Journal of Health Policy and Management (JHPM) is an electronic, open-access, double-blind and peer-reviewed international multidisciplinary and integrative journal, focusing on health policy, health system, and healthcare management. It began its publication on October 21, 2015. The journal is published twice yearly. It aims to improve the design and implementation of health policies, health systems, and healthcare management, primarily in low- and middle-income countries. Abstract

Background: Given the number of patients failing to achieve control of Diabetes Mellitus (DM), it causes an increase in the incidence of DM complications. Along with the rapid deve­lopment of technology in this era, this study aimed to prove the effectiveness of technology-based health management compared to usual treatment for levels glycated hemoglobin (HbA1c) in type 2 diabetes mellitus patients.Subjects and Method: This was a meta-ana­lysis using a randomized controlled trial. Arti­cles were obtained from PubMed, Google Scholar, and ResearchGate databases. The arti­cles used in this study were those published from 2012-2021. The search article was carried out by considering the eligibility of the criteria determined using the PICO model. Population: type 2 DM patients (HbA1c>7%), Intervention: health management-assisted technology, Com­parison: usual care Outcome: HbA1c levels. There were 10 articles used with a sample size of 1693 people who were divided into two groups (845 people in the health management-assisted technology group and 848 people in the group usual care). Articles were analyzed using Review Manager 5.3 Appli­cation to determine the Standard Mean Diffe­rence (SMD) and heterogeneity of the study sample.Results: From 10 articles that were processed using RevMan 5.3, significant results were obtained, this is indicated by the overall effect (diamond) which does not touch the vertical line H0 (d= 0) and can also be seen from the 95% CI range of -0.62 to -0.13 which shows significant because it does not pass the number 0 (SMD= -0.37; 95% CI= -0.62 to -0.13; p= 0.003). The heterogeneity of the research data shows I2 = 82% so that the distribution of the data is very heterogeneous (random effects model).Conclusion: Using technology to help health management of patients with type 2 diabetes mellitus can reduce HbA1c levels compared to usual care.Keywords: Health management, technology, diabetes mellitus, HbA1cCorrespondence: Fajar Novianto. Abstract

Background: patient-oriented health services are the primary strategy for health service organiza­tions. A good service is determined by the fact that the services provided can meet the patient's needs. Patient satisfac­tion is an integral part of the quality assurance of health service. This study aims to analyze the quality of tangible health ser­vices with patient satisfaction in receiving health services.Subjects and Method: Meta-analysis was performed by searching journals from direct databases such as Google Scholar, Science Direct, and PubMed using the key­words service quality, patient satisfaction, and heat service. The inclusion criteria in this study were full text from 2010-2021, with a cross-sectional study. The data analysis was carried out by RevMan 5.3.Results: Nine articles have been analyzed, showing that the quality of service had direct evidence of an effect on patient satis­faction in accessing health services (aOR= 4.17; 95% CI= 1.78 to 9.78; p <0.001).Conclusion: The quality of direct evidence (tangible) affects patient satisfaction in accessing health services.Keywords: tangible, service, patient satisfaction.Correspondence: Siti Mar'atul Mu­na­waroh. Abstract

Background: The COVID-19 outbreak rapidly spread worldwide, with a significant number of cases and deaths causing anxiety, especially among the sufferers. Biologically, males and females show different psychological responses to the events at hand. This study aimed to examine the association between gender and anxiety among COVID-19 patients.Subjects and Method: This study was a meta-analysis. The articles were obtained from data­bases PubMed and Scopus using keywords "Anxiety" OR "anxiety disorder" AND "COVID-19 Patients" OR "Patients with COVID-19" OR "Hospitalized" AND "Risk factors" OR "deter­mi­­nants" OR "Gender" OR "Factors associa­ted". The articles included in this study were full-text with a cross-sectional design, dis­cuss-ed the association between gender and anxiety among COVID-19 patients, published in Eng­lish, studies located in Asia. The final results were pre­sented in Adjust Odds Ratio (aOR) or Standar­dized Mean Difference (SMD). Data analysis was performed using Revman 5.3 software, with generic inverse variance data type for categorical outcome group (CtOG) articles and continuous data type for continue outcome group (CnOG).Results: 9 articles were analyzed, including 4 articles CtOG and 5 articles CnOG. The  forest plot of both categorical and continue outcomes showed consistent result that among COVID-19 patients  who hospitalized with mild to severe symptoms. Female patients significantly had higher anxiety event (aOR= 2.19; 95% CI= 1.60 to 2.99; p<0.001; I2= 0% and SMD= 0.35; 95% CI= 0.21 to 0.49; p<0.001; I2= 12%).Conclusion: Among COVID-19 patients, the female gender has a significantly higher risk of suffering from anxiety than the male gender.Keywords: anxiety, COVID-19Correspondence: Zuraida Zulkarnain. Abstract

