Abstract

Tetralogy of Fallot (TOF) is the most common cyanotic heart disease with anomalies consist of: ventricular septal defect (VSD), overriding aorta, pulmonary stenosis and hypertrophic of the right ventricular. It also has various anomalies type such as TOF with absent pulmonary valve, TOF with pulmonary atresia, TOF with atrioventricular septal defect, TOF with coronary artery anomalies, etc which will be discussed in detail. Proper diagnosis is required when dealing with the TOF continue with radiological examination. Adequate history taking and correct physical examination are necessary in dealing with congenital heart defects, especially TOF case. In this overview, we also discuss about the comprehensive management, from tet spell management, palliative management of TOF that required shunt with various technique, and surgical total corrective of TOF with many approach such as transannular accompany with right ventriculotomy or transatrial with transpulmonary or beating heart surgery technique to prevent reperfusion injury complication.

Abstract

Background: Coronavirus 2019 (COVID-19) is a global pandemic with various clinical manifestations and is affected by multifactor. Epidemiological data of COVID-19 in Indonesia, especially in Surabaya have not been well established yet. Purpose: This study aims to provide the COVID-19 patients profile in Surabaya City, Indonesia. Method: The study data were retrospectively collected from electrical medical records in Primasatya Husada Citra (PHC) Hospital of Surabaya, one of the referral hospitals for COVID-19 in Surabaya. Descriptive and Spearman correlation statistics were done for data analysis. Results: Between 1 July – 31 August 2020, 456 subjects were diagnosed positive for COVID-19 by real-time polymerase chain reaction and enrolled in the study. Study subjects were dominated by the group age of 21-30 (26.75%), male (60.30%), living in East Surabaya (16.22%), and mild to moderate disease severity (47.59%) according to the patient’s clinical manifestation, respiration rate, and peripheral oxygen saturation. Subjects with symptoms were majorly having cough (37.93%), fever (29.38%), malaise (28.07%), and dyspnoea (25.00%). Positive correlation was found between disease severity and group of age (r=0.35; p=0.01), gender (r = 0.13; p=0.01), comorbidities (diabetes mellitus, hypertension, and cardiovascular disease), and X-ray findings (r=0.14; p=0.02). A negative correlation was found between disease severity and the region of domicile (r=-0.15; p=0.01). Conclusion: This epidemiological data of COVID-19 patients in Surabaya city, Indonesia, may help in diagnosing COVID-19 patients and improving health management strategies during a pandemic. Patients need to be aware of COVID-19 and understand disease transmission, especially with symptoms and risk factors.

Abstract

One of the health problems that occur in Indonesia is the increasing number of NCD (Non-Communicable Disease) such as heart attack and cardiovascular disease. There are two factors that cause cardiovascular disease, i.e. factor that can be changed and cannot be changed. This study aim to analyze the best performance of several classification algorithms such as k-nearest neighbors algorithm (k-NN), stochastic gradient descent (SGD), random forest (RF), neural network (NN) and logistic regression (LR) in classifying cardiovascular based on factors that caused those diseases. There are two aspects that need to be examined, the performance of each algorithm which is evaluated using the Confusion matrix method with the parameters of accuracy, precision, recall and AUC (Area Under the Curve). The dataset uses 425.195 samples from result data of cardiovascular disease diagnosed. The testing mode uses percentage split and cross-validation technique. The experimental results show that the performance of NN algorithms produces the best prediction accuracy compared to other algorithms, which is accuracy of 89.60%, AUC of 0.873, precision of 0.877, and recall of 0.896 using percentage split and cross-validation testing mode using Orange. For the accuracy of 89.46%, AUC of 0.865, precision of 0.875, and recall of 0.895 using cross-validation testing mode using Weka. By KNIME, the result of accuracy value is 88.55%, AUC value is 0.768, precision value is 0.854, and recall value is 0.886 using cross-validation testing mode.

