Abstract

Brain-derived neurotrophic factor (BDNF) merupakan salah satu molekular neurotropin dalam sistem saraf yang berperan dalam modulasi dan perkembangan sinapsis serta sebagai regulator penting dalam proses transmisi di hipokampus, neokorteks, otak kecil, striatum, dan amigdala yang berpengaruh dalam proses neuroregenerasi, neuroprotektif, dan neurogenesis. Tingginya angka depresi pada sebagian besar usia dewasa menetapkan bahwa depresi menjadi salah satu perhatian penting dalam masalah kesehatan mental sehingga banyak penelitian meneliti tentang masalah ini pada manusia. Pada penelitian ini digunakan metode narrative review dengan cara mencari artikel dari search engine Pubmed dan Google Scholar dengan kata kunci “BDNF” dan “nutrition” dan “probiotic” dan “depression” dan “vitamin”. Kriteria inklusi berupa artikel penelitian 10 tahun terakhir, bahasa Inggris, full text dapat diakses. Pada narrative review ini, didapatkan lima artikel penelitian yang menguraikan tentang hubungan sintesis BDNF di hipokampus dan daerah lainnya dengan perbaikan gejala pada pasien depresi melalui pemberian jus jeruk murni dengan kandungan flavonoid, suplementasi probiotik, konsumsi minyak zaitun, suplementasi vitamin D dan magnesium, serta konsumsi vitamin C. Hasil telaah menunjukkan terdapat peningkatan kadar BDNF di hipokampus, serta peningkatan modulasi jaringan saraf, neuroplastisitas, neuromaturasi, dan pembentukan sinapsis pada uji coba otak hewan pengerat pada pemberian flavonoid, probiotik, minyak zaitun, suplemen vitamin D dan magnesium, serta vitamin C. Oleh karena itu, pemberian flavonoid,probiotik,  minyak zaitun, vitamin D dan magnesium, serta vitamin C dapat menurunkan tingkat dan gejala depresi dengan cara meningkatkan sintesis BDNF pada hipokampus dan daerah otak lainnya

Abstract

Depresi pasca persalinan merupakan kondisi mental yang ditunjukkan oleh perasaan yang konstan merasa rendah diri pada ibu yang baru melahirkan, dengan disertai perasaan sedih, penurunan harga diri, dan rasa putus asa. Estrogen memiliki efek indirek terhadap suasana hati melalui pengaruhnya pada fungsi endokrin, proses peradangan selama masa nifas, serta memodulasi aktivitas molekuler otak seperti brain-derived neurotrophic factor (BDNF) dan cyclic AMP response element binding protein (CREB). Perubahan hormonal yang signifikan pada ibu yang baru melahirkan memicu para peneliti untuk mengetahui pengaruh estrogen pada depresi pasca persalinan dan peluangnya dalam pengobatan. Tujuan dari telaah literatur ini adalah mengetahui pengaruh estrogen terhadap depresi pasca persalinan. Metode penelitian adalah telaah delapan artikel yang diperoleh dari PubMed dan BMJ journal dengan kata kunci “estrogen” dan “postpartum depression” dan dipilih berdasarkan kriteria inklusi seperti tahun terbit (2014-2024) dan isi artikel. Hasil telaah pada delapan artikel penelitian yang diperoleh menunjukan lima penelitian menunjukkan tidak ada pengaruh pemberian estrogen terhadap depresi pasca persalinan, sedangkan tiga penelitian menunjukan ada pengaruh. Depresi pasca persalinan bukan hanya karena faktor hormon estrogen, namun masih banyak faktor lain yang menjadi etiologinya. Kesimpulan telaah ini, sampai saat ini tidak ada bukti langsung pengaruh terapi estrogen untuk depresi pasca persalinan, namun masih memiliki potensi untuk dilakukan penelitian lebih lanjut sebagai terapi ajuvan.

Abstract

BackgroundSkin aging is a complex biological phenomenon influenced by intrinsic and extrinsic factors. Photoaging can be prevented by applying phytochemicals that have sun-protective properties. This study aimed to evaluate the effect of saffron serum to restore collagen density and autophagy processes and reduce inflammatory gene expression in UVB-exposed Wistar rats. MethodsAn experimental laboratory study was conducted involving 20 male Wistar rats that were divided into 4 groups: control, UVB, UVB + base serum, UVB + saffron serum exposed to UVB radiation for 5 weeks with a total dose of 3100 mJ/cm2. The skin was extracted then underwent Masson Trichrome staining and real-time PCR to obtain collagen density and gene expression. ResultsThe gene expression of MMP1, IL6, TNFá, LC3, and p62 was significantly increased in the UVB group compared to the control group. Topical administration of saffron serum significantly increased collagen density (p=0.001). Induction by UVB significantly increased LC3 (p=0.020) and p62 (p=0.030) gene expression, indicating an inhibition of autophagy. The saffron serum might modulate autophagy by increasing LC3, but not significant (p=0.495) and significantly decreasing p62 gene expression (p=0.001). As for MMP1, IL6, and TNFá, no significant decrease in gene expression was found in the UVB + saffron serum group compared to the UVB group. ConclusionSaffron serum increases collagen density and modulates autophagy in the skin of UVB-exposed Wistar rats. Inflammatory markers were increased after UVB induction, but no differences were found after saffron serum topical administration.

