Abstract

Background: Electrosurgery is a common surgical technique used to treat skin tumors. This procedure produces necrotic skin, which might impair the wound healing process, prompting physicians to prescribe topical antibiotics to prevent infection. This study aims to analyze the efficacy of sodium fusidate ointment compared to petrolatum for wound healing after electrosurgery. Methods: We conducted a double-blind, randomized, controlled study with a within-person design. The inclusion criteria were adult subjects with seborrheic keratosis or acrochordon with a size of 4 mm to 10 mm on the face and neck, with a minimum of 2 lesions. Electrodessication was performed on the subject lesions. The patients were then randomized to receive and sodium fusidate ointment or petrolatum. Follow-up was done for up to 14 days, during which wound healing score, erythema, edema, crusts, re-epithelialization, incidence of infection, and subjective symptoms were assessed. This study is registered in ClinicalTrials.gov (NCT05353374). Results: Twenty-two subjects with 90 wounds were enrolled. The subjects had a mean age of 48.18 ± 11.25 years old and 86.4% of them had seborrheic keratosis. Statistical analysis revealed no significant difference in erythema, edema, crusts, re-epithelialization, infection, subjective symptoms, and total wound healing score appearance on days 3, 7, and 14 following the procedure. Conclusion: Both sodium fusidate ointment and petrolatum showed similar efficacy for wound healing after electrosurgery.

Abstract

Background: Epidemiological data on occupational hand eczema in Indonesia is still limited, partly because there are no valid and reliable instruments in the Indonesian language as a means of survey. This study aims to translate the Nordic Occupational Skin Questionnaire 2002 (NOSQ-2002) into the Indonesian language and assess the validity and reliability of the Indonesian NOSQ-2002 as an instrument for epidemiological surveys and screening of occupational hand eczema. Methods: The original English version of NOSQ-2002 was translated into the Indonesian language following the standard procedure of translation. The Indonesian NOSQ-2002 version, which had been collectively approved, was subsequently completed by a group of 194 textile employees from PT. Panca Persada Mulia-PANDATEX in Magelang, Central Java, Indonesia. Validity was assessed using Pearson correlation for each question with the total score. Reliability was evaluated using Cronbach's alpha values. The sensitivity and specificity of the Indonesian NOSQ-2002 screening were determined by comparing results with those of examination by the examining physician as the gold standard. Results: Pearson correlation values for each question range from 0.252 to 0.905, all surpassing the r table value, indicating questions in NOSQ-2002 are valid. Reliability NOSQ-2002 rated good (reliable), with a Cronbach's alpha of 0.933. The Indonesian NOSQ-2002 version demonstrates a sensitivity of 93.3% and specificity of 98.8% for screening occupational hand eczema. Conclusion: The Indonesian version of NOSQ-2002 is a valid and reliable instrument for use in epidemiological surveys and screening of occupational hand eczema.

Abstract

Background: Leprosy is a chronic infection caused by Mycobacterium leprae. Leprosy reactions, including erythema nodosum leprosum (ENL), are induced by immune responses. ENL can cause neurological damage and deformities that result in disabilities. This study aimed to identify factors associated with the severity of ENL to help control the leprosy reactions. Methods: This was an analytical study on 148 patients with ENL reactions, who were divided into two groups based on the severity of their reactions. A retrospective cross–sectional study was conducted in the medical record unit of Sitanala Hospital, Tangerang, Banten, Indonesia, from May 2022 to April 2024. Data evaluated included age, gender, leprosy types, treatment status, duration of illness, clinical manifestations, involvement of other organs, disability, comorbidities, acid-fast bacilli, and laboratory examination. Categorical data was analyzed with chi-square test or Fisher’s exact test, and numerical data was analyzed with the Mann-Whitney test. Significance is determined at p < 0.05. Results: Characteristics including age, neuritis, skin ulcers, and comorbidities differ significantly between mild and severe ENL (p = 0.034; < 0.001; < 0.001; and 0.003, respectively). Severe ENL reactions tend to occur more frequently in older patients, and is associated with higher bacterial index (p = 0.004) and lower sodium and albumin levels (p = 0.014 and < 0.001, respectively). Conclusion: Age, neuritis, skin ulcer, comorbidities, bacterial index, as well as sodium and albumin levels were associated with the severity of ENL reactions. Appropriate management is required to prevent further complications.

