Abstract

Background: Obesity is still one of the problems in Indonesia. The obesity problem has grown into an epidemic, with more than 4 million people dying each year. Factors causing obesity such as lifestyle, lack of physical activity, poor diet habits, low family income, sleep duration, and others. This study aims to estimate the effect of sleep duration, exercise, and family income on obesity in children. Subjects and Method: A systematic review and meta-analysis were conducted using PRISMA guidelines and the PICO model. Population= children of 2-18 years. Intervention= short sleep duration, exercise, and high family income. Comparison= long sleep duration, no exercise, and low family income. Outcome= Obesity. Articles were collected from databases such as Google Scholar, PubMed, Science Direct, and ResearchGate. The literature search used the keywords “family income" AND “exercise” AND "sleep duration" AND “obesity” AND “children” AND "cross-sectional". A total of 19 articles met the inclusion criteria for the meta-analysis, and subsequently were assessed using RevMan 5.4. Results: A meta-analysis of studies from Japan, Korea, Ethiopia, China, Morocco, Saudi Arabia, and Ghana showed that children with short sleep duration (aOR= 1.83; CI 95%= 1.22 to 2.73; p= o.003), children with high family income (aOR= 1.18; CI 95%= 1.03 to 1.36; p= 0.020) could increase the incidence of obesity, and those results were statistically significant. Exercise can reduce the incidence of obesity in children, but this risk of reduction was not statistically significant (aOR= 0.80; CI 95%= 0.59 to 1.09; p= 0.15o). Conclusion: Short sleep duration and high family income increase the incidence of obesity in children, and these results are statistically significant. Exercise decreases the incidence of obesity in children, but the risk of decline is not statistically significant. Abstract

Background: Tuberculosis is an infectious disease caused by mycobacterium tuberculosis by air-borne transmission. TB patients are at risk of psychosocial disorders during treatment and post-treatment. This study aims to analyze the effect of psychoeducation on the level of anxiety experienced relapse in patients after Tuberculosis treatment.Subjects and Method: This study used a quasi-experimental design of a posttest pretest at the Baki Health Center, Sukoharjo Regency, Central Java, Indonesia from June to July 2023. The sample was 30 people in the treatment group, namely post-tuberculosis treatment patients, and 30 people in the control group by paying attention to the inclusion criteria, the sample was selected in total sampling. The dependent variable is anxiety and the independent variable is psychoeducation. Psychoeducation was provided with intervention for 3 sessions with a time of 25-30 minutes and anxiety was measured by the HALS questionnaire. The data was analyzed by the Mann-Whitney test.Results: After the intervention, the psychoeducational group had a decrease in anxiety scores (Mean = 7; SD= 4.68) than the control group (Mean= 4.07; SD= 3.14) with p=0.006.Conclusion: Psychoeducation for post-tuberculosis treatment patients can continue to be carried out to reduce the level of anxiety about tuberculosis recurrence and awareness of the impact of the disease.

Abstract

Background: Hypertension remains a major public health challenge in Indonesia, with prevalence continuing to rise across different population groups. The burden of hypertension varies across settings, including urban and rural areas, socioeconomic levels, and regional contexts. This study aimed to examine how district- and city-level contextual factors influence the risk of hypertension among adults in Indonesia. Subjects and Method: A cross-sectional study included a sample of 48,878 adults who participated in the 2023 Indonesian Health Survey (Survei Kesehatan Indonesia). The data were analyzed using multilevel multiple linear regression. Results: Multilevel analysis showed that the risk of hypertension increased among individuals aged ≥60 years (b= 4.12; 95% CI= 3.76 to 4.48; p < 0.001), compared with those aged <20 years. Senior high school or higher education (b= –0.40; 95% CI= –0.48 to –0.31; p <0.001) was associated with a lower risk of hypertension compared with individuals with no formal education. Higher BMI was associated with an increased risk of hypertension among individuals with BMI 18.5–25 kg/m² (b= 0.19; 95% CI= 0.09 to 0.30; p < 0.001) and BMI ≥25 kg/m² (b= 0.92; 95% CI= 0.82 to 1.03; p < 0.001). Female sex (b= 0.21; p < 0.001), urban residence (b= 0.06; p= 0.021), and high Gini Ratio (b = 0.17; p < 0.001) increased the risk of hypertension, whereas high physical activity and higher provincial minimum wage reduced the risk. The ICC of 0.37% indicated low variation across districts/cities. Conclusion: Hypertension risk among Indonesian adults is mainly driven by individual factors, while income inequality is the only contextual factor with a significant effect. These results emphasize the need for interventions targeting both personal health behaviors and socioeconomic disparities.

