Abstract

Psoriasis is an autoimmune disease affecting the integumentary and organ systems. It impacts daily activities and degrades self-esteem. Meanwhile, there is a minimum amount of literature about self-care management education for Psoriasis. Pocketbooks can be used as an alternate learning tool for self-care management by making them more portable among patients. The aims are to foster a positive perception of illness and self-determination regarding Psoriasis. This study evaluated the effectiveness of providing self-care management education using a pocketbook on the illness perception of Psoriasis. The research employed a mixed-methods design, combining quasi-experimental and qualitative approaches. The study involves 30 participants diagnosed with Psoriasis. Participants are divided into two groups: a control and an intervention group. The Brief Illness Perception Questionnaire (B-IPQ) was utilized to assess the participants' perceptions of their illness before and after the intervention. A paired t-test was used to compare pre-test and post-test scores within each group. A two-sample t-test was employed to compare the mean differences between the control and intervention groups. The significance level was set at a p-value of 0.05. The interview data were transcribed verbatim using thematic analysis. The study indicates a difference in the illness perception of patients between control and intervention groups with a significance value of 0.000. Patients report that understanding triggers and management strategies allow for better health. Pocketbook is a practical education approach improving illness perceptions of Psoriasis. Recognizing triggers and adopting management techniques, patients are able to take steps to deal with flare-ups and improve their well-being.

Abstract

Obesity is associated with muscle function impairments and can progress to functional domain disorders including strength, mobility, and postural balance deficits. Exercise helps with weight management and enhances mass of muscle. Two physical exercise programs available are Elevated-Intensity Interval Exercise and Moderate-Intensity Constant Exercise. Choosing the right type of physical exercise for increasing muscle mass is very important for people with obesity. This study aimed to determine muscle mass changes following physical exercise intervention and compare muscle mass increases in obese women between interval versus constant exercise. The research design was a Randomized Controlled Trial (RCT) with 30 subjects divided into two groups (15 per group) using purposive sampling. Each participant performed cycling-based physical activity using a cycle ergometer for 12 weeks, three times weekly. Muscle mass was measured using Bioelectrical Impedance Analysis (BIA) before and after intervention. Data were analyzed using T-test. Paired T-test results showed significant muscle mass increases in both intervention models (p=0.000). Elevated-intensity interval exercise increased muscle mass by 1.2 ± 0.75, while moderate-intensity constant exercise increased it by 0.98 ± 0.54. Unpaired T-test revealed no significant difference between the two groups' mean muscle mass changes (p=0.341). Both moderate-intensity constant exercise and elevated-intensity interval exercise effectively increase muscle mass in obese women, with both training modalities having equal effectiveness in increasing muscle mass.

Abstract

Untreated depression in HIV patients is associated with lower CD4 cell counts attributed to the excessive HPA axis activity, which leads to increased cortisol secretion. This can accelerate the progression of HIV through shifting cytokine production from Th1 to Th2, which triggers CD4 cell destruction and stimulates HIV replication. This study aimed to determine the relationship between depression levels and CD4 counts in HIV patients at Dr. Saiful Anwar General Hospital Malang. This study used a quantitative method with an analytical observational study design and a cross-sectional approach. A total of 30 participants were selected through purposive sampling. The Beck Depression Inventory-II (BDI-II) questionnaire was used to evaluate the participants' depression levels. Six participants were identified with moderate depression, while 24 participants exhibited either no depression or minimal depression. The Spearman's Rank Correlation Coefficient test showed p=0.183, p>0.05, with a correlation coefficient of -0.250, indicating no significant relationship between the depression levels and CD4 cell count in HIV patients at Dr. Saiful Anwar General Hospital Malang. Nevertheless, among the study subjects with moderate depression (6 participants), low CD4 counts (≤345cells/mm3) were observed, suggesting a tendency for depression levels to be associated with CD4 count.