Abstract

Background Acute kidney injury (AKI) is still diagnosed by measuring the estimated creatinine clearance (eCCl), despite the fact that it may not change until 50% or more of kidney function has been lost. AKI after cardiac surgery is related to prolonged intensive care, decreased quality of life, and increased long term mortality. Neutrophil gelatinase-associated lipocalin (NGAL) represents an early biomarker of AKI, which may be useful for assessing AKI in cardiac patients.Objective To determine the validity of urinary and plasma NGAL as biomarkers for AKI in children after cardiac surgery.Methods Subjects were children who underwent cardiac surgery in Dr. Soetomo Hospital, Surabaya, Indonesia from August 2013 to January 2014. Serial urine and blood samples were analyzed for NGAL before surgery, as well as at 2h, 4h, 12h, and 24h after surgery. The AKI was established based on pRIFLE criteria. Estimated creatinine clearance (eCCl) was calculated from the estimated glomerular filtration rate (eGFR), according to age by the traditional Schwartz formula. Serum creatinine was assayed by the Jaffe method before surgery, as well as at 12h, 24h, 48h, and 72h after surgery.Results Of 20 subjects, 5 developed AKI. Urinary and plasma NGAL increased markedly at 2h postoperatively, as compared to eGFR which showed a rise at 12-48 h after cardiac surgery. Analysis of 2h post-operative urinary NGAL at a cut off value of 11.270ng/mL yielded an area under the curve (AUC) of 1.00 (95%CI 2.63 to 12.13), with sensitivity and specificity of 100% each for AKI. In addition, 2h post-operative plasma NGAL at a cut off value of 8.385 ng/mL yielded an AUC of 1.00 (95%CI 3.71 to 12.15) with sensitivity and specificity of 100% each for AKI.Conclusion Urinary and plasma NGAL are valid as early biomarkers for AKI in children after cardiac surgery.

Abstract

Introduction: One of the abnormalities in the prostate gland is prostate adenocarcinoma. Prostate adenocarcinoma expresses tumor antigen in the form of Prostate Specific Antigen (PSA) which will then be recognized by the immune system. The immune system then responds by infiltrating the tumor tissue. Tumor Infiltrating Lymphocytes (TILs) are defined as the infiltration of lymphocytes into the tumor microenvironment. T lymphocytes have a role in recognizing and killing tumor cells. The Gleason grading system together with serum PSA assessment and clinical staging are key in determining appropriate therapy and prognosis in prostate adenocarcinoma. The Gleason grading system is used to assess the degrees of glandular differentiation in prostate adenocarcinoma. An increase in the gleason score indicates poor gland differentiation.Purpose: To assess the correlation between the degrees of Tumor Infiltrating Lymphocytes (TILs) with gleason score in prostate adenocarcinoma.Method: This retrospective study used a correlation analytic method with a cross sectional study design that measured the degrees of Tumor Infiltrating Lymphocytes (TILs) in each gleason score of prostateadenocarcinoma that was observed once at a time. The research samples obtained from the patient's medical records included the gleason score and histopathological slides of the Transurethral Resection of Prostate (TURP) with hematoxylin eosin (HE) staining which were diagnosed as prostate adenocarcinoma according to the inclusion criteria using the non-probability total sampling method at Dr.RamelanNavy’s Central Hospital and analyzed using SPSS version 25 application.Result: Spearman correlation test to assess the correlation between the degree of stromal TILs with gleason score obtained p-value = 0.345 (p >0.05) which indicates there is nocorrelation.Conclusion: There is nocorrelation between high grade stromal TILs and high Gleason Score in prostate adenocarcinoma

Abstract

More than 85% of congenital heart disease in children survive to adulthood according to recent advances in pediatric cardiology and heart surgery practice. Fifty percent of  this population is made up of women, most of whom have reached child bearing age. In this group of patients, pregnancy represents a new challenge, whose natural history has been modified by surgery. In Fontan circulation,  the ventricle unable to compensate for the major restriction in the bottleneck and non physiological suction have to pull blood through the lungs, although the ventricles provide the driving force for the circuit.  Women with an adequate Fontan circulation can become pregnant without clinically significant long term sequele.  Pre-marital and pre-pregnancy consultation is important. A close collaboration between cardiologist, obstetrician and anaesthesiologist is mandatory in order to get the best outcome of pregnancy.

Abstract

Introduction: Necrotizing Fasciitis (NF) is bacterial infection spreading rapidly affecting soft tissue and muscular fascia. It's life threatening and fatal, leads to tissue necrosis, sepsis, and death. It has high mortality number especially in elderly.Purpose: To report a rare case of odontogenic NF in elderly patient extending to the neck, left upper limb and anterior chest wall, diagnosed and treated in district hospital with limited facilities.Case Report: A 65 years old male with painful left neck swelling and fluctuating gangrenous appearance start form the mastoid bone to infraclavicular area crossing to the right close with sternal area. Swelling and fluctuating area also found in left axillar and left arm. Thirteen hours after hospital admission, the patient was sent to operating room to do the wound debridement. Wound care was carried out daily in intensive care unit and inpatient ward. 14 days postoperatively patient was send out to continue the wound care in outpatient department. 28 days postoperatively, skin grafting was done.Conclusion: Fourteen days postoperatively patient was sent to outpatient department to continue his wound care. After wound care in order to wound prepare for skin grafting. 28 days postoperatively, 14 days after patient discharge from hospital, skin grafting was done to close the wound defect.

