Abstract

Background. The most common intracranial neoplasms worldwide is meningioma. It has been classified into three grades by World Health Organization (WHO) in which each grade has several histological variants. The prognosis of meningioma is affected by many factors including histopathological type of tumor, age, and gender. Objective. The aim of this study is to determine the frequency of meningioma based on histopathological type, age, and gender from 2016 to 2020 in Saiful Anwar General Hospital Malang. Methods. This study was a quantitative descriptive study and data was collected from medical records from 2016 to 2020 in Department of Anatomic Pathology, Medical Faculty, Brawijaya University, Saiful Anwar General Hospital Malang. Histopathological type of meningioma, age, and gender were acquired. Results and Discussion. From 2016 to 2020, there were 96 eligible cases of meningioma. Ratio of men to women was 1:12.7. Meningioma was most commonly found in age group 41­50 years old (43.75%). The most common histopathological type of meningioma was meningotheliomatous meningioma (37.5%). The average of meningioma cases was 19 per year. In this study, there was an increase in incidence rate of meningioma in 2017, and decreased in 2020. It may happen because The government has a better referral system to provide health care, thus meningioma can be detected earlier. Conclusion. Frequency of meningioma is higher in women than men, and increases with age until 50 years, which then decreases. Meningotheliomatous meningioma is the most common histopathological type in this study.

Abstract

Background: Mediastinal germ cell tumors are uncommon, representing less than 1% of all malignancies. They mostly occur in young men after puberty and have a poor prognosis with over all long term survival rate 42%. Case Report: We reported a case report in Saiful Anwar Malang Hospital, Indonesia, a young man, 26 years old with an anterior mediastinum tumor and VCSS The CT scan of the chest showed an anterosuperior mediastinal mass which compress the superior vena cava. The result from transthoracic FNAB with USG guidance showed an extragonadal germ cell tumor which resemble an Embryonal Carcinoma from mediastinum. The AFP serum level was elevated. Patient underwent a radiotherapy and a surgical debulking. The biopsy showed an Endodermal Sinus Tumor (Yolk Sac). Immunohistochemical stain for cytokeratin was positive in the tumor cells while stain for CD30 was negative. These were confirming the diagnosis of primary mediastinal yolk sac tumor. Patient received a combination chemotherapy for a total 6 cycles.Conclusion: Some procedures like biopsy, tumor marker, immunohistochemical stain, could differentiate the subtype of mediastinal germ cell tumor. The multimodality treatment by combining chemotherapy with surgical therapy or radiotherapy could increase the survival outcome of patients.

A woman with complaints of shortness of breath, cough, fever, night sweating, decrease of body weight and swelling in face, neck and right arm. Chest x-rays and ct thorax revealed a mass in mediastinum.Case Description: Patient is a 24 years old female with complaints of shortness of breath, cough, fever, night sweating, decrease of body weight and swelling in face, neck and right arm. Chest x rays and CT thorax revealed a mass in mediastinum. Open biopsy on right neck mass and transthoracic FNAB resulting of chronic inflammation, so the patient was diagnosed with glandular tuberculosis and Mediastinal Tumor. Patient was getting worse after one month consuming OAT. Patient was consulted to Cardiovascular and thorax Surgery Department. Patient was performed thoracotomy and debulking. Anatomical pathology was examined from debulking mass, the result of immunohistochemistry was found nodular mediastinal gray zone lymphoma.Discussion: this case it is very difficult to enforce the diagnosis. Patients have done neck FNAB, transthoracic FNAB, open biopsy right neck mass and FOB have not been able to establish diagnosis. Until finally done thoracotomy and debulking in this patient, from immunohistochemistry results obtained nodular sclerosis Hodgkin lymphoma. Collaboration from pulmonologist, radiologist, cardiovascular and thorax surgeon, and patology anatomy specialist  is needed to confirm the diagnosis in these patients.Keywords: mediastinal tumor, mediastinal gray zone lymphoma

Abstract

Lung adenocarcinoma with malignant pericardial effusion and also metastases to mediastinum has great impact on morbidity and mortality of patient and it is rare case. Most adenocarcinoma in the mediastinum are metastases from the lung, gastrointestinal tract, pancreas, kidney or even the pituitary gland. Cardiac metastases in lung cancer may occur due to retrograde lymphatic spread or by hematogenous dissemination of tumour cells. Pericardium is frequently involved in direct cardiac invasion by adjacent lung cancer. Pericardial effusion, pericarditis, and tamponade are common and life threatening presentation in such cases. A 42-years-old male with superior mediastinal mass has malignant pericardial effusion, cytology shows adenocarcinoma. The patient undergo transthoracic fine needle aspiration biopsy. The result from biopsy is adenocarcinoma. This is a rare case, patient with primary lung adenocarcinoma and malignant pericardial effusion also metastases to mediastinum. To determine the diagnosis requires a comprehensive clinical evaluation and multidisciplinary approach.

