Abstract

Neonatal mortality is an important issue to receive attention because neonatal death not only affects infant mortality rates but also has an impact on society. This research wants to examine the determinants of neonatal death in the Mother and Child Hospital (RSIA) Aceh. This type of research is analytic with a case-control design. The case population in this study were all babies who were born and died at the age of 0-28 days, both those born at the Aceh Mother and Child Hospital (RSIA) and those referred to the Aceh Mother and Child Hospital (RSIA) from January to October 2022. totaling 108 people. The sample in this study was 216 babies consisting of 108 case babies and 108 control babies. Data analysis used a logistic regression test. The results showed that the factors that had a significant value (P value <0.05) with neonatal mortality were low birth weight (OR= 5.5; 95% CI 2.35 – 12.23), premature (OR= 0.36; 95% CI 0.13 – 0.96), jaundice (OR= 0.20; 95% CI 0.09 – 0.43), and sepsis (OR= 9; 95% CI 4.41 – 20.89), age mother > 35 years (OR 0.43; 95% CI 0.19 – 0.94), neonatal referral (OR= 0.01; 95% CI 0.00 – 0.17) and long-distance ANC (OR= 6 .6; 95% CI 2.82 – 15.63), length of stay in the emergency room ≥ 30 minutes (OR= 0.15; 95% CI 0.73 – 0.31), length of stay 1-5 days (OR= 4.3; 95% CI 2.17 – 8.78). This study concludes that sepsis is the most associated factor with neonatal mortality in RSIA. It is recommended that related parties to improve the quality of ANC services according to standards and improve the quality of PONEK (Comprehensive Emergency Neonatal Midwifery Services).

Abstract

Non-adherence of hypertensive patients in the treatment of their disease has an impact on uncontrolled blood pressure which can worsen the patient's health status and even death. One important factor that must be considered in efforts to improve hypertension compliance is patient knowledge about the disease and the necessary management. This study aimed to determine the relationship between knowledge in supporting practice and compliance with hypertension sufferers in taking antihypertensive drugs. The research design used was cross-sectional descriptive analysis. The sample size for this study was 220 respondents who were recruited using purposive sampling techniques. Analysis of research data was carried out using the chi-square statistical test. A total of 51.8% of hypertensive patients had sufficient knowledge and 65.9% of hypertensive patients admitted that they were not compliant in taking the recommended medication. The value of the test results < α 0.05 (p= 0.000), which means that knowledge is significantly related to adherence to taking medication. Knowledge is the most important component that sufferers must have so that they can be actively involved in compliance practices. It is hoped that these findings can be used as material for consideration in formulating treatment interventions for people with hypertension about efforts to increase patient compliance in terms of taking anti-hypertensive medication.

Abstract

Background: Pregnancy is a very important period, the state of the mother and fetus are related to one another. While pregnant women are often found to have iron deficiency anemia. One of the efforts to overcome the problem of anemia in pregnant women is by giving iron supplements. However, in Riau Province pregnant women who received FE3 decreased, namely in 2018 as much as 79% decreased to 65% in 2019. Therefore, an educational method is needed to increase knowledge, and adherence to taking iron supplements, and hemoglobin (Hb) levels in pregnant women. Objectives: To increase knowledge and adherence of taking iron supplements and hemoglobin (Hb) levels in pregnant women. Methods: This study is a pre-experimental study with one group pretest and post-test to evaluate the effect of nutrition education with video media on knowledge, adherence to taking iron supplements, and Hb levels in late-trimester pregnant women. The educational videos include material on the importance of taking iron supplements, how to consume them, and balanced nutrition for pregnant women. Result: There is a statistically significant difference between the knowledge and the adherence of pregnant women taking iron supplements before and after treatment or intervention with a significance value of 0.000 (<0.05). Changes in Hb levels before and after treatment or intervention in the form of Educational Video for Iron Supplements (TTD) and Nutrition for Pregnant Women did not show a statistically significant difference with a significance value of 0.085 (> 0.05).Conclusion: Educational videos on iron supplements and nutrition for Pregnant Women affect knowledge and adherence of pregnant women taking iron supplements.

