Abstract

Introduction: Obesity is one of global epidemic health problems. Obesity has been suggested to be associated with a state of chronic low-grade inflammation, which is characterized by abnormal cytokine production, increased acutephase reactants and activation of inflammatory signaling pathways. NLR (neutrofil to lymphocyte ratio) a simple and reliable indicator of inflammation, have been reported to be associatied with metabolyc syndrom, insulin resitence, and obeity in more study. Ferritin is a marker of inflammation contribute in obesity, and have been reported that ferritin is a marker of inflammation rather than iron status in overweight and obese people. Relationship between anemia and obesity may be due to fat a ccumulation and chronic inflammation in adipose tissue can be decreased iron absorbtion.Methods: Cross-sectional study of 50 obesity subject, based on Riskesdas criteria, on Mei – September 2018. Marker of inflammation is ferritin serum was measured by ELISA (enzyme linked immunosorbent assay), and NLR and haemoglobin was perfomed from haematology analyzer. Spearman test was perfomed for analyzing data with p <0,05 was significantResults: No correlation between NLR and haemoglobin, r= 0,067 and p= 0,642, and significant strong positive correlation between ferritin and haemoglobin, r= 0,630 and p < 0,000.Conclusion: There was no correlation between NLR and haemoglobin, and there was significant strong positive correlation between ferritin and haemoglobin in obesity. Future research need to observe with evaluate hepcidin, other iron status parameter and marker of inflammation.

Abstract

Background: Obesity is excessive and abnormal fat accumulation thus harmful to health. Increase in body mass index (BMI) in obesity, also accompanied by increase hormon which synthesized and released by adipose tissue informs the brain for controlling hunger and food intake that is Leptin. Leptin plays a role in chronic inflammatory process of obesity. Leptin receptor was detected in human platelets and high concentration of leptin reported to promote platelet aggregation. The platelet parameters examination is a cheap, easy, and routinly done in daily basis, which consist of platelet counts, mean platelet volume (MPV), platelet distribution width (PDW), platelet large cell ratio (PLCR), plateletcrit (PCT), which indicates platelet proliferation. The aim of research was to look the relationship of leptin with thrombocyte parameter in obesity people.Methods: Cross sectional study of 45 obese adults, based on RISKESDAS criteria  with  BMI>27, on Mei-September 2018 was done. Venous blood collected in EDTA and plain tube. The EDTA sample was used for complete hematological examination to obtain platelet parameters by Hematology analayzer with impendace method. Serum from plain tube used for leptin examination was measured by ELISA. Data were analyzed by Pearson and Spearman correlation test. P-value < 0,05 was consideres as significant.Results: There is no correlation between leptin and platelet count (r=0,051, p=0,740), MPV (r=0,021, p=0,891), PDW (r=-0,093, p=0,541),  P-LCR (r=0,082,  p=0,593), and PCT  (r=0,079, p=0,605). Conclusion: There is no correlation between leptin and platelet parameters in obesity. Further studies need to be done with adding another characteristic such as inflammation markers, metabolic syndrome parameters and platelet activation

Abstract

Background: Sepsis is a condition in systemic infection associated with organdysfunction. Interleukin-17 is a pro-inflammatory cytokine produced by Th-17 cells. C-reactive protein and neutrophil-lymphocyte ratio (NLR) have been widely used asmarkers of inflammation. The relationship between IL-17 as a proinflammatorycytokine with CRP and NLR has not been reported.Objective: This study aims to prove the correlation between IL-17 with CRP and NLRin septic patients.Methods: Analytical observational study with a cross-sectional approach was carriedout on 40 septic patients in July–December 2020. IL-17 levels were obtained by theELISA method while CRP levels were obtained using the immunoturbidimetric method.NLR was the result of dividing the absolute number of neutrophils and lymphocytesfrom the automatic hematology analyzer. Correlation between variables was performedusing Spearman correlation test.Results: The median levels of IL-17, CPR, and NLR were 363.55 (11.4-1695.80)pg/mL, 13.25 (0.43-53.87) mg/L, and 12.00 (2.26–48.5), respectively. The Spearmancorrelation test between IL-17 levels and CRP obtained p = 0.019, r = 0.37, and NLR p= 0.425, r = 0.13.Conclusion: There is a weak positive correlation between IL-17 levels and CRP inseptic patients. There is no correlation between IL-17 levels and NLR in septic patients.

