Abstract

Background: Inappropriate use of antibiotics could increase the incidence of antibiotic resistance, hence increasing morbidity, mortality, and health cost. WHO has recommended to conduct a study of rational use of antibiotics in order to overcome this problem. Aim: To analyze the quality and quantity of antibiotic use in inpatient care facility at Diponegoro National Hospital (RSND). Methods: Quality and quantity assessment conducted by the use of DDD/100 patient–days and the Gyssens criteria. The study was conducted retrospectively at 86 patients’ medical records that were obtained by consecutive sampling method. Results: There were 141 antibiotics prescriptions from 86 medical records. Azithromycin, levofloxacin, and ceftriaxone were the most commonly used and had 48,12; 44,01; and 21,13 DDD/100 patient–days. By Gyssens crieteria, 35,4% of antibiotic usage was appropriate, 51,1% of antibiotic usage was inappropriate, and 13,5% no indication of the use of antibiotic. Based on the type of therapy, ADE 89,4% and ADET 10,6%. Conclusion: In terms of quantity, the most commonly used antibiotic was azithromycin. In terms of quality, 35,4% antibiotic were used appropriately according to Gyssens criteria. Keywords: Antibiotic usage; internal medicine; DDD/100 patient–days; Gyssens criteria

Abstract

Background Osteoporosis is a complication that often occurs in thalassemia major patients. This complication is still not widely known. This community service program aims to increase the knowledge of families of major thalassemia patients in Rembang Regency and Grobogan Regency regarding the complications of osteoporosis in thalassemia patients. Methods Education about osteoporosis, risk of osteoporosis in thalassemia patients, and prevention of osteoporosis. Extension method with presentation and discussion. The activity was attended by 20 thalassemia families in the Rembang Regency and 21 families of thalassemia patients in the Grobogan Regency. Before and after counseling, pretest and posttest questionnaires were completed to measure the knowledge of families of thalassemia major patients regarding bone complications in thalassemia patients. Results and Discussion Nearly half (46.34%) of respondents did not know the risk of osteoporosis in thalassemia. After counseling, 82.93% of respondents knew the risk of osteoporosis in thalassemia, but there were still 17.07% of respondents who still did not know this risk. Conclusion There is a significant increase in family knowledge of thalassemia major patients about the risk of osteoporosis in thalassemia. Further efforts to increase knowledge are needed with written information media (leaflets) given to families of thalassemia major patients

Abstract

Background: Electric cigarette which contained nicotine affected bad impact to oral cavity, since the nicotine contained on e-liquid electric cigarette could reduce function of salivary gland. This condition decreases the salivary flow rate and increase salivary viscosity. could impact to less activity of self-cleansing and antimicrobial of saliva. Objectives: To prove the difference of salivary viscosity and salivary flow rate on nicotine electric smokers and non-smokers. Methods: This research was an analytic descriptive research which exerted cross sectional research design. As many as 60 people were selected to be the research subjects who were then divided into 30 electric smokers and 30 non-smokers. The saliva on each subject was collected in a beaker cup stimulated for 8 minutes, and continued to the viscosity measurement to be valued both visually and subjectively. The data was analyzed through Mann-Whitney test to identify the difference between viscosity and salivary flow rate on nicotine electric smokers and non-smokers. Results: The result of Mann-Whitney test showed a significant difference between salivary flow rate (p<0,05) and salivary viscosity of nicotine electric smokers and non-smokers (p<0,05). Conclusion: This research indicated the difference of viscosity and salivary flow rate between electric smokers and non-smokers. The electric smokers have thicker viscosity and lower flow rate compared to the non-smokers.

Abstract

BACKGROUND: Hyperglycemia causes dyslipidemia in type 2 diabetes mellitus (T2DM). Metformin monotherapy is known to be less effective at improving glycemic status, insulin function, and lipid profiles. Lycopene is a potential antioxidant and has been shown to be hypoglycemic and hypocholesterolemic. However, the effects lycopene and metformin combination are still up for debate. This study was conducted to determine the potential of lycopene in enhancing the ability of metformin to improve glycemic status, insulin resistance, beta cell capacity, and lipid profile of T2DM rats.METHODS: Thirty male Wistar rats were randomly divided into six groups: control (N), T2DM-untreated (D), T2DM + metformin (DM), T2DM + metformin + 10 mg/kgBW lycopene (DMLy-10), T2DM + metformin + 20 mg/kgBW lycopene (DMLy-20), and T2DM + metformin + 40 mg/kgBW lycopene (DMLy-40). The treatment was administered once daily through oral route and lasted for 28 days, before blood samples were collected. Fasting blood glucose (FBG) was assessed by oxidase-peroxidase method, fasting serum insulin and HbA1c were measured using enzyme-linked immunosorbent assay (ELISA), while lipid profile was determined using enzymatic methods. The homeostatic model assessment for insulin resistance (Homa-IR) as well as the homeostatic model evaluation of β-cell function (Homa-B) were then calculated.RESULTS: Fasting serum insulin levels increased significantly (p<10.05) in the DMLy-20 and DMLy-40 groups, but Homa-B or high-density lipoprotein (HDL) did not significantly increase. Additionally, the FBG, HbA1c, Homa-IR, total cholesterol, triglyceride, and low-density lipoprotein levels were not significantly decreased than in the group treated with metformin alone.CONCLUSION: Lycopene can enhance the ability of metformin to improve the glycemic status, insulin resistance, beta-cell capacity, and lipid profile of T2DM rats.KEYWORDS: dyslipidemia, Homa-B, insulin resistance, lycopene, metformin, type 2 diabetes mellitus

