Background: Progression and expansion of skin fibrosis are the most important characteristics in determining clinical responses and prognosis of Systemic Sclerosis(SSc). Using modified Rodnan skin score (mRSS) can not rapidly detect a slight changes of skin fibrosis in SSc patients. Biomarker assessment is needed to make a more objective, quantitative and rapid evaluation of the changes. Suggested potential useful biomarker isB-cell Activating Factor (BAFF), a positive regulator of B cell survival and maturation process. This study aimed to evaluate correlation between skin fibrosis based on mRSS and BAFF serum in SSc patients.Methods: We used cross sectional methods. Enrolled all patients who met ACR EULAR 2013 criteria for SSc in Rheumatology Clinic Hasan Sadikin Hospital, Bandung, from November 2015 to March 2016. Subjects underwent medical record review, physical examination,mRSS measurement by rheumathologist, and blood tests. Data were analized using Rank-Spearman Correlation.Results: Thirty seven subjects, with mean age 40+10 years old. Subjects consisted of 23(62.2%) limited SSc and 14(37.8%) diffuse SSc. Mean BAFF serum was 1160.2+424.7 pg/mL, no statistical difference were found between limited and diffuse type (p=0.662).Median mRSS results was 16 ranged from 2 to 36.Correlation between mRSS and BAFF serum was not significant (r=0.077; p=0.326).Conclusion: There is no correlation between mRSS and BAFF serum in systemic sclerosis at Hasan Sadikin Hospital.Keywords: mRSS, BAFF, Systemic Sclerosis

Background: Osteoarthritis (OA) is the most common musculoskeletal disease. World Health Organization (WHO) estimates that 10% of the aged over 60 year population have this disease. The aim of OA treatment is to reduce pain, which is the most OA patients chiefcomplaint. People in Indonesia are very interested in use of herbal therapies from original traditional plant to treat pain now, one of the traditional plants that are known havea benefit is Phaleria macrocarpa or Mahkota Dewa fruit. Phaleria macrocarpa has been shown to decrease the degree of inflammation of OA animal model experiments. In order to know what is the effect of this fruit extract to reduce degrees of pain and change the levels of IL-1, IL-6, TNF-α in the blood as the marker of inflammation of patients with knee OA, and what is the effects to liver, kidney and haematology in the Indonesian population hasnot been investigated Methods: The research method is an experimental study and the research design is PROBE (Prospective Randomized Open End Blinded Evaluation), to evaluate the efficacy of the extract Mahkota Dewa fruit (Phaleria macrocarpa fructus) 330 mg (Super Mahkota POMTR 053 345 491) compared to meloxicam 7.5 mg in patients with knee osteoarthritis. The study population was outpatients with knee OA at Rheumatology Clinic Dr Hasan Sadikin Hospital Bandung. Patients are given Phaleria macrocarpa 330 mg or meloxicam 7.5 mg once a day for 14 days. Observations were made to evaluate the degree of pain as measured by VAS and Lequesne index at day 0, day 14 and measured again at day 28,after they are not taking the extract anymore.Results: Phaleria macrocarpa 330 mg is equal to meloxicam 7.5 mg in reducing the degree of pain as measured by VAS (p=0.78) and the Lequesne index (p=0.51). Our finding, t here is no effect of decreasing the proinflammatory cytokine IL-1 (p=0.72), IL-6(p=0.53) and TNF-α (p=0.07) in the blood of both groups. Safety analysis shown that this extract is safe for consumption.Conclusions: Phaleria macrocarpa 330 mg equal to meloxicam 7.5 mg in reducing the degree of pain however there is no effect on reducing proinflammatory cytokines in the blood of OA patients who had received therapy for 14 days in both groups. There is no adverse effects found on hematological, liver function and kidney function after consumption this plantâs fruit extract.Keywords: osteoarthritis, mahkota dewa, phaleria macrocarpa

