Abstract

Resep adalah proses transaksi dalam terapi yang penting antara dokter, apoteker, dan pasien. Resep yang berkualitas baik adalah faktor yang penting untuk meminimalkan kesalahan dalam pelayanan obat sehingga resep yang baik harus sesuai dengan pedoman penulisan resep untuk menjamin efektifitas dan keamanan pengobatan. Penelitian ini dilakukan untuk menganalisis kualitas penulisan resep untuk pasien anak di instalasi rawat jalan rumah sakit di Surakarta. Penelitian ini merupakan penelitian deskriptif cross-sectional yang dilakukan di dua rumah sakit. Sebanyak 368 resep untuk pasien pediatrik yang dikumpulkan di bulan Februari 2017 dari instalasi rawat jalan di rumah sakit dianalisis. Ketidaklengkapan penulisan berat badan (33,15%), usia (2,98%) dan jenis kelamin pasien (7,34%) teridentifikasi, padahal informasi tersebut merupakan hal tentang pasien yang penting untuk diketahui. Sebanyak 110 (29,89%) resep juga tidak mempunyai informasi kekuatan konsentrasi obat dalam sediaan. Dua ratus lima puluh tujuh resep (69,83%) tidak mencantumkan data nomer surat ijin praktek dokter dan 33 (8,97%) resep tidak mempunyai paraf dokter. Kualitas peresepan di instalasi rawat jalan masih belum memuaskan karena kurangnya informasi yang penting untuk sebuah resep yang berkualitas. Untuk meningkatkan kualitas dalam penulisan resep, kami menyarankan agar rumah sakit menyediakan formulir resep standar yang berisi informasi yang diperlukan untuk mengidentifikasi data pasien dan dokter.

Abstract

Usia geriatri akan merubah kondisi fisik dan profil farmakokinetik farmakodinamik. Meningkatnya sarana kesehatan (klinik dan apotek) yang terdapat di Kabupaten Sukoharjo berdampak pada penggunaan obat masyarakat yang secara bebas melakukan upaya untuk memelihara dan meningkatkan kesehatan sendiri (swamedikasi). Swamedikasi tanpa dasar pengetahuan yang tepat dapat meningkatkan kemungkinan terjadinya medication error. Intervensi edukasi obat oleh Apotekerterbukti dapat menurunkn angka kejadian medication error. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh sesi edukasi oleh apoteker terhadap pengetahuan masyarakat Dusun Karangtengah Ngadirejo Kartasura Sukoharjo tentang penatalaksanaan nyeri pada penyakit otot dan tulang dengan obat dan tanpa obat serta bagaimana cara memilih obat penghilang rasa nyeri yang aman. Penelitian dilakukan dengan cara memberikan intervensi edukasi kepada respoden. Evaluasi tertulis dengan memberikan kuesioner dilakukan sebelum dan sesudah intervensi. Kuesioner berisi pertanyaan tertutup tentang penyebab gout, rematik, dan osteoarthritis, perbedaan antara ketiganya; serta penatalaksanaan nyeri karena penyakit otot dan tulang. Analisis data dilakukan dengan membandingkan skor pengetahuan sebelum dan sesudah intervensi dengan uji t berpasangan. Hasil penelitian menunjukkan adanya peningkatan pengetahuan masyarakat tentang penyakit rematik dan penatalaksanaan nyeri sebelum dan sesudah edukasi secara signifikan (uji t berpasangan p< 0,001) dengan rata-rata nilai pengetahuan sebelum penyuluhan adalah 55,0±23,1 dan sesudah penyuluhan adalah 82,0±15,4. Berdasarkan hasil penelitian tersebut terbukti bahwa sesi edukasi oleh apoteker dapat meningkatkan pengetahuan masyarakat tentang penatalaksanaan nyeri secara signifikan. Hal ini diharapkan akan memunculkan kesadaran yang dapat merubah tingkah laku atau pola pikir seseorang agar dapat memperoleh obat yang efektif dan aman serta dapat menggunakan obat nyeri dengan baik dan benar.