Abstract

Background: BPaL is a new oral drug combination consisting of Bedaquiline (B), Pretomanid (Pa) and Linezolid (L) for multi-drug resistance tuberculosis. In Indonesia BPaL has been officially launched by the government as a therapeutic regimen for multidrug resistant tuberculosis patients since August 2023. One of the drugs in this regimen has the side effect of anemia. Until now, there are still no references explaining the effect of BPaL on the hematological profile.Objective: To determine the differences in the hematological profile of multidrug resistance tuberculosis patients before receiving the multi drug resistance regimen BPaL anti-tuberculosis drug regimen and while receiving the multi drug resistance regimen BPaL anti-tuberculosis drug regimen.Methods: This research was a retrospective observational analytical study with a retrospective cohort design involving patients with multidrug-resistant tuberculosis in Kariadi Hospital based on medical records. The difference test used the paired T-test and the Wilcoxon test.Results: 40 subjects were recruited, there were significant differences in hemoglobin level (p=0.003), hematocrit (p=0.002), red blood cells count (p=<0.001), white blood cells count (p=0.004), platelet count (p=<0.001) and RDW (p=<0.001) before therapy with the BPaL regimen and during therapy with the BPaL regimen.Conclusion: There are significant differences in the hematological profile of multidrug resistant patients, before treatment with the BPaL regimen and while receiving the BPaL regimen.

Abstract

Latar Belakang: Corona Virus Disease-19 (COVID-19) merupakan masalah kesehatan dunia yang sangat serius. Gejala yang berat dapat berujung kematian terutama pada individu yang memiliki komorbid seperti penyakit jantung koroner, diabetes melitus, hipertensi dan pada kondisi imunosupresi seperti pada pasien dengan keganasan. Studi terdahulu menunjukkan peningkatan risiko mortalitas pada pasien keganasan yang terinfeksi COVID-19. Penelitian ini bertujuan untuk menganalisis hubungan antara jenis kelamin, jenis keganasan, hipertensi, diabetes melitus, derajat keparahan penyakit, nilai lymphocyte-to-CRP ratio (LCR), dan kadar kreatinin serum dengan tingkat mortalitas pada pasien COVID-19 dengan keganasan non-hematologi.Metode: Penelitian ini merupakan penelitian cross-sectional retrospektif yang dilaksanakan pada bulan Mei 2023 hingga Juni 2023 dengan mengambil data pasien COVID-19 dengan keganasan non-hematologi di RSUP Dr. Kariadi Semarang pada periode bulan Maret 2020 hingga Desember 2022. Semua data diambil melalui rekam medis elektronik (RME) RSUP Dr. Kariadi Sema-rang. Nilai cut-off untuk LCR dan kadar kreatinin serum masing-masing adalah 100 dan 1,3 mg/dL. Data kategorik dianalisis menggunakan uji Chi-square untuk mendapatkan nilai p dan Prevalence Ratio (PR). Uji regresi logistik digu-nakan untuk mengetahui hubungan independen antara derajat keparahan penya-kit dan kadar kreatinin serum terhadap tingkat mortalitas.Hasil: Seratus satu pasien diikutkan dalam penelitian dengan 48 kasus mortalitas. Hubungan antara jenis kelamin, jenis keganasan, hipertensi, diabetes melitus, dan nilai LCR dengan tingkat mortalitas didapatkan masing-masing nilai p = 0,924; p = 0,847; p = 0,101; p = 0,916; p = 0,143. Hubungan antara derajat keparahan penyakit dan kadar kreatinin serum dengan tingkat mortalitas didapatkan masing-masing nilai p < 0,001 (PR = 22,148) dan p = 0,009 (PR = 3,604). Hasil analisis mutivariat menunjukkan bahwa nilai PR dari hubungan antara derajat keparahan penyakit dan kadar kreatinin serum dengan tingkat mortalitas pasien masing-masing adalah 27,547 dan 7,090.Kesimpulan: Jenis kelamin, jenis keganasan, hipertensi, diabetes melitus, dan nilai LCR tidak berhubungan dengan tingkat mortalitas. Derajat keparahan penyakit dan peningkatan kadar kreatinin serum berhubungan dengan tingkat mortalitas.
