Abstract

Kanker nasofaring adalah tumor ganas dari sel-sel epitel yang melapisi nasofaring. Pada tahun 2020, kanker nasofaring menempati urutan ke-5 di dunia. Tujuan penelitian ini adalah untuk mengetahui profil klinikopatologi kasus karsinoma nasofaring di RSUD Dr. Saiful Anwar dari tahun 2018 hingga 2020.  Penelitian ini adalah penelitian deskriptif analitik dengan pendekatan cross sectional untuk mengetahui jumlah kasus karsinoma nasofaring di RSUD Dr. Saiful Anwar dari tahun 2018 hingga 2020. Data yang dikumpulkan berupa spesimen biopsi kanker nasofaring dari laboratorium patologi anatomi meliputi jenis kelamin, umur, dan tipe histopatologi. Terdapat 557 kasus dari biopsi organ nasofaring yang terdiri dari 282 kasus ganas, 21 kasus suspek ganas, dan 216 kasus non-ganas. Kasus keganasan nasofaring pada laki-laki sebesar 69,86%. Jumlah kasus tertinggi terdapat pada rentang usia lebih dari 50 tahun. Tipe histopatologi yang paling banyak adalah karsinoma tidak berdiferensiasi (undifferentiated carcinoma) sebanyak 244 kasus.  Dapat disimpulkan bahwa kasus karsinoma nasofaring masih cukup tinggi di RSUD Dr. Saiful Anwar Malang dengan profil klinikopatologi yaitu mayoritas pasiennya adalah laki-laki, usia tua, dengan tipe histopatologi karsinoma tidak berdiferensiasi.

Abstract

ABSTRAK Latar belakang Astrositoma adalah tumor dari sel astrosit dengan variasi derajat histopatologik, disertai ukuran edema peritumoral. Peningkatan derajat keganasan, ukuran tumor dan pembentukan edema peritumoral diperlukan proses angiogenesis. Vascular Endothelial Growth Factor (VEGF) adalah salah satu faktor penting dalam proses angiogenesis. VEGF diperlukan dalam proliferasi, survival dan migrasi sel endotel dan permiabilitas vaskuler. Peranan VEGF dalam pembentukan edema peritumoral masih dalam perdebatan. Penelitian ini bertujuan untuk menganalisis hubungan antara pola ekspresi VEGF dan peningkatan derajat histopatologik, ukuran tumor dan insiden edema peritumoral pada astrositoma. Metode Arsip histopatologik astrositoma derajat II, III, IV (Glioblastoma) dari RSUD Dr. Soetomo Surabaya selama kurun waktu Januari 2009 hingga Januari 2012 dikumpulkan. Pada tiga puluh kasus astrositoma yang memenuhi kriteria inklusi dilakukan pemeriksaan imunohistokimia dengan VEGF Rabbit. Hubungan pola ekspresi VEGF dan derajat histopatologik, ukuran tumor dan kejadian edema peritumoral dianalisis menggunakan uji Spearman. Hasil Sebanyak 29 dari 30 kasus menunjukkan ekspresi VEGF positif. Dari 29 kasus dengan ekspresi VEGF positif tersebut, 13 kasus adalah astrositoma derajat IV, 22 kasus dengan ukuran tumor > 2cm, dan 17 kasus disertai edema peritumoral. Analisis statistik ekspresi VEGF memiliki pengaruh terhadap peningkatan derajat histopatologik (p=0,023), rs=0,413) dan ukuran tumor (p=0,005, rs= 0,499) namun tidak berpengaruh terhadap kejadian edema peritumoral (p=0,273). Kesimpulan Penelitian ini mengungkapkan bahwa ekspresi VEGF dapat meningkatkan derajat histopatologik dan besar tumor, namun tidak berpengaruh terhadap pembentukan edema peritumoral pada astrositoma. ABSTRACT Background Astrocytoma is a glial tumor that derived from astrocyte cells with various tumor size, histopathologic grading and peritumoral edema. Angiogenesis plays role in tumor grading, tumor size and formation of peritumoral edema. VEGF is needed for proliferation, survival, migration of endothel cell and vascular permiability. The role of VEGF to regulate peritumoral edema in brain tumor is still debated. The purpose of this study was to analyze the relationship between VEGF expression and tumor size, histopathologic grading and peritumoral edema of astrocytoma. Methods Pathology archives of grade II, III and IV astrocytomas were retrieved at Dr Soetomo Hospital Surabaya between Januari 2009-Januari 2012. Thirty cases of astrocytomas that fulfilled the conclusion criteria were stained with VEGF rabbit monoclonal antibody (clone EP1176Y). Relationship between VEGF expression and histopathologic grading, tumor size and peritumoral edema were analyzed using Spearman correlation test. Results There were 29 of 30 cases of astrocytom showing positif VEGF expression, 13 cases of grade IV astrocytoma, 22 cases had tumor size more than 2 cm, and 17 cases showing peritumoral edema. Statistical analyzis revealed that VEGF expression had a significant association with histopathologic grading ( p=0.023; rs=0.413) and tumor size (p=0.005; rs=0.499). In contrast, VEGF expression had no significant assoiation with peritumoral edema (p=0.273). Conclusion This study revealed that VEGF plays role in increasing histopathologic grading and tumor size in astrocytoma but its role in peritumoral edema is not significant. Abstract

