Abstract

The social stigma attached to TB patients hampers efforts to control pulmonary TB. Health cadres at the primary care level are vital and often limited in their capacity, especially in dealing with the social stigma experienced by TB patients. Cadres are more often provided with basic medical training in identifying new cases rather than communication skills and advocacy strategies to reduce stigma, which makes patients reluctant to report and delay treatment. The purposeof this sudy is to increase the capacity of health cadres through training and strengthening the SI Peduli TBC peer group as a strategy to reduce stigma in pulmonary TB patients in the work area of the Janti Community Health Center in Malang City. Preparations were carried out in coordination with stakeholders, identification of active cadres and TB support communities, formation of an implementation team and peer group facilitator, namely the Si Peduli TBC Community Team of 19 people. This was followed by collecting data on cases and stigma levels. The implementation of the activity included providing training materials covering an introduction to TB and Stigma, the role of cadres in reducing stigma through effective communication, and practice and action planning for the Si Peduli TBC Community Peer Group. The final stage of knowledge evaluation was through a post-test to measure the increase in cadre knowledge regarding TB and stigma, evaluation of the quality of interaction in peer groups, and joint reflection with cadres regarding the effectiveness of stigma reduction strategies. The results of the community service activity involved three sub-districts within the Janti Community Health Center's work area, namely Bandungrejosari, Sukun, and Tanjungrejo. The preparation stage with coordination took place on August 15, 2025, and August 22, 2025. The training was implemented on August 23-24, 2025, along with the socialization and formation of the Si Peduli TB Community peer group. The activity outputs were educational videos and measurements of the level of knowledge of the concept of Pulmonary TB, TB stigma, and effective communication methods, which increased by 63.2%, 68.4%, and 63.3%, respectively. It can be concluded that the Si Peduli TB Community program has been proven to increase the knowledge of cadres, volunteers, and survivors and strengthen the Si Peduli TB Community peer group as a strategy to reduce stigma in pulmonary TB patients in the Janti Community Health Center's work area, Malang City.  Keywords: Pulmonary Tuberculosis, Stigma, Health Cadres, Peer Group, Community TB Care

Abstract

Kata Kunci: Tuberkulosis Paru, Deteksi Dini, Pelatihan Si Peduli TBC Komunitas, Stigma TBC Paru, Literasi Digital     ABSTRACT Tuberculosis (TB) remains a major public health problem in Indonesia. TB control efforts remain a serious national concern through the TOSS TB (Temukan, Obati Sampai Sembuh) strategy, which emphasizes early detection, treatment adherence, and community involvement. Various obstacles are still faced, including low public health literacy, stigma against sufferers, and limited health workers in actively finding cases. Janti Community Health Center, as a first-level health facility, plays an important role in actively finding TB cases. However, the achievement of early detection is still hampered by fear, stigma, and a lack of public understanding regarding TB symptoms. The purpose of this community service is to analyze the effect of providing "Si Peduli TB Komunitas" training on the ability to detect pulmonary TB early. The implementation method uses a Peer Education Approach, Blended Education and Cross-Sector Collaboration, tracing assistance and creating educational content & screening campaigns through social media. This activity was carried out on August 15-31, 2025, involving community leaders, families, and local organizations. The number of participants was 19 people from 3 sub-districts in the Janti Health Center working area, namely Bandungrejosari, Sukun, and Tanjungrejo. Through the Wilcoxon test, it was found that the results of the activity were 19 people who experienced an increase in the ability to detect TB early after being given the Community TB Care Training intervention. The asymp.sig. (2-tailed) value is 0.000, where this value is smaller than the alpha value of 5% (0.05), so it can be interpreted that there is a difference between before and after the provision of the Community TB Care Training intervention on Early Detection Ability. Increasing the ability of participants in creating and disseminating educational media through various digital platforms strengthens the role of the community in reducing stigma while encouraging early detection of pulmonary TB in the working area of the Janti Health Center in Malang City. Keywords: Pulmonary Tuberculosis, Early Detection, Community TB Care Training, Pulmonary TB Stigma, Digital Literacy

Abstract

ntroduction: Tuberculosis (TB) remains a major global public health problem in Indonesia, where household contact serves as the primary transmission pathway. Families, as the closest social units, play a crucial role in preventing TB spread; however, preventive behaviors often remain suboptimal due to fragmented and unsustained community interventions. Objective: This study aimed to develop and evaluate an integrative, family-based prevention model by positioning personal counseling as a mediator linking threat perception, coping appraisal, and family support with preventive behaviors. Methods: A two-phase quantitative study was conducted in Madiun City. Phase I involved 387 families of TB patients to analyze the relationships among variables using structural equation modeling–partial least squares (SEM-PLS). Phase II adopted a quasi-experimental design with intervention and control groups (n = 30 each) to assess the effectiveness of the integrative module. Statistical analyses included 95% confidence intervals (CIs) for all effect estimates. Ethical approval was granted by the Health Research Ethics Committee of Universitas Strada Indonesia (No. 2781/KEPK/IX/2023). Results: SEM-PLS analysis revealed that threat perception, coping appraisal, and family support significantly influenced TB preventive behaviors directly and indirectly through personal counseling (? = 0.01–0.35; 95% CI = 0.02–0.40; p < 0.05). The intervention significantly improved preventive behavior in the intervention group compared with the control group (mean increase = +7.5; 95% CI = +5.9–+9.1; p < 0.001). Conclusion: The integrative family-based model incorporating personal counseling effectively strengthened TB preventive behaviors among families. Theoretically, this study contributes to the integration of Social Cognitive Theory and Protection Motivation Theory in TB prevention, highlighting personal counseling as a behavioral mediator. Practically, this model can be adopted by primary healthcare providers and community health cadres to enhance family engagement and support Indonesia’s TB elimination targets by 2030. Study limitations include reliance on self-reported measures and a localized setting, which may constrain generalizability.

