Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

Thalassemia is a hereditary form of anemia that affects the synthesis of hemoglobin. The management of therapy in patients with b-thalassemia major which patients should receive continuous blood transfusions and increased iron absorption from the digestive tract causes excess iron in the body. This will lead to an increase of free iron level that triggers Radical Oxygen Species (ROS). Increased level of ROS can initiate lipid peroxidation which used as an indicator of oxidative stress in cells and tissues and produce reactive carbonyl, mainly malondialdehyde (MDA). Thus, MDA measurement is widely used as an indicator of lipid peroxidation. On the other hand, the risk of oxidative damage can be reduced by antioxidant, one of them is Vitamin E that is a fat-soluble vitamin with high potential antioxidant. The objective of this study was to analyze the effect of the dl-a-tocopherol (Vitamin E) administration on decrease of MDA serum level on pediatric patients with b-thalassemia major. This was a longitudinal observational study design for one group without comparison was conducted to examine the use of vitamin E to decreased MDA serum level on children patients with b-thalassemia major. The inclusion criteria were patients who rely on blood transfusions, patients who received only one type of iron chelating agents during the study period, the clinical condition is stable, agrees, and has completed the informed consent. In the course of the study of 21 patients there were variations in patient compliance in taking vitamin E tablet dosage 200 IU once-daily for one month: only 11 out of 21 patients consumed 30 tablets of vitamin E 200 IU (total dose of 6000 IU) in the 1-month study, and only data from those 11 samples will be analysed further. MDA serum level was measured pre- and post-administration of vitamin E and patient's characteristics of subjects was obtained for additional information. Pre-administration of vitamin E, serum level of MDA was 1239.4 ± 502.55 ng/mL with a range of 216.95 to 2297.3 ng/mL, whereas in the group post administration of vitamin E, MDA serum level was 786.49 ± 704.88 ng/mL with a range of 6.5380 to 1958.6 ng/mL. In conclusion, there was no significant difference in MDA serum level in the group pre- and post- administration of vitamin E (p = 0.15).

Abstract

Treatment with RT Inhibitors has been used as first line program in Mimika. Regular use of antiretroviral drugs can lower the amount of the virus, but after that HIV can become resistant to the drugs given. Viral resistance to the drugs can occur because of a mutation. If the resistance-associated mutations occur in the DNA of the virus, then the ARV provided will no longer be effective. The aim of this study was to identify the presence of resistance-associated mutations in DNA fragment that encodes the protease and reverse transcriptase. This study used cross sectional design with 84 subjects who had received antiretroviral for > 6 months. The study was conducted in Mitra Masyarakat Mimika Hospital. Laboratory process included extraction, RT-PCR, electrophoresis and sequencing. Data analysis used resistance interpretation algorithms in HIV Database. Results showed that 1 subject who did not receive protease therapy was identified as having minor mutation L10V, 1 subjects receiving NRTI inhibitors had mutation M184V motive and 1 subjects with M41L motive and 1 subjects who received NNRTI inhibitor therapy identified as having mutated Y181C and V108I motive. In conclusion, mutant HIV-1 related to ARV resistance has been identified in two subjects who had received antiretroviral therapy in Mitra Masyarakat Mimika Hospital.

Abstract

Cisplatin is one of platinum cytostatic drug for the medication of solid cancers, one of which is head and neck cancer. Adverse event that resulted during drug treatment was acute or chronic nephrotoxicity. Cisplatin concentration in proximal tubular epithelial cells is about 5 times the serum concentration. Platinum exposure on renal tubular cells bonding covalent complex which stimulate production of inflammatory factors that lead to apoptosis and necrosis cell. Cisplatin nephrotoxicity can be prevented by aggressive hydration or alternate method of administration. The aim of this study was to analyze the effectiveness of NaCl-Mannitol hydration on renal function of head and neck cancer patients receiving cisplatin 100 mg/m2 chemotherapy combination with 5FU or paclitaxel. This was a cohort, prospective, and observational study to analyze renal function of head and neck cancer patients receiving cisplatin 100 mg/m2 chemotherapy combination with 5FU or paclitaxel. Inclusion criteria were BUN 7-18 mg/dl and serum creatinine < 2 mg/dl of any cycle. All patients received infuse NaCl-Mannitol hydration with term that provided in Surgeon Departement of Dr. Soetomo General Hospital. Data obtained were BUN, SCr, and eClCr Cockroft-Gault, each was measured pre- and post-hydration. In cisplatin and 5FU chemotherapy combination value BUN pre-hydration (11,99 + 4,62) mg/dl, value BUN post-hydration (12,14 + 4,74) mg/dl and value serum creatinine pre-hydration (0,97 + 0,34) mg/dl, value serum creatinine post-hydration (1,02 + 0,37) mg/dl. Meanwhile to the combination of cisplatin and paclitaxel chemotherapy, value BUN pre-hydration (10,19 + 2,58) mg/dl, value of BUN post-hydration (10,43 + 2,31) mg/dl and value of serum creatinine post- hydration (0,98 + 0,26) mg/dl. In conclusion, NaCl-Mannitol hydration administration is adequate which is shown by BUN and serum creatinine in pre- and post-hydration data within normal limits.