Abstract

Background: Color changes in nanohibrid composite resins can occur due to 2 factors, intrinsic and extrinsic factors. Beverage can be the one of the extrinsic factor such as orange juice and fermented milk. The pH content of this both of drink will affect the change in color of the composite resin. Low pH conditions will cause the release of H+ which has an impact on the degradation and breaking of polymer chain bonds. Aim: To determine the effect of orange juice and fermented milk on the color change of nanohibrid composite resin. Methods: An experimental study with the post test control group design and a total sample of 6 samples. Soaking for 24 hours at 37ºC in artificial saliva, orange juice, and fermented milk. After 24 hours the color changes were measured using a colorimeter. Statistical test using Independent T-test.Results : Significant differences in color changes in nanohibrid composite resins between the two sample groups (p = 0.00). Independent T-test showed the results of a significant difference between 2 groups.Conclusion: Orange juice affect the change in color of nanohibrid composite resin. Abstract

INTRODUCTION:  Platelet and lymphocyte are significant in acute myocardial infarction (AMI) pathogenesis. Platelet-lymphocyte ratio (PLR) is a systemic biomarker which indicates thrombosis and inflammation. The diagnosis of AMI can be made by detecting elevated blood troponin I levels. The correlation between PLR and troponin I levels in ST-segment elevation myocardial infarction (STEMI) patients has not been widely known. AIM                : The objective of this study is to determine the correlation between PLR and troponin I levels in STEMI patients. METHODS    : This study was a cross-sectional study which data were obtained from the medical records of patients with STEMI admitted to Dr. Kariadi General Hospital Semarang from 2019 to 2020. Serum Troponin I levels were measured with Enzyme-Linked Fluorescent Assay (ELFA) method in less than 24 hours from symptoms onset. Complete blood counts were performed immediately on hospital admission. Correlations between variables were obtained using Spearman's test. RESULTS      : A total of 28 STEMI patients were included, 82.1% were male, mean age was 55.81 ± 2.13 years old. Mean symptoms onset was 7.52 ± 0.81 hours prior to admission. Mean PLR and troponin I levels were 233.32 ± 25.54 and 14.44 ± 3.16 μg/l, respectively. A moderately significant correlation was detected between troponin I levels and PLR in STEMI patients (r=0.333; p=0.042). Symptoms onset in STEMI patients had significant correlation to troponin I levels (r=0.596; p=0.001). CONCLUSION : A moderate positive correlation between PLR and troponin I levels is observed within 24 hours from symptoms onset in STEMI patients. Abstract

Introduction: Bacterial infection induces inflammation in human body. This process produceshumoral and cellular immune responses. Cinnamomum burmaniigrows very vast in Indonesia and contains cinnamaldehyde known to have an anti-inflammatory effect.Objective: To prove the effect of C. burmanii bark extract on CRP level, leukocyte count and differential blood count.Methods: Aposttest-only controlled group design with 25 Wistar Rats divided into 5 groups was employed. The CN-G group was giventhe standard feed, the CP-G group was given the standard feed and levamisole 2.5 mg/KgBW, while the CBE-100, CBE-200, and CBE-400 groups were respectively given the standard feed and cinnamon bark extract 100 mg/kgBW, 200 mg/KgBW and 400 mg/KgBW. The treatmentswereconducted for 7 consecutive days.On day 8, all rats were injected with the suspense of S. aureus intraperitoneally. The blood wasthen drawn on day 9, followed with CRP level measurement using the ELISA method. The total leukocyte count and differential blood count weremanually measured.Results: There is no significant difference in the value of CRP level (One Way ANOVA; p = 0.749) with the total counts of leukocytes(p=0.685), monocytes (p=0.769), and eosinophil(p=0.123) between groups. The neutrophils and lymphocytes of CBE-100 group aresignificantly differentfrom the other groups.Conclusion: C. burmanii extract has a potential benefit as immunomodulator.