Abstract

Background: Rehabilitation services for stroke patients decreased during the Covid-19 pandemic, and patients mainly stayed home without rehabilitation. As a result, the patient experiences recurrent strokes, contractures, malnutrition, decubitus, and reduced cognitive function. Therefore, the repair is crucial to achieving maximum recovery and improving one's physical, mental, social, and speech abilities. Purpose: To find out the rehabilitation for stroke patients during the Covid-19 pandemic. Method: This review used a modified version of the integrative review research methodology developed by Whittemore and Knafl (2005). The literature search was conducted on five databases, namely PubMed, Science Direct, ProQuest, Cochrane Library, and DOAJ, with inclusion criteria in English, focusing on the rehabilitation of stroke patients, the Covid-19 pandemic, and original research published between 2020 and 2022. Evaluation of publications used Bowling's critical assessment model (2001). Results: Ten articles were included in the analysis. As a result, an overview of the rehabilitation of stroke patients during the Covid-19 pandemic was obtained, which included, first: Changes in the rehabilitation service system. Second: the obstacles to the rehabilitation of stroke patients during the Covid-19 pandemic, such as increasing family workload, lack of social support, and economic factors/financial constraints. Third: Changes in the functional status of stroke patients.Conclusion: The overview of the rehabilitation of stroke patients during the Covid-19 pandemic includes changes in the rehabilitation service system, obstacles to the restoration of stroke patients during the COVID-19 pandemic, and changes in the functional status of stroke patients.

Abstract

The decrease in the hormone estrogen during menopause causes the quality of sleep to decrease. Sleep quality affects physical and psychological health which can reduce the quality of life of menopause. This study aims to prove that acupressure therapy can improve sleep quality and estrogen hormone levels in postmenopausal women. The design of this study is a Quasi experiment, which is by using existing groups. The number of samples was 40 respondents, selected using the Consecutive Sampling technique. The intervention group was given acupressure therapy on HT 7, PC 6, and GV 20 points 6 times for 2 weeks. Sleep quality was measured by the Pittsburgh Sleep Quality Index (PSQI) questionnaire and estrogen levels were measured by the ELISA method. Data analysis using Paired Sample Test and independent T-Test. Data analysis showed a change in the mean score of sleep quality and estrogen levels in the intervention group after being given acupressure therapy intervention (p = 0.000) (effect size = 0.90). Hasil dari penelitian ini terdapat perbedaan rerata skor kualitas tidur dan skor kadar estrogen antara kelompok intervensi dan kontrol (p = 0,000) (effect size=1.17). The conclusion of this study is acupressure therapy at points HT 7, PC 6, and GV 20 can improve sleep quality and estrogen hormone in menopausal women. This study implies that acupressure therapy interventions can be applied as part of obstetric care in menopausal women.

Abstract

Preeclampsia is hypertension at 20 weeks of gestation or after delivery with blood pressure ≥ 140/90 mmHg accompanied by protein in the urine and edema. Preeclampsia can occur during pregnancy, childbirth, and puerperium. During pregnancy, preeclampsia occurs at the age of 20 weeks and above. The study aimed to analyze the factors associated with the occurrence of preeclampsia. The research method was analytic with a cross-sectional approach. The independent variables are age, education, occupation, anxiety, and adherence to antenatal care, the dependent variable is preeclampsia. The population is 42 children with a sample of 38 respondents. Simple random sampling technique, statistical test using Spearman Rank. The data analysis used was univariate with percentage, and bivariate with Spearman Rank. The statistical test results obtained P-value (0.010) < α (0.05) anxiety, P-value (0.001) < α (0.05) adherence to antenatal care, age P-value (0.000), occupation P-value (0.003) and P-value (0.04) with the incidence of preeclampsia. Efforts to reduce the incidence of preeclampsia are by encouraging pregnant women to carry out regular and active Ante Natal Care examinations to attend classes for pregnant women. Early detection of preeclampsia is still being carried out for all pregnant women to prevent preeclampsia.

