Abstract

ABSTRACTHypertension is one of the diseases that can cause people’s death and it usually does not give any symptoms. Hypertension often occurs to the pilgrims. Knowledge and proper attitudes about hypertension of prospective pilgrims from Indonesia, especially in Malang city (East Java), are expected to reduce morbidity and mortality rates because they can manage their illness, properly. The purpose of this study is to describe the level of hypertension-related knowledge and attitudes of prospective pilgrims from KBHI 2019. The method used in this study is a descriptive cross-sectional study. 48 participants have participated in this study. They are the 2019 pilgrims from Indonesia. The results showed that 18 people (38%) had good knowledge about hypertension, 17 people (35%) are intermediately educated and 13 people (27 %) are lack of knowledge. Furthermore, this study has indicated that 25 people (52%) have positive attitudes and 23 people (48%) have negative attitudes about hypertension. This study concluded that in the majority, the 2019 prospective pilgrims from Malang have good knowledge and acceptable attitudes about hypertension.Keywords: Hajj pilgrims, hypertension, knowledge, attitude

Abstract

Estrogen Alfa (ERα) is a receptor used as the main marker to identify the presence of tumors in the breast.compounds Gendarusin A-E have anticancer activity by inhibiting the poliferation of cancer cells and inducing apoptosis. The purpose of this research are to predict the cytotoxic activity, physicochemical properties, and toxicity of the gendarusin A-E compound. The predictions of physicochemical properties were tested in compliance with the Five Lipinski Rules and the results of the ADME process (absorption, distribution, metabolism, and excretion) using the application pkCSM Online tool. Prediction of cytotoxic activity using Molegro Virtual Docker (MVD) by validating receptors and molecular docking. Cancer receptor protein used in Estrogen Alfa with PDB code 2JF9. Toxicity prediction using the Protox II Online tool. The results of this study indicate that the Gendarusin A-E compound didn’t completed the Five Lipinski Rules. Gendarusin A-E compounds had activity against receptors Estrogen Alpha which is shown by the results of RMSD 2 and Gendarusin A compounds had the smallest Rerank Score of -70.9817 compared to other compounds. Gendarusin B compound had the highest LD50  1212 mg / kg and classified in grade 4.

Abstract

The number of pilgrims of various ages and levels of education experience many health problems such as hypertension, respiratory infections and heart problems. From these problems encourage researchers to determine the response of prospective pilgrims to health services in Pademawu Pamekasan. The object of this study is the pilgrims in Pademawu Pamekasan Regency. The purpose of this study was to determine the response of the pilgrims in Pademawu District, Pamekasan Regency. This research uses a quantitative approach that is data collection using a questionnaire instrument. The sampling technique uses a total sampling population of 70 pilgrims. The study was conducted in the District of Pademawu Pamekasan to the Hajj pilgrims in 2018. The results of the study of the pilgrims' response to the quality of service include cognitive, affective, conative, age and education levels. Overall, the response of the haj pilgrims to the quality of service on the cognitive aspect was 29.5%. In the affective aspect of 4.69%. In the conative aspect 2.05%. At the age factor of 4.51% and at the aspect of Education level as much as 2.19%. The quality of health services affects the response of the pilgrims to the cognitive, affective and conative aspects but is not influenced by the age and level of education of the pilgrims.Keywords: hajj response, service, health

Abstract

Estrogen deficiency causes various health problems in postmenopausal women, including neurodegenerative disease. Phytoestrogens emerged as a group of compounds that can replace the estrogen function in the body, and prevent the neurodegenerative disease to occur. Semanggi (Marsilea crenata Presl.) is a typical plant in Surabaya, Indonesia, that contain phytoestrogens. The aim of this research was to determine the metabolite profile of n-hexane fraction of Semanggi leaves using UPLC-QToF-MS/MS, and then to predict the neuroprotective activity of compounds with in silico study using PyRX 0.8 software. The 100 ppm of n-hexane fraction of Semanggi leaves in DCM and methanol were injected 5 µl each into the UPLC-QToF-MS/MS, and then analyzed by Masslynx 4.1 software to determine the compounds. The compounds from metabolite profiling then prepared with SwissADME webtool and Avogadro 1.90.0 software, molecular docking was done using Autodock Vina and Biovia Discovery Studio Visualizer 2016 to 3OLS protein. Metabolite profiling process shows a total of 34 predictable compounds and 28 unknown compounds. From in silico study, it shows a total of 7 compounds that are predicted to have activities similar to estrogen. This result indicate that n-hexane fraction of Semanggi leaves has potential as a neuroprotective agent for treatment for postmenopausal women who experience estrogen deficiency.Keywords: Marsilea crenata Presl., phytoestrogens, metabolite profiling, in silico, neuroprotective