Abstract

This study aims to determine the potency of CV 12 acupuncture as prevention of weight gain in ovariectomizedmice (OVX). Female mice strain BALB/c, 28 mice, 3-4 weeks, 25-35 grams were divided into four groups; namely K1 (n = 8, sham and vehiculum injection), K2 (n = 8, OVX and vehiculum injection), K3 (n = 7, OVX and injection of estrogen intraperitoneal 10 mg/kg/day) and K4 (n = 6, OVX and CV12 acupuncture with a frequency of 2 Hz in 10 mA of continuous spike current wave).Injection and acupuncture given every day for four weeks. Weight change in K1 (0.75 ± 2.8) grams, K2 (2.71 ± 2.4) grams, K3 (0.71 ± 0.95) g and K4 (0.93 ± 2.40) gram with a significant difference in the K2 and K4 (p = 0.011). a receptor expression in K1 (12.25 ± 2.96), K2 (5.43 ± 2.64), K3 (18.86 ± 1.57), K4 (17 ± 2.00) with a significant difference to the K2 K1, K3 and K4 with p<0.05. b receptor expression in K1 (11.38 ± 1.92), K2 (4 ± 1.41), K3 (16.86 ± 1.86), K4 (14.17 ± 5.24) with significantly different a receptor expression in visceral fat mass on the group K2 to K1, K3 and K4 with p<0.05. CV 12 acupuncture increases the expression of a and b estrogen receptor in visceral adipose tissue of OVX mice equivalently and able to prevent the increase of weight due to the estrogen withdrawal. This point could potentially be used as one of the management ofobesity prevention in menopause.

Abstract

Delivery, induction, stimulation, cesarean section with the aid of a vacuum extraction, for example, can reduce maternal confidence on the smooth delivery process, as well as improve postpartum stress. These stressors stimulate the HPA axis (hypothalamus-pituitary-adrenal), so that the adrenal cortex produces more cortisol hormone, it can increase postpartum blues. The objective of this study was to analyze the effect of delivery stimulation on protracted labor on cortisol levels and the occurrence of postpartum baby blues. This was an experimental study of non-randomized post-test control group. The subject of the study 30 patients in labor taken consecutive sampling, divided into 2 groups (normal delivery and stimulation) each group consist of 15 patients. In stimulation delivery group, it is examined of cortisol serum levels after five days and then continued for postpartum blues occurrence measurement. Statistical analysis using t-test for differences in levels of cortisol and chi square for analyzing the effect on the occurrence of post-partum blues (a=0.05). Mean level of cortisol in delivery stimulation group is 40.29 ± 5.58, in normal delivery is 33.59 ± 11.17, with p=0.047, meaning there are significant differences both study groups. Stimulation delivery increases the occurrence of post-partum blues 5.50 times compared to normal delivery (OR=5.50 and p=0.028). Mean cortisol levels on post-partum blues higher at 42.90 ± 6.97 compared to no post-partum blues 30.14 ± 6.66, p=0:00, which means there are significant differences both groups. In conclusion, there was significant relationship between stimulation in protrated labor that increases cortisol serum level and post partum blues incidence.

Abstract

The objective of this study was to analyze the elevation of osteoblast activity in bone marrow mesenchymal stem cells (BM-MSCs) in osteogenic medium by physiological doses of melatonin administration by measuring alkaline phosphatase (ALP) and osteocalcin level.This studyused BM-MSCs from Rattusnorvegiccus femur bone. Rat BM-MSCs were cultured in a-Mem medium, differentiated in osteogenic medium, and administrated melatonin up to 21 days. This study was divided into 4 groups, K0 (control group), K1 (administrated of 25 nM melatonin), K2 (administrated of 50 nM melatonin), and K3 (administrated of 100 nM melatonin). Rat BM-MSCs were characterized CD 45- and CD 105+ marker using imunocytochemistry analysis and stained with Alizarin red after 15 days treatment. ALP and osteocalcin level were measured using ELISA Kit in days 21st.There weren't differences ofALP level beetwen groups and there are differences ofosteocalcin level between control groups (K0) withK1, K2, dan K3, and beetwen K1 and K2. The conclusion of this study was that there were an elevation of osteoblast activity in rat BM-MSCs in osteogenic medium by physiological doses of melatonin administration characterized by the elevation of osteocalcin level.