Background: Vitamin D is an immune-regulator that influences the neuro-inflammation process in schizophrenia. The study aimed to explore the vitamin D serum level in schizophrenic patients at the outpatient clinic of the Provincial Mental Hospital, West Java, Indonesia.Methods: A quantitative descriptive study was conducted in November-December 2019 using secondary data of all-male schizophrenic patients at the Provincial Mental Hospital. Data collected were demographic characteristics, clinical characteristics, and vitamin D Level. Scoring was then performed using Positive and Negative Syndrome Scale (PANSS) and Montreal Cognitive Assessment (MoCA) to assess the positive and negative symptoms as well as the cognitive symptoms based on the serum vitamin D levels.Result: All male schizophrenic patients had a low serum vitamin D level (mean16.67±5.6ng/ml) with 64.52% of them had vitamin D deficiency (<20 ng/ml). Interestingly, patients with vitamin D deficiency had a negative symptom (mean22.1±5.6ng/ml) whereas those with vitamin D insufficiency (20-29 ng/ml) had a positive symptom (mean 18.55±5.6ng/ml). Furthermore, patients with vitamin D deficiency had a lower impaired cognitive function value (mean 19.7±3.4ng/ml) compared to those with vitamin D insufficiency.Conclusions: A low vitamin D serum level in schizophrenic patients may play a role in the pathogenesis of this disorder. Further studies are needed to confirm this finding.

Background: Food safety is defined as the confidence level that food will not cause sickness or harm to consumer when it is prepared, served, and eaten according to its intended use. This study was conducted to describe the pattern of knowledge, attitude, and practice on food safety among high school students in Jatinangor.Methods: One hundred and two respondents were involved in this study. The data were collected from the high school students in Jatinangor, West Java from August to November 2013 through the methods of questionnaire after the informed consent was obtained from the respondents. The questionnaire was translated from the Five Keys to Safer Food Manual by World Health Organization. The collected data were then presented in percentage.Results: Overall, 60.78% of respondents had moderate knowledge, 85.29% had good attitude, and 71.57% had good practice in food safety. Among the five main points on food safety in all aspects, most of the respondents were unaware of the safe temperature to store food; hence, they did not practice it.Conclusions: Even though most of the students have the knowledge, attitude, and practice on food safety, a need for relevant and motivating education should be performed for spesific indicators of food safety. Background: Studies have shown that muscle strength will decline with aging and inactivity, with 1% to 3% loss of muscle strength each day. Nevertheless, different lifestyles have different effect on muscle strength. This study was conducted to analyze the differences in quadriceps and hamstring muscle strength in elderly from 60–79 years olds according to lifestyle.Methods: A cross-sectional study was conducted from September to November 2015. The subjects were 31 older persons from community who voluntarily participated the research procedure and were divided into 2 groups, active group and sedentary group according to International Physical Activity Questionnaire (IPAQ). Quadriceps and hamstring muscle strengths were measured using the push and pull dynamometer. The collected data were analyzed using the Wilcoxon rank sum test and the unpaired t-test. Results: This study discovered that the mean difference in hamstring muscle strength was 6.09 ± 0.70, p< 0.001; median difference in Quadriceps muscle strength was 5.00 (13.00, 6.50), p<0.001. Conclusions: There is a higher Quadriceps and Hamstring muscle strength in elderly active lifestyle compared to elderly sedentary lifestyle.

Background: Knee osteoarthritis (OA) is a degenerative disease of the knee joints characterized by progressive softening and disintegration of articular cartilage. In OA, which is influenced by several risk factors, free radicals are increased by local ischemia in the cartilage. As an exogenous antioxidant, vitamin C also plays an important role in collagen and glycosaminoglycan synthesis. This study was carried out to identify vitamin C intake as well as risk factors in knee OA. Methods: The study population was determined by non-probability sampling with convenient approach to knee OA patients at the Rheumatology Outpatient Clinic of Dr. Hasan Sadikin General Hospital in October–November 2013. Data were obtained through questionnaire interview about risk factors, severity index of OA and vitamin C intake profile. Data presentation was conducted by descriptive method.Results: There were 47 patients diagnosed with knee OA in the Rheumatology Outpatient Clinic. The result showed that 7 patients (14.9%) had low vitamin C intake for the last 3 months. There were 30 patients with family history of OA (63.8%). Thirty two patients (68.1%) were passive smokers, 44 patients (93.6%) had history of repeated use of knee joints and majority of respondents had obesity. Conclusions: Most of the subjects have sufficient vitamin C intake and more than half have risk factors that may contribute to the incidence of knee OA. DOI: 10.15850/amj.v4n2.1067