Background: Indonesia is one of the coun­tries in the world that has a commitment to Universal Health Coverage in 2019. Indo­ne­sia has made various efforts to realize this commitment, one of which is through Natio­nal health insurance. The presence of the National Administrative Body is new hope for the Indonesian people for the quality of health services that will be provided. The quality of service can be perceived as good and satisfies the patient if the service received is appropriate or exceeds what is expected. So it is necessary to apply a solution in training or training related to existing resources.Subjects and Method: This study is a syste­matic review and meta-analysis. The articles used in this study were obtained from Pub­med and Scopus. The articles used in this research are articles published from 2016-2021. The article search was carried out by considering the eligibility criteria defined using the PICO model. P: JKN participants, I: adequate resources, C: insufficient resources, O: patient satisfaction. The articles included in this study are articles full text with a study design Cross-sectional. Articles were ana­lyzed using the application Review Manager 5.3.Results: Funnel plot of comparisons showed a publication bias, as evidenced by the unbalanced number of plots on the left and right sides. The distance between the plots was far. Forest plot of national health insu­rance (JKN) patients with sufficient resour­ces has 5.56 to experience patient satisfaction compared to patients with insufficient resources.Conclusion: Publication bias in funnel plots is evidenced by the unbalanced number of left and right-sided plots. Whereas in the forest plot, JKN patients with adequate resources had a higher score, namely 5.56, to experience patient satisfaction than patients with inadequate resources. Abstract

Background: Health care renunciation aims to identify unmet care needs that a health con­dition would have justified. This behavior appears to be more common in women than in men. The objective of this work was to analyze the determinants of the health care renuncia­tion among women in the city of Abidjan.Subjects and Method: We carried out a cross-sectional study from March to May 2019 in Anonkoi-3, a peri-urban district of the muni­cipality of Abobo, in the north of the city of Abidjan. Questionnaires served to collect infor­mations. Bivariate analyzes and a multiple logistic regression were used to measure the association between the different types of renunciation and the characteristics of women. Dependant variable was healthcare renuncia­tion. Independent variables were socio-demo­graphic, economic, health status characte­ristics and reasons for renouncing to health care.Results: The population sample consisted of 423 women (with mean age= 32; SD= 12 years). The renunciation on consultations with the general practitioner concerned, 72.34% of women. Regarding consultation with the spe­cialist, the ophthalmologist (25.05%), the dentist (21.99%), and the gynecologist (14.89%) were those mostly renounced by the women. After consultation, 31.2% of them renounced to pursuing other treatment. They most often renounced to buying drugs from conventional medicine (19.62%) and preferred to use street drugs and traditional drugs (87.71%). All things being equal, women aged 28 to 38 (OR= 2.45; 95% CI= 1.31 to 4.68; p= 0.013), artisans and traders (OR= 3.22; 95% CI= 1.48 to 7.38; p= 0.004) and those in trade learning (OR= 2.42; 95% CI= 1.12 to 5.49; p= 0.028) significantly renouncing more on health care.Conclusion: In addition to financial reasons, the renunciation on health care can be explained by individual and social behaviors specific to individuals.Keywords: women, healthcare renunciation, precariousness, social inequalities, ivory coastCorrespondence: Jérôme Kouame, Faculty of Pharmaceutical and Biological Sciences, Department of Public Health, Hydrology and Toxicology, University Félix Houphouët Boigny, BPV 34 Abidjan 01, Ivory Coast. Abstract