Abstract

Jumlah AIDS di Jawa Tengah merupakan peringkat 3 terbanyak dari seluruh provinsi di Indonesia. Terapi yang digunakan untuk mencegah AIDS pada pasien HIV yaitu dengan pemberian ARV. Kepatuhan terapi ARV (antiretroviral) sangat penting karena berkaitan erat dengan Parameter keberhasilan terapi dan tingkat penularan dari penyakit HIV. Penelitian ini bertujuan untuk mengidentifikasi profil kepatuhan penggunaan obat antiretroviral pada pasien HIV/AIDS yang menjalani rawat jalan di sebuah rumah sakit umum Jawa Tengah. Jenis penelitian ini adalah cross-sectional dengan pengambilan data secara retrospektif dari rekam medis pasien yang memenuhi kriteria inklusi yaitu pasien dari pasien di sebuah rumah sakit Jawa Tengah yang nama pasien ditandai dengan inisial untuk menjaga kerahasiaan pasien. Ukuran kepatuhan dihitung dengan medication possession ratio (MPR). Untuk mengetahui pengaruh variabel lain (usia, jenis kelamin, pekerjaan, status pernikahan, pendidikan terakhir, dan regimen awal ARV) terhadap kepatuhan, dilakukan uji bivariat. Dari 96 pasien, jumlah pasien yang patuh sebesar 64,58% dengan sebagian besar regimen TDF+3TC+EFV 55,21%. Setelah dilakukan uji bivariat, variabel usia, jenis kelamin, pekerjaan, status pernikahan, pendidikan terakhir, dan regimen awal ARV tidak berpengaruh terhadap tingkat kepatuhan pasien.

Abstract

The incidence of HIV infections in Papua is currently on the rise, primarily due to restricted access to antiretroviral treatment and insufficient support and care. Additionally, there is a lack of awareness among residents in rural or mountainous areas about HIV/AIDS infection, as provided by healthcare professionals. Until now, research data have been scarce on the demographic characteristics, clinical features, and treatment approaches for HIV/AIDS patients in the interior regions of Papua. This data is crucial for devising strategies aimed at enhancing healthcare services for individuals affected by HIV/AIDS.Consequently, this study endeavors to gain insights into the demographic characteristics, clinical features, and treatment modalities for HIV/AIDS patients at the Regional General Hospital of the Central Papua Region. It employs an observational descriptive approach, utilizing medical records of HIV/AIDS patients who visited the Central Papua Regional Government Hospital in June 2023. The findings indicate that, based on demographic data, the majority of patients were, on average, ≤25 years old (60.8%), male (52.5%), married (56.7%), and the highest number of HIV/AIDS cases were among secondary education graduates (40.8%). The majority were employed (67.5%) with incomes below the minimum wage threshold (69.2%). All heterosexual patients exhibited behavioral risk factors, and 4.2% had comorbidities such as hypertension. Tuberculosis was the most common opportunistic infection (17.5%). The highest proportion of individuals with HIV/AIDS were in stage II (66.7%). The predominant antiretroviral treatment regimen was a fixed-dose combination (FDC) containing Tenofovir 300mg, Lamivudine 300mg, and Efavirenz 600mg. The duration of use of ARV therapy in people with HIV / AIDS is at most 1 month.

Abstract

The use of adjuvant endocrine therapy (AET) tamoxifen in the treatment of breast cancer has been associated with adverse drug reactions (ADRs). This study aims to assess the picture of ADRs in breast cancer patients receiving Tamoxifen Adjuvant Endocrine Therapy (AET) and the factors that influence it in Semarang private hospitals. This study was conducted with a cross-sectional design that was analyzed quantitatively and the sampling technique was carried out by purposive sampling. The inclusion criteria are adult breast cancer patients who receive Adjuvant Endocrine Therapy (AET) tamoxifen at least in 1 month, patients are willing to be respondents to fill out questionnaires by signing informed consent. Evaluation of the incidence of ADRs identified by the pharmacist. A total of 87 respondents experienced 18 types of ADRs. The three most common ADR events experienced by patients in the last 7 days were hormonal reactions to painful breasts (25.68%), hot or warm sensations in the upper body (chest, neck, face) (13.51%), vaginal dryness (10.14%). There is an incidence of ADRs in breast cancer patients receiving Adjuvant Endocrine Therapy (AET) tamoxifen. The three most common ADR events experienced by patients in the last 7 days were hormonal reactions to painful breasts (25.68%), hot or warm sensations in the upper body (chest, neck, face) (13.51%), vaginal dryness (10.14%). It is known that age, stage, and consumption of other drugs are not significantly associated with the incidence of ADR.

