Abstract

The human respiratory system can be infected with coronaviruses (CoVs), one of the families of viruses that can lead to symptoms ranging from mild to fatal. Seven genera of CVS have been identified as having human infection to date. The virus can spread as fine, liquid particles from the mouth or nose of an infected person when coughing, sneezing, speaking, or breathing. The diameter of droplet particles ranges from large to small. In this review, we summarize vaccine development for COVID-19. We anticipate this study will be a valuable resource for any researcher interested in creating a COVID-19 vaccine.

Abstract

The use of marijuana/cannabis for therapy, especially for certain neurological diseases, is still causing controversy in various countries, including Indonesia. Various studies continue to be carried out while certain parties feel they have the right to get treatment through marijuana based on testimonials and previous research. The urge to legalize the use of marijuana as an alternative therapy is getting tougher, including in Indonesia. Judging from medical ethics, therapy can be applied if its use is following medical indications, the benefits outweigh the risks, its use respects human dignity, and has been clinically tested with scientific research. This research used literature studies from various references to analyze the medical and ethical aspects of using marijuana as a therapy. Potentially Cannabidiol in cannabis can be an alternative therapy, provided that it has passed the correct and ethical scientific research, which is not yet complete. Thus, the legalization of the use of cannabis/marijuana as a therapy still requires stronger and more valid scientific evidence so that the use of cannabis does not reduce human dignity, especially patients as vulnerable people under the pretext of humanity.

Abstract

It has been a concern that using antifungals may induce some fungi to develop antifungal resistance in the future. Therefore, systemic anti-candidiasis agents should become a focus in controlling antifungal drugs since it is quite commonly used. There are currently three approaches to using systemic anti-candidiasis agents based on their indication, i.e. definitive, empiric and pre-emptive indication. These can be applied by observing supportive findings such as the presence of Candida sp infection or colonization, the severity of the infection and the patient's risk factors. The severity of invasive candidiasis is usually severe, and various risk factors need to be considered, such as Total Parenteral Nutrition (TPN), catheterization including deep vein catheter, central venous catheter (CVC), etc.  Antifungal stewardship program, including management of systemic anti-candidiasis, is essential nowadays. Therefore, it is necessary to have a program that can serve as a guideline for clinicians to implement a treatment approach for systemic anti-candidiasis. RASPRO Alur Anti Candida Sistemik (RASCANDIS) 1.0 form or the Indonesian Regulation on the Prospective Antimicrobial System on Systemic Anti-Candidiasis Flowchart 1.0 form is an actual implementation to provide guidelines for clinicians to administer systemic anti-candidiasis agents for non-transplant patients. The form is not a diagnostic tool, but it is more likely to serve as a review and summary of knowledge obtained from various scientific journals, which is expected that it can be proposed as an effort to administer therapeutic management of systemic anti-candidiasis appropriately.

Abstract

Background Inflammation is one problem in the oral cavity that patients often complain about. Anti-inflammatory drugs are generally used to treat inflammation but these drugs have side effects therefore currently many anti-inflammatory drugs are developed from natural ingredients, one of which is Tamarillo. The flavonoid of Tamarillo fruit peel can inhibit inflammation. Carrageenan is an irritant that is often used as an indicator of inflammation because it has many advantages. Macrophages are innate immune cells that are important in inflammation. Macrophage cluster of differentiation 68 or CD68 expression can be used to detect inflammation activity. The objective of this study is to detect CD68 expression on macrophages as the anti-inflammatory effect of Tamarillo fruit peel (Solanum betaceum Cav.) ethanol extract. Methods This study used biological specimens of in vivo experimental research by observing immunohistochemical preparations of 5 sample groups, diclofenac sodium as a positive control, NaCl as a negative control, and Tamarillo fruit peel extract groups at doses of 70, 140, and 280 mg/kg.BW. Statistical analysis was performed using the one-way ANOVA test. Results There were significant differences (p< 0.05) in CD68 expression between each treatment group on the 24, 48, and 72 hours with the lowest number of expressions on the 72 hours of 280 mg/kg.BW group. Conclusions Ethanol extract of Tamarillo (Solanum betaceum Cav.) fruit peel has an effect on CD68 expression of macrophage cells in the buccal mucosa of carrageenan-induced rats with the most optimal dose of 280 mg/kg.BW on 72 hours.

