Abstract

Recently, Pejawaran District has high IMR minimal 5 year and higher than national average. The aims of this research are to evaluate health care, community behavior, and environment in Pejawaran District. This research uses observational method with survey approach. The 97 sample are women who ever deliver the life baby and live in Pejawaran for 5 years. Maternal-neonatal health cares in Pejawaran are good, which are enough in available, acceptable-continuely, accessible, affordable, and good quality. Health behaviors are various and some components should be more concerned (proper washing hand, brushing teeth, “kadarzi”, birth attendance, health insurance and smoking). Illness behaviors are still low, especially mother knowledge. Sick role behaviors are various especially the community efforts to take medication. Achievement is good in the knowledge of health care places and the understanding of the rights of sick people although the understanding of the duties is low. Unfulfilled health housing component are ratio wide house with family members (22,7%), floor (51,5%), wall (49,5%), roof (3,1%), ventilation (66%), lighting (20,6%), and cages of animal (51,6%). Unfulfilled health basic sanitation are the drinking water source (100%), excreta disposals (87%), garbage bin (4,1%), andwaste water disposal (50,5%). The conclution are maternal-neonatal health care in Pejawaran District have fulled requirement; health related behaviors achievement are various, but mostly must be increased; and the majority environment archievement haven ’t fulled health requirements yet.Kecamatan Pejawaran memiliki AKB tinggi selama 5 tahun terakhir dan lebih tinggi dibanding rata-rata nasional. Tujuan penelitian ini adalah mengevaluasi pelayanan kesehatan, perilaku masyarakat, dan lingkungan di Kecamatan Pejawaran. Penelitian ini menggunakan metode observasional dengan pendekatan survei. Sampel sebanyak 97 yaitu ibu yang pernah melahirkan 1 bayi lahir hidup dan tinggal di Pejawaran minimal 5 tahun. Pelayanan kesehatan maternal-neonatal cukup tersedia, dapat diterima-berkelanjutan, mudah dicapai, dapat dijangkau, dan berkualitas. Perilaku sehat masyarakat bervariasi, komponen yang masih memerlukan perhatian adalah mencuci tangan dengan benar, menggosok gigi, kadarzi, penolong persalinan, asuransi kesehatan dan merokok. Perilaku sakit masih rendah khususnya pengetahuan ibu. Perilaku peran sakit beragam khususnya perilaku mencari kesembuhan. Pengetahuan adanya tempat pelayanan kesehatan dan kepemahaman terhadap hak orang sakit tercapai dengan baik walaupun kepemahaman terhadap kewajiban orang sakit masih rendah. Komponen rumah yang belum mencapat syarat kesehatan adalah kepadatan rumah (22,7%), lantai (51,5%), dinding (49,5%), langit-langit (3,1%), ventilasi (66%), pencahayaan (20,6%), dan kandang ternak (51,6%). Sanitasi dasar yang belum mencapat syarat kesehatan adalah sumber air bersih (100%), sarana pembuangan kotoran (87%), sarana pembuangan sampah (4,15%), dan sarana pembuangan limbah (50,5%). Kesimpulan yang diperoleh adalah pelayanan kesehatan maternal-neonatal Kecamatan Pejawaran sudah memenuhi syarat; cakupan perilaku masyarakat terkait kesehatan beragam, namun sebagian besar harus diperbaiki; dan cakupan komponen lingkungan sebagian besar belum memenuhi syarat kesehatan.

