Abstract

Introduction: Intense and severe pain is the most common symptom of acute gout arthritis (GA) flare. A recent study revealed that opioids are commonly prescribed for acute gout flare. Objective: This review aims to explain current evidence of opioid usage in the management of pain for acute GA flare. Method: The literature search was conducted on PubMed and Google Scholar in June 2022. Relevant articles published in the last 10 years were included. Results: The evidence of opioid use in managing acute gout arthritis flares is scarce. Currently, no guidelines recommend opioids as an initial analgesic choice in managing acute GA flare. Opioids are indicated in the flare with severe kidney dysfunction if only oral corticosteroids and intra-articular corticosteroids are not effective in reducing pain. Conclusion: Opioids should be used as only adjuvant and last choice in selected cases.

Abstract

The Provincial Association of All-Indonesian Football Associations in East Java as the Association of Football Associations, has duties and responsibilities in managing, developing and advancing East Java football. One of the problems faced by the Provincial Association is the first treatment of sports injuries. New football coaches receive training on Basic Life Support and First Aid for injuries/traumas when participating in C-licenced trainer certification while becoming a coach at a football school does not require a C-licence qualification. The purpose of this study was to determine the skills and abilities of football coaches regarding Basic Life Support and First Aid for injuries to players when the medical team is not available so that permanent disability and athlete death can be avoided. The method used is a survey method by assessing the skills of Football School coaches in understanding Basic Life Support and First Aid. The results show that only 12 have heard/done 38.7% of First Aid training. Of the 31 coaches, 18 people understand what to do to check consciousness when a player faints are 58.1%. How to free the airway in players suspected of having neck injuries, only 13 coaches knew about 41.9%. Only 12 coaches knew how to free the airway in players without neck injuries by 38.7%. 15 trainers who know the maximum time for breathing examinations are 48.4%.

Abstract

Acute gout flare is one spectrum of gout disease. The flare is characterized by severe acute pain with signs of inflammation at the joint. Currently, there are three primary guidelines in managing gout flares, namely American College of Rheumatology (ACR), European League Against Rheumatism (EULAR), and Indonesian Rheumatology Association (IRA). The objective of this review is to analyze the management of gout flares among these guidelines. We compared three guildelines on managing acute gout flare based on their acute therapy initiation time, first-line treatment, additional treatment, and urate-lowering therapy initiation. Additional literature search was also conducted on PubMed and ScienceDirect to provide additional information to the guideline. All guidelines agreed that management of gout flares should be initiated as early as possible with colchicine or NSAIDs based on patient condition and contraindications. Oral or injected corticosteroid should be considered in patient with renal impairment or contraindications to colchicine or NSAIDs. In conclusion, most recommendations from three guidelines were similar. IRA 2018 are still suitable for managing acute gout flare for Indonesian population