Epidermolysis Bullosa Acquisita (EBA) is a rare, chronic autoimmune subepidermal bullous disease and has been noted to be associated with systemic lupus erythematosus (SLE). The incidence of EBA and SLE in one patient within the period of 1980-1990 found only 7 published case reports. A 23 years old woman with exfoliate skin since 12 years ago. Initially itchy on her buttock then appeared small blister. Blister spread almost the entire body and rupture. This complaint got worsening in a year accompanied with hair loss, weight loss, and oral ulcer. Dermatological examination showed patch eritematosa, hyperpigmentation, hypopigmentation, erotion with erythematous base, yellow brownish crust. Also obtained sclerodactyli toes and nail fingers. Laboratory examination anemia gravis, hypoalbuminemia, Coombs test +2, ANA Test negatif, dsDNA IgG 32,80. Histopathology examination showed blister subepidermal, no vacuolar degeneration, no superficial and deep infiltrat, and minimal lymphocyte. Patient had diagnosed SLE from Internal Department based on MEX-SLEDAI score. The patient was treated with metylprednisolone intravenous pulse dose 500 mg on 3 days then tappering off and wound care. Epidermolysis Bullosa Acquisita immunogenetically related with MHC class II haplotype in particular  HLA-DR2. This factor suggest playing role in the development of  EBA to express more aggresive SLE.

The high mortality rate of patients with NPC is caused by recurrence and metastases early in the disease process, even though they have been given a combination of standard NPC therapy, namely radiotherapy and chemotherapy. Recurrences generally occur after the initial modalities of radiotherapy. Toxicity due to therapy given over a long time can also increase mortality. This study aims to determine the expression of BCL11B in undifferentiated NPC and its correlation with LMP-1 so that it can provide an overview of the nature of CSC in NPC, which is thought to cause recurrence and metastases and provide a poor prognosis in patients with NPC. Methods: This study was an analytical observational study using a cross-sectional approach using 30 samples from nasopharyngeal biopsy tissue diagnosed with undifferentiated NPC at the anatomical pathology installation of Dr. Saiful Anwar General Hospital, Malang, Indonesia, between 2018-2020. LMP-1 and BCL11B expression was examined using the immunohistochemical method. Data analysis was performed univariate and bivariate with the help of SPSS software. Results: Statistical analysis using the Spearman correlation test between LMP-1 and BCL11B expression in undifferentiated NPC biopsy tissue showed a significant correlation (p=0.004) with a correlation coefficient of r=-0.511. Conclusion: The higher the expression of LMP-1, the lower the expression of BCL11B in nasopharyngeal biopsies of patients with undifferentiated.