Abstract

Every year, the working area of Gunung Toar Community Health Center views an increase in the number of patients suffering from diabetes mellitus and hypertension. However not everyone with hypertension and diabetes mellitus received prolanis, and neither the Gunung Toar Community Health Center nor BPJS Health have evaluated the quality of prolanis services. The research's objective was to use the Importance Performance Analysis (IPA) method to assess participant satisfaction and the quality of services provided by the chronic disease management program.  It is a mixed-methods research design. The study was carried out in May and June of 2022 in the working area of the Gunung Toar Community Health Center in Kuantan Singingi Regency. There were 533 individuals in the population. Importance Performance Analysis was used for both quantitative analysis, with a sample of 100 respondents, and qualitative analysis, with five informants conducted through in-depth interviews. Based on the study's findings, it was discovered that participant satisfaction was 96.51% overall, officer reliability was 92.68%, responsiveness was 97.45%, physical evidence was 99.50%, assurance was 98.18%, and empathy was 96.02%. More personnel in charge of prolanis in each club, a stronger prolanis team, increased cross-sectoral support, better management of accountability document archiving, and regular, quarterly, and annual evaluations of prolanis operations are still required.

Abstract

Early Initiation of Breastfeeding is the process of breastfeeding the baby immediately after birth, where the baby is left to find his own mother's nipple which is done at least one hour immediately after birth. Nationally, the percentage of newborns who received IMD was 82.7% and in North Lampung Regency it reached 95.1%, and at RSD Mayjend HM Hospital. Ryacudu only 33.04% of mothers who successfully perform IMD. Unsuccessful implementation of IMD can have an impact on increasing the high mortality and morbidity rates. This study aimed to determine the factors related to the implementation of IMD at RSD Mayjend HM. Ryacudu, North Lampung Regency in 2023. This research was quantitative with a cross-sectional design. The population in this study were all mothers giving birth at RSD Mayjend HM. Ryacudu, North Lampung Regency in February-March 2023, with a total of 93 people. The sample in this study used a total sampling of 93 people. The research instrument used was a questionnaire and an observation sheet. Data analysis in this study used univariate, bivariate (chi-square), and multivariate (multiple logistic regression). The results of this study indicate that there was a relationship between education (p-value 0.040 and OR 2.630), type of delivery (p-value 0.005 and OR 3.860), knowledge (p-value 0.027 and OR 2.856), attitude (p-value 0.020 and OR 2.951), and support health workers (p-value 0.000 and 5.438) with implementation. There was no relationship between parity (p-value 0.427) and family support (p-value 0.138) with the implementation of IMD. The dominant factor most related to the implementation of IMD was the variable type of delivery with an OR of 12.535. Advice for mothers giving birth either by sectio caesarea or vaginally can play an active role in carrying out IMD, that Mothers can also find information from both print and online media.

Abstract

Implementing evidence-based nursing is an effort to improve the quality of nursing care by applying the best research results in carrying out patient care. However, its implementation still needs to be improved. This study aims to identify barriers to nurses' implementation of EBN. Descriptive research was conducted on 49 nurses in the surgical inpatient room of one of the government hospitals in Pekanbaru City. Data were collected using a questionnaire with a Likert scale. Data were analyzed univariately using frequency distribution. The results of the study found that the main things that hinder the implementation of evidence-based nursing are the number of patients to be treated (69.4%), lack of fluency in English (61.2%), lack of time to study research (53.1%) and lack of knowledge of nurses about scientific evidence-based care (51%).  Workload and personal aspects become obstacles in the implementation of evidence-based nursing by nurses; it may be necessary to recalculate the workload so that the distribution of nurses becomes better and consideration to carry out training related to evidence-based nursing to improve knowledge and practice of evidence-based nursing by nurses.

Abstract

The decrease in the Community Satisfaction Index from 2018 to 2021 from 89% to 79% shows the dissatisfaction with services at the Public Health Center, so it is necessary to manage the improvement of service quality through behavioral attitudes, officer expertise, and process quality in providing services. The purpose of this study was to prove the effect of health service quality on trust through patient satisfaction at the  Public Health Center in Jember Regency. The research was conducted through a survey using a questionnaire on 204 patients from 6 selected health centers with consecutive sampling techniques. Data analysis techniques using Structural Equation Model (SEM) with Partial Least Square (PLS). The results show that satisfaction is influenced by attitudes and behaviors and process quality, but not expertise. Attitude, behavior, and satisfaction have a direct effect on trust, while process quality has an indirect effect through satisfaction. Trust was not influenced by expertise. The attitude and behavior of health workers greatly affect satisfaction and trust, so officers at the  Public Health Center must have good attitudes and behavior in serving patients.
