Abstract

Magnetic Resonance Imaging (MRI) is a medical diagnostic imaging technique that works on the principle of nuclear magnetic resonance. MRI aircraft can produce diagnostic images of the inside of the human body in both sagittal, coronal, and transverse cuts, Magnetic resonance imaging (MRI) is a widely accepted non-invasive technique for the evaluation of cartilage lesions. The femur, tibia, fibula, and patella are united into one complex group by ligaments. According to Moeller, Genu Examination uses STIR Sequence, Sagital T2 Weighted Fat saturated, Sagittal 3D GRE, Proton Density weighted fat saturated T1 Coronal. The author's purpose in conducting this study is because Genu Examination at one of the Jakarta Hospital in the Case of Soft Tissue Tumor, Meniscus Tear and Oesteoarthritis added the Axial 3D SPGR sequence. The research method used is descriptive research with an observational approach. Data collection was carried out on June 5 to August 31 using Mri GE aircraft with a power of 1.5 Tesla. The patient samples used in this study were three patients with clinical Soft Tissue Tumor, Meniscus Tear, and Osteoarthritis. Data collection was carried out through observation, interviews and Genu Examination Documentation with the addition of Axial 3D SPGR. The examination is carried out using Quad Knee Coil, the patient's position is supine, feet first. The sequences used in the examination at one of the Jakarta hospitals are Axial Proton Density Fat saturated, Sagital Proton Density Fat saturated, Sagital T2, Coronal Proton Density Fat saturated, Coronal STIR. The role of the Axial 3D SPGR sequence is to evaluate articular cartilage due to its clear depiction of cartilage morphology and high signal to noise ratio (SNR) images.

Abstract

Magnetic Resonance Imaging (MRI) is a diagnostic examination modality in medical science using a magnetic field without using X-rays. MRI has the advantage of being able to create images and showing clearer and more sensitive anatomical differences in soft tissues in the body such as the extremities, brain, bone marrow and musculoskeletal to establish a clinical diagnosis, one of which is Supraspinatus Injury to the Shoulder. Supraspinatus is a tear or rupture of the tendon of the supraspinatus muscle. The supraspinatus is part of the shoulder rotator cuff. Most of the time, it is accompanied by another rotator cuff muscle tear. This can occur due to trauma or repeated microtrauma and appears as a partial or full thickness tear. The rotator cuff is a fibrous connective tissue that surrounds the top of the humerus bone and functions to maintain the stability of the glenohumeral joint by pulling the humerus towards the scapula. The research method used is qualitative descriptive research with an observational approach. Data collection was carried out from 08 August 2023 to 07 September 2023 using the Mri Philips Achieva 1.5 Hestia aircraft. The patient sample used in this study was a patient with shoulder complaints. Data collection was carried out through observation, interviews and documentation. Abstract

Latar Belakang: Lymph node cancer in the case of Hodkin's Lymphoma is a type of cancer. Lymphoma is a solid tumor of the immune system. Hodgkin lymphoma (HL) accounts for about 10% of all lymphomas. The Intensity Modulated Radiation Therapy (IMRT) technique is one of the cancer treatment techniques in Hodgkin's Lymphoma cases. The aim of this research is to determine the procedure and reasons for using the IMRT technique in lymph node cancer in Hodkin's Lymphoma cases.Metode: This research is qualitative research with a case study approach using retrospective data. The research time is from 24 to 30 May 2023 and the research location is in the Department of Radiation Oncology, RSUP Dr. Hasan Sadikin Bandung. The research subjects consisted of Radiation Oncologists, Medical Physics, Radiotherapists. Researchers collected data from patient medical records, made interview transcripts and carried out data reduction and processed it using open coding, and provided it in the form of quotations and then drew conclusions.Hasil: The procedure for irradiating Hodgkin's Lymphoma using the IMRT technique at the RSHS Department of Radiation Oncology begins with positioning the patient on the examination table with a fixation device and arranging the patient according to the reference point of the CT Simulator and making a thermoplastic mask then carrying out verification and irradiation. The IMRT technique has the advantage of using MLC so that the radiation is modulated according to the contour of the target and the disadvantage of precise dose distribution in limiting the radiation dose to organs at risk.Kesimpulan: In the case of the Department of Radiation Oncology, Dr. Hasan Sadikin Hospital, Bandung, the IMRT technique was used with consideration, namely that the IMRT technique already uses MLC which creates individual blocks automatically according to the projections depicted on the TPS.

