Abstract

D-dimer is a degradation of soluble fibrin which is the result of systematic degradation of vascular thrombus through a fibrinolytic mechanism. The purpose of this study was to look at sociodemographic factors and other diseases that affect the d-dimer value of COVID-19 patients. This study uses a retrospective cohort method with the collection of sampling is convenience sampling. There were 137 patients who met the study criteria. Age, BMI, blood pressure and respiration rate are sociodemographics that affect the D-dimer lab value. For comorbidities that affect the D-dimer lab value, there are CKD, anemia, hypertension and heart disease. A high D-dimer value also has a significant relationship with the severity of the COVID-19 infection. In addition, patients who died were found to have high D-dimer values. By this research, patients with high age, BMI, blood pressure and respiration rate can increase patient mortality and for comorbidities that have an important role in increasing mortality in Covid-19 patients are CKD, anemia, hypertension and heart disease.

Abstract

Abstract: In reducing the risk of transmission to health workers, the Government has given the 3rd Booster Covid-19 Vaccination since August 2021. This study uses the Astra Zeneca Vaccine, Moderna Vaccine and Pfizer Vaccine. The purpose of this study was to evaluate the effectiveness of the 3rd Booster vaccine and the clinical outcomes of post-vaccination health workers at West Kalimantan Hospital. Research method Observational Cross-sectional Prospective by conducting questionnaires and interviewing health workers. Samples were taken by convenience sampling and statistical analysis of chis-square. The research instrument used a questionnaire distributed in the hospital. In the study, it was found that as many as 12.60% (14 out of 118 health workers) were infected with Covid-19. The habit that influenced the effectiveness of the 3rd booster vaccination was sunbathing (p <0.05). The low severity of health workers infected with Covid-19 after the 3rd booster vaccine can be seen from the place of treatment, namely 97% independent isolation, 60% without using antivirals and 77% without using antibiotics. The type of vitamin that is most widely used is vitamin C. Hence with this booster vaccination aims to prevent and reduce the severity of patients who contract Covid-19 after vaccination.

Abstract

Vitamin D plays a crucial role in calcium and phosphorus metabolism, bone health, and immune system function. Deficiency of vitamin D in adolescents increases the risk of impaired bone growth and various chronic diseases. This community service activity aimed to enhance the understanding of 11th-grade students at SMK N 31 Jakarta regarding the importance of vitamin D in disease prevention. The method involved an interactive educational session using PowerPoint presentations, accompanied by pre-test and post-test assessments to evaluate knowledge improvement. The results showed an increase in the average post-test score to 88.07%, compared to 86.16% in the pre-test, indicating a positive improvement in students' understanding. The program effectively raised students’ awareness of the importance of vitamin D intake through sun exposure and a nutritious diet. It also demonstrated the effectiveness of interactive health education in increasing adolescents health knowledge.

Abstract

Tuberkulosis (TB) paru tetap menjadi tantangan besar dalam kesehatan masyarakat di Indonesia. Meskipun program pengobatan telah distandarisasi, variasi dalam lama pengobatan masih ditemukan, yang kemungkinan dipengaruhi oleh gejala klinis dan persepsi kesehatan pasien. Penelitian ini bertujuan untuk mengkaji hubungan antara gejala klinis, persepsi kesehatan, dan faktor perilaku dengan durasi pengobatan TB paru. Studi kuantitatif dengan desain potong lintang (cross-sectional) ini melibatkan 111 pasien TB paru yang memenuhi kriteria inklusi. Data dikumpulkan melalui kuesioner terstruktur dan rekam medis, kemudian dianalisis menggunakan uji korelasi Spearman, ANOVA, dan regresi linier. Dari hasil penelitian menunjukkan 60,4% laki-laki dan 39.6% perempuan dan adanya korelasi negatif yang signifikan antara gejala batuk (r = –0,358; p < 0,001), sesak napas (r = –0,327; p < 0,001), dan persepsi kesehatan (r = –0,212; p = 0,025) terhadap lama pengobatan. Variabel kelelahan juga menunjukkan pengaruh signifikan terhadap durasi pengobatan (B = 0,272; p = 0,039). Selain itu, semua variabel prediktor secara simultan memiliki hubungan yang signifikan terhadap lama pengobatan, ditunjukkan oleh nilai F hitung sebesar 3,679 dengan tingkat signifikansi < 0,001. Hasil ini menunjukan bahwa terdapat hubungan yang signifikan antara faktor klinis, sosiodemografi dan presepsi kesehatan terhadap lama pengobatan TB di Puskesmas Kota Ambon.

