, sehingga diperlukan bahan alternatif yang memiliki efek terapi antiinflamasi. Salah satu tanaman yang memiliki efek antiinflamasi adalah daun Singkong. Tujuan penelitian adalah menganalisis potensi ekstrak daun Singkong sebagai antiinflamasi terhadap profil leukosit darah tepi model tikus disfungsi ovarium dan periodontitis. Metode: Jenis penelitian eksperimental laboratoris dengan post-test only control group design. Sampel penelitian sebanyak 18 ekor tikus betina yang dibagi menjadi dua kelompok besar yakni kelompok disfungsi ovarium dan kelompok kecil periodontitis. Pembuatan model tikus disfungsi ovarium dan tikus periodontitis dilakukan selama 28 hari. Setelah masing-masing kelompok dibagi menjadi 3 kelompok yang diberikan perlakuan aquades, metronidazole dan ekstrak daun Singkong selama 7 hari. Setiap tikus diambil darahnya melalui plexus infraorbitalis pada hari ke-1, hari ke-3 dan hari ke-7. Darah yang diambil selanjutnya dilakukan perhitungan jumlah sel leukosit darah tepi menggunakan kamar hitung dan perhitungan jenis dengan cara visual. Hasil: Hasil uji LSD menunjukkan bahwa terdapat perbedaan bermakna jumlah leukosit darah tepi antar hari pada kelompok tikus yang mengalami disfungsi ovarium (p≤0,05). Simpulan: Ekstrak daun Singkong (Manihot esculenta Crantz) dapat menurunkan profil leukosit perifer model tikus disfungsi ovarium dan periodontitis.Kata kunci: Disfungsi ovarium, periodontitis, profil leukosit perifer, ekstrak daun singkong. ABSTRACTIntroduction: Ovarian dysfunction is when the ovaries fail in the secretion of steroid sex hormones. Decreased secretion of hormones, especially estrogen, can trigger an increase in pro-inflammatory cytokines such as IL-1, IL-6 and TNF-α, which play a role in bone resorption and periodontal inflammatory reactions. One drug of choice for periodontitis is metronidazole. Systemic use of metronidazole can cause side effects, so an alternative material with a therapeutic anti-inflammatory effect is needed. One of the plants that have an anti-inflammatory effect is cassava leaves. The research objective was to analyse the potential of cassava leaf extract as an anti-inflammatory against the peripheral blood leukocyte profile in a mouse model of ovarian dysfunction and periodontitis. Methods: This was an experimental laboratory study with a post-test only control group design. The research sample consisted of 18 female rats divided into two large groups: the ovarian dysfunction group and the small periodontitis group. The making of mouse models of ovarian dysfunction and periodontitis rats was carried out for 28 days. After each group was divided into three groups treated with distilled water, metronidazole and cassava leaf extract for seven days. Blood was drawn from each mouse through the infraorbital plexus on day 1, day three, and 7. The blood that was taken was then performed to calculate the number of peripheral blood leukocytes using the counting room and the calculation of the type by visual means. Results: The results of the LSD test showed that there was a significant difference in the number of peripheral blood leukocytes between days in the group of rats with ovarian dysfunction (p≤0.05). Conclusion: Cassava leaf extract (Manihot esculenta Crantz) can reduce the profile of peripheral leukocytes in a mouse model of ovarian dysfunction and periodontitis.Keywords: Ovarian dysfunction, periodontitis, peripheral leukocytes profile, cassava leaf extract.

Abstract

Perimenopause is trantition phase of nomal ovulation cicle to menopause that signed decreasingestrogen hormone production. In this phase, women will be physiology and psychology alteration related fluctuationof estrogen hormone. The estrogen level alteration is one of cause reducing of bone density in perimenopausewomen. When estrogen level decreases, it will induce osteoclast activity and finally cause bone resorption. In boneresorption osteoclast secretes protease that can dissolve collagen which is in organic matrix and free bone mineral.Mature collagen is bound by pyridinium crosslink that it will be degraded in bone resorption process, such asdeoxypyridinoline. Deoxypyridinoline have suggested as collagen degradtion marker of bone resorption in serumand urine. This study was objective to identify deoxypyridinoline in saliva of perimenopause women. The subjects ofthis study were perimenopause women who were 40-55 years old. The subjects had to fulfill informed consent and beundergone intra oral examination, including caries and periodontal index index. After that the subjects collectedtheir whole saliva. Deoxypyridinoline in saliva was identified by Liquid chromatography Mass Spectroscopy. The resultshowed that deoxypyridinoline level of perimenopause women was about 23.802 ppm. This level was higher thanproductive women. The conclusion was deoxypyridinoline level in saliva of permenopause women

Abstract

Stres caused homeostasis disturbance. Stres induced glucocorticoid and catecholamine releasing. These hormone influenced lipid metabolism. The aim of this study is to know effect electrical foot shock to total cholesterol level in serum of male rats (Rattus norvegicus) strain Wistar. This study was experimental laboratory with the post test only kontrol group design. This study used 14 male rats that divided into 2 groups, kontrol and treatment. The treatment group, samples got stres from electrical foot shock for 14 days. On the 15th day, blood of samples was taken from intracardial. Then, blood samples were tested cholesterol total level by CHOD-PAP in photometric system. The result was analyzed by t-test. The result showed there was significant different between kontrol and treatment group. The conclusion describe that cholesterol total level in treatment group is higher than kontrol group.