Abstract

Background: Associated with sepsis is the illness known as acute kidney injury (AKI). AKI may be mediated by uric acid, and blood creatinine levels can be utilized to diagnose the condition to measure kidney function. Sambiloto (Andrographis paniculata) is a traditional medicine that has flavonoid compounds that can reduce creatinine levels and Xanthine Oxidase Inhibitors which can reduce uric acid levels.Objective: Septic model rats generated by lipopolysaccharide were used to test the effects of sambiloto (Andrographis paniculata) leaf extract on serum creatinine and uric acid levels (LPS).Methods: This study was experimental employing 25 rats split into 5 groups as the post-test alone control group: healthy control with standard feed (HC), negative control with LPS injection(NC), Treatment (T)1 (A.Paniculata 200 mg/kgBW+LPS), T2(A.paniculata 400 mg/kgBW+LPS), and T3(A.paniculata 500mg/kgBW+ LPS). A.paniculata leaf extract was given via oral gavage on day 8-21. Intraperitoneal injection of LPS 5 mg/kgBW was given on day 22. On the 25th day, the blood serum was analyzed for creatinine levels using Jaffe method and uric acid was analyzed using the enzymatic photometric method. One-way analysis of variance (Kruskal-Wallis) and the Kruskal-Wallis test were used to evaluate the data.Results: The mean creatinine levels of HC,NC,T1,T2,T3 were 0.7±0.01;3.5±0.04;2.9±0.03;1.9±0.05; 1.3 ±0.04 mg/dl respectively. The mean uric acid levels of HC.NC.T1.T2.T3 were 1.7±0.05;8.2±0.11; 4.5±0.03;4.0±0.12;3.0±0.19 mg/dl respectively. There were significant differences (p<0.05) in creatinine levels in groups T2(p=0.031) and T3(p=0.001) to NC group and serum uric acid levels in groups T1(p<0.001), T2(p<0.001), and T3(p<0.001) to NC group which creatinine and uric acid levels were lower than NC group.Conclusion: Andrographis paniculata leaf extract has renoprotective effect against AKI in LPS-induced septic ratsKeywords: Andrographis paniculata, creatinine, uric acid, lipopolysaccharide, AKI, sepsis

Abstract

Coronavirus 2019 (COVID-19) is an infectious disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Inflammation occurs when the body is infected with the virus. Platelets play a role in immune response and immunomodulation by activating P-Selectin Glycoprotein (PSGL) to the site of inflammation. Lymphocytes play a role through CD4 T-cells, B-cells producing specific viral antibodies, and CD8 cytotoxic T-cells by directly killing the virus in infected cells. This study aimed to prove the correlation between PLR and CRP as inflammation markers in COVID-19 patients. This study was a retrospective observational study with the cross-sectional approach at Dr. Kariadi Hospital, Semarang, for the period March-August 2020. Spearman test performed for analyzing data with p<0.05 was significant. Thirty-three confirmed COVID-19 patients with median value of PLR 218 (103-1609) and CRP 15.94 (1.24-200) mg/L were tested for correlation with a value of p=0.013 and r=0.427. The increase of PLR and CRP in COVID-19 patients was caused by an inflammatory process mediated by the immune response. High values in the blood were associated with disease severity and poor prognosis. There was a statistically significant moderate positive correlation between PLR and CRP in COVID-19 patients.

