Abstract

Background: Patients with chronic kidney failure report the sight of blood-filled dialysis tubes, the burden of routine therapy costs, fear of invasive measures, and the uncertainty of recovery triggers their anxiety. This anxiety also carries over into the home, causing difficulty sleeping at night and ongoing feelings of restlessness. Purpose. The Effect of the Combination of Mindfulness based on Breathing Exercise with Lavender Aromatherapy on Anxiety and Sleep Quality in Hemodialysis Patients Methods: This study employed a quasi-experimental pretest–posttest control-group design and was reported in accordance with the TREND guidelines for non-randomized intervention research. The target population comprised 90 hemodialysis patients at Bhayangkara Kediri Hospital, of whom 40 were selected using purposive sampling. The intervention consisted of a combination of mindfulness-based breathing exercises and lavender aromatherapy, with anxiety and sleep quality as outcome variables. Anxiety was measured using the Self-Rating Anxiety Scale (SAS), and sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), both of which have demonstrated acceptable validity and reliability in prior research. Data analysis included descriptive statistics and the Wilcoxon signed-rank test to determine pre-post differences between intervention and control groups, with a significance level set at p < 0.05. Result: The general characteristics of respondents were comparable between the intervention and control groups with respect to age, sex, duration of hemodialysis, and comorbid conditions. Specific baseline characteristics related to anxiety levels and sleep disturbances were also similar across groups, indicating initial equivalence. The Wilcoxon signed-rank test showed a statistically significant improvement in anxiety (p < 0.001) and sleep quality (p < 0.001) following the intervention. These findings suggest that combining mindfulness-based breathing exercises with lavender aromatherapy may reduce anxiety and enhance sleep quality among hemodialysis patients. Conclusion: The combination of mindfulness-based breathing exercises with Lavender Aromatherapy reduces Anxiety and improves Sleep Quality in hemodialysis patients. This intervention is very easy to implement, so it can be used as a nursing intervention in the hemodialysis unit to advance nursing science.

Abstract

Background: People with diabetes mellitus are increasing in number. Controlling blood sugar levels is an action that must be taken to get normal and stable blood sugar levels. Self-control takes precedence over treatment. Compliance with diet is one of the important things in controlling blood sugar levels. Raflesia Room as one of the internal medicine inpatients in dr. SoedomoTrenggalek, received inpatient care for the majority of diabetes mellitus patients. Purpose : The purpose of this study was to analyze the relationship between diet adherence and blood sugar levels in people with diabetes mellitus. Methods : The research design used descriptive analytic with cross sectional approach. The population in this study were patients with diabetes mellitus in the Raflesia room of Dr. SoedomoTrenggalek Hospital in January 2017 with 30 patients. With a simple random sampling technique so that 29 samples were obtained. Result : The results showed that almost all respondents, namely 25 (86%) respondents, had abnormal blood sugar levels. Almost all respondents, 23 (79%) did not comply with the diet. Chi square test results obtained 0.004 which means there is a relationship between dietary compliance with blood sugar levels of diabetes mellitus patients in Raflesia room Dr. Soedomo Hospital Trenggalek in 2017. Conclusion : Old age is an age that is susceptible to a decrease in body function, especially hormonal causes of decreased pancreatic function resulting in a decrease in insulin sensitivity which causes high blood sugar levels. The level of education has an important role in diet compliance where patients are required to understand 3J "Types, Amounts and Schedules" so that the lower level of education causes lower diet compliance. Disobedience to diets causes sufferers to consume irregular foods which cause a pile of glucose and blood so that hyperglycemia occurs.

Abstract

Pre-anesthetic initial assessment is a key element in anesthetic risk management, particularly in major surgical patients with high clinical complexity. Variability in the implementation of initial assessment and limited understanding among policy stakeholders may reduce the effectiveness of preventing anesthetic complications. Within the framework of regional health service governance, the Health Office plays a strategic role as a regulator in ensuring the standardized implementation of pre-anesthetic initial assessment oriented toward patient safety.This activity aimed to improve the understanding of staff of the West Nusa Tenggara Provincial Health Office regarding the concept and role of pre-anesthetic initial assessment as a foundation for strengthening anesthetic service governance at the West Nusa Tenggara Provincial General Hospital. This community service activity was conducted in the form of socialization sessions, including focused discussions and knowledge evaluation through pre-test and post-test. A total of ten staff members of the West Nusa Tenggara Provincial Health Office participated in this activity. The results showed an improvement in participants knowledge levels from poor to good knowledge following the educational intervention. This improvement reflects a more comprehensive understanding of the function of pre-anesthetic initial assessment as an instrument for preventing anesthetic complications, as well as a basis for guidance and supervision of health service quality. It can be concluded that strengthening the Health Office’s understanding of pre-anesthetic initial assessment has the potential to serve as a governance foundation for supporting the development of an initial assessment–based model to prevent anesthetic complications in major surgical patients.

Abstract

Praktik penunjang disertasi ini dilaksanakan di Dinas Kesehatan Provinsi Nusa Tenggara Barat sebagai lembaga pembina dan pengendali tata kelola sumber daya manusia RSUD Provinsi NTB. Tujuan praktik ini adalah mengembangkan model peningkatan kepuasan kerja perawat melalui penguatan komunikasi internal dan partisipasi kerja dengan motivasi kerja sebagai variabel mediasi. Metode yang digunakan meliputi pengkajian sistem pembinaan, analisis Fishbone, penentuan prioritas masalah menggunakan USG, serta analisis SWOT sebagai dasar perumusan intervensi kebijakan. Hasil pengkajian menunjukkan bahwa rendahnya kepuasan kerja perawat RSUD Provinsi NTB tidak hanya dipengaruhi oleh faktor individu, tetapi juga oleh lemahnya sistem komunikasi internal dan rendahnya tingkat partisipasi perawat dalam pengambilan keputusan. Kondisi ini berdampak pada menurunnya motivasi kerja dan keterikatan perawat terhadap organisasi. Sebagai solusi dikembangkan Provincial Internal Communication and Participation Enhancement System (PICAPES–NTB), yaitu model pembinaan komunikasi internal dan partisipasi kerja yang diarahkan untuk meningkatkan motivasi dan kepuasan kerja perawat. Model ini menempatkan Dinas Kesehatan Provinsi NTB sebagai pengendali kebijakan komunikasi dan partisipasi di lingkungan RSUD Provinsi NTB. Implementasi awal PICAPES–NTB menunjukkan peningkatan keterbukaan komunikasi, keterlibatan perawat dalam proses kerja, serta penguatan motivasi yang berdampak positif terhadap kepuasan kerja. Praktik ini menegaskan bahwa penguatan tata kelola komunikasi dan partisipasi pada level regulator merupakan kunci dalam membangun lingkungan kerja perawat yang lebih sehat dan produktif