Abstract

Abstract: Sperm centrifugation is an important process to obtain good spermatozoa for assisted reproductive techniques. However, the centrifugation process has additional impacts such as increased reactive oxygen species (ROS) and oxidative stress affecting sperm viability due to damage of spermatozoa membrane. The use of vitamin C (ascorbic acid) can reduce elevated ROS levels, thereby protecting DNA and spermatozoa from damage caused by oxidative stress, meanwhile albumin can reduce oxidative stress and maintain the integrity of spermatozoa membrane. This study aimed to obtain the effect of combination of vitamin C and albumin on spermatozoa viability in centrifugation process. This was a true experimental in vitro study using post-test-only control design. There were four groups in this study, as follows: the control group (K); the treatment group (P1), centrifugation without vitamin C and albumin; (P2), centrifugation with vitamin C; and (P3), centrifugation with a combination of vitamin C and albumin. The Mann-Whitney test for each group showed significantly differences in spermatozoa viability (p<0.05) among them. The treatment group receiving supplementation with a combination vitamin C and albumin produced the highest viability after the control group. In conclusion, supplementation of combination of vitamin C and albumin is capable in maintaining spermatozoa in centrifugation process. Keywords: assisted reproduction technique; centrifugation; spermatozoa viability; supplementation    Abstrak: Sentrifugasi sperma merupakan proses penting untuk mendapatkan spermatozoa yang baik dalam keperluan teknik bantu reproduksi, namun proses sentrifugasi memberi dampak lain yaitu terjadinya peningkatan reactive oxygen spesies (ROS) dan stres oksidatif yang memengaruhi viabilitas spermatozoa akibat rusaknya membran spermatozoa. Pemberian vitamin C (asam askorbat) dapat menurunkan kadar ROS yang meningkat sehingga melindungi DNA dan spermatozoa dari kerusakan akibat stres oksidatif sedangkan albumin mampu mengurangi stres oksidatif dan mempertahankan integritas membran spermatozoa. Penelitian ini bertujuan untuk mengevaluasi pengaruh kombinasi vitamin C dan albumin terhadap viabilitas spermatozoa. Jenis penelitian ialah true experimental in vitro menggunakan post test only control design dengan pengamatan pada kelompok kontrol (K); kelompok perlakuan (P1) sentrifugasi tanpa vitamin C dan albumin; (P2) sentrifugasi dengan vitamin C; dan (P3) sentrifugasi dengan kombinasi vitamin C dan albumin. Hasil uji Mann-Whitney dari masing-masing kelompok menunjukkan viabilitas spermatozoa yang berbeda bermakna (p<0,05) antar satu sama lain. Kelompok perlakuan yang mendapat suplementasi kombinasi vitamin C dan albumin menghasilkan viabilitas tertinggi setelah kelompok kontrol. Simpulan penelitian ini ialah suplementasi kombinasi vitamin C dan albumin mampu mempertahankan viabilitas spermatozoa dalam proses sentrifugasi. Abstract

Pelaksanaan belajar dari rumah meningkatkan paparan anak pada gawai. Penggunaan gawai oleh anak usia sekolah, perlu dibarengi dengan pendampingan orang tua agar dapat mengendalikan durasi dan konten yang diakses. Penggunaan gawai berlebihan berpotensi menimbulkan gangguan tajam penglihatan (miopia). Anak dengan gangguan tajam penglihatan perlu terapi kacamata agar dapat berfungsi dengan baik di lingkungannya dan mencegah perburukan penyakit. Pada umumnya anak tidak mengeluhkan tentang gangguan penglihatan, tetapi ada tanda yang dapat diperhatikan oleh orang tua atau guru yaitu sering menyipitkan/mengusap mata, melihat dengan jarak sangat dekat atau sering mengeluh pusing atau mual. Tujuan pengabdian kepada masyarakat ini adalah pemeliharaan kesehatan mata siswa kelas 1 di SD Kr. Baptis Surabaya. Metode yang digunakan adalah dengan diskusi kelompok terfokus, webinar tentang kesehatan mata, cara menjaga dan mencegah perburukan gangguan penglihatan pada guru dan wali murid, pemeriksaan tajam penglihatan dan koreksi kacamata untuk murid. Prevalensi gangguan tajam penglihatan pada murid sebesar 81.25% dan 50% diantaranya menderita astigmatisma miopia simpleks. Miopia pada anak perlu diterapi dengan kacamata untuk mencegah terjadinya miopia berat saat anak bertumbuh dewasa dan meningkatkan prestasi akademik serta membuka peluang karier masa depan anak. Program skrining tajam penglihatan mata anak di sekolah adalah langkah mudah deteksi dini gangguan tajam penglihatan untuk optimalisasi perkembangan anak bangsa.

Abstract

Injuries to sports activities can have a negative impact on physical activities. Sports injuries include physical injuries that usually occur during physical activity, whether amateur or professional. Football is a type of sport that has a high intensity and requires good physical endurance. This sport is very susceptible to injury. First aid training in the form of handling injuries can be seen as an important skill for the formation and training of needed experts, in this case trainers. The purpose of first aid techniques in the form of handling injuries are as follows; protect the further life of the individual, prevent adverse injury situations, and recover in the form of rehabilitation. The activity was carried out on February 16, 2022 at the Kampi Hotel Surabaya. The number of participants who took part in this community service activity was 73 football coaches, members of PSSI East Java.