Abstract

Background: Facial rejuvenation has become a global trend in cosmetic and aesthetic dermatology to restore youthful skin. The combination of 1064 nm Q-Switched Neodymium:Yttrium Aluminum Garnet (QS Nd:YAG) laser and exosomes as a facial rejuvenation procedure restores youthfulness by increasing collagen and elastin. Case Illustration: This case series evaluated the effectiveness of 1064 nm QS Nd:YAG laser with and without human amniotic fluid stem cell-derived exosomes (hAFSC-exo) for facial rejuvenation in six adults aged 24–39 years with Fitzpatrick skin types III-IV. Participants were randomized to receive either laser treatment alone or combined with intradermal hAFSC-exo injections every two weeks for three sessions, with outcomes assessed through blinded objective and subjective evaluations. Objective assessments using a skin analyzer showed improvements for both groups, with consistently greater improvement in the combination therapy group. Meanwhile, subjective evaluations using the GAIS scale indicated that combination therapy recipients experienced “very much improved” outcomes, minimal side effects, and good overall tolerance. Discussion: Facial rejuvenation using 1064 nm QS Nd:YAG laser and hAFSC-exo was evaluated in this case series. Both treatments improved skin quality, but combination therapy demonstrated superior results. However, improvements in pigmentation were limited, and subjective assessments may have been influenced by response bias. The short follow-up period limited the evaluation of long-term skin changes; extended studies are warranted to assess sustained outcomes. Conclusion: Combination therapy of 1064 nm QS Nd:YAG laser and exosomes has a favorable result for facial rejuvenation.

Abstract

Background: Latent varicella-zoster virus (VZV) in nerve ganglia may reactivate into herpes zoster infection. The coronavirus disease (COVID-19) vaccine is suspected of triggering VZV reactivation, although the exact cause is unclear. This case report aims to raise awareness of the potential complications induced by the COVID-19 mRNA vaccine, particularly in elderly patients. Case Illustration: A 62-year-old woman complained of itchy and painful unilateral dermatomal herpetiform blisters six days after mRNA COVID-19 vaccination. The vesicle first appeared on the left breast, spreading to the back. The patient has a history of hypertension. After the administration of 800 mg of acyclovir five times a day for seven days, the vesicles dried up, and no new vesicles emerged. Discussion: The incidence of herpes zoster has been widely reported to occur within 5-7 days after the COVID-19 vaccination, mostly due to the mRNA vaccine. The dysregulated cell-mediated immune system may trigger VZV reactivation. The vaccines have been shown to induce a cellular response with increased CD8+ T-cells and T-helper type 1 CD4+ T cells. The VZV-specific CD8+ cells cannot control VZV after the massive shift of naïve CD8+ T-cells in the setting of COVID-19 vaccination. Hypertension is also allegedly the main comorbid factor for herpes zoster reactivation after COVID-19 vaccination. Conclusion: Increased awareness and early recognition of potential COVID-19 vaccine-related complications, especially in elderly individuals with comorbidities, are important for timely management and prevention of further issues.

Abstract

Background: Acne conglobata (AC) is a rare and severe acne characterized by interconnected nodules, cysts, and abscesses. Mesenchymal stem cell-derived exosomes (MSC-exosomes) have immunosuppressive properties that alleviate inflammation and immunological reactions. We present a case of AC in a patient who received topical administration of exosomes. Case Illustration: A 20-year-old male was diagnosed with AC, presenting as inflammatory papules and cysts covered with yellowish crusts that had been present for five months. Exosomes (ASCE+®, Seoul, South Korea) were topically administered at a dose of 5 mL, applied evenly to the face. The patient underwent three treatment sessions, with a one-week interval between sessions. Photographic images were captured before treatment, immediately after the final session, followed by one week and then three weeks after the final treatment session. The patient expressed complete satisfaction with the implemented treatment and disclosed no adverse effects. Discussion: The present case report illustrates the therapeutic advantages associated with the use of exosomes in a patient with AC. The therapeutic targeting of inflammatory regulators is crucial in treating AC. Studies have shown that MSCs possess inherent immunosuppressive properties that can help alleviate inflammation and immunological reactions. MSC-exosomes offer additional benefits, including angiogenic capacity and collagen synthesis. The study's limitations include the short follow-up duration and the lack of a control group. Conclusion: Exosomes have the potential to serve as cosmeceutical in the treatment of AC, due to their anti-inflammatory, regenerative, and antimicrobial properties.