Abstract

This study was to approve the increased secretion of Hsp 70, DNA damage, and inhibitor apoptosis protein in cisplatin therapy which influence apoptosis of oral cancer cell and to know mechanism of molecular pathology. This study was an in vitro experimental laboratory using Randomized Block Design. Cell culture of oral cancer divided from cisplatin resistance cancer cell and cancer cell never induce cisplatin. Two group of cancer cell would be given cisplatin therapy. Secretion of Hsp 70, DNA damage, Inhibitor of apoptosis protein, and apoptosis index would be examined. Cisplatin resistance cancer cell group showed lower apoptosis than never induce cisplatin cancer cell. Elevated secretion of Hsp 70 in cisplatin therapy group (p= 0.000, b=0.881). Lower secretion of DNA damage protein in cisplatin resistance cancer cell and it was not going apoptosis. In path regression analysis, cisplatin was significans through IAP pathway (p=0.000, b=0.726) to apoptosis. All type of cell cultures were also significans through IAP pathway (p=0.000, b=0.496) to apoptosis. Elevated IAP secretion influenced apoptosis (b= 1.000). In conclusion, cisplatin used IAP pathway to apoptosis. All type of cell cultures also used IAP pathway to apoptosis. Cisplatin resistance cell culture had stronger effect to IAP and IAP increased inhibition to apoptosis.

Abstract

Pain is the most common complaints in everyday practice. Pain is a clinical manifestation caused by stimuli due to potential tissue damage. Low level laser therapy (LLLT) has been used in reducing pain. One of indicator tissue healing is activation of transforming growth factor -b1(TGF-b1). Tissue healing will eliminate the pain. Do LLLT stimulate TGF-b1 expression? The objective of this study was to investigate the expression of TGF-b1 in low level laser therapy. Thirty male Wistar rats, body weight 250 – 300 gram, 3 month old, were divided into 3 groups, negative control (n=10), positive control (n=10) and treatment groups (n=10). The positive control and treatment groups were injected with Complete's Freund Adjuvant (CFA) 100 mL at left footpad. The treatment group treated with low level laser therapy Lasermed type 2100 wavelength 905 nm, out power 25-500 mW, dose 1 joule/cm2;5 times with interval 24 hours on left footpad. Samples were taken from the skin of footpad and immunohistochemical examination with monoclonal antibody anti- TGF-b1. Calculations carried out on brown reaction in macrophage cell. Data were analyzed using the Games Howell with SPSS 17. There were significant differences in the expression of TGF-b1 in treatment group (15.8±3.1) compared with the positive control group (3.9±1.1) and negative control group (4.6±2.1). There were no significant differences between expression TGF-b1 in positive control group and negative control group. In conclusion, low level laser therapy increased the expression of TGF-b1.

Abstract

Bone graft use in maxillofacial surgery is currently developing, though some problems still persist. One of promising solutions to the problems is application of platelet rich plasma which may accelerate soft tissue healing and bone formation. This study aims to provide basic understanding to the development of PRP application in bone healing where bone graft is applied on human maxillae. Bone defects were made on maxillae of albino rabbits, with an application of bone graft in control group and bone graft plus PRP in treatment group. Histopathological analysis was performed in both groups to assess osteoblast density and collagen tissue area. On day 21st, more osteoblast density and collagen area in treatment group are significantly observed (p<0.05, 95% confidence interval). Observations were made on hard callus formation. In conclusions, platelet rich plasma may increase density of osteoblast in rabbit maxillary bone graft.

Abstract

Cancer patients have a tendency to experience mental disorders, because the physical changes cause frustration and despair. It impacts on anxiety, depression, and social isolation. The purpose of this study was to analyze the influence of mindfullness meditation to stress levels and coping mechanisms in cancer patients. This study used pre experimental approach with one group pre-test post-test design. Samples were 32 cancer patients with composmentis condition in Indonesian Cancer Foundation East Java Branch, taken by total sampling. The instrument was Perceived Stress Scale and Coping Strategies Inventory. Statistics was verified with Wilcoxon Sign Rank Test and Paired T-test P < 0.05. Results showed the effect of Mindfulness Meditation on the level of stress p = 0.001, the adaptive of coping mechanisms p = 0.001 and maldaptive p = 0.003. In conclusion, Mindfulness meditation can cause the respondent to concentrate and focus on themselves, and also strengthen brain regions at the center of attention and executive function, interoception and mental flexibility.