Abstract

ABSTRACT Drug Related Problems (DRPs) are treatment conditions that cause or have the potential to interfere with clinical outcomes and result in therapy failure. The use of prophylactic antibiotics often experiences inaccuracies that cause DRPs. The aim of this research was to determine the problems and causes of DRPs in the use of prophylactic antibiotics in clean orthopedic surgery patients using a retrospective approach. The inclusion criteria applied were clean orthopedic surgery patients aged >18 years. and exclusion criteria are patients with Diabetes Mellitus, immune diseases, and other infectious diseases and age > 60 years. The use of prophylactic antibiotics was analyzed based on the Regulation of the Minister of Health of the Republic of Indonesia concerning Guidelines for the Use of Antibiotics and then classified with PCNE V9.1. Of 264 medical records of orthopedic patients with clean surgery, the highest choice of prophylactic antibiotic therapy was ceftriaxone at 49.6%. The highest DRPs problems are related to safety issues (P2) 48.4% in the form of potentially harmful drugs occurring (P2.1), other problems (P3) 40% related to unnecessary drug administration (P3.1) and effectiveness 10.6% related to effects medication is not optimal (P1.2). The highest cause of DRPs, drug selection factor (C1) 67% (no drug indication (C1.2) = 36.7%, drug selection not in accordance with guidelines (C1.1) = 30.3%), drug use factor (C6) 24.9% in the form of an inappropriate administration time (C6.1), and dose selection factor (C3) 8.1% in the form of an inappropriate dose (C3.1). Keywords: Antibiotics, Prophylaxis, DRPs, PCNE, Orthopedics Surgery  ABSTRAK Drug Related Problems (DRPs) merupakan kondisi pengobatan yang menyebabkan atau berpotensi mengganggu hasil klinis hingga kegagalan terapi. Penggunaan antibiotik profilaksis seringkali mengalami ketidaktepatan yang menyebabkan terjadinya DRPs. Penelitian dilakukan bertujuan untuk mengetahui masalah dan penyebab terjadinya DRPs penggunaan antibiotik profilaksis pada pasien operasi bersih ortopedi dengan pendekatan retrospektif. Kriteria inklusi diterapkan yaitu pasien operasi bersih ortopedi yang usia >18 tahun. dan kriteria eksklusi adalah pasien dengan Diabetes Mellitus, penyakit imunitas, dan penyakit infeksi lainnya serta usia >60 tahun. Penggunaan antibiotik profilaksis dianalisis berdasarkan Peraturan Menteri Kesehatan Republik Indonesia tentang Pedoman Penggunaan Antibiotik kemudian diklasifikasikan dengan PCNE V9.1. Dari 264 rekam medik pasien ortopedi dengan tindakan operasi bersih, pemilihan terapi antibiotik profilaksis tertinggi adalah ceftriaxone sebesar 49,6%. Masalah DRPs tertinggi terkait masalah keamanan (P2) 48,4% berupa obat merugikan berpotensi terjadi (P2.1),  masalah lain (P3) 40% terkait pemberian obat yang tidak perlu (P3.1) dan efektivitas 10,6% terkait efek obat tidak optimal (P1.2). Penyebab DRPs tertinggi, faktor pemilihan obat (C1) 67% (tidak adanya indikasi obat (C1.2) = 36,7%, pemilihan obat yang tidak sesuai dengan pedoman (C1.1) = 30,3%),faktor penggunaan obat (C6) 24,9% berupa waktu pemberian yang tidak sesuai (C6.1), dan faktor pemilihan dosis (C3) 8,1% berupa dosis yang tidak tepat (C3.1). Kata Kunci: Antibiotik, Profilaksis, DRPs, PCNE, Operasi Ortopedi