Abstract

Background: Nevus sebaceous of Jadassohn (NSJ) is a non-hereditary hamartoma of the skin’s adnexal structures. The incidence of NSJ in newborns is estimated at 0.1-0.3% with no gender or ethnic propensity. Surgical excision is a common treatment for NSJ, although there is no definite consensus on the timing or recommendations for the removal. This article aims to present the advantages of serial excision therapy in a case of NSJ. Case Illustration: A 14-year-old male presented with a complaint of a bump on the scalp since birth, which had progressively enlarged during puberty. Dermatological examination revealed a well-demarcated, yellow-orange verrucous plaque with irregular borders measuring 3x3.5 cm. The diagnosis was confirmed through dermoscopy and histopathology. The patient underwent a three-stage serial excision at 3-4-week intervals. Discussion: The excision technique for NSJ excision was selected based on the location and size of the lesion. Serial excision is a widely used and straightforward method. This method was chosen due to the lesion’s large area. However, NSJ excision on the scalp presents its own challenges due to limited soft tissue flexibility, hair-bearing skin, and a convex surface, which may affect reconstruction outcomes. Conclusion: The three-stage serial excision resulted in complete removal of the NSJ, minimal scarring without significant alopecia, and no recurrence during the 6-month follow-up.

Abstract

Background: Psoriasis is a dermatological condition influenced by genetic and epigenetic factors, environmental triggers, as well as immune responses. Genomics refers to the study of individual genes (genomes) and their interactions with environmental influences. In the context of psoriasis genomics, the interplay between genetic predisposition and environmental factors contributes to the manifestation and variations of the disease. With advancements in molecular genotyping technology and genome-wide association studies (GWASs), researchers have identified 50 genetic markers linked to psoriasis. Discussion: Several genetic loci associated with psoriasis have been identified, including PSORS 1-15. The genetics of psoriasis is also linked to its onset, precipitating factors, associated comorbidities, and clinical variations. Advances in psoriasis genomics are leading to more targeted and innovative treatment approaches. The latest therapeutic developments for psoriasis mainly focus on the interleukin-23 (IL-23)/T-helper 17 (Th17) pathways. Conclusion: The development of GWASs provides new knowledge about psoriasis, including its pathogenesis and specific treatments.

Abstract

Background: Snakehead fish (Channa striata, CS) has traditionally been used in Indonesia as a nutrient to promote wound healing. It contains protein highly similar to human protein, which may be involved in wound healing via biological pathways. We aim to determine the effectiveness of the topical 5% CS extract gel in promoting wound healing. Methods: A double-blind, placebo-controlled randomized clinical trial was conducted from June to August 2022 at the Department of Dermatology, Venereology, and Aesthetics of Dr. Mohammad Hoesin Hospital. We allocated 32 post-skin surgery participants with grade I acute superficial wounds on the face, neck, trunk, and extremities into 2 groups with one receiving 5% CS (n=16) and the other receiving placebo (n=16). Both groups applied the treatments twice daily. Assessments of wound healing using modified REEDA (redness, edema, ecchymosis, drainage, approximation) score, wound size, and adverse effects were done at follow-up days 1, 3, and 7. Results: Both groups showed REEDA score improvements on days 1, 3, and 7, with significant difference between both groups identified on day 3 (p=0.038). Wound size improved at each follow-up in all participants, with the 5% CS group being slightly better than the placebo group on day 3 (p>0.05). A mild adverse event was found in only one participant in the 5% CS group (3.1%). Conclusion: Both 5% CS extract gel and placebo had good safety profile and were effective in promoting wound healing. 5% CS extract gel was better at reducing REEDA score.

Abstract

Background: Skin aging is characterized by a gradual loss of tissue and organ function. Skin aging can be classified into two types: intrinsic aging, associated with the aging process itself, and extrinsic aging, which is linked to external factors. Microbotox—an intradermal injection of diluted botulinum toxin—has emerged as a promising technique for rejuvenation by improving skin texture, pore size, and sebum control. This review aims to provide a comprehensive overview of the microbotox technique in the context of skin aging, emphasizing its mechanisms, indications, and clinical outcomes. Discussion: In Asia, intradermal injection of botulinum toxin or microbotox has been widely adopted. The technique involves injecting microdroplets of onabotulinumtoxin type A (ONA) into the intradermal layer at lower concentrations compared to conventional methods. Microbotox is highly sought after for its ability to improve skin texture, address enlarged pores, control sebum, and rejuvenate the skin without the stiffness often associated with conventional botox. Conclusion: Microbotox provides multiple dermatological benefits, including improved skin texture, pore reduction, oil control, and fine line softening, with minimal side effects. As an intradermal technique, it offers a safer and subtler alternative to traditional botox. It provides rapid results lasting up to 3-4 months, making it a valuable tool in aesthetic dermatology. This has made microbotox one of the most popular skin rejuvenation treatments today.