Abstract

Background: The strategy to reduce the risk of transmission of COVID-19 is to reduce the salivary SARS-CoV-2 viral load. Chlorhexidine and povidone iodine mouthwash are common active ingredients in oral antiseptics that have efficient viral activity against salivary SARS-CoV-2. This study aims to combine the results of several effect sizes regarding the effect of using chlorhexidine and povidone iodine mouthwash on the salivary SARS-CoV-2 viral load from various countries.Subject and Method: This study is a meta-analysis with the following PICO model, P: COVID-19 patients. I: use of chlorhexidine and povidone iodine mouthwash. C: no mouthwash. O: salivary SARS-CoV-2 viral load. A search for the articles used in this study was carried out using the keywords “COVID-19” OR “SARS-CoV-2” OR “viral load” OR “SARS-Cov-2 viral load” OR “Chlorhexidine mouthrinse” OR “Povidone Iodine mouthrinse ” OR “Randomized Controlled Trial” OR “RCT” between 2012-2022 from the PubMed, Springerlink, Elsevier, Google Scholar and Wiley Online Library databases. The inclusion criteria used in this study were full-text articles using a Randomized Controlled Trial (RCT) design. The analysis used was multivariate with Standardized Mean Difference (SMD). The articles collected were then critically reviewed using the PRISMA checklist, then the data were analyzed using the Review Manager 5.4 tool.Results: This meta-analysis examined 10 articles with a Randomized Controlled Trial (RCT) study design originating from Singapore, Saudi Arabia, Iran, Brazil, Italy, South Korea and Malaysia. A meta-analysis of 7 articles showed that the use of chlorhexidine mouthwash could reduce salivary SARS-CoV-2 viral load by 0.12 units lower than without the use of mouthwash (SMD= -0.12; 95% CI= -0.33 to 0.09; p=0.250). Meanwhile, 7 articles showed that the use of povidone iodine mouthwash could reduce the salivary SARS-CoV-2 viral load by 0.64 units lower than without the use of mouthwash (SMD= -0.64; 95% CI= -1.51 to 0.23; p=0.150).Conclusion: The use of chlorhexidine and povidone iodine mouthwashes can reduce the amount of salivary SARS-CoV-2 viral Keywords: mouthwash, chlorhexidine, povidone iodine, COVID-19, salivary SARS-CoV-2 viral load.Correspondence: Danti Narulita. Abstract

Background:  Tuberculosis (TB) is the most common cause of death in patients with the Human Immunodeficiency Virus (HIV). Understanding CD4 as a predictor of mortality from TBHIV coinfection is critical to improving disease management and minimizing mortality. This study aims to examine the mortality risk of TBHIV-coinfected patients based on CD4 values.Subjects and Method:  Metaanalysis was performed according to the PRISMA flow chart and the PICO model (Population: TBHIV coinfected patients, Intervention: CD4 value <200 cells/µL, Comparison: CD4≥200 cells/µL, Outcome: mortality). The databases used are Google Scholar, PubMed, Scopus, Proquest, and Science Direct. Keywords used (coinfection OR "mixed infection") AND (HIV OR "Human Immunodeficiency Virus") AND (TB OR “tuberculosis”) AND ("CD4 count" OR "T4 lymphocyte") AND “mortality”. The inclusion criteria were full-text articles with cohort studies published in 20122022, articles in English, and multivariate analysis using the adjusted hazard ratio. Analysis was performed with Revman 5.3.Results:  17 articles with cohort design originating from Ethiopia, Khayelitsha, Kenya, Cape Town, Uganda, Brazil, Suriname, Sao Paulo, Guinea Bissau, Myanmar, Durban, Ireland, China, and multicountry studies between Europe and Latin America totaling 24,514 research sample. A meta-analysis of 10 study cohorts concluded that CD4<200 cells/µL had a 2.00 times risk of mortality compared with CD4 values ≥200 cells/µL (aHR=2.00; 95% CI 1.44 to 2.78; p<0.001). A meta-analysis of 7 study cohorts concluded that CD4<100 cells/µL had a 2.40 time risk of mortality compared with CD4 values≥200 cells/µL (aHR=2.40; 95% CI 1.61 to 3.57; p<0.001). A meta-analysis of 7 study cohorts concluded that CD4<50 cells/µL had a 3.12 times risk of mortality compared with CD4 values≥200 cells/μL (aHR=3.12; 95% CI 1.51 to 6.46; p<0.001).Conclusion:  Decreased CD4 values increase the risk of mortality.Keywords:  tuberculosis, HIV, CD4, mortality, meta-analysisCorrespondence: Victoria Husadani Permata Sari. Abstract

Background: Acupuncture therapy is an effective treatment for reducing headache intensity, migraine recurrence, and improving quality of life. This study aims to analyze and estimate the effect of acupuncture therapy on reducing migraine recurrence.Subjects and Method: This study used a systematic review and meta-analysis by following PICO, Population: migraine patients. Intervention: acupuncture therapy. Comparison: no acupuncture therapy. Result: reduction in migraine recurrence. The data used were obtained from scientific research articles from the Pubmed, Google Scholar, Science Direct, and SpringerLink electronic databases with a Randomized Control Trial (RCT) design from 2011 to 2022 which reports effect sizes with Mean and SD. The keywords used in the search for scientific articles are “Acupuncture” AND “Profilaxis Migraine” AND “Migraine Frequency”. The selection of articles was carried out using the PRISMA flowchart. Data were analyzed using Review Manager software version 5.4.1.Results: A total of 9 articles with qualified RCT designs from Asia, America, Australia, and Europe were selected for the systematic review and meta-analysis. The results of the meta-analysis showed that patients with migraine who received acupuncture therapy experienced migraine recurrence 0.31 units lower than those who were not treated with acupuncture (SMD= -0.31; 95% CI= -0.61 to -0.01; p=0.040).Conclusion: Acupuncture therapy reduces migraine recurrence.Keywords: acupuncture, relapse, migraine.Correspondence: Elsa Tursina.