Abstract

Doctor patient communication is the important basis of diagnosis, therapy and preventive process. In a few cases in Primary, Care, most of doctrs do wrong diagnosis because of inhibition in communication. In a busy doctor practice, communication takes short time. Patient are dissatisfied to doctor care because of less attention and opportunity to express their feeling. The objective of this study is to determine the correlation between patient satisfaction and duration of doctor patient communication in Community Health Centre. The research method used cross sectional design with analytic type. The collected data using satisfaction questionnaire that filled directly by respondent and direct observation by researcher to duration of doctor patient communication. Sixty-four respondents selected at random from eight Community Health Centres in Primary Care where doctor care the patient. Spearman Correlation test is used to analyze the data. The study finds out that the duration of doctor patient communication is 2,96±1,27 minutes on average. Based on eight categories (greeting, non verbal, empathy, language, listening, giving information, summarizing, motivation) of patient satisfaction level, the highest score is empathy and the lowest score is greeting. Patient satisfaction score to female doctor is higher than male (p=0,026). There is moderate correlation (r=0,444, p0,005) between patient satisfaction and duration of doctor patient communication in Community Health Centre. In conclusion, there is moderate correlation between patient satisfaction and duration of doctor patient communication in Community Health Centre.Komunikasi dokter pasien merupakan landasan yang penting dalam proses diagnosis, terapi maupun pencegahan penyakit. Dalam beberapa kasus di Primary Care banyak dijumpai dokter yang keliru mendiagnosis karena adanya hambatan dalam komunikasi. Pada praktik dokter yang sibuk komunikasi yang terjadi hanya sangat singkat. Ketidakpuasan sebagian pasien terhadap layanan dokter karena merasa kurang diperhatikan dan tidak ada kesempatan dalam mengungkapkan perasaannya. Tujuan dari penelitian ini adalah untuk mengetahui hubungan antara kepuasan pasien dengan durasi komunikasi dokter pasien di Puskesmas. Metode penelitian menggunakan rancangan cross sectional dengan jenis penelitian analitik. Pengumpulan data menggunakan kuisioner kepuasan yang diisi langsung oleh responden dan observasi langsung oleh peneliti terhadap durasi komunikasi dokter pasien. Enampuluh empat responden pasien terpilih secara acak dari delapan Puskesmas tempat dokter Primary Care memberikan pelayanan. Analisis data menggunakan uji Spearman Correlation. Hasil penelitian didapatkan rata rata durasi komunikasi dokter pasien 2,96±1,27 menit. Dari delapan komponen kategori (greeting, non verbal, empathy, language, listening, giving information, summarizing, motivation) tingkat kepuasan pasien yang tertinggi pada empathy dan yang terendah pada greeting. Skor kepuasan pasien terhadap dokter perempuan lebih tinggi daripada dokter laki laki (p= 0,026). Terdapat korelasi sedang (r = 0,444, p0,005) antara kepuasan pasien dengan komunikasi dokter pasien di Puskesmas. Dari hasil tersebut dapat disimpulkan bahwa terdapat hubungan sedang antara kepuasan pasien dengan komunikasi dokter pasien di Puskesmas.

Abstract

Job analysis and performance appraisals are the basis of developing a remuneration system consisting of fees for position, fees for service, and fees for people. Clinic Pratama receives payment for health services for patients participating in the National Health Insurance in the form of a capitation package. The capitation package distribution has not been regulated based on the position value index and performance appraisal according to the performance indicators of each employee’s detailed job descriptions, responsibilities and authorities. The purpose of this community service activity is to develop a remuneration system which can then be implemented in KPF as a strategy in human resource management facing the era of National Health Insurance (JKN) to achieve Universal Health Coverage (UHC). The method used is a qualitative method through a Focus Group Discussion (FGD) approach, interviews and documentation studies. The results of community service show that the Remuneration System can be developed based on Job Analysis and Performance Appraisals

Abstract

Latar belakang : Tindakan kateterisasi jantung dapat menimbulkan ketidaknyamanan / nyeri, nyeri dapat diturunkanmenggunakan terapi musik dan teknik relaksasi nafas dalam. Terapi musik adalah Pemberian tindakan mandirikeperawatan menggunakan MP3 dan headphone dengan mendengarkan musik yang telah dipilih oleh responden sesuaistandar terapi musik selama 15 menit minimal satu kali dan teknik relaksasi nafas dalam adalah pemberian metodebernafas secara mendalam dengan frekuensi kurang dari 10 x/menit yang dilakukan sambil mendengarkan musikselama 15 menit dengan mengacu panduan kombinasi terapi musik dan relaksasi nafas dalam yang dibimbing olehpeneliti/asisten peneliti.Tujuan : Untuk menganalisis efektivitas kombinasi terapi musik dan teknik relaksasi nafas dalam terhadap penurunanintensitas nyeri pada pasien post kateterisasi jantung.Metode: Desain penelitian ini adalah quasi eksperimen dengan pre - post test with control group design, denganpendekatan consecutive sampling. Jumlah sampel 38 responden. Penilaian nyeri menggunakan Numeric Rating Scale(NRS), analisa data menggunakan uji statistik wilcoxon, dan mann-whitney.Hasil: Hasil uji statistik wilcoxon menunjukkan p value < 0.05 sehingga kombinasi terapi musik dan teknik relaksasinafas dalam secara signifikan dapat menurunkan intensitas nyeri pasien post kateterisasi jantung, kemudian dilakukananalisis perbedaan masing-masing variabel ditambah dengan protap rumah sakit sebagai kontrol dengan uji mannwhitney, hasil p value < 0.05 dan dilanjutkan uji mann-whitney tes untuk melihat perbandingan penurunan dua variabeldengan masing-masing p value < 0.05 dengan demikian secara statistik ada perbedaan antara kelompok intervensi dankelompok kontrol.Kesimpulan: Kombinasi terapi musik dan teknik relaksasi nafas dalam efektif dalam menurunkan nyeri pada pasienpost kateterisasi jantung.