Abstract

Stroke and chronic kidney disease are major health problems worldwide. Acute ischemic stroke is a significant complication in patients with end-stage renal disease undergoing hemodialysis. The incidence of stroke is about 8-10 times higher in chronic renal failure patients on hemodialysis compared to the general population. The annual incidence rate of acute ischemic stroke is reported to be about 4% in chronic renal failure patients on hemodialysis. Initiation of dialysis in chronic renal failure patients is associated with an increased risk of stroke. Chronic renal failure disease is an independent risk factor for stroke. A 57-year-old woman complained of left-side weakness during hemodialysis. Paresis of cranial nerve VII and XII and grade 3 flaccid hemiparesis sinistra were found. Electrocardiogram showed normal atrial fibrillation response. Non-contrast head CT scan showed subacute ischemic cerebral infarction in the right internal capsule. Factors that cause stroke in chronic renal failure are endothelial dysfunction, accelerated arteriosclerosis, and impaired cerebral autoregulation.

Abstract

Cytoglobin (Cygb) is a new member of the globin protein family, following the discovery of other globin proteins such as hemoglobin (Hb), myoglobin (Mb), and neuroglobin (Ngb). In 2001, Kawada et al. identified Cygb in hepatic stellate cells (HSCs) that play an important role in the repair of damaged liver cell tissue, hence the name stellate cell activation-associated protein (STAP). Cygb has a more universal role than Hb, Mb, and Ngb, and is expressed throughout mammalian tissues. Cygb is a globin with 6 coordination bonds (hexacoordinate globin). Cygb has been shown to play an important role in the normal cell respiratory chain, including oxygen storage, destruction of reactive oxygen species (ROS), terminal oxidase activity, regulation of fibrogenesis, and regulation of apoptosis. The role of Cygb in the respiratory process has been studied because it is associated with the globin family, and because of the upregulation of Cygb during hypoxia, but its specific role has not been elaborated. Recent research reports that Cygb has several implications for cancer. In most cancer cells, Cygb expression is upregulated by hypermethylation, suggesting epigenetic control. In cancer cells, downregulation of Cygb occurs which indicates a possible role as a tumor suppressor gene. In some malignancies, on the contrary, Cygb upregulation may be associated with resistance to hypoxia which indicates Cygb has two sides or Janus faces related to its role in cancer cells.

Abstract

Background Student medical is one major favorite where his students are confronted with various relative constraints difficult to take, like many it's a mandatory academic target resolved and prosecuted adapt from transition high school to college sure height different from facet academic, then environment social making student stressed and triggered it happened stress.  stress will raise problems for students when not managed with ok. The spiritual Quotient (SQ) is one factor that plays a role in controlling stress. Students who have a High Spiritual Quotient will be capable of controlling stress. Methods Research this characteristic quantitative, observational analysis with an approach cross-sectional. The instrument used consists of a questionnaire from the level Spiritual Quotient with the title “questionnaire spiritual intelligence” and level stress with the title "MSSQ questionnaire" and well both have been already through validity and test reliability. The analysis study use analysis bivariate with the Rank Spearman correlation test. The sample study is student-level beginning Faculty Medical Unimus class of 2021 as many as 115 of the 150 students taken with the Simple Random Sampling technique. Results Connection significant (p=0.001) and strong correlation (r=0.568) between the connection Spiritual Quotient with level stress on Unimus FK Students Class of 2021. In part, students own High Spiritual Quotient in circumstances stress moderate (55.7%), stress weight (3.5%), and stress mild (0.9%). Conclusions Increasingly tall Spiritual Quotient possessed, than the more low-level stress experienced by students level beginning class of 2021.