Abstract

Kurangnya pengetahuan masyarakat mengenai faktor risiko kanker serviks merupakan salah satu penyebab tingginya prevalensi penyakit ini. Penelitian ini bertujuan untuk menganalisis tingkat pengetahuan mahasiswa Universitas 17 Agustus 1945 Jakarta tentang penyakit hpv (kanker serviks) dan rekomendasi apa yang diberikan mahasiswa terhadap penyakit hpv (kanker serviks). Penelitian ini menggunakan metode kuantitatif dengan desain prospektif cross-sectional, melibatkan populasi mahasiswa aktif Universitas 17 Agustus 1945 Jakarta dengan sampel sebanyak 470 responden. Pengumpulan data dilakukan menggunakan kuesioner daring melalui Google Form, dengan kriteria inklusi mahasiswa aktif dan eksklusi mahasiswa yang tidak aktif. Analisis data melibatkan distribusi frekuensi dan pengujian hubungan antar variabel. Hasil penelitian menunjukkan bahwa responden perempuan memiliki tingkat pengetahuan yang lebih baik dibandingkan laki-laki, dan responden pada kelompok usia 18-25 tahun lebih aktif memberikan rekomendasi yang efektif dibandingkan kelompok usia lainnya. Kesimpulannya, terdapat hubungan signifikan antara tingkat pengetahuan mahasiswa terhadap pengidap HPV (kanker serviks), dan rekomendasi yang diberikan mahasiswa terbukti efektif dalam upaya pencegahan kanker serviks. Penelitian ini menegaskan pentingnya edukasi yang berkelanjutan untuk meningkatkan kesadaran dan pengetahuan mengenai pencegahan kanker serviks.

Abstract

Metformin is a first-line therapy for type 2 diabetes mellitus (Type 2 DM), but its primary excretion through the kidneys raises concerns about the risk of nephrotoxicity, particularly in geriatric patients. This study aims to assess the relationship between metformin use and renal function in geriatric patients with Type 2 DM at Prof. Dr. J. A. Latumeten Ambon Level II Hospital. This study used a retrospective and prospective observational design of 150 geriatric patients receiving metformin. Data were collected through medical records and questionnaires. Kidney function parameters are assessed through GFR values and creatinine levels. The results of the study showed that the majority of respondents were aged 66–74 years (88.7%), with a prevalence of metformin use of 100%. No significant association was found between the duration of metformin use and a drastic decline in kidney function (the average creatinine value remained < 1.3 mg/dL). However, comorbidities and age were found to be factors that affect kidney function more significantly. Metformin is relatively safe to use in geriatric patients with close monitoring of renal function. The decline in kidney function is more influenced by age factors and comorbidities than by the use of metformin itself.

Abstract

Penyakit ginjal kronik (PGK) merupakan salah satu masalah kesehatan global dengan prevalensi yang terus meningkat, terutama di Indonesia. Faktor risiko seperti hipertensi, diabetes, pola makan tidak sehat, serta kurangnya literasi kesehatan masyarakat menjadi penyebab utama tingginya kasus PGK. Kegiatan pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan masyarakat mengenai pola hidup sehat sebagai upaya pencegahan penyakit ginjal. Kegiatan dilakukan di RPTRA Kampung Indah Lestari melalui metode penyuluhan interaktif yang mencakup presentasi, diskusi, pre-test, dan post-test. Hasil evaluasi menunjukkan adanya peningkatan signifikan dalam tingkat pengetahuan peserta, dari 60% kategori “baik” sebelum kegiatan menjadi 90% setelah kegiatan. Edukasi mencakup pentingnya konsumsi air putih yang cukup, pengendalian asupan gula dan garam, serta penggunaan obat secara bijak. Peningkatan ini menunjukkan efektivitas pendekatan edukatif dalam meningkatkan kesadaran masyarakat terhadap pencegahan PGK. Diharapkan kegiatan serupa dapat dilakukan secara berkelanjutan untuk memperluas jangkauan manfaatnya.

Abstract

This study aims to determine the effectiveness of clinical pathway implementation in pediatric patients with acute diarrhea with mild-moderate dehydration on clinical outcomes, length of stay, use of quality and quantity of antibiotics at North Jakarta Teaching Hospital. This study is a quantitative study using a retrospective cohort case study method. The samples in this study were pediatric patients with a diagnosis of acute diarrhea with mild-moderate dehydration who were hospitalized at the North Jakarta Teaching Hospital for the period January-December 2020 and JanuaryDecember 2021 before and after the implementation of the clinical pathway, totaling 274 patients. The data used is secondary data in the form of medical records of pediatric patients with acute diarrhea with mild-moderate dehydration who were hospitalized at the North Jakarta Teaching Hospital. The data that has been collected is then processed and analyzed using the chi-square test. The results showed Qualitative use of antibiotics with the Gyssens method after the application of clinical pathway is better than before the application of clinical pathway. Quantitative antibiotic administration with the DDD method in pediatric patients with acute diarrhea with dehydration before and after the application of clinical pathway the highest was ceftriaxone antibiotics after the application of clinical pathway with DDD/100 patient-days value increased compared to before clinical pathway.