Abstract

The pattern of payment that is applied by Jaminan Kesehatan Nasional (JKN) in advanced health facilities (hospitals) is a prospective payment pattern. Prospective Payment pattern known as casemix is a grouping diagnoses and procedures with reference to clinical characteristics which is similar and resource used or maintenance cost is similar. The grouping is done by software groupers. Bagas Waras hospital in making diagnosis grouping sometimes experiences the negative difference (loss) between the real cost of hospital fees in accordance with the Indonesia Case Based Group (INA CBGs) cost package approved by Badan Penyelenggara Jaminan Sosial (BPJS).This study is to determine the cause of the change file BPJS claims that the negative consequences of coding problems after qualitycontrol of INA-CBGs coding in Bagas Waras hospital Klaten. This study was a qualitative research design action (Action Research). This study used a model of action research (Action Research) of S Kemmis and R McTaggart which is designed through two cycles of the procedure: (1) planning , (2) action (3) observation, (4) reflection  in each cycle. The research instrument used a questionnaire. Analysis of the data used qualitative analysis and quantitative analysis.The results showed that the frequency of claims BPJS start of the cycle I, II and III, there was a decreasing in the proportion of the, it is due to an error coding, Leght OfStay (LOS) extends, the cost of medical action that is excessive, and Excess Cost investigations. In improving the ability of diagnosis coding, Bagas Waras hospitalcontinuously strives to improve their ability to apply the Hospital Information System (SIRS).The cause of negative claim in a file BPJS after INA-CBGs control coding in Bagas Waras hospital  is the employees do not understand the overall coding system, and  do not run the monitoring and evaluation system of coding.  Keyword : Negative Claim of BPJS, Coding INA CBG's

Abstract

Death and complication due to surgeries or surgical actions is a global health problem. The WHO estimates that at least half a million deaths that are caused by surgeries can be prevented. On 2008, the WHO release a campaign about safe surgery and released the surgical safety checklist to improve the quality of our surgery cases and decrease the number of complication and deaths caused by surgeries. Hospital accreditation is used to improve the quality of hospital management, including in reducing numbers of death and complication due to surgeries. This is a quantitative descriptive approach study. The study population used were all surgical safety checklist. Sample number of 75 surgical safety checklist, 15 surgical safety checklist each from June 2017, August 2017, January 2018, June 2018, and January 2019. Data analysis is shown by statistic table and percentage. There was a change of number in completing compliance of the surgical safety checklist before and after hospital accreditation. It showed that nearing hospital accreditation, the compliance rate was almost 100% on all the items. This continued a month after, but some of the items had a lower rate of compliance further after the hospital accreditation. The Conclusion is the number of completing compliance of the surgical safety checklist nearing hospital accreditation is higher than after hospital accreditation. The sign in the item has the highest rate of completing compliance whereas the sign out item had the lowest number of compliance.