Abstract

Pseudomonas putida is a Gram-negative bacterium that can cause nosocomial infections, particularly in neonates. This study aimed to identify the characteristics of neonates with P. putida infection, the antibiotics administered, and the treatment outcomes during a P. putida outbreak. This descriptive study collected data from the medical records of neonates diagnosed with neonatal sepsis admitted to Ulin Hospital, Banjarmasin, between August and October 2022. Data collected included patient characteristics, clinical profiles, laboratory results, antibiotic therapy, P. putida antibiotic sensitivity results, and bacterial culture results from samples taken in the NICU during the outbreak period. Fourteen neonates (6.5%) had positive P. putida during the outbreak of blood cultures, including 13 neonates born at Ulin Hospital and one referred from another health facility. Neonates with P. putida infection exhibited leukopenia, thrombocytopenia, and elevated CRP levels. After obtaining the culture results and antibiotic sensitivity tests, the choice of antibiotics was adjusted based on the bacteria's sensitivity. The antibiotics found compelling during treatment included ciprofloxacin, cefepime, tazobactam, gentamicin, a combination of ciprofloxacin and amikacin, and a combination of piperacillin. Most of the clinical and laboratory characteristics improved following treatment, but three neonates unfortunately died during hospitalization. Overall, there were clinical and laboratory improvements after administering antibiotics according to the culture-based antibiotic resistance results.

Abstract

Stroke and Chronic Obstrutive Pulmonary Disease (COPD) are two diseases that have high mortality and disability rates in the world. Smoking is one of the risk factors that cause these two diseases. The occurrence of Non-Hemorrhagic Stroke (NHS) and COPD simultaneously in one patient will cause a poor prognosis and more comprehensive treatment. In addition, there are no therapeutic guidelines for these two diseases if they occur together. This case report reports a case of NHS and COPD in a 64-year-old man who received aminophylline therapy. The patient experienced hypotension on the second day of treatment induced by aminophylline administration. Discontinuation of aminophylline therapy in the patient and administration of other supportive therapies for NHS and COPD showed improvements in blood pressure and other medical conditions related to the patient's disease on the sixth day of treatment. A decrease in blood pressure by >15% in the acute phase of NHS is a quite dangerous condition because it causes a lack of collateral blood flow which results in a lack of blood flow in the penumbra area. Administration of aminophylline has the effect of lowering blood pressure because it increases the release of catecholamines that stimulate beta adrenergic to reduce vascular resistance. Aminophylline should be given >48 hours after the attack to prevent the expansion of cerebral infarction and recurrent NHS attacks and strict monitoring is required when administering COPD therapy accompanied by NHS in relation to the effects caused by the administration of this therapy.

Abstract

Recent global statistics indicate approximately 527,624 new cases of cervical cancer and 265,672 deaths annually. Additionally, recent research has identified a correlation between cervical cancer incidence and the use of hormonal or non-hormonal contraceptives. This systematic review aims to investigate this correlation. Articles meeting inclusion criteria from Google Scholar and PubMed within the last five years were searched. Eight eligible studies were included: six on hormonal contraceptives and two on non-hormonal contraceptives. The review findings suggest that the use of both non-hormonal and hormonal contraceptives increases the risk of cervical cancer. Non-hormonal contraception was associated with a slightly elevated risk, while hormonal contraceptives, especially oral contraceptive tablets used over an extended period, showed a clear link to increased cervical cancer risk.

Abstract

Cesarean section (C-section) is one of the main medical procedures performed at Dr. Saiful Anwar Regional Hospital in Malang. Implementing a clinical pathway (CP) as a standard of evidence-based care can influence the length of hospital stay for patients. This study aims to analyze the relationship between CP implementation and the length of hospital stay for C-section patients at Dr. Saiful Anwar Regional Hospital. The analysis was based on data from CP forms and patient medical records. The lowest CP implementation score in this study was 89.86%. A correlation test across all samples (n=97) revealed a significant relationship between CP implementation and length of stay (correlation coefficient: 0.246, p=0.008). Dividing the samples into four groups based on the most frequent diagnosis categories (placenta previa, PROM (Premature Rupture of Membranes), preeclampsia, and placenta accreta) showed significant differences in the length of stay between patients with PROM and those with preeclampsia (p=0.0001) and placenta accreta (p=0.002). The CP implementation rate for all C-section patients was above 80% and was significantly correlated with the length of hospital stay. In the future, CPs specifically designed for different C-section indications may be developed to accommodate various patient conditions.