Abstract

Introduction: Since Brescia and Cimino(1966) posted AV Fistula technique, there has been increase of long term survival of patients with End Stage Renal Disease (ESRD). AV fistula has its own set of indications, benefits, and complications. Purpose: To show AV Fistula creation outcomes, complications and its related comorbidities in Ben Mboi District Hospital, Ruteng Flores IndonesiaMethod: An analytical study design using a cross sectional approach was employed in this study. Using medical record data from 2019 to 2022, all men and women who received hemodialysis through AV Fistula were included. Bivariate analysis was carried out from baseline data such as age, sex, comorbidities, location of an av fistula (AVF), previous dialysis insertion with complications that arise using Chi-square. Results: Age, sex, hypertension and AV Fistula creation location has no significant correlation with complications such as failed to mature, thrombosis, infection, venous hypertension. Diabetes mellitus has a significant correlation with occurrence of failed to mature (p=0.012), thrombosis (p=0.014), venous hypertension (p=0.08). Obesity has a significant correlation with occurrence of failed to mature (p=0.000), thrombosis (p=0.000). Previous insertion of CDL in right subclavian vein has a significant correlation with occurrence of venous hypertension (p=0.000).Conclusion: Diabetes mellitus has a significant correlation with occurrence of complications such as failed to mature, thrombosis and venous hypertension. Obesity has a significant correlation with occurrence of failed to mature and thrombosis. History of CDL insertion in right subclavian vein has a significant correlation with venous hypertension.

Abstract

Introduction: Acne vulgaris (AV) is a chronic inflammation of the pilosebaceous follicles. The four main pathologies underlying the occurrence of acne vulgaris are follicular epidermal hyperproliferation, increased sebum production, colonization of Propionibacterium acnes and inflammatory processes. Triglycerides are a component of sebum which plays an important role in the inflammatory process.Objective: to determine the relationship between triglyceride levels in the blood and acne vulgaris in young adults.Method : this research is analytic. The research design used was cross sectional. The population of this study were all patients who came to the clinic. The sample in this study used an accidental sampling technique.Results: The study showed that the characteristics of the respondents based on the Chi-Square test for the relationship between triglyceride levels and the incidence of Acne vulgaris showed that there was a statistically significant relationship between triglyceride levels and the incidence of Acne vulgaris.Conclusion: There is a relationship between triglyceride levels in the blood and the incidence of acne vulgaris in young adults.

Abstract

Background: Tuberculosis (TB) is ranked as the second deadliest infectious disease in the world. There is a drastic increase in TB cases by 2023 with 92% of these cases being pulmonary TB infections. Thoracic photography is a standard examination performed to determine the development of tuberculosis disease through reviewing populations with TB risk factors, one of which is patients with MDR-TB (Multi-Drug Resistance) can result in increased opacity in the lower lung lobes with pulmonary, cavity, and parenchymal lesions. Thoracic photo scoring was performed to determine the severity of lesions in the thoracic photo images of TB patients with MDR (Multi-Drug Resistance) and DS (Drug-Sensitive).Objective: to compare the scoring of thoracic photographs between of TB patients with MDR-TB and DS-TB.Method: using observational analytic method with cross-sectional research design. The sampling technique used purposive sampling method through medical records with analysis using two tests, namely Wilcoxon and Mann-Whitney comparative test. Radiologic findings that we analyzed were infiltrate, consolidation, cavity, ground glass opacity, fibrosis, bronchiectasis, calcification, node, atelectasis, bullae, emphysema, and other nonlung parenchymal findings.Results: The samples to be analyzed were 55 samples had completed anti-tuberculosis therapy in the 6th month and had thoracic photographs. The Wilcoxon Comparative Test on two paired groups showed a significant difference between the TB group with MDR (p=0.008) and DS (p=0.000). In the Mann-Whitney comparative test on two unpaired groups, there were no significant results either TB groups with MDR and DS (p=0.282). Scoring results in MDR-TB patients after therapy showed mild (3.70%), moderate (44.44%), severe (51.85%) categories. In DS-TB patients, showed mild (37.03%), moderate (48.15%), severe (14.81%) categories.Conclusion: There is a significant difference between the severity of thoracic photographs in TB patients with MDR and without MDR (Drug-Sensitive).