Abstract

The Quick Sequential Organ Failure Assessment (qSOFA) is a simple parameter, however, its sensitivity as a mortality predictor is low. This study aimed to improve the predictor performance of qSOFA for in-hospital mortality. This study was a retrospective single-centered cohort using medical record data. This study included 150 patients aged 18-80 years old, who experienced sepsis and received ICU care between September 2021 and August 2022. qSOFA and modified ROX index (mROX) were calculated based on the most severe condition recorded in the emergency department (ED). Each variable's area under the receiver operating characteristic (AUROC) curve, sensitivity, and specificity were compared to predict in-hospital mortality. qSOFA scores ≥2 and mROX values ≤3.20 were independent factors that increased the risk of in-hospital mortality (OR 3.69 and 21.50; p 0.004 and 0.005, respectively). The combination of qSOFA scores ≥2 and mROX value ≤3.20 as in-hospital mortality predictors resulted in AUROC 0.791 with a sensitivity of 71.7% and specificity of 75.7%. The AUROC, sensitivity, and specificity of the combination of qSOFA and mROX were higher than qSOFA (0.766, 70.8%, 70.3%) or mROX (0.760, 68.1%, 67.6%) alone. In conclusion, the combination of qSOFA scores ≥2 and mROX values ≤3.20 increase the sensitivity and specificity for predicting in-hospital mortality in sepsis patients.

Abstract

Background: Chronic Energy Deficiency (CED) during pregnancy represents a critical health challenge, especially in developing regions, where it significantly threatens maternal and fetal well-being. Docosahexaenoic Acid (DHA), an essential omega-3 fatty acid, plays a pivotal role in fetal neurodevelopment and maternal health. However, the impact of DHA supplementation on maternal calcium levels, particularly among pregnant women affected by CED, has not been extensively studied. This study evaluate the correlation between DHA supplementation and calcium levels in third-trimester pregnant women with CED, with the goal of improving outcomes for this vulnerable population. Method and Results: This research uses experimental cohort study design with a quantitative approach, using non-probability sampling techniques such as quota and purposive sampling, and involving a total of 24 participants. Statistical analysis revealed a significant increase in calcium levels following DHA supplementation, with a p-value of 0.007 (<0.05), indicating a notable impact of the intervention. The mean calcium levels post-treatment were higher than pre-treatment, demonstrating the efficacy of DHA supplementation in improving calcium levels. Further analysis showed no significant correlation between pre-supplementation calcium levels and Body Mass Index (BMI) (p = 0.622, > 0.05). However, a significant correlation was found between post-supplementation calcium levels and BMI (p = 0.002, < 0.05). These findings indicate a positive correlation between DHA supplementation and calcium levels in pregnant women with CED during the third trimester. However, further investigation is needed to fully understand the relationship between calcium levels and BMI. Conclusion: This study lays the groundwork for future research aimed at optimizing therapeutic interventions or supplementation strategies to improve the nutritional status of underweight pregnant women.

Abstract

In the last decades, traditional herbal medicine for gout has been rapidly advanced. Numerous herbs have been identified for their hypouricidal effect. This review aims to explain the potential and mechanisms of traditional herbal medicine in reducing serum uric acid (sUA). This review included five RCTs and four traditional herbal medicine formulas from a previous systematic review. Articles published in non-English language were excluded for further analysis. Despite limited RCTs design, the formula of Chuanhu; Tufu, and ZinutriK® were beneficial for hyperuricemic individuals. While Yellow-dragon Wonderful-seed mixture showed limited hypouricemic feature. Based on these findings, traditional herbal medicine holds promising potential in treating hyperuricemia and gout. However, further studies are required.

Abstract

Gout is the most common inflammatory arthritis in the world, including in the Asia region. Gout is a debilitating, yet preventable disease. The general principle of gout prevention is to regulate circulating UA concentration and other related comorbidities. Identifying gout modifiable risk factors is paramount in gout prevention. This review aims to explain modifiable risk factors for gout, especially in the Asia region. Apart from urate-lowering therapy, modifying gout risk factors consisted of a low-purine diet, mild to moderate exercise, adequate sleep, elimination of smoking exposure, and regulation of comorbidities such as hypertension, obesity, metabolic syndrome, and kidney diseases, lowered the risk of hyperuricemia and gout in Asian population.