Abstract

Background: Coronary Artery Disease (CAD) is a blockage or narrowing of the coronary arteries that carry blood to the heart muscle. The results of the 2018 Basic Health Research based on doctors' diagnoses stated that coronary heart disease in Indonesia was 1.5% with a prevalence in the DKI Jakarta area reaching 1.9%. Percutaneous Coronary Intervention (PCI) is a procedure or act of widening narrowed coronary arteries using a balloon aimed/directed across a catheter. Objective: To determine the application of the radiographer's SKKNI in PCI procedures in the Intervention Room at the Bhayangkara TK.I Pusdokkes Polri East Jakarta Hospital. Method: The research used is a descriptive qualitative method, namely describing the results of observations. This research was conducted in July 2023 in the Intervention Room at Bhayangkara TK.I Pusdokkes Polri East Jakarta Hospital. The subjects in this study were three radiographers in the Intervention Room at the Bhayangkara TK.I Pusdokkes Polri East Jakarta Hospital. The data collection methods used in this research are observation, interviews and documentation. The data method and analysis uses an interactive model method with stages of data collection, data processing, and data presentation, and discussion and conclusions. Results: Of the 25 criteria for work on the SKKNI radiographer, there are 20 criteria for appropriate work and 5 criteria for inappropriate work. The criteria for inappropriate work are the radiographer not preparing the injector machine, not preparing the emergency trolley and its equipment (medicines and others), did not educate the patient, did not re-check the informed consent and did not regulate the injector machine. Elements of radiographer competency in the SKKNI include pre-examination procedures, equipment and supplies preparation, patient preparation, examination management procedures and post-examination procedures.

Abstract

Magnetic Resonance Imaging (MRI) is the gold standard for diagnosing Hernia Nucleus Pulposus. Hernia of the Nucleus Pulposus (HNP) is a condition in which there is bulging of the nucleus pulposus. This study aims to determine the management of lumbar MRI examinations in cases of Nucleus Pulposus Hernia and the role of the Dixon sequence in cases of lumbar HNP. The Dixon sequence is an MRI method used for fat suppression and produces 4 contrasts in one scanning, including in phase, opposed phase, water and fat. The research method used is descriptive research with a case study approach. Data collection was carried out in January 2023 – February 2023 using a Philips 3 Tesla MRI aircraft. Subjects consisted of ten patients with clinical HNP. Data collection was carried out through observation, interviews and documentation. The research results were obtained according to theory using the sequence T1 SE/TSE sagittal, T2 SE/TSE axial, SE/TSE T1 axial, Short Inversion Recovery (STIR) while in the field using the sequence Survey, Myelo Radial, T2 Weighted Dixon Sagittal (T2 Fat Saturation Sagittal, T2 Sagittal), T1 Weighted TSE Sagittal, STIR TSE PSS Coronal, T1 Weighted TSE Axial, T2 Weighted TSE Axial.

Abstract

Protokol Survey_Full, Survey_Right, T2W_TSE_Tra, T2W_TSE_tra, T2W_TSE_Sag, T2W_SPIR, T1W_TSE_sag, T2W_TSE_sag, T2W_SPIR_cor sering digunakan untuk pemeriksaan MRI Knee Joint di RSUD Provinsi NTB, di Instalasi Radiologi RSUD Provinsi Nusa Tenggara Barat sekuens T2*GRE tidak menjadi sekuens rutin, sehingga penulis tertarik ingin membandingan hasil citra MRI Knee Joint T2W TSE dengan T2*GRE pada kasus ruptur ACL. Jenis penelitian ini adalah kuantitatif deskriptif. Penulis mengumpulkan data berupa 12 pasien hasil citra hasil radiograf dari pemeriksaan MRI Knee Joint terhadap 12 jumlah sampel pasien menggunakan sekuens T2W TSE dan T2*GRE Kemudian dari 12 hasil citra dilakukan penilaian berupa check list oleh responden yaitu 2 dokter radiolog. Data kemudian diolah dengan menggunakan spss. Hasil check list yang sudah diolah menggunakan spss, menurut hasil uji wilcoxon bahwa adanya perbedaan informasi anatomi ACL dengan sekuens T2W TSE dan T2*GRE sehingga diambil kesimpulan dengan hasil mean rank 2.00 jika sekuens T2W TSE lebih informatif dalam menilai anatomi ACL dari pada sekuens T2*GRE.