Abstract

Introduction: Community health cadres serve as vital front-line actors in improving maternal health outcomes, particularly in the early identification of high-risk pregnancies. Operating as motivators, facilitators, and educators, their performance is influenced by internal attributes such as knowledge, skills, competence, and motivation, alongside external elements including social support. However, limited evidence exists on how these factors interact to influence their effectiveness. This study aimed to develop a structural model to analyze the relationships between these internal and external factors and their impact on cadre performance in early risk detection. Methods: A cross-sectional study with a correlational approach was conducted from January to May 2025. A total of 330 active health cadres from 37 community health centers in Kediri Regency, Indonesia, were selected using proportional random sampling. Data were collected through a structured questionnaire and analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) to explore both direct and mediated effects among variables. Results: Knowledge (? = 0.044, p = 0.004), skills (? = 0.329, p < 0.001), and social support (? = 0.639, p < 0.001) significantly enhanced competence. Competence, in turn, strengthened motivation (? = 0.546, p < 0.001), which had the strongest direct effect on performance (? = 0.319, p = 0.043). Social support and skills indirectly improved performance through competence and motivation. The final model demonstrated good fit (SRMR = 0.046; NFI = 0.91; RMS_theta = 0.08) and explained 68% of the variance in cadre performance (R² = 0.68). Conclusion: Competence and motivation act as key mediators linking individual capacities and social support to performance. Strengthening these aspects through structured training and continuous social reinforcement is essential to enhance early detection of high-risk pregnancies and improve maternal health outcomes.

Abstract

Digital transformation in the health sector requires improved data utilization, particularly at the level of primary care services. Community health centers, as the frontline of health service delivery, face challenges such as uneven digital literacy among health workers, inadequate infrastructure, and a weak culture of data-driven decision-making. A training program on the use of e-Puskesmas data was conducted at Puskesmas Kendalkerep to strengthen the capacity of health workers in integrating data into service planning processes. This study employed a quasi-experimental one-group pretest–posttest design involving 52 participants selected through total sampling. Data were collected through pretest–posttest assessments, observations, and a questionnaire based on six dimensions of planning optimization: data utilization, program effectiveness, decision-making quality, coordination and collaboration, monitoring and evaluation, and human resource and infrastructure capacity. The analysis revealed significant improvements across all dimensions after the training. The average pretest scores, which ranged from 45.50 to 57.50, increased to 82.50–92.50 in the posttest. The paired sample t-test produced t-values of 6.04–9.44, exceeding the critical value of 2.01 (p < 0.05), indicating a statistically significant difference before and after the training. Cohen’s d values ranged from 0.81 to 1.58, indicating large effect sizes, with the strongest impact observed in the dimension of coordination and collaboration. Observational findings also showed enhanced technical data-entry skills, greater participation in discussions, and increased awareness of the importance of data-driven planning.

Abstract

Background: Diabetes mellitus (DM) is a chronic metabolic disease frequently accompanied by complications such as peripheral circulatory disorders and neuropathy, which contribute to decreased lower-extremity function. This condition is still often found in DM patients at Toto Kabila Regional Hospital, highlighting the need for effective non-pharmacological interventions. Purpose: To analyze the effectiveness of buerger allen exercise and diabetic foot exercise in improving peripheral circulation and neuropathic symptoms among patients with diabetes mellitus. Method: A quasi-experimental two-group pretest–posttest design involving 60 patients who were randomly assigned into two intervention groups. Both exercises were administered for four weeks. Data were collected using standardized operational procedures for each exercise along with instruments assessing peripheral circulation and neuropathic symptoms. The Wilcoxon test was used to analyze within-group effects, while the Mann–Whitney test was applied to compare differences between groups. Results: Both interventions significantly improved peripheral circulation and reduced neuropathic symptoms (p = 0.000). Between-group analysis showed that the Buerger Allen Exercise produced greater improvement in peripheral circulation, particularly in the left extremity (p = 0.019), and demonstrated a more substantial reduction in neuropathic symptoms compared to diabetic foot exercise (p = 0.000). Conclusion: Both Buerger Allen Exercise and diabetic foot exercise are effective in enhancing peripheral circulation and reducing neuropathic symptoms in patients with diabetes mellitus. However, Buerger Allen Exercise shows superior effectiveness and may be recommended as a primary non-pharmacological intervention to improve vascular and sensory function in diabetic patients.   Keywords: Buerger Allen Exercise; Diabetes Mellitus; Diabetic Foot Exercise; Diabetic Neuropathy; Peripheral Circulation.