Abstract

Background: Malignant mixed germ cell tumors account for 13-25% of all non seminoma germ cell tumors, almost all cases were found in males (85%), increased in the third decade, and had an average survival rate of 40-45%. With modern management the average 5-year survival rate can be over 80%.Case Report: We reported a case report in Saiful Anwar Malang Hospital, Indonesia, a young man, 26 years old with an malignant epithelial tumor derived from mediastinal or lung. The CT scan of the chest showed an lobulated solid mass of iso-hipodens on the anterior superior mediastinum, mass encompassing the aortic arch, left pulmonalis artery, subclavian artery. The result from transthoracic FNAB with USG guidance showed an malignant epithelial tumor derived from mediastinal or lung, impression of an adeno squamous carcinoma. The AFP, LDH, and NSE serum level was elevated. Patient underwent a surgical sternotomy and tumor debulking. The biopsy showed an malignant mixed germ cell tumor (yolksac and embrional carcinoma). These were confirming the diagnosis of malignant mixed germ cell mediastinal tumor (yolksac and embrinonal carcinoma). Patient received a combination bleomycin, etoposid, cisplatin 3 series chemoteraphy  and evaluated.Conclusion: Some procedures like biopsy, tumor marker, could differentiate the subtype of mediastinal germ cell tumor. The multimodality treatment by combining surgical therapy (surgical sternotomy and tumor debulking) with chemotherapy could increase the survival rate of patients.

Abstract

Background: Nevus sebaceous of Jadassohn (NSJ) is a non-hereditary hamartoma of the skin’s adnexal structures. The incidence of NSJ in newborns is estimated at 0.1-0.3% with no gender or ethnic propensity. Surgical excision is a common treatment for NSJ, although there is no definite consensus on the timing or recommendations for the removal. This article aims to present the advantages of serial excision therapy in a case of NSJ. Case Illustration: A 14-year-old male presented with a complaint of a bump on the scalp since birth, which had progressively enlarged during puberty. Dermatological examination revealed a well-demarcated, yellow-orange verrucous plaque with irregular borders measuring 3x3.5 cm. The diagnosis was confirmed through dermoscopy and histopathology. The patient underwent a three-stage serial excision at 3-4-week intervals. Discussion: The excision technique for NSJ excision was selected based on the location and size of the lesion. Serial excision is a widely used and straightforward method. This method was chosen due to the lesion’s large area. However, NSJ excision on the scalp presents its own challenges due to limited soft tissue flexibility, hair-bearing skin, and a convex surface, which may affect reconstruction outcomes. Conclusion: The three-stage serial excision resulted in complete removal of the NSJ, minimal scarring without significant alopecia, and no recurrence during the 6-month follow-up.

Abstract

The use of dyes in food today is very apprehensive. Many foods contain dyes that are prohibited to be used in food. The addition of the dye will have an impact on health. The aim of this research was to know the effectiveness of the administration of extra virgin olive oil to the number of granulosa cell and 17β estradiol level of Rats exposed by rhodamin B. The design of this study was true experimental with post test approach of group design control. The subject of the study was 30 white mice Rattus novergicus in the age of 12 weeks weight average 220 grams and in healthy condition. Rats were divided into 5 groups. Control group without treatment, positive control treatment with rhodamine B 18 mg / 200gr. Treatment 1 administration of rhodamine B 18 mg / 200gr and EVOO (Extra Virgin Olive Oil) 1.5 ml / KgBB, treatment 2 rhodamine B 18 mg / 200gr and EVOO 3 ml / KgBB, treatment 3 rhodamine B 18 mg / 200gr and EVOO 4,5 ml / KgBB. The number of granulosa cells was determined by HE staining, the level of 17β estradiol with ELISA. The data was analyzed by Kruskal Wallis test, Spearman Rank and Simple Linear Regression with degree of significance α <0.05. The average number of granulosa cells and the lowest levels of 17β estradiol were found in the positive control group and the highest average was in the treatment group 3. There was one dose that could significantly increase granulosa cells and the beta estradiol levels compared with the treatment group the other with niali α <0.05. The administration of EVOO affected the number of granulosa cells and the levels of 17β estradiol in rats exposed by rhodamine B.