Background: Systemic sclerosis (SSc) is a multisystem autoimmune illness with a wide range of clinical symptoms. The pulmonary organ manifestations frequently occur, but the symptoms are non-specific. Radiological examination and pulmonary function tests (spirometry) are needed to detect lung abnormalities in SSc patients. This study aimed to obtain information about the overview of lung abnormalities in SSc patients through spirometry, chest x-ray, and high-resolution computed tomography (HRCT) scan examination.Methods: A descriptive qualitative study was conducted on 75 SSc patients registered in Dr. Hasan Sadikin General Hospital Bandung from January 2019 to December 2020. Data were collected with a total sampling method and presented in proportions and percentages.Results: The majority of subjects were affected by cutaneous 73 (97%), pulmonary 29 (39%), and musculoskeletal 17 (23%) involvement. Spirometry revealed that 43 subjects (57%) had restrictive lung disease, with one false-positive case and two false-negative cases. On a chest x-ray, 45 (60%) of subjects had abnormalities. The majority of subjects were found to have Ground-glass opacities on HRCT scans. Ground-glass opacities were discovered in 46 subjects (82%) and 27 subjects (60%) were identified as having severe fibrosis scores.Conclusion: According to spirometry results and abnormalities on chest x-ray and HRCT scans, the majority of SSc patients have restrictive lung disease.

It has become a habit of self-medication with non-prescription drugs done by most Indonesians. In the Regulation of the Minister of Health No. 924 / MENKES / PER / X / 1993 Regarding the Medication List of Pharmacies regulates certain hard drugs that pharmacists can give to people without a prescription. The purpose of the publication of Permenkes is to swamedikasi, a person can self- edicate rationally and supported by the pharmacy's mandatory drugs, although in general pharmacists can not sell hard drugs without a prescription, but there are certain hard drugs based on Permenkes may be sold without a doctor's prescription . The issue raised is how medikolegal swamedikasi in the perspective of healing disease? as well as how are the barriers and medicolegal swamedikasi solutions in the perspective of curing diseases? swamedikasi most places done in pharmacies, knowing drugs purchased through news media such as advertising the reason is the first attempt before treatment to health care facilities, swamedikasi actors get an explanation of doses and how to drink by pharmacist / clerk / drug seller, surveillance of swamedikasi drug use carried out with how to monitor / inspect pharmacies, the results obtained are frequent pharmacists are not in place at open pharmacy hours, selling OWA (Pharmacy Drug Compensation) without the presence of pharmacists and administrative violations. The obstacles are not yet optimal supervision from the government solution with counseling about the rule of law of drug delivery, the second barrier of the lack of awareness of drug sellers in pharmacies that do not fit the provisions of the solution by giving counseling to the owner of pharmacy facilities so as not to pursue Profit Oriented but pay attention to Patient Oriented, the third obstacle of lack of knowledge the swamediciator of the drug classes that the solution can buy is the government to increase socialization or counseling to the community about proper and responsible swamedication.

Evil is not reduced but is increasing, this is not exactly user policy and laws, if the law itself becomes a factor kriminogen and viktimogen. Imprisonment sanction policies relating to drug abuse for yourself stipulated in Article 127 paragraph (1) letter a, b, c of Law 35 of 2009 and against the abuse of psychotropic substances for selfregulated in Article 60 paragraph (5) of the Constitution of Number 5 of 1997, using the drug for themselves to have the intent that drug use is done without a doctor's supervision. Drug abusers for themselves viewed as criminals and victims, therefore, in formulating sanctions to be applied against drug abusers for themselves should be pursued through rational policy. In the prevention of drug abuse for yourself, where criminal sanctions are still needed, although it focuses on the repressive nature of which is after the crime occurred. However, criminal sanctions is an effort to substitute the latter when other efforts have been inadequate.

In providing legal assurance and protection for both health service providers or doctors and the receivers of health services or patients, a set of laws is required to be the sign for them. How is the legal resolution when there is a alleged malpractice performed by a doctor ? The Indonesian Medical Disciplinary Board is the authorized institution to determine the presence of error (alleged malpractice) performed by a doctor in applying the medical science and decide the sanction. In terms of the negligence performed by medical practitioners in their profession, it must be settled first through mediation. In this case, the health law and the doctrine of health services is as the norm of Lex spesialis.