Abstract

Penelitian terkait penggunaan obat off label di Indonesia mayoritas terfokus pada pasien pediatri daripada pasien obstetri dan ginekologi. Padahal penggunaan off label pada pasien obstetri dan ginekologi perlu mendapat perhatian khusus karena perlu memperhatikan banyak aspek kondisi pasien. Pada kondisi hamil umumnya obat-obatan dapat melintasi plasenta sehingga dapat memengaruhi kondisi janin (Wibowo et al. 2021).  Penelitian di Inggris menemukan mayoritas resep yang digunakan secara off label beresiko tinggi bagi pasien dan dokter yang meresepkan (Herring, McManus, and Weeks 2010). Kemungkinan terjadinya pravalensi tinggi terkait penggunaan obat-obat off label pada pasien obstetri dan ginekologi sehingga perlu dilakukan penelitian terkait  persepsi dan kecenderungan keterlibatan apoteker pada peresepan obat - obat off label pada pasien obstetri dan ginekologi. Penelitian ini bertujuan untuk mengembangkan alat evaluasi terhadap persepsi dan kecenderungan apoteker dalam peresepan obat-obat off label pada pasien obstetri dan ginekologi, dalam hal ini berupa kuesioner. Kuesioner  dikembangkan berdasarkan beberapa aspek yang ditemukan dalam kajian pustaka terpublikasi. Uji validitas rupa dan uji validitas konten kuesioner dilakukan dengan penilaian expert judgement, sedangkan uji validasi konstruk dilakukan dengan uji korelasi. Uji reliabilitas kuesioner dilakukan dengan menggunakan nilai Cronbach’s Alpha. Penelitian ini menghasilkan kuesioner yang terdiri dari 46 item pernyataan yang dibagi menjadi beberapa variasi. Setelah melalui beberapa uji validitas dan reliabilitas didapatkan kuesioner yang valid dan reliabel dengan nilai R Corrected item-Total Correlation > 0,200 dari masing-masing pernyataan, diperoleh nilai Cronbach’s Alpha sebesar 0,906 sehingga dapat digunakan dalam pengujian selanjutnya.Kata kunci: Off label, Obstetri dan Ginekologi, Persepsi, Apoteker

Abstract

Intravenous admixtures are sterile solutions prepared by mixing one or more different drugs with a suitable solvent and given to patients via the intravenous route. One of the factors causing contamination of the intravenous preparation mixture is an error in the aseptic technique during the preparation phase. Errors in aseptic techniques can cause infection. Newborns (neonates) are children under the age of 28 days. During 28 days of birth, have the highest risk of dying. The rate of potential adverse drug events (ADE) is three times higher in children than adults and substantially higher in neonates receiving intensive care. Intravenous admixture therapy, often performed in general wards, is highly susceptible to bacterial contamination which can result from aseptic technique errors. Bacterial contamination in intravenous preparations can cause infection which can trigger septic shock to death. The purpose of this study was to evaluate the suitability of the mixing process and the process of administering intravenous admixture drugs to infant patients. This study was conducted on infant or neonatal patients who were hospitalized in general wards and special intensive wards for babies in hospitals. Dr. Moewardi Surakarta. The research took place from February to March 2020 and was carried out using a prospective observational method. The inclusion criteria for this study were patients receiving parenteral drug therapy with complete medical records. Analysis of drug miscibility/insolubility was carried out using Handbook an Injectable Drugs edition 17 as the main reference and ASHP Drug information handbook 2021 edition. The results of this study, the percentage of solubility of injectable drugs that dissolved was 44 times administration (36.7%). For the administration of insoluble drugs 3 times (2.5%) and 73 (60.8%) administrations there was no information regarding the solubility of the drugs given. The study concludes that although there is incompatibility potential information references, at the direct observation was no incompatibility found. The administration of the two potentially insoluble drugs was carried out with different syringe containers.

Abstract

Background: Drug interactions represent a significant medication safety concern among pediatric inpatients. Pediatric patients possess distinct pharmacokinetic characteristics compared to adults, rendering them more vulnerable to adverse effects from concurrent drug administration. Objective: This study aimed to analyze potential drug-drug interactions (DDIs) in the prescriptions of pediatric inpatients at Dr. H. Andi Abdurrahman Noor Regional Hospital, Tanah Bumbu Regency. Method: A descriptive cross-sectional study with retrospective data collection was conducted on 143 patients meeting the inclusion criteria from a total of 221 patients during the period of September–November 2025. Drug interaction identification was performed using the drugs.com database. Results: Results showed that of the 143 analyzed prescriptions, 81 (56.6%) contained potential drug interactions, comprising 27 interacting drug pairs and a total of 107 interaction events. By mechanism, pharmacokinetic interactions predominated at 55.1%, followed by pharmacodynamic interactions at 44.9%. By severity, moderate interactions were most prevalent (54.2%), followed by minor (36.4%) and major (9.3%). The most frequently interacting drug pairs were paracetamol–ranitidine (18.7%), cefotaxime–gentamicin (13.1%), and diazepam–valproic acid (8.4%). The most clinically significant major interactions included diazepam–phenobarbital, gentamicin–furosemide, furosemide–propranolol, and dexamethasone–phenobarbital.Conclusion: These findings highlight the high prevalence of potential DDIs in pediatric inpatients and underscore the necessity for rigorous therapeutic monitoring and active clinical pharmacist involvement in the prescribing process.

