Abstract

Indonesia has the second-highest number of tuberculosis (TB) cases in the world. Many TB patients did not take their medicine at health care centers causing an increase in taking medication. This study aimed to develop a simple model for describing non-adherence to medication in TB patients. This study used an analytical with a cross-sectional approach. The following criteria were used in selecting the respondent: adolescent, able to read and write, and willingness to be a respondent. A total of 93 participants were recruited randomly. Data were collected through questionnaires and analyzed using logistic regression. The predisposing factors were knowledge, attitude, belief (p = 0.000), trust (p = 0.013), and values (p = 0.001). Family support (p = 0.034) and healthcare personnel support (p = 0.022) were reinforcing factors. The enabling factor was the healthcare facility (p = 0.000). This study found that the most dominant factor is knowledge (B = 56.4). The model was Logit (Pi) = 22.363 + 56.4 knowledge (1) + 22.56 belief (1) + 2.9 family support (1) + 0.577 healthcare support (1) + 0.061 healthcare facility (1). This study suggests that TB patients’ knowledge must be increased to improve their TB treatment and management.

Abstract

Introduction: Bronchitis is characterized by excessive sputum production due to an infection in the bronchi. In patients with bronchitis, it can cause nursing problems in the form of ineffective airway clearance. Ineffective airway clearance is the inability to clear the airway which is characterized by changes in frequency, pattern, and breath sounds. Objective: The purpose of this study was to apply nursing care to patients with the application of Clapping, Vibrating, Postural Drainage, and Effective Coughing to overcome ineffective airway clearance in the Azzahra 1 Room, RSI Jemursari Surabaya. Methods: The design of this research is a case study; the time of research is 13-15 October 2022 in the Azzahra 1 Room, RSI Jemursari Surabaya. The subject used is 1 bronchitis patient (Mr. B) with a nursing diagnosis of ineffective airway clearance. The research was conducted using interviews accompanied by physical examination, observation, and documentation. Nursing care given to patients with a nursing care process approach includes assessment, nursing diagnosis, intervention, implementation, and evaluation. Results: The results of the case study found that the patient had a cough with phlegm for 2 weeks, chest pain when coughing, increased sputum production, had rhonchi, and the patient was an active smoker. Evaluation of the results of nursing care with effective Clapping, Vibrating, Postural Drainage, and Coughing for 3 days once a day showed decreased rhonchi, decreased sputum production, and patients can cough effectively independently. Conclusions: Clapping, vibrating, postural drainage and effective coughing can help clear secretions to maintain a patent airway. Nurses are expected to be able to apply effective chest and cough physiotherapy to patients so that optimal action is achieved and the patient performs independently.

Abstract

Background: Inadequate airway clearance is one of the nursing issues that arises during the pulmonary tuberculosis disease process. If untreated, ineffective airway clearance can result in hypoxemia, decreased consciousness, and even cell death. Objective: The purpose of this study is to see that simple inhalation therapy can help pulmonary tuberculosis patients at the Banyu Urip Community Health Center to overcome ineffective airway clearance. Methods: The research methodology combines a case study approach with a family nursing process approach, with one case as the unit of analysis, Mrs. K, a pulmonary tuberculosis patient who get treatment at the Banyu Urip Community Health Center working area. Data was gathered through interviews, physical examinations, supporting examinations, and a literature review. In the study, 350 ml of hot water steam and 5 drops of eucalyptus oil were applied twice a day for three days, for 10-15 minutes. Result: The research results of simple inhalation therapy proved to have a positive effect on Mrs. K, as evidenced by a reduction in verbal complaints, including ease in driving out secretions, respiratory frequency was within normal limits, and breathing became easier, because the cineol content of eucalyptus oil has mucolytic, bronchodilator, and anti-inflammatory functions. Conclusion: To overcome airway ineffectiveness and improve health status, simple inhalation therapy combined with eucalyptus oil can be used as an alternative. Non-pharmacological techniques that are simple, easy to use but provide numerous benefits in health care facilities, such as simple inhalation therapy, are expected of health care workers.

Abstract

Introduction: Diverticulitis is a disease caused by an unhealthy lifestyle, lack of dietary fiber intake and a high-fat diet as well as a lack of physical activity are the main triggers for this disease. In diverticulitis sufferers, anxiety problems often arise due to concerns about their health condition in the future and what their health status will be if surgery is performed. Objective: This study aims to determine the description of nursing care and analyze the application of progressive muscle relaxation therapy in diverticulitis patients with anxiety problems. Methods: This research uses a case study method by providing nursing care through a nursing process approach. The research was conducted at Darmo Hospital Surabaya on 23-25 September 2022 by applying progressive muscle relaxation therapy to 1 respondent for 3 days with an intensity of 2x/day with a duration of 15-20 minutes, evaluating the patient using the Hamilton Rating Scale for Anxiety instrument (HARS). Results: The results of this case study show a decrease in the level of anxiety felt by the patient after being given progressive muscle relaxation therapy. The level of anxiety decreased from moderate anxiety (score 22) to mild anxiety (score 14), the patient appeared calm and relaxed, and the patient's pulse decreased. This research concludes that the application of progressive muscle relaxation therapy can be an alternative to reduce anxiety in diverticulitis patients. Conclusions: However, the findings should be interpreted with cautiously due to the study's reliance on a single case and the short intervention period. Future research should explore the long-term effects of progressive muscle relaxation therapy on a larger and more diverse patient population.
