Abstract

One of the most critical issues affecting older adults is the increasing use of potentially inappropriate medication (PIMs). The use of PIMs is associated with drug-related problems, as well as undesirable drug effects. This study aimed to assess the prevalence of potentially inappropriate medication and the most commonly prescribed PIMs category in older adults. This research method is observational with a cross-sectional design. Drugs were analyzed using the 2019 Beers criteria compiled by The American Geriatrics Society (AGS), classifying PIMs into five categories. The inclusion criteria in this study were patients aged ≥65 years, using at least one drug, with a duration of drug use of at least one month, and analyzing this research data descriptively. The results showed that out of 134 prescriptions consisting of 773 drug items, 95 drug items were potentially inappropriate for use in older adults. The prevalence of PIMs was 12%, and if classified by category, then partly in order, inappropriate medication included category 1 (8.2%); category 2 (0.6%); category 3 (2.8%); category 4 (0.3%); category 5 (0.1%). The most common drugs included in category 1 PIMs criteria were proton pump inhibitors (PPIs), namely lansoprazole and omeprazole. Conclusion: In this study, the prevalence of potentially inappropriate Medication in older adults was 95 drug items (12%), and the more commonly prescribed category of PIMs is category one drugs and should avoid category one drugs in older adults.

Abstract

Kelompok Wanita Tani (KWT) Sri Rejeki in Cabeyan Village, Sukoharjo, is a community of women actively engaged in plant cultivation and the production of instant herbal products such as ginger and temulawak (curcuma). However, their product marketing activities are still limited to conventional methods and have not yet reached digital marketing platforms. This community service activity aimed to enhance the knowledge and skills of KWT members in digital product marketing through educational sessions and practical training. The methods used included interactive lectures, group discussions, and hands-on practice related to the use of social media and e-commerce platforms. The educational materials covered product marketing and basic understanding of Break Even Point (BEP) calculation. A total of 18 active KWT members and 5 teenagers participated in this activity. The participants' knowledge test results before (pre-test) and after (post-test) the educational session showed an increase in the average knowledge level, from the fair category (63.77%) to the good category (97.10%). KWT members began to understand the importance of market segmentation, slogan development, and the use of digital platforms such as WhatsApp Business, Instagram, and Shopee. This activity demonstrated that participatory training could equip participants with practical skills to improve their product marketing strategies. It is recommended that an internal marketing team be formed and continuous mentoring provided to optimize digital marketing efforts and product legality aspects such as PIRT (Home Industry Food Permit) and halal certification.

Abstract

The use of the docetaxel-doxorubicin regimen in breast cancer treatment has been associated with Adverse Drug Reactions (ADRs). This study aims to analyze the occurrence of ADRs and their severity in breast cancer patients undergoing therapy with the docetaxel-doxorubicin regimen at the hospital. The research design employed a quantitative cross-sectional approach, and purposive sampling was used as the sampling technique. Six primary Adverse Drug Reactions (ADRs) were identified among breast cancer patients undergoing chemotherapy with the docetaxel-doxorubicin regimen: nausea (21,32%), anemia (13,71%), peripheral neuropathy (7,45%), alopecia (23,01%), cardiac disorders (7,11%), and anorexia (9.81%). Breast cancer patients treated with the docetaxel doxorubicin regimen experienced various levels of ADR severity. Among the 591 adverse drug reaction (ADR) incidents recorded, the majority (55.6%) were classified as mild (severity level 1), with nausea reported as the most common side effect. At severity level 2 (moderate), alopecia was predominant, while at severity level 3 (severe), anemia was the most common reaction. There were no ADR incidents reported at severity levels 4 and 5

Abstract

Background: Despite available treatment options, diabetes mellitus (DM) management faces significant barriers affecting patient outcomes. While most research focuses on patient factors, limited attention has been given to healthcare provider perspectives, particularly hospital pharmacists who are key stakeholders in diabetes care. Objective: This study explored treatment barriers and solutions from hospital pharmacists’ perspectives in Indonesia. Methods: This qualitative phenomenological study involved 18 hospital pharmacists at Cilacap Regional General Hospital, Central Java, Indonesia. Participants were selected through purposive sampling and had minimum one-year experience in diabetes patient care. Semi-structured interviews were conducted from January to February 2025, audio-recorded, transcribed verbatim in Bahasa Indonesia, and analyzed using thematic analysis. All interviews were conducted in Bahasa Indonesia and participant quotes were translated to English while preserving original meaning and cultural context. Results: Analysis of three predetermined barrier themes revealed: adverse drug reactions (ADRs) (particularly metformin-related gastrointestinal effects and insulin-associated hypoglycemia), medication non-adherence (influenced by feeling healed, treatment boredom, age-related denial, and therapy burden), and monthly follow-up challenges (geographic barriers, transportation difficulties, physical limitations, and insufficient family support). Pharmacists proposed comprehensive solutions including patient and family education, interprofessional collaboration, technology integration through WhatsApp support, and flexible monitoring approaches. Conclusion: Pharmacist-identified barriers operate as interconnected system components requiring multi-level interventions. Evidence-based solutions emphasizing education, collaboration, family engagement, and technological support can enhance diabetes management outcomes while respecting cultural contexts and resource constraints.

Abstract

Background: Poor medication adherence is common among hemodialysis patients and is associated with adverse clinical outcomes. However, factors such as health-related quality of life (QoL) and medication beliefs remain underexplored as determinants of non-adherence in this population. Objective: This systematic review aims to synthesise the evidence regarding the relationship between QoL, medication beliefs, and pharmacological non-adherence in patients undergoing hemodialysis (HD). Methods: A comprehensive literature search was conducted using electronic databases (PubMed, Scopus, and Google Scholar) for observational studies published between January 2015 and July 2, 2025. Studies were included if they involved adult hemodialysis patients and examined QoL and/or medication beliefs concerning pharmacological non-adherence. Results: Six observational studies, encompassing 1,698 hemodialysis patients from various countries, met the inclusion criteria. The synthesised evidence indicated that poorer QoL (particularly depression and poor sleep quality), higher burden of physical symptoms, and specific medication beliefs (e.g., concerns about side effects and low perceived necessity of medications) were significantly associated with increased non-adherence. Conclusion: Non-adherence among hemodialysis patients is influenced by the complex interaction of psychosocial factors, including QoL and individual medication beliefs. To improve adherence, interventions should be developed to target these factors through health education, psychological support, and patient-centred care. Further integrative and contextual research is needed to strengthen understanding and develop more effective interventions.