Abstract

Tahapan untuk menjadi dokter perlu melewati tahap sarjana kedokteran lalu program profesi dokter dan diakhiri dengan ujian UKMPPD sebelum akhirnya disumpah dokter. Pada kenyataannya terdapat banyak mahasiswa fakultas kedokteran yang tidak lulus UKMPPD sementara berdasarkan IPK pada tahap sarjana kedokteran dan program profesi dokter dinyatakan lulus. Tujuan penelitian ini adalah mengetahui ada tidaknya korelasi antara IPK sarjana kedokteran dan program profesi dengan nilai CBT UKMPPD. Data diambil pada 2022 di Fakultas Kedokteran Universitas Kristen Maranatha. Penelitian ini melibatkan 1.183 mahasiswa Fakultas Kedokteran Universitas Kristen Maranatha yang mengikuti UKMPPD pada tahun 2016 sampai tahun 2022. Pengolahan data sekunder dilakukan dengan metode analisis bivariat dan multivariat. IPK sarjana kedokteran dan IPK program profesi dokter memiliki korelasi signifikan dengan nilai CBT UKMPPD. Pada tahap Pendidikan profesi seharusnya dalam hal praktik tetap melakukan tindakan sesuai dengan teori yang ada agar sesuai dan relevan dengan pembelajaran teori pada tahap akademik dan CBT UKMPPD.

Abstract

Introduction: In the field of medical education, the evaluation process of communication and clinical skills training is known as Objective Structured Clinical Examination (OSCE). OSCE can be assessed using the global rating method, which is a method of subjective evaluation by examiners and standardized patients of student performance. Objective: The study aims to examine the relationship between the subjectivity of examiners and standardized patients in OSCE scores using the global rating method. Methods: The study involved 985 students from the Faculty of Medicine at Maranatha Christian University who participated in the Local OSCE from December 2021 until January 2022. Secondary data were analyzed using logistic regression in SPSS software and presented in tables and multivariate analysis. Results: The global rating assessment by examiners showed a significant relationship with OSCE scores (p<0.001), while the global rating assessment by standardized patients did not show a significant relationship with OSCE scores (p>0.05). This indicates that the evaluation by examiners has a greater influence on OSCE results compared to the assessment by standardized patients. Recommendations: More comprehensive training is needed for standardized patients in conducting assessments using the global rating method to minimize bias. Furthermore, future research is recommended to explore other factors that may influence OSCE assessments to improve the validity and reliability of the evaluation.

Abstract

Minat terhadap tindakan kosmetik telah mengalami peningkatan dalam beberapa tahun terakhir. Namun tindakan kosmetik dapat menimbulkan rasa nyeri yang berlebih. Telah tersedia berbagai metode untuk mengurangi rasa nyeri, salah satunya adalah pemberian stimulus vibrasi. Tujuan dari literature review ini adalah untuk mengetahui efek dan signifikansi anestesi vibratorik dalam mengurangi rasa nyeri pada tindakan kosmetik. Metode yang digunakan adalah review terhadap tujuh artikel dari jurnal internasional yang diterbitkan pada tahun 2014-2024. Artikel didapatkan dari mesin pencarian PubMed dengan kata kunci ‘vibratory anesthesia’, ‘cosmetic procedure’, dan ‘vibration anesthesia device’. Dari studi pustaka didapatkan hasil bahwa anestesi vibratorik aman dan efektif dalam mengurangi nyeri pada berbagai tindakan kosmetik seperti dermal filler di wajah, injeksi steroid interlesional pada keloid, staple removal, dan suture removal. Stimulasi vibrasi dapat diberikan secara tunggal maupun bersamaan dengan metode analgesik lain, baik dalam waktu bersamaan maupun beberapa saat sebelum dilakukan tindakan. Stimulai vibrasi diberikan dengan alat Vibration Anesthesia Device (VAD).