Abstract

Mycosis Fungoides  adalah keganasan sel limfosit T yang langka dan bersifat indolen pada kulit dengan evolusi klinis berupa bercak, plak dan tumor. Mycosis Fungoides fase dini  sulit terdeteksi karena kemiripannya dengan keradangan pada kulit  berpola likenoid.  Mycosis Fungoides dan dermatitis likenoid memiliki terapi dan prognosis  yang berbeda sehingga harus dapat dilakukan diagnosis yang akurat. Kami melaporkan serial kasus  Mycosis Fungoides dari dua orang pasien wanita yang terdiagnosis pada fase dini dengan gejala klinis berupa bercak dan plak multipel disertai gatal menahun. Kasus pertama terjadi pada usia 60 tahun dengan gejala selama 8 tahun sedangkan kasus kedua terjadi pada usia 58 tahun dengan gejala selama 6 tahun.  Bercak dan plak pada kedua pasien tersebut berlokasi di area yang terlindung dari sinar matahari disertai skuama halus, sebagian hiperpigmentasi, bervariasi bentuk dan ukuran. Hasil pemeriksaan histopatologi dari sediaan biopsi kulit kedua penderita berupa reaksi likenoid yang mengandung sejumlah  sel limfoid atipik, serebriformis. Hasil  pewarnaan imunohistokimia menunjukkan CD3 positif membentuk epidermotropisme, rasio CD4/CD8 8:1, Ki67 positif sekitar  25%  dan CD20 negatif.  Dari kedua kasus ini dapat disimpulkan bahwa pemeriksaan imunohistokimia dapat membatu menegakkan diagnosis Mycosis Fungoides pada dua orang wanita tua dengan keluhan gatal disertai bercak atau plak yang menahun dengan gambaran histopatologik berupa reaksi likenoid dengan  epidermotropisme.

Abstract

Kanker kulit yang tersering dikelompokkan menjadi kanker kulit melanositik dan nonmelanositik. Kanker kulit nonmelanositik menempati urutan kelima terbanyak dari seluruh jenis kanker di dunia dengan jenis yang paling sering adalah karsinoma sel basal. Secara histopatologis karsinoma sel basal dibagi menjadi dua kelompok, yaitu kelompok risiko rekurensi rendah dan tinggi. Sifat Karsinoma sel basal yang proliferatif dan invasif membutuhkan marker yang dapat memprediksi keduanya, di antaranya adalah cyclin D1 dan smooth muscle actin (SMA). Penelitian ini untuk membuktikan adanya hubungan antara ekspresi cyclin D1 dengan karsinoma sel basal kelompok risiko rendah dan tinggi. Desain penelitian adalah observasional analitik dengan metode cross-sectional, sampel penelitian karsinoma sel basal ditetapkan sebanyak 30 sampel, terdiri dari 15 sampel kelompok risiko rekurensi rendah dan 15 sampel kelompok risiko rekurensi tinggi. Ekspresi cyclin D1 dan SMA pada sediaan imunohistokimia dinilai secara semi kuantitatif pada masing-masing kelompok kemudian akan dilakukan uji chi-square. Hasil penelitian ini menunjukkan bahwa ekspresi cyclin D1 memiliki hubungan dengan karsinoma sel basal kelompok risiko rekurensi rendah dan tinggi (p = 0,008 < 0,05), namun tidak berhubungan dengan ekspresi SMA (p = 0,389 > 0,05). Ekspresi cyclin D1 pada karsinoma sel basal kelompok risiko rendah memiliki skor yang lebih rendah dibandingkan dengan kelompok risiko rekurensi tinggi. Sementara pada SMA tidak didapatkan perbedaan yang bermakna. Kesimpulan penelitian ini, ekspresi cyclin D1 memiliki hubungan dengan karsinoma sel basal kelompok risiko rekurensi rendah dan tinggi, sedangkan SMA tidak berhubungan.