Abstract

Latar Belakang: Indonesia menjadi peringkat ketiga Asia kematian korban kecelakaan lalu lintas. Penatalaksanaan patah tulang dilakukan dengan cara membatasi pergerakan tulang (imobilisasi) dapat dilakukan dengan cara Open Reduction Internal Fixation (ORIF). Managemen nyeri post operasi fraktur merupakan tindakan sangat penting dapat berupa nafas dalam dan mendengarkan musik gamelan.Tujuan: Untuk menganalisis pengaruh nafas dalam dan mendengarkan music gamelan terhadap tingkat nyeri pasien post operasi frakturMetode: Penelitian dengan Jenis   kuantitatif   pendekatan quasi ekeperimen ( pre-post test with control group design ). Populasi penelitian ini adalah pasien post operasi ORIF (Open Reduksi Internal fixation) yang mengalami rawat inap dengan sampling menggunakan teknik  simple random sampling sejumlah  30 responden. Kelompok intervensi diberikan nafas dalam dan mendengarkan musik gamelan, kelompok kontrol dengan nafas dalam dan mendengarkan musik  instrumental.Penilitian dilaksanakan bulan Agustus – September 2018Hasil: Uji statistik yang digunakan adalah uji t- berpasangan yaitu paired t-test dan independent t-test. Penelitian menunjukkan bahwa niali p value 0,05 sedangkan untuk nafas dalam dan mendengarkan musik gamelan serta nafas dalam dan mendengarkan musik instrumental dengan nilai p value 0,000, Pada independent t-test nilai p = 0,03. Nilai p0,05 Sehingga nafas dalam dan mendengarkan musik gamelan berpengaruh terhadap perubahan tingkat nyeri secara bermakna pada pasien post operasi ORIFKesimpulan: Terdapat pengaruh nafas dalam dan mendengarkan musik gamelan terhadap perubahan tingkat nyeri pasien post operasi di RSUD dr. Postoperative fracture pain management is a very important action that can take the form of deep breathing and listen to gamelan music.Objective: To analyze the effect of a deep breath and listen to gamelan music on the pain level of post-fracture surgery patientsMethod: Research with quantitative type quasi-experimental approach (pre-post test with control group design). The population of this study was patients post-ORIF (Open Reduction Internal Fixation) surgery who experienced hospitalization by sampling using a simple random sampling technique of 30 respondents. The intervention group was given a deep breath and listened to gamelan music, the control group with deep breathing and listening to instrumental music. The study was held in August - September 2018Results: The statistical tests used were paired t-test and independent t-test. Research shows that the value of p 0.05 value while for a deep breath and listening to gamelan music and deep breath and listening to instrumental music with a p-value of 0.000, In the independent t-test the value of p = 0.03. Value p 0.05 so that deep breath and listening to gamelan music have a significant effect on changes in pain levels in patients post-ORIF surgeryConclusion: There is the influence of a deep breath and listening to gamelan music on changes in the level of pain of postoperative patients in Dr. Soediran General Hospital Sumanguno Wonohiri. Keywords: gamelan music, pain

Abstract

Komunikasi terapeutik diperlukan perawat, pasien kanker dan keluarganya dalam pelayanan kanker dan paliatif dalam memberikan informasi dan memenuhi kebutuhan pasien. Artikel ini untuk mereview artikel yang menyediakan informasi tentang Komunikasi terapeutik dalam pelayanan kanker dan paliatif. Metode: Pencarian dari 3 database yaitu PubMed, Ebscohost, dan ProQuest, serta dari Google Scholar search engine di cari menggunakan kata kunci : Komunikasi terapeutikor effective communication or therapeutic communication or discussion or conversation, Paliatif or Palliative care or terminal care, cancer care, Nurse*, Cancer patient*. Hasil : 17 artikel yang terinklude dalam review ini . Karakteristik komunikasi terapeutik: menunjukan empati dan dukungan emosional, rasa hormat or dignity, informasi yang jelas, terbuka dan jujur, mengklarifikasi dan fokus pada informasi yang lebih disukai dan dibutuhkan pasien dan keluarga, menghindari pemberian harapan palsu dan kata-kata pelembut, menggunakan bahasa yang mudah dimengerti dan penggunaan nonverbal, pendengar secara aktif dan baik. Manfaat komunikasi terapeutik yaitu meningkatkan kepuasaan pasien dan keluarga dan membangun hubungan interpersonal. Hambatan berasal dari perawat, pasien dan institusional. Strateginya yaitu training skill communication bagi perawat. Kesimpulan : perawat perlu mengetahui karakteristik, hambatan, manfaat serta strategi berkomunikasi terapeutik karena komunikasi tersebut adalah inti dari pelayanan kanker dan paliatif.