Abstract

The global prevalence of postpartum depression is increasing annually. This condition has been associated with polymorphisms in the catechol-O-methyltransferase (COMT) enzyme, which plays a critical role in the degradation of catecholamines. Therefore, a literature review was conducted to examine the influence of COMT enzyme variants on postpartum depression, with a focus on specific time periods and genetic variants implicated in the disorder. The review methodology involved a systematic search of articles on the PubMed database using the keywords "catechol-O-methyltransferase," "postpartum depression," and "role of COMT in postpartum depression." Inclusion criteria required full-text availability in either Indonesian or English. Four relevant studies were identified that investigated the relationship between COMT enzyme variants and postpartum depression. Evidence suggests that the COMT Val158Met polymorphism may affect the risk of postpartum depression in the context of elevated estrogen levels, as high estrogen concentrations are known to downregulate COMT expression. Additionally, certain single nucleotide polymorphisms (SNPs), including rs2020917 (TT genotype), rs737865 (GG genotype), and the COMT rs4680 Met variant allele, have been shown to modulate the influence of COMT on postpartum depression independently of estrogen levels, possibly through interactions with other enzymatic pathways.

Abstract

Preeklamsia merupakan kondisi terjadinya disfungsi plasenta karena adanya inflamasi yang bersifat sistemik, yang dapat ditandai secara umum dengan adanya hipertensi dan gangguan organ. Homosistein merupakan salah satu faktor yang berperan dalam terjadinya disfungsi endotel. Pada beberapa penelitian yang ada, dipercaya asam folat dapat menurunkan kejadian preeklamsia pada ibu hamil. Studi pustaka ini dibuat untuk membahas efek asam folat terhadap pencegahan terhadap kejadian preeklamsia. Penelitian ini dijalankan dengan metode literature review, artikel didapatkan melalui PubMed dengan kata kunci preeklamsia, dan asam folat. Ditemukan terdapat 6 penelitian yang membahas mengenai suplementasi asam folat terhadap pencegahan terhadap preeklamsia. Studi pustaka ini menyimpulkan bahwa pemberian asam folat dosis tinggi 4 hingga 5 mg memilki efek terhadap penurunan insidensi preeklamsia, tetapi juga terdapat penelitian yang menyatakan tidak terdapat pengaruh pemberian asam folat terhadap pencegahan preeklamsia. Waktu pemberian asam folat yang paling efektif pada usaha pencegahan preeklamsia masih perlu diteliti lebih lanjut.

Abstract

Background: The government implements a high-quality assurance system in the health sector to strengthen the national education system. One of the steps is the Indonesian Medical Doctor National Competency Examination (known as UKMPPD in Indonesia), which follows regulations on higher education and health workers. The entrance exam for medical students at the Faculty of Medicine, Maranatha Christian University, consists of a biology test and an academic potential test.Aims: The purpose of this study was to determine the relationship between the biology entrance exam, academic potential test, and grade point average with the final score of UKMPPD CBT at the Faculty of Medicine, Maranatha Christian University. Methods: The samples collected consisted of students from the Faculty of Medicine at Maranatha Christian University, from the 2010 to 2016 classes, who took the UKMPPD from 2016 to 2022. Data were analyzed using the Spearman test and examined for both bivariate and multivariate relationships.Results: This study showed that the biology entrance examination, medical undergraduate GPA, and medical professional GPA influenced the UKMPPD CBT final score of Maranatha Christian University Faculty of Medicine students. Nevertheless, there is no relationship between students’ TPA score and UKMPPD CBT.Conclusion: Medical undergraduate GPA had the strongest relationship with CBT scores, compared to medical professional GPA. This study also highlighted the importance of non-cognitive aspects of input selection tests besides the improvement of the quality of the clinical education evaluation process.

Abstract

Background Protein is essential for skin integrity, while exercise modulates cellular and molecular processes that regulate autophagy, mitochondrial biogenesis, and collagen synthesis. This study evaluates the combined effects of dietary protein restriction and varying exercise intensities on epidermal thickness, autophagy markers (LC3, p62), mitochondrial biogenesis (PGC1α), and collagen synthesis (Col1a1, Col3a1) in male Wistar rats.    Methods An experimental laboratory study was conducted involving 32 male Wistar rats. They were randomized into 8 groups: control, low protein, low intensity, group IV, moderate intensity, moderate intensity low protein, high intensity, and high intensity low protein. Low protein diet (5%) compared to the control (24%) was given for 60 days, while the exercise regimen was eight weeks of treadmill exercise (low/10 m per minute, moderate/20 m per minute, and high intensity/30 m per minute) for 30 minutes a day, five days a week. Epidermal thickness, LC3, p62, PGC1α, Col1a1, and Col3a1 mRNA expression were measured at the end of the study. One-way ANOVA/Kruskal-Wallis followed by Tukey HSD/Mann–Whitney test was used to analyze the data.   Results A low-protein diet significantly reduced epidermal thickness (p=0.048), increased LC3 (p=0.043) and p62 (p=0.011), indicating impaired skin regeneration and autophagy. However, exercise, especially at high intensities with low protein, mitigated this effect by increasing both LC3 (p=0.011) and PGC1α (p=0.011).   Conclusions These findings highlight the compensatory role of exercise in maintaining tissue integrity under protein-deprived conditions in male rats. High-intensity exercise potentially enhances autophagy and mitochondrial biogenesis.
