Abstract

Background: No drug with a liver antifibrotic effect for treating non-alcoholic fatty liver disease (NAFLD) has been approved. Physalis angulata Linn., better known to Indonesian as ciplukan, has natural abilities in various metabolic and inflammatory diseases. This study aims to determine the effect of ethyl acetate fraction of P. angulata in the NAFLD rat model by examining alanine aminotransferase (ALT), cholesterol levels, and liver histopathological features, which are methods to evaluate the course of the disease and the potential antifibrotic effect.Method: This research is an in vivo study on male Wistar rats conducted at the Animal Laboratory, Faculty of Medicine, Universitas Padjadjaran, from September to November 2020. Rats were grouped randomly into seven groups of 5 each. The NAFLD models were created by giving a diet containing 20% margarine for four weeks. The intervention groups were given vitamin E, ethyl acetate fraction of P. angulata, and both combinations. The statistical analysis examined differences in each group based on their histopathological features, ALT, and cholesterol levels. Results: Histopathological results in the group given P. angulata at a dose of 0.32 mg resembled normal liver, and the ALT level was similar to vitamin E. The administration of P. angulata at 0.16 mg dose improved cholesterol levels. Conclusions: P. angulata ethyl acetate fraction at a dose of 0.32 mg improved the histopathological and serum ALT levels in the NAFLD rat model, which could be the basis for the mechanism of P. angulata's antifibrotic ability in NAFLD conditions.

Abstract

Such occurrences are relatively common in Indonesia’s healthcare system, where doctors, nurses, and other medical personnel often act beyond their prescribed authority. Various factors, including lack of awareness, insufficient training, and the pressure to complete tasks quickly, may contribute to these actions. The purpose of this paper is to analyze the legal repercussions for healthcare professionals who exceed their authority and to provide a deeper understanding of the regulations governing the professional authority of healthcare providers in Indonesia. It is hoped that this article will offer valuable insights and enhance healthcare professionals’ understanding of the importance of adhering to legal guidelines in medical practice.

Abstract

The misuse of authority by law enforcement officers in handling public demonstrations remains a recurring issue within Indonesia’s legal enforcement practices. Such incidents are frequently manifested through arbitrary arrests, excessive use of force, and the criminalization of peaceful protesters. In fact, the right to express opinions publicly is explicitly guaranteed under the 1945 Constitution and Law No. 9 of 1998. Repressive actions of this nature not only constitute legal violations but also erode public trust in the police institution. From a criminal law perspective, officers who exceed their authority may be held accountable under provisions of the Indonesian Penal Code, such as Article 170 or 351 on acts of violence, as well as through internal disciplinary sanctions. This study employs both normative and empirical approaches, examining recent cases including the May Day 2025 demonstrations and the Kanjuruhan tragedy to highlight weaknesses in oversight mechanisms, both internal and external. Findings indicate that a culture of impunity persists, allowing officers to often escape criminal liability despite evidence of misconduct. Therefore, comprehensive reforms are urgently needed, including strengthening oversight systems, ensuring consistent law enforcement, and fostering cultural transformation within the police force to ensure accountability and justice are genuinely upheld.

Abstract

Abstract

Objective: To identify auto-antibodies in systemic sclerosis with interstitial lung disease (ILD). Method: This was a descriptive categorical study on auto-antibody profile in systemic sclerosis patients visiting the Rheumatology Clinic of Dr. Hasan Sadikin General Hospital, West Java, and Bandung during the period of January 2018 to December 2019 who were registered in the West Java Systemic Sclerosis Registry. Auto-antibody identification was performed using the Euroline immunoblot assays. Results: Thirty six cases were identified during the study period with most of the cases involved women (n=35, 97.2%). The average age of patients participating in this study was 40 years, with an average duration of disease of 18 months. Diffuse cutaneous systemic sclerosis was found in 22 (61.1%) cases and limited cutaneous systemic sclerosis was observed in 14 (38.9%) cases. Specific autoantibodies were positive in 33 (91.6%) cases, with anti-topoisomerase I as the largest group, positive in 22 (52.9.3%) cases. This was followed by anti-Th/To in eight (15.7%) cases; anti-Ro52 in four (7.8%) cases; anti-centromere in three (5.9%) cases; anti-RNA polymerase in three (5.9%) cases; anti-fibrillarin in three (5.9%) cases; anti-Ku in two (3.9%) cases; and anti-PDGF in one (2.0%) case. High-resolution computed tomography of the lung showed 34 (94.4%) cases with ILD and 22 (61.1%) cases with severe lung fibrosis. Usual interstitial pneumonia was seen in 19 (52.8%) cases and non-specific interstitial pneumonia in 15 (41.7%) cases. Conclusion: Anti-topoisomerase I, anti-Th/To, and anti-Ro52 are the most common autoantibodies observed in systemic sclerosis patients with ILD as the most prevalent feature detected with lung HRCT.

Abstract

Background: Systemic Sclerosis (SSc) or Scleroderma is a systemic autoimmune rheumatic disease characterized by dysregulation of the immune system, fibrosis of the skin and visceral organs and vasculopathy. The global prevalence of SSc reaches 17.6 per 100,000 population and the incidence reaches 1.4 per 100,000 people per year. The prevalence and incidence of SSc in Asia reaches 6.8 per 100,000 and 0.9 per 100,000, which is lower than other continents. Women have a prevalence and incidence of SSc five times higher than men. The prevalence and incidence of SSc depends greatly on the results of the study. There has not been much research on the epidemiological studies of SSc in Indonesia. Methods: Twelve selected rheumatologists from the Indonesian Rheumatologist Association (IRA) made recommendations based on key questions formed by a steering committee from IRA. These recommendation materials were taken from several online databases such as PubMed, Science Direct, and Google Scholar. Level of evidence and grades of recommendation were then assigned, and every member of the panelist team will assign a score for level of agreement. Results: A total of 50 recommendations that discussed the diagnosis, pharmacological and non- pharmacological therapy, and prognosis for SSc were made. Conclusion: These recommendations can be used to help clinicians correctly diagnose and choose the most suitable therapy for SSc patients. All recommendation statements were created with the consideration of Indonesia’s clinical setting, facility, and drug availability.