Abstract

Background: Isometric handgrip exercise is an alternative to decrease blood pressure using handgrip performed with static exercise on contracting muscles, without any changes in muscle length or joint movement. Objective: The research objective to know the comparison of isometric handgrip exercise and walking towards to systolic and diastolic blood pressure to patient with hypertension. Method: This research was quasi experiment design, divided into two intervention groups A (isometric handgrip exercise and walking) and intervention B (walking). The subjects of the study were patients with primary hypertension with the number of each intervention group as many as 22 respondents, so there were 44 respondents totally. Data were collected using simple random sampling where all the patients, with hypertension who lived in the work area of Community Health Center Pohjarak. then analyzed using wilcoxon and Mann-Withney tests on dependent variable. Results: The analysis results of difference in the intervention groups A and B towards decreasing blood pressure, showed that the systolic blood pressure was obtained p value 0.026, indicating that there was difference, whereas the diastolic blood pressure was obtained p value 0.031, indicating that there was difference. Conclusion: Implementation of isometric handgrip exercise and walking can decrease systolic and diastolic blood pressure to patients without hypertention.

Abstract

COVID-19 outbreak is a challenge for health facilities in providing guarantees for patient safety and works safety. The dialysis clinic has a high risk and vulnerable environment to the spread of COVID-19, but it has to keep providing services to patients with chronic kidney disease. One way to ensure quality and patient safety is to apply isolation precautions. This type of study was is mixed-method research, with a sequential explanatory approach. The population in this study was nurses and doctors at the HD Clinic of Nitipuran. The sampling technique in the quantitative research used total sampling; while qualitative research used the purposive sampling technique. Quantitative data analysis was Spearman rank analysis, while qualitative research used Miles and Huberman analysis. The behavior of implementing isolation precautions in the hemodialysis services at the HD Clinic of Nitipuran was carried out more strictly, supported by a high level of hygiene awareness, supervision and management support. However, the high workload and inadequate infrastructure are obstacles was carried out isolation precautions. Factors related to the behavior of isolation precautions were namely education, knowledge, training, attitudes, cultural values, motivation, supervision, management support and environmental facilities. The strategy for implementing isolation precautions was namely the SO strategy was in the first quadrant which had the highest score, with priorities in making policies and efforts to prevent post-exposure infections, increasing the efficiency of facilities and infrastructure and always reviewing policies and internal guidelines.Abstrak: Wabah COVID-19 menjadi tantangan bagi fasilitas kesehatan dalam memberikan jaminan keselamatan pasien dan keselamatan kerja. Klinik dialysis memiliki lingkungan yang berpotensi tinggi dan rawan penyebaran COVID-19, namun harus tetap memberikan pelayanan pada penderita penyakit ginjal kronik. Salah satu cara untuk menjamin mutu dan keselamatan pasien adalah dengan menerapan kewaspadaan isolasi. Jenis penelitian ini merupakan jenis penelitian mix method, dengan pendekatan sequential explanatory. Populasi dalam penelitian ini adalah perawat dan dokter klinik HD Nitipuran. Teknik sampling pada penelitian kuantitatif menggunakan total sampling dan penelitian kualitatif menggunakan teknik purposive sampling. Analisis data kuantitatif  dengan analisis spearman rank, sedangkan penelitian kualitatif menggunakan analisis miles and huberman. Perilaku penerapan kewaspadaan isolasi pada pelayanan hemodialysis di Klinik HD Nitipuran dilakukan lebih ketat, didukung kesadaran kebersihan dan pengawasan serta dukungan manajemen yang tinggi. Beban kerja yang tinggi dan sarana prasana yang belum memadai menjadi penghambat dalam melakukan kewaspadaan isolasi. Faktor yang berhubungan dengan perilaku kewaspadaan isolasi yaitu factor pendidikan, pengetahuan, pelatihan, sikap, nilai budaya, motivasi, supervise, dukungan manajemen dan sarana lingkungan. Strategi penerapan kewaspadaan isolasi diketahui bahwa strategi SO berada di kuadran pertama memiliki skor tertinggi, dengan prioritas membuat kebijakan dan upaya dalam pencegahan terjadinya infeksi paska pajanan, meningkatkan daya guna fasilitas sarana dan prasarana serta selalu melakukan peninjauan kembali terkait kebijakan dan pedoman internal.