Abstract

ABSTRACT During the COVID-19 pandemic, people with diabetes and hypertension are especially vulnerable to this virus. Of 1488 cases, 50.5% of patients had hypertension, 34.5% had diabetes, with mortality rates of 13.2% and 11.6%, respectively.  This study aims to evaluate the clinical laboratory parameters of diabetes and hypertensive diabetes patients  who have been vaccinated with Sinovac Vaccine. This study used retrospective and prospective cohort methods with convenience sampling. Inclusion criteria included patients over 18 years of age who were diagnosed with diabetes and diabetes with hypertension, had been vaccinated with two doses of Sinovac, and had filled out informed consent. From the 300 research samples, there were 169 women (56.3%). The majority of patients (94.4%) were over 40 years old, with 36.0% having comorbid DM hypertension with cholesterol, and 26.6% were smokers. The results of the analysis showed that age (P = 0.009), gender (P = 0.000), BMI (P = 0.000), and time of diagnosis (P = 0.000) were significantly correlated with EGFR clinical results. BMI (P=0.007) was also significantly associated with creatinine. Diabetes medication use showed  significant association with  GDS (P = 0.000) and HbA1c (P = 0.000). There is a significant relationship between gender, age, BMI, and time diagnosed with EGFR. Patients with a diagnosis period of more than 10 years suffer from stage 3 kidney failure reaching 9% and stages 4 and 5 reaching 4%. The use of metformin, sulfonyl urea, insulin glulisine, and insulin glargine provides a controlled effect on GDS and HbA1c. Keywords: Diabetes, Hypertension, Laboratory, Sinovac, Sosiodemography  ABSTRAK Selama pandemi COVID-19, penderita diabetes dan hipertensi sangat rentan terhadap virus ini. Dari 1488 kasus, 50,5% pasien memiliki hipertensi, 34,5% menderita diabetes, dengan angka kematian masing-masing 13,2% dan 11,6%.   Penelitian ini bertujuan mengevaluasi hasil klinis laboratorium pasien Diabetes dan Diabetes dengan Hipertensi yang telah divaksinasi dengan Vaksin Sinovac. Penelitian ini menggunakan metode kohort retrospektif dan prospektif dengan pengambilan sampel secara convenience sampling. Kriteria inklusi mencakup pasien berusia di atas 18 tahun yang terdiagnosis diabetes dan diabetes dengan hipertensi, telah divaksinasi Vaksin Sinovac dua dosis, dan telah mengisi informed consent. Dari 300 sampel penelitian, perempuan berjumlah 169 (56.3%). Mayoritas pasien 94.4% berusia di atas 40 tahun dengan 36,0% memiliki komorbiditas DM hipertensi dengan kolesterol, dan 26.6% adalah perokok. Hasil analisis menunjukkan bahwa usia (P = 0,009), jenis kelamin (P = 0,000), IMT (P = 0,000), dan lama terdiagnosis (P=0,000) berkorelasi secara signifikan dengan hasil klinis EGFR. IMT (P-0,007) juga berhubungan secara signifikan dengan hasil klinis kreatinin. Penggunaan obat diabetes menunjukkan hubungan yang signifikan dengan hasil klinis GDS (P = 0,000) dan HbA1c (P = 0,000).  Terdapat hubungan yang signifikan antara jenis kelamin, usia, Indeks Masa Tubuh (IMT), dan lama terdiagnosis dengan gagal ginjal (EGFR). Pasien dengan lama terdiagnosis lebih dari 10 tahun menderita gagal ginjal stadium 3 mencapai 9% dan stadium 4 dan 5 mencapai 4%. Penggunaan obat diabetes, metformin, sulfonil urea, insulin glulisine, dan insulin glargine memberikan efek terkontrol pada GDS dan HbA1c. Kata Kunci: Diabetes, Hipertensi, Laboratorium, Sinovac, Sosiodemografi

Abstract

Multidrug-resistant tuberculosis (MDR-TB) poses a significant challenge to TB control efforts in Indonesia, primarily due to its complex, long-term treatment and significant drug side effects. To date, there is no comprehensive evaluation data on treatment outcomes and quality of life of MDR-TB patients at Raden Mattaher Regional Hospital, Jambi Province. This study aims to evaluate treatment outcomes and health-related quality of life in MDR-TB patients, as well as to identify predictors influencing therapy outcomes. This study used an observational   design with a retrospective approach, analyzing medical records of MDR-TB patients from 2020 to 2024. The WHOQOL-BREF instrument was used to measure quality of life. Bivariate and multivariate analyses were conducted to evaluate the association between patient characteristics and treatment outcomes. Most MDR-TB patients demonstrated successful treatment outcomes, but quality of life in the physical (score 45.2) and environmental (score 48.5) domains was relatively low. Side effects of second-line drugs, such as nausea, neuropathy, and hearing loss, contributed to the low physical domain scores. Multivariate test results showed that comorbid diabetes mellitus (DM) was a significant predictor of treatment outcomes (p = 0.007). The quality of life of MDR-TB patients at Raden Mattaher Regional Hospital in Jambi remains low, primarily influenced by side effects of therapy and the presence of comorbidities such as DM, which have been shown to impact treatment outcomes. More integrated interventions between TB management and comorbidities are urgently needed. Patients should be educated about side effects and the importance of treatment adherence. Hospitals are advised to improve side effect monitoring and integrate comorbidity management.