Abstract

Tuberculosis is a systemic infectious disease caused by Mycobacterium tuberculosis. Pediatric tuberculosis patients often have reduced immunity due to malnutrition. Vitamin D supplementation improves host cell antimicrobial immune response, improving clinical outcomes. A study in Iran found that intramuscular cholecalciferol administration improved tuberculosis patients' outcomes, including weight gain and arm circumference when administered alongside tuberculosis therapy for 6 or 9 months. This study focuses on oral cholecalciferol for pediatric tuberculosis patients, analyzing its effects on clinical improvement, radiological features, laboratory findings, and its impact on weight gain, height, and vitamin D levels. The chi-square test was used to analyze the effect of vitamin D on the improvement of fever (p=0.638), cough (p=1.00), nutritional status (p=0.388), radiological features (p=0.510), decrease in leukocytes (p=1.00), lymphocytes (p=0.262), monocytes (p=0.575), and increase in vitamin D levels (p=0.149). The Mann-Whitney test was used to analyze the effect of vitamin D on the average increase in weight gain (p=0.392), height (p=0.044), vitamin D levels (p=0.770), and the average decrease in leukocytes (p=0.260), lymphocytes (p=0.055), and monocytes (p=0.687). The confidence level is 95%. Most clinical, radiographic, and laboratory outcomes in pediatric TB patients did not significantly change when vitamin D therapy was used. However, it markedly increased the patients' development, indicating possible advantages for these individuals.

Abstract

Candida sp. is renowned for causing oral candidiasis. The prevalence of this disease is around 20-25% and the prevalence can increase in patients with systemic factors (40.2,%). Meanwhile, resistance to antifungal drugs can worsen the prognosis, especially in patients who have systemic risk factors. Therefore, this study aimed to analyze the resistance profile of Candida sp. as a cause of oral candidiasis. The scoping review method was used based on Preferred Reporting Items for Systematic Reviews and Meta-Analyse Extensions for Scoping Reviews. Data were collected through the Pubmed, Springer Open, and Google Scholar databases. Based on the inclusion and exclusion criteria, a total of 18 articles were analyzed. The results showed that antifungal drug resistance test profile depended on the species of Candida and the status of comorbid disease. Based on species, antifungal drug resistance was higher in non-Candida albicans than in Candida albicans. In the comorbid group, Candida sp. resistance to antifungal drugs was higher in the azole group. Meanwhile, in the group without comorbidities, antifungal drug resistance in groups with comorbidities is higher in flucytosine, econazole, and ketoconazole groups than in nystatin, caspofungin, and miconazole. This suggest that the use of nystatin is still effective for treating oral candidiasis. Healthcare provider may need consider prescribing nystatin, especially for patients with systemic risk factors.

Abstract

Tuberculosis (TB) is still the main cause of death in people living with HIV (PLHIV). TB preventive therapy (TPT) and ARVs can reduce morbidity, mortality, and incidence of TB in PLHIV. With the severity of the immunodeficiency, extrapulmonary TB or disseminated TB occurs more often. Disseminated TB occurs due to the hematogenous spread of Mycobacterium tuberculosis, which occurs as a result of progressive primary infection or reactivation of latent TB infection. Disseminated TB can involve many organs such as the lungs, liver, and spleen. Delays in diagnosis often occur due to non-specific clinical manifestations; thus, the diagnosis needs to be supported by radiological and microbiological examination, as well as definite histopathological diagnosis. Treatment is given according to existing therapy guidelines with a more extensive duration and regard to the patient's clinical condition.