Abstract

Tujuan dari penelitian ini adalah untuk menganalisis CT Scan Cardia Klinis Chest Pain di rumah sakit Bhayangkara Makassar. Penelitian yang digunakan adalah jenis penelitian kualitatif dengan pendekatan studi kasus pada Pemeriksaan CT Scan Cardiac dengan klinis Chest Pain di Instalasi Radiologi Rumah Sakit Bhayangkara Makassar. Hasil dari penelitian ini yaitu bahwa teknik pemeriksaan CT Scan cardiac pada klinis Chest Pain di Instalasi Radiologi RS. Bhayangkara Makassar, sudah dapat mendiagnosis kasus Chest Pain dimana dilakukan serangkaian persiapan pasien dan teknik pemeriksaan antara lain yang perlu diperhatikan dalam CT Scan Cardiac yaitu Heart Rate pasien yang disarankan 50 – 70 bpm karena sangat berpengaruh untuk hasil citra nya dan dalam pemasukan bahan kontras menggunakan Teknik bolus tracking sebagai teknik pemasukan media kontras karena kemampuan dari teknik ini lebih sederhana dan sudah dapat menghasilkan gambaran pembuluh darah jantung dan penentuan nilai HU sudah tercover system pada komputer CT Scan serta untuk hasil citra dilakukan berbagai macam rekonstruksi seperti Retro Recon, Calcium Score, MIP, VRT, 3D, Coronary Tree, Angio View, Vessel Analisis.

Abstract

Background: Analysis of Radiation Exposure Levels in the Thyroid Area of ​​Interventional Cardiology Doctors During Percutaneous Coronary Intervention (PCI) Diagnostic Examinations in the Cath Lab Room at Dr Soedarso Pontianak Hospital, is research related to patient examinations in the Percutaneous Coronary Intervention Diagnostic Room. The main object in this research is the doctor who examines or treats patients in the room. Researchers wanted to see how many doses of radiation exposure were received by officers examining patients. Method: The research used in this research is quantitative methods. Quantitative research is research that emphasizes analysis on numerical data processed using statistical methods. Based on the type of research, this quantitative research leads to analytical (inferential) research, namely analyzing the relationship between variables by testing hypotheses. The object of focus in this research is an Interventional Cardiology doctor, using 30 patients who have received treatment from a doctor in the cath lab room then the data analysis process uses the measurement results from the pen dose, by conducting research for ten days from 10 August 2023 - 17 August 2023 Results: The results obtained from the average value of radiation dose exposure received by Dr. Interventional Cardiology in the Cath Lab Room at Dr. Soedarso Pontianak Hospital while examining 30 patients, were 0 Sv or the equivalent of 0.000 mSv. Based on these results, it was concluded that the PPE used in the cath lab room at Soedarso Regional Hospital was in accordance with applicable regulations. Conclusion: Based on the data that has been analyzed, it can be concluded that the average value of radiation dose exposure received by Dr. Interventional Cardiology in the Cath Lab Room at Dr. Soedarso Pontianak Hospital while examining 30 patients, is 0 Sv, and is equivalent to 0.000 mSv. Based on the average value of the exposure dose received, which is 0 Sv, the PPE used can reduce the radiation exposure received by doctors. The personal protective equipment includes: Hat, Google Glasses, Thyroid, Apron and protective screen used in accordance with applicable regulations can reduce radiation exposure received by cardiology doctors in the cath lab room at Dr Soedarso Hospital Pontianak.