Abstract

ABSTRACT Drug-Related Problems (DRPs) are a common issue in the treatment of childhood tuberculosis due to the complexity of treatment regimens, the long duration of therapy, and the immature physiological condition of children. The occurrence of DRPs has the potential to reduce treatment success and increase the risk of side effects and drug resistance. Objective: To systematically review the incidence, types, severity, and risk factors of DRPs in pediatric patients with tuberculosis. Methods: This systematic review was compiled based on the Preferred Reporting Items For Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A literature search was conducted thru the PubMed, ScienceDirect, and Google Scholar databases for publications from the past 15 years. The included studies involved pediatric patients aged 0–18 years and reported data related to DRPs, primarily Adverse Drug Reactions (ADRs). Data analysis was conducted descriptively and narratively. Results: A total of 6 studies met the inclusion criteria. The prevalence of DRPs in children with tuberculosis ranges from 13.2% to 44.7%. The most frequently reported type of DRP is Adverse Drug Reactions (ADRs), particularly gastrointestinal disorders and hepatotoxicity. Most DRPs are classified as mild to moderate. The main risk factors for DRP include poor nutritional status, lack of patient or parent education, high-dose use without individual adjustment, and the use of combination drugs. Some studies report that educational interventions can improve adherence and reduce the incidence of DRP. Conclusion: Drug-related problems (DRPs) in children remain a serious challenge in the treatment of tuberculosis in children. Increased therapy monitoring, patient and family education, and clinical pharmacist involvement are essential for effectively preventing and managing DRPs to improve therapeutic outcomes. Keywords: Tuberculosis, Children, Drug-Related Problems, Adverse Drug Reactions, Anti-Tuberculosis Drugs.  ABSTRAK Drug-Related Problems (DRPs) merupakan permasalahan yang sering terjadi pada terapi tuberkulosis anak akibat kompleksitas regimen pengobatan, durasi terapi panjang, dan kondisi fisiologis anak yang masih berkembang. Kejadian DRPs berpotensi menurunkan keberhasilan terapi dan meningkatkan risiko efek samping serta resistensi obat. Tujuan: Meninjau secara sistematis kejadian, jenis, tingkat keparahan, dan faktor risiko DRPs pada pasien anak dengan tuberkulosis.  berpotensi menurunkan keberhasilan terapi. Metode:Tinjauan sistematis ini disusun berdasarkan pedoman Preferred Reporting Items For Systematic Reviews and Meta-Analyses (PRISMA). Pencarian literatur dilakukan melalui basis data PubMed, ScienceDirect, dan Google Scholar untuk 15 tahun terakhir. Artikel yang disertakan melibatkan pasien anak berusia 0–18 tahun dan melaporkan data terkait DRPs, terutama adverse drug reactions (ADR). Analisis data dilakukan secara deskriptif naratif. Hasil: Sebanyak 6 artikel memenuhi kriteria inklusi. Prevalensi DRPs pada anak dengan tuberkulosis berkisar antara 13,2% hingga 44,7%. Jenis DRPs yang sering dilaporkan yaitu Adverse Drug Reactions (ADR) terutama gangguan saluran cerna dan hepatotoksisitas. Sebagian besar DRPs diklasifikasikan sebagai ringan hingga sedang. Faktor risiko utama DRPs meliputi status gizi buruk, kurangnya edukasi pasien atau orang tua, penggunaan dosis tinggi tanpa penyesuaian individual, dan terapi kombinasi obat. Beberapa artikel melaporkan bahwa intervensi edukatif dapat meningkatkan kepatuhan dan menurunkan kejadian DRPs. Kesimpulan:Drug-Related Problems (DRPs) pada anak masih menjadi tantangan serius dalam terapi tuberkulosis pada anak. Peningkatan pemantauan terapi, edukasi pasien dan keluarga, serta keterlibatan apoteker klinis sangat diperlukan untuk mencegah dan mengelola DRPs secara efektif guna meningkatkan keberhasilan terapi.  Kata Kunci: Tuberkulosis, Anak, Drug-Related Problems, Adverse Drug Reactions, Obat Anti-Tuberkulosis.