Abstract

Latar Belakang : Diabetes melitus merupakan kumpulan gejala gangguan metabolik dengan prevalensi penyandang yang masih terus meningkat. Faktor lingkungan, gaya hidup, maupun genetik dapat menyebabkan kerusakan sel pankreas maupun resistensi insulin yang kemudian bermanifestasi sebagai diabetes melitus. Daun wungu (Graphytophyllum pictum (L.) Griff) mengandung metabolit aktif seperti flavonoid yang merupakan antioksidan untuk mencegah destruksi dan meregenerasi sel pankreas penderita diabetes melitus. Tujuan : Membuktikan adanya perbedaan ukuran luas islet pankreas tikus Wistar diabetes melitus yang mendapatkan ekstrak daun wungu (Graphtophyllum pictum (L.) Griff) setelah perlakuan dengan kontrol. Metode : Penelitian true experimental dengan menggunakan desain Post-Test Only Control Group Design. Sampel 24 ekor tikus Wistar jantan dengan kriteria tertentu dibagi secara acak menjadi 4 kelompok. Kontrol negatif (K-1) diberikan aloksan dan kelompok perlakuan 1 (K-2), perlakuan 2 (K-3), dan perlakuan 3 (K-4) masing-masing diberikan aloksan dan ekstrak daun wungu 50 mg/kgBB, 100 mg/kgBB, dan 200 mg/kgBB. Glukosa darah diperiksa pada hari ke-4 untuk memastikan tikus sudah diabetes melitus dan dilanjutkan perlakuan selama 14 hari. Terminasi dan pengambilan organ dilakukan pada hari ke-15. Data berupa luas islet pankreas kemudian dianalisis menggunakan uji Kruskal-Wallis dan uji Mann-Whitney. Hasil: Berdasarkan uji Kruskal-Wallis didapatkan perbedaan luas islet pankreas yang bermakna (p = 0,000) antar kelompok. Pada uji Mann-Whitney didapatkan peningkatan luas islet pankreas yang signifikan (p = 0,000) pada seluruh kelompok perlakuan terhadap kontrol negatif. Rerata luas islet pankreas berbanding lurus dengan peningkatan dosis pemberian ekstrak daun wungu. Kesimpulan : Terdapat perbedaan ukuran luas islet pankreas yang signifikan pada seluruh kelompok perlakuan terhadap kontrol negatif. Kata Kunci : Daun wungu; luas islet pankreas; diabetes melitus

Abstract

Latar Belakang: Diabetes melitus (DM) merupakan penyakit kronik dengan gejala glukosa darah yang tinggi (hyperglycaemia) dimana terjadi peningkatan biomarker stress oksidatif yaitu MDA. Daun wungu (Graptophyllum pictum (L.) Griff) memiliki alkaloid dan flavonoid yang bersifat antioksidan untuk mengurangi radikal bebas.Tujuan: Mengetahui pengaruh ekstrak daun wungu (Graptophyllum pictum (L.) Griff) terhadap kadar malondialdehid darah pada tikus Wistar yang diinduksi aloksan.Metode: Desain percobaan menggunakan post-test only control group design. Penelitian ini menggunakan tikus wistar jantan sebanyak 24 ekor yang dibagi menjadi 6 tikus setiap kelompok. Kelompok kontrol negatif diberikan aloksan, kelompok perlakuan 1, kelompok perlakuan 2, dan kelompok perlakuan 3 diberikan ekstrak daun wungu dengan dosis 50, 100 dan 200 mg/kgBB peroral kemudian kadar glukosa darah diperiksa untuk memastikan bahwa tikus sudah diabetes melitus. Setelah tikus diabetes melitus, dilakukan tindakan tiap kelompok. Pada hari ke-14 tikus diambil data kadar malondialdehid darahnya. Data diuji dengan uji normalitas Saphiro-Wilk. Pada penelitian ini data terdistribusi normal (p>0.05) sehingga dilakukan uji ANOVA kemudian dilakukan uji post-hoc. Hasil: Didapatkan penurunan kadar MDA darah pada tikus yang diberikan daun wungu. Hasil signifikan didapatkan antara kelompok perlakuan 3 dengan kelompok kontrol negatif dan kelompok perlakuan lainnya. Kesimpulan: Daun wungu dengan berbagai dosis dapat menurunkan kadar malondialdehid darah pada tikus DM.