Abstract

Dosis radiasi yang diterima pasien pada pemeriksaan Thorax bisa bervariasi antara satu rumah sakit dengan rumah sakit lainnya. Untuk itu, pemerintah Indonesia melalui BAPETEN telah menetapkan Indonesian Diagnostic Reference Levels tahun 2021 sebagai referensi dosis radiasi dalam pencitraan medis. Penelitian ini bertujuan untuk mengetahui apakah ada perbedaan signifikan pada dosis radiasi pemeriksaan Thorax di rumah sakit yang berbeda, serta membandingkan dosis tersebut dengan referensi IDRL 2021. Penelitian ini merupakan penelitian kuantitatif dengan pendekatan eksperimen dan survei yang dilaksanakan di 2 rumah sakit pusat rujukan di Bali. Nilai dosis radiasi berupa Entrance Surface Air Kerma (ESAK) dikumpulkan di setiap rumah sakit. Nilai ESAK didapatkan melalui simulasi atau pengukuran dengan menggunakan dosimeter digital yang diletakkan pada permukaan water phantom untuk pemeriksaan Thorax. Adapun parameter pemeriksaan yang dipakai adalah parameter standar untuk pemeriksaan Thorax dewasa pada pasien dengan berat badan 50-70 kg. Secara keseluruhan, 60 nilai ESAK dikumpulkan dari dua rumah sakit. Hasil penelitian menunjukkan adanya variasi signifikan pada dosis radiasi pemeriksaan Thorax di rumah sakit yang berbeda dengan rata-rata dosis sebesar 0,033 mGy untuk RS A, dan 0,03 mGy untuk RS B. Sedangkan nilai median ESAK adalah 0.032 mGy (RS A) dan 0.027 mGy (RS B). Meski demikian, penelitian menunjukkan bahwa nilai median ESAK pemeriksaan Thorax di kedua rumah sakit lokasi penelitian masih dalam batas rekomendasi IDRL 2021 yaitu dibawah 0,4 mGy. Penelitian ini menunjukkan pentingnya upaya optimasi berkelanjutan untuk menjamin keselamatan radiasi pada pasien yang menjalani pemeriksaan radiologi diagnostik.

Abstract

Stenosis pylorus adalah kondisi penyempitan pada bagian pylorus gaster yang menghambat pengosongan lambung ke usus. Penyakit ini lebih sering terjadi pada bayi dan jarang ditemukan pada orang dewasa. Pemeriksaan CT Scan abdomen dengan kontras merupakan modalitas efektif untuk menegakkan diagnosis stenosis pylorus karena mampu memberikan visualisasi anatomi lambung dan pylorus yang lebih baik dibandingkan modalitas lain. Penelitian ini bertujuan untuk mengetahui prosedur pemeriksaan CT Scan abdomen, serta penggunaan media kontras oral dan intravena pada kasus susp stenosis pylorus gaster di RSUP Persahabatan. Penelitian ini menggunakan metode kualitatif dengan pendekatan studi kasus, yang dilaksanakan pada Juli–Agustus 2024. Data dikumpulkan melalui observasi, wawancara, dan dokumentasi. Analisis data dilakukan dengan mentranskrip hasil wawancara, mereduksi informasi relevan, dan melakukan coding terbuka untuk menarik kesimpulan. Prosedur pemeriksaan CT Scan abdomen dimulai dengan pemindaian topogram dari diafragma hingga symphysis pubis, diikuti dengan scan pre-kontras, post-kontras, dan scan full-blass. Media kontras oral berupa campuran 150 ml iodine dan 450 ml air mineral diberikan sekali sebelum pemeriksaan, sedangkan media kontras intravena sebanyak 50 ml dengan flow rate 2,5 ml/detik diberikan setelah scan pre-kontras selesai. Meskipun metode ini berbeda dari teori yang melibatkan tiga jenis kontras (oral, anal, dan intravena), pendekatan yang digunakan di RSUP Persahabatan tetap memberikan visualisasi anatomi yang memadai untuk menegakkan diagnosis stenosis pylorus.