Abstract

Statins are generally recognised as being safe and well tolerated cholesterol-lowering drug and have been successfully used for prevention of primary and secondary cardiovascular disease. However, Some researchers have reported diabetes development in patients taking statins. A number of meta-analyses conducted in recent years have proved that the association is real even though its causality have not been fully elucidated. Various pathophysiological mechanisms that could explain the increased risk of diabetes in patients treated with statin have been described. These are mainly responsible for impairment of β -cell insulin secretion and alteration of intercellular signaling through depletion of important downstream products.  This review aims to examine the relationship between statin treatment and the presence of diabetes. Previous clinical reviews of the evidence and pathophysiological mechanisms involved may also be explained. Furthermore, many studies have concluded that Pitavastatin and pravastatinom do not affect glycemic control and may be a beneficial treatment option in patients with, or at risk for, type 2 diabetes.

Abstract

Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) merupakan reaksi obat berat pada kulit, idiosinkratik yang ditandai dengan demam, ruam dan kegagalan multiorgan akibat induksi obat. Stevens Johnson Syndrome (SJS) / Toxic Epidermal Necrolysis (TEN) adalah kelainan kulit berupa lepuh pada perbatasan dermal dan epidermal yang diakibatkan karena kematian keratinosit. Perbedaan antara SJS/TEN dilihat berdasarkan luas area epidermis yang terlepas (epidermolisis), yaitu: 10% untuk SJS, 10-30% untuk SJS overlap TEN dan 30% untuk TEN. Dari gejala yang ditimbulkan maka perlu dilakukan penelitian tentang penilaian kualitas hidup pasien yang mengalami DRESS dan SJS/TEN akibat penggunaan obat. Metode yang digunakan pada penelitian non-experimental observasional ini dilakukan dengan rancangan cross sectional. Pemilihan subjek penelitian menggunakan populasi pasien yang mengalami DRESS dan SJS/TEN berdasarkan kriteria inklusi. Pengambilan data sekunder melalui rekam medis secara retrospektif dan penilaian kualitas hidup menggunakan kuesioner EQ5D secara prospektif. Dari penilaian kualitas hidup yang dinilai menggunakan kuesioner EQ5D, didapatkan nilai signifikansi ρ 0,05 pada semua domain. Nilai signifikansi (ρ) pada semua domain menyatakan bahwa terdapat perbedaan kualitas hidup yang bermakna antara pasien DRESS dan SJS/TEN akibat penggunaan obat. Untuk hasil analisis EQ5D VAS, didapatkan nilai rata-rata 76,3636 ± 14.48988 pada pasien DRESS dan 56,7647 ± 26.45414 pada pasien SJS/TEN. Nilai tersebut membuktikan bahwa pasien DRESS memiliki tingkat kualitas hidup lebih baik bila dibandingkan dengan pasien SJS/TEN. Nilai signifikansi pada EQ5D VAS ρ 0,05. Nilai signifikansi (ρ) pada EQ5D VAS menyatakan bahwa terdapat perbedaan kualitas hidup yang bermakna antara pasien DRESS dan SJS/TEN akibat penggunaan obat. Penilaian kualitas hidup menggunakan kuesioner EQ5D menunjukkan adanya perbedaan kualitas hidup yang signifikan pada pasien DRESS dan SJS/TEN akibat penggunaan obat. Kata kunci: Kualitas hidup, DRESS (Drug Reaction with Eosinophilia and Systemic Symptom, SJS (Stevens Johnson Syndrome), TEN (Toxic Epidermal Necrolysis), akibat penggunaan obat.