Abstract

Background: Langerhans Cell Histiocytosis (LCH) has diverse manifestations, from asymptomatic to aggressive, which involves many organs. Histopathological examination playsa crucialrole as a basic diagnostic standard for LCH. Writing Group of the Histiocyte Society proposes a guideline for diagnosing LCH, divided into presumptive, designated, and definitive diagnosis. Case Illustration: Two cases of a 14 month-old girl and an 18 month-old girl presented similar clinical manifestation and multi-organ involvement. Dermatological examination revealed red papules and plaques covered by brownish scales and crusts on the scalp and body, erosion in some folds of the body. Histopathological examination of the first case revealed an early purpuric phase. S100 immunostaining just revealed hyperplasia of Langerhans cell but still could not support the diagnosis of LCH. Fine Needle Aspiration Biopsy of the enlarged submandibular lymph node after two months ofobservation suggested LCH. In the second case, histopathological examination revealed proliferation of round-oval nucleated cells, pleomorphic, some reniform nuclei, with amphophilic cytoplasm. S100 and CD1a immunostaining revealed a positive reaction in the proliferative cells. Discussion: Patients aged 14 and 18 monthsoldindicatedalmost similar clinical manifestations leading to LCH diagnosis, with different histopathological pictures. The first patient was presumptively diagnosedas high-risk multisystem LCH, but theinitial histopathology results did not support LCH diagnosis. On the other hand, the second patient was definitively diagnosed with high-risk multisystem LCH. Conclusion: Patientswith clinically suspected LCH without histopathological confirmation should be observed at least six months to reassess the necessity of a follow-up biopsy.

Abstract

Background: Meningioma is the most common intracranial tumor in adults. Progesterone plays a role in the cell development and proliferation of hormone-associated tumors, such as meningiomas. Significant morbidity and mortality can occur, when the lesion is localized in an area that is difficult to undergo surgical resection or is highly infiltrative in the surrounding tissue. Many studies have been published on the molecular properties associated with meningioma aggressive behavior, including matrix metalloproteinase (MMP), but research on the relationship between PR expression and MMP-9 has not been widely carried out. Methods: This study used a cross-sectional analytic observational study design. The analysis used in this study includes descriptive analysis and bivariate analysis. The data obtained was processed using SPSS software. Results: Spearman's correlation test for PR expression with WHO grading in meningioma, obtained p = 0.492 and correlation coefficient -0.117. This shows a correlation that is not significant. Spearman's correlation test for MMP expression with WHO grading in meningioma, obtained p = 0.16 and a correlation coefficient of 0.236. This shows a correlation that is not significant. Spearman's correlation test between MMP9 expression and WHO grading in meningioma obtained p = 0.008 which indicated that the correlation between PR and MMP9 values ​​was significant with a correlation value of -0.427. Conclusion: There is no significant correlation between PR expression and WHO grading. There was no significant correlation between MMP9 expression and WHO grading. There is a significant correlation between PR expression and MMP9.

Abstract

Giant condyloma acuminata (GCA) is one of sexually transmitted infections caused by the Human Papilloma Virus (HPV) infection, specifically HPV types 6 and 11. Giant condyloma acuminata is a rare disease with an incidence of around 0.1%. The age distribution peaked in 25-29 years. The higher prevalence of infection is associated with number of sexual partners, history of chlamydia and gonorrhea infection, smoking and human immunodeficiency virus (HIV) infection. A 24 years old male presented with a cauliflower-like mass on the penis that has been growing slowly for 2 years. Physical examination of the penis revealed a pedunculated reddish verrucous mass, mobile, well-defined, 5x2x1.5 cm in size, and positive acetowhite test. The histopathological picture is in accordance with condyloma acuminata. Surgery with wide margin excision was performed to remove the lump. After 14 days post-surgery, the wound was healed completely without any sign of recurrence.