Background: The infusion procedure was the most traumatic experience and can cause anxiety and pain for more than 50% of pediatric patients during hospitalization. This study was to determine the effect of hand puppet game and cold compress on pain and anxiety of pre­school age children undergoing infusion.Subjects and Method: This study used a quasi-experimental research design with the type of non-equivalent control group posttest-only design. It was conducted in the Pediatric Care Room at Sumbawa Hospital in July-Sep­tember 2020. The total number of respondents who were hospitalized in the Pediatric Care Room at Sumbawa Hospital selected by con­se­cutive sampling. They were divided into two, 38 people in each intervention and control group. The intervention group was given cold com­press on the infusion area and the hand puppet game, while the control group was given treat­ment according to hospital standards. The dependent variables of the study were pain level and anxiety level, while the independent variables included hand puppet game and ice pack (cold compress). In addition, the instru­ments used were pain and anxiety instruments. Data analysis was performed using the Mann-Whitney test.Results: Based on the results of the Mann-Whitney test, there was a change of pain levels (p<0.001) and anxiety levels (p<0.001) after receiving hand puppet game and cold compress during infusion compared to children with normal treatment.Conclusion: Hand puppet game and cold compress administration can be applied to reduce pain and anxiety levels in preschool age children during the infusion procedure.Keywords: children, distraction, anxiety, pain, infusionCorrespondence: Alfia Safitri. Abstract

Background: On March 11, 2020, the World Health Organization (WHO) has declared an outbreak of a new coronavirus (COVID-19) as a global pandemic. In responding to the threat of the COVID-19 virus pandemic, policymakers have implemented a policy lockdown that can cause psychological distress, one of which is anxiety. This study aims to examine the effect of the policy lockdown on anxiety in adults using a meta-analysis.Subjects and Method: Meta-analysis was performed by searching for articles from the database, namely PubMed, Google Scholar, Sci­ence Direct, Scopus, and Crossref. The key­words used are "Anxiety" AND "Adult" AND "COVID-19" OR "SARS-CoV-2" AND "Lock­down" OR "Movement Restriction" OR "qua­ran­tine" OR "shutdown". The inclusion criteria in this study were full-text articles with a cross-sectional design, in English, published in the period 2019-2021. Analysis of the article using RevMan 5.3.Results: Four articles were analyzed. The results of the meta-analysis showed that anxiety in adults with the implementation of the policy lockdown was 1.94 times more at risk than those who did not implement the policy lockdown, but it was not significant in incre­asing anxiety in adults (aOR= 1.94; 95% CI= 0.79 to 4.74; p= 0.150).Conclusion: Anxiety in adults with the imple­mentation of the policy is lockdown more at risk than those who have not implemented the policy lockdown, but it is not significant in increasing anxiety in adults. Keywords: Anxiety, lockdown, covid-19 Correspondence: Isna Nur Rohmah. Abstract

Background: The Covid-19 pandemic caused great stress, especially for health workers which resulted in moderate or severe symptoms of depression, anxiety, insomnia, and more serious stress. Gender is very influential, given the differences in biological and social roles between men and women. Gender differences between men and women allow for variations in strategies in responding to mental health to increase awareness and self-management, so as to avoid mental health problems during the Covid-19 pandemic.Subjects and Method: Meta-analysis was performed by searching articles from Google Scholar, PubMed, Scopus, Springer Link, and health-related databases. The keywords used in the article search were "health workers" AND "covid-19" AND "mental health". The inclusion of the criteria in this study is a full-text article, from 2019-2021, with a study design cross-sectional. The article analysis was carried out using RevMan 5.3. Results: There were 5 articles analyzed, the results showed no significant difference between gender and mental health responses among health workers during the Covid-19 pandemic (aOR= 1.12; 95% CI= 0.59-2.13; p=0.01).Conclusion: Not significant between primary studies regarding gender differences in mental health responses to health workers during the Covid-19 pandemic. Keywords: health workers, covid-19, mental healthCorrespondence: Riyesti Hero Fresna, Purwantoro Special Edu­cational School.