Abstract

This study aims to measure the performance of pharmacy inan in the (National Health Insurance) JKN era based ona financial perspective with parameters the net profit margin and pharmacy income. And the perspective customer is within the parameters of the quality pharmacy service in this matter tangibles  , reliability, responsiveness, assurance, and empathy in a pharmacy in Makassar city on customer satisfaction pharmacy and the number of customer visits to the pharmacy. This study employed analytic quantitave and qualitative methods. The research sample consisted of 5 apothecary 99 pharmacy customers. The study results on the financial perspektive show that the net profit margin pharmachie’s 2014 average value was 6,31% and that by 2018 it was 6,93% and the pharmacies were virtually in good condition. In the pharmacies income there are 2 pharmachies that have suffered a decline in the pharmacy’s turnover and 3 pharmachies have experienced an increase in the 2018 over 2014. As for the number of pharmcy customers, there is 1 pharmacy that has a decline and 4 pharmacy that have seen an increase in the number of customer visits to the pharmcy in 2018 over 2014.And the customer perspective, the test F show that the F Count of 5,111 and F Tables 2,31, so that it can be reached the conclusion that the quality of service made up of both reliabilty, tangible, responsiveness, assurance and emphaty affect the  satisfaction of the pharmacies customers in the city of Makassar.   Keywords: financial perspective, customer  satisfaction, pharmacy service’s quality, National Health Insurance

Abstract

Kesambi (Schleichera oleosa) is one of the forest plants from Indonesia that has potential as medicine. Kesambi has active compounds that act as antioxidants. The purpose of this study was to determine the potential of kesambi leaf as an antioxidant using the DPPH method. Samples of kesambi leaf were extracted in methanol, ethanol, and water solvents by the maceration method. The class of compounds contained in kesambi leaf was tested by the colour reaction test method for alkaloids, flavonoids, triterpenoids, phenols, steroids, saponins, and tannins. Antioxidant activity was tested by the DPPH method to determine IC50 values. Based on the results of the colour reaction test showed that methanol and ethanol solvents were able to bind 6 classes of compounds (alkaloids, flavonoids, triterpenoid, phenols, saponins, and tannins) and water solvents were only able to bind 5 classes of compounds (flavonoids, triterpenoid, phenols, saponins, and tannins) contained in kesambi leaf. The results of antioxidant activity tests using the DPPH method showed the lowest IC50 value was methanol extract (16,12µg/ml) compared to ethanol extract (20,43µg/ml) and water (904,28µg/ml). It was concluded that the leaf extract of kesambi has potential as an antioxidant and which has the best antioxidant ability was extracted in methanol. The class of compounds which were thought to be responsible for the antioxidant activity of the kesambi leaf extract based on the color reaction test were phenols, flavonoids and tannins.Keywords: antioxidant, kesambi, Schleichera oleosa, DPPH

Abstract

The route of administration of transdermal drugs is preferred because it is easy to use. However, there are limitations associated with the difficulty of drugs penetrating into the skin. This is caused by the presence of the stratum corneum which is the main barrier for drug entry into the skin. Penetration of the drug into the skin can be through the trans-epidermal (transcellular and paracellular) route and the trans-appendegeal route depending on the dosage form. To increase the penetration ability of transdermal drugs, it can use chemical enhancers and physical enhancers. Chemical enhancers can be classified based on chemical structure or based on the mechanism of action. Chemical compounds that have the same functional groups can have different mechanisms of action depending on the physical-chemical nature. Chemical enhancers are categorized based on their chemical structure, including: water, alcohol, amides, esters, alcohol ethers, pyrrolidone, hydrocarbons, sulfides, surfactants, terpenes, phospholipids, vesicles. Whereas physical enhancers in the form of drug delivery use external energy to encourage or physically damage the stratum corneum. Abstract

Glucocorticoids (GCs), a class of corticosteroids that widely used a variety of diseases, associated physiological processes in the body [1,2]. It has structurally and pharmacologically similar to the endogenous hormone cortisol with various beneficial functions like anti-inflammatory, immunosuppressive, anti-proliferative, and vasoconstrictive effects. As an anti-inflammatory and immunosuppressive effect, corticosteroids through a combination of both inhibition upregulation of gene transcription [3-5]. Despite beneficial effects, they also have side effects that depend on the dose, type of steroid and length of treatments [6]. Short courses of high-dose GCs usually are safe and reasonably well-tolerated, but they do have numerous potential adverse effects [7]. Many of the severe complications occur in long-term use at doses greater than 20 mg of prednisone per day for three weeks or more causes tertiary adrenal insufficiency due to the HPA-axis (Hypothalamic Pituitary Adrenal Axis) suppression by endogenous GCs [8-10]. In this article, we’ll discuss how to manage GCs tapering and when is the right time to use GCs.