Abstract

Background Hyperglycemia is a medical condition that increases blood glucose levels beyond normal limits because the body lacks enough insulin or insulin does not work properly and is known to trigger oxidative stress. In cells sensitive to Hyperglycemia, such as endothelial cells, excessive glucose loads trigger the formation of Reactive Oxygen Species (ROS) in mitochondria, impairing mitochondrial function. ROS are essential mediators for activating pro-inflammatory signaling pathways; obesity and hyperglycemia-induced ROS production may support the induction of proinflammatory macrophages such as M1 during the onset and development of diabetes. Consumption of olive oil can increase plasma antioxidant capacity and reduce oxidative stress markers. Methods This study aimed to determine the effectiveness of olive oil antioxidants against Malondialdehyde (MDA) in hyperglycemic rats. This is an experimental design with a post-test-only group design. Adult male Wistar rats were divided into three groups (n = 8): negative control (K-), positive control (K+), and treatment (P). The normality test was conducted using the Shapiro-Wilk test, followed by Kruskal Wallis. Results The olive oil was proven to reduce levels of Malondialdehyde in the P group, with an average of 2.29 nmol/mL lower than the K+ group. Although the olive oil reduced the average MDA level in hyperglycemic rats, the post-hoc test showed no difference in MDA between the K+ and P groups (p-value = 0.226). Conclusions Antioxidants in olive oil may effectively lower blood glucose levels, reducing levels of Malondialdehyde in hyperglycemic rats.

Abstract

Sepsis is an emergency condition as a result of host dysregulation systemic immune response to infection that is related to the end stage of organ dysfunction. Both sepsis and septic shock conditions are the main problems in ICU especially, those that affect millions of people in the whole world every year. Studies in line with immune-neuroendocrine related to sepsis get high attention about factors that play roles in sepsis pathogenesis and prognosis, like correlation to gender, hormones, and other factors. In this case, lots of experimental studies and clinical studies showed that sepsis has significant sexual dysmorphic. The female gender has proven protective against sepsis, meanwhile, males could have worse sepsis because of decreasing immunologic response that mediates cell and cardiovascular function. Estrogen is a hormone in women that plays important roles in not only reproductive function but also non-reproductive function. Physiological estrogen in women is divided into three forms: estrone (E1), estradiol (E2 or 17β- estradiol), and estriol (E3). Several experimental studies in animals showed that estradiol has a protective response when infection occurs. Estrogen generally stimulates cytokine release, induction of HO-1, and restoration of organ function due to sepsis. Potential pathogenesis for this condition is a specific expression of cytokine pro and anti-inflammation. This pathological inflammatory condition is related to gender that is found in surgical patients at the molecular level.

Abstract

Background High blood glucose and cholesterol are risk factors for hypertension. This study aims to determine the correlation between blood glucose and cholesterol levels with blood pressure in the normal blood pressure (NBP), controlled hypertension (CHT), and uncontrolled hypertension groups (UHT). Methods The study used a cross-sectional design with analytic observations on subjects aged 36 years or older. Ninety-five subjects were divided into three groups: NBP, CHT, and UHT. Subjects were men and women, without chronic heart failure or chronic renal failure. Samples were taken by consecutive random sampling. Blood pressure, body mass index, random BG, and lipid profile (triglycerides, HDL, LDL, total cholesterol) were measured. Statistical test using Spearman correlation test with p-value <0.05 significantly. Results There were 95 subjects with a range age of 36-81 years old. There were 30 NBP subjects, 34 CHT subjects, and 31 UHT subjects. There was a weak positive correlation between HDL level and diastolic BP in the NBP group (r=0.391;p=0.032). There was no correlation between blood glucose and other lipid profiles with BP in the three groups. Conclusions The increase in HDL is accompanied by an increase in diastolic blood pressure in NT but not with random blood sugar and other lipid profiles in all three blood pressure groups.
